2.Relationship between expression of sex hormone receptor and tumor central artery in meningiomas

Objective To investigate the expression of sex hormone receptor (SHR) in meningioma and its relationships to the origin of tumor central artery.Methods The expressions of sex hormone receptors, including progesterone receptor (PR), androgen receptor (AR) and estrogen receptor (ER) in 43 cases with meningioma were detected by immunohistochemically streptavidin-peroxidase (SP) method. These data combined with the characteristics of digital subtraction angiography (DSA) were analyzed.Results The expression of ER in meningioma was not related to the origin of tumor central artery. AIM: To examine COX-2 expression in esophageal carcinoma,and to study relationships between COX-2 expression and clinicopathological features and prognosis of esophageal carcinoma patients.METHODS: 89 paraffin-embedded tissue samples from patients with esophageal carcinoma were collected,its clinicopathological features such as tumour differentiation,depth of invasion,length and site of the tumor,regional lymph node metastases,distant metastasis were recorded.Survival time of 81 cases were also recorded.By SP immunohistochemistry method,the expression of COX-2 in tumor samples was examined.RESULTS: COX-2 expression in esophageal carcinoma was markedly higher than that in nomal esophagus,the expression was higher in less differentiated and deeper invaded cases(P0.05).Cases of esophageal carcinoma with lower COX-2 expression had longer survival time than those with higher COX-2 expression(P

Objective To discuss the diagnostic value of dynamic contrast-enhanced MRI in differentiating vertebral metastatic tumors from benign deseases after studying the early changes of signal intensity.Methods Twenty patients were scanned by dynamic contrast-enhanced MRI.The parameters of signal intensity were measured and calculated,and time-intensity cures(TICs)were drawn.Then,they were compared with those of normal verte- braes,the characteristics were analyzed.Results The parameters of vertebral metastatic tumors:peak signal intensi- ty,signal intensity minimum signal intensity of post-peak,peak slope,initial ratio of enhancement,maximum ratio of enhancement was respectively(50.61?11.38),(46.75?10.23),(0.73?0.12),(0.33?0.08),(1.03?0.31); that of normal vertebraes was respectively(40.53?12.12),(34.72?14.06),(0.31?0.11),(-0.25?0.05), (0.69?0.28);and ten normal cases showed negative in initial percent of enhancement,but only two abnormal ver- tebraes showed that.All of the above had significant statistical difference.Four types of TICs were concluded:early rapid rise and early rapid descent followed by steady phase(type A),persistently rise(type B),rise phase followed by steady phase(type C),rise phase followed by rapid descent phase(type D).TICs of abnormal vertebraes were respec- tively 6,5,5,5 cases;types of normal vertebraes were mainly type C,15 cases.Type A and type B and the type with the characteristic of early rapid descent phase were features of abnormal vertebraes;type C was the feature of normal ones.Conclusions Dynamic contrast-enhanced MRI could quantitatively show the characteristics of early changes of signal intensity of vertebral metastatic tumors,which were significant statistically different from those of normal ver- tebraes.Dynamic contrast-enhanced MRI was worth performing in the early diagnosis of vertebral metastatic tu- mors.
6.Surgical treatment of ulnar lateral mass fractures of distal radius.

OBJECTIVETo investigate the type and corresponding operation methods of ulnar lateral mass in distal radial fractures.
METHODSFrom January 2004 to September 2010,32 cases of distal radial fractures with ulnar lateral mass were treated by surgical incision and relocate ulnar lateral mass,including 23 males and 9 females with an average age of (34 +/- 9) years old ranging from 28 to 65 years old. According to the classification of Melone-Doi, 2 cases with type I, 24 cases of type II, 6 cases with type III. The operative appoach involved palm-radial, palm-ulnar, radial-dorsal. Among them, 28 patients with closed fractures were fixed with plate, 4 cases (including 2 cases of open fractures) were fixed by Kirschner pin and braces.
RESULTSThirty-two patients were followed up for 8 to 18 months with an average of 12 months. All fractures were healing without complications. According to modified Gartland-Werley scoring system (GWSS), the total score was 1.12 +/- 0.45, the remain deformity was 0, subjective evaluation was 0.50 +/- 0.30, objective evaluation was 0.30 +/- 0.21, complications was 0.40 +/- 0.09; the result was excellent in 21 cases, good in 11 cases.
CONCLUSIONThe location of ulnar lateral mass in distal radial fractures is one of the important prognostic factors in wrist function and surgical treatment is an effective method of fixing ulnar lateral mass in distal radial fractures.

7.The efficacy of different embolic agents to block bronchial artery for massive hemoptysis secondary to bronchiectasis or pulmonary tuberculosis

Objective To investigate the efficacy of gelatin sponge particles(GSP)or polyvinyl alcohol particles (PVA) for hemoptysis secondary to bronchiectasis or pulmonary tuberculosis. Methods The clinical data on 271 patients with bronchiectasis- or tuberculosis-induced hemoptysis were retrospectively analyzed. The efficacy and rates of recurrence and complications were analyzed. Results A total 271 patients were included in this study, 176 of whom suffered from bronchiectasis and the rest 95 had tuberculosis. One-week cure rate was signifi-cantly higher in bronchiectasis group than in tuberculosis group(73.3%vs. 46.3%,P<0.05),and one-year recur-rence rate was significantly lower in bronchiectasis group(17.6%vs. 26.3%,P<0.05). One-year recurrence rate was slightly higher in patients receiving GSP than in those undergoing PVA(bronchiectasis group:22.2%vs. 10.3%, P<0.05;tuberculosis group:28.8%vs. 22.2%,P>0.05). No severe complications occurred. Conclusions In-terventional artery embolization therapy for hemoptysis secondary to bronchiectasis is better than tuberculosis-induced hemoptysis,and PVA is more effective than GSP. Recurrence of massive hemoptysis mostly occurrs within one month ,and most of the patients are complicated with blood supply and have a history of hemoptysis.
8.Back skin donor site:A good choice for repairing Ⅲ? burn wound in children

Objective: It is difficult to repair large-area Ⅲ?burn wound in children.Here we introduce the advantages of the back skin donor site in repairing Ⅲ?burn wound in children.Methods: This clinical study included 15 children with Ⅲ?burn,ranging in age from 18 months to 11 years(mean 7.6 years),with the Ⅲ?burn wound area averaging at 12.6% of the total body surface.The back skin donor site was used to repair the wound.The functional site,such as the joint,was preferentially considered if the skin graft was not enough for all the wounds.Results: Most of the skin grafts survived and the donor sites on the back healed very well.The patients were followed up for six months to three years,which revealed that the skin grafts remained soft without dysfunction and the scar hyperplasia was mild on the donor site.Conclusion: The back donor site supplies larger-area skin with good quality.The donor site heals fast.After healing,patients could sleep on back and compress the scar so as to prevent scar hyperplasia,and the scar does not affect the patient's appearance.With all these advantages,the back donor site is a good choice for repairing Ⅲ?burn wound in children.
9.Comparative study on flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy for treating renal pelvis calculus

Objective To investigate the effects of flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy for treating renal pelvis calculus.Methods The clinical diagnosis and treatment data in 65 patients with single renal pelvis calculus treated in department of urology surgery of our hospital from Jan.2013 to Jan.2016.Among them,33 casesas the group A adopted flexible ureteroscopic lithotripsy and 32 cases as the group B adopted percutaneous nephrolithotomy.The operative time,lithotripsy success rate,complications and average hospitalization time were compared between the two groups.Results In the renal pelvis calculus diameter ≥ 2 cm,the operation time and lithotripsy success rate in the group B were significantly better than those in the group A,but the intraoperative blood loss and average hospitalization time in the group A were significantly better than those in the group B,the differences were statistically significant (P<0.05).In renal pelvis calculus diameter ≤2 cm,the lithotripsy successful rate had no statistically significant difference between the two groups (P>0.05),but the group A was significantly better than the group B in the operative time,intraoperative blood loss and average hospitalization time (P<0.05).Conclusion Renal pelvis calculus diameter <2 cm is suitable for selecting flexible ureteroscopic lithotripsy,but which ≥2 cm is suitable to select percutaneous nephrolithotomy.
10.Management of lactational infection after breast augmentation by polyacrylamide hydrogel injection

Objective: Breast augmentation by polyacrylamide hydrogel(PAHG) injection has resulted in a series of adverse events in many female patients.This article summarizes the authors' experience in the diagnosis and treatment of lactational infection after breast augmentation by PAHG injection and discusses related preventive measures.Methods: This study included 2 patients who had developed infection during lactation 5 years after breast augmentation with PAHG.We identified the exact position of the PAHG and vomica by color Doppler,computerized tomography and magnetic resonance.We made incisions in the submammary fold or mammary areola to take out the PAHG,drain the pus and place the vacuum pressure tube,followed by application of pressure bandages.Results: One of the patients healed without any complication,while the other developed latex leak in both breasts,but finally cured after milk regurgitation and repeated changing of dressings.Conclusion: Lactational infection after breast augmentation by PAHG injection is detrimental to female patients.Preventive measures should be taken as early as possible.For patients with this infection,operation should be performed to take out the PAHG and drain the pus.Sufficient draining and milk regurgitation are necessary to avoid latex leak.
