1.Digital rapid prototyping implant template assists anterior tooth restoration in maxillary esthetic zone:a follow-up of 12 months

BACKGROUND:Dental implantation in maxilary esthetic zone is difficult to achieve desired outcomes.
OBJECTIVE:To observe the feasibility of digital rapid prototyping implant template for anterior implantation.
METHODS: Eighty patients scheduled to receive anterior implant treatment were enroled. According to the wishes of patients, they were divided into two groups, with 40 cases in each group, and were given routine implant treatment (control group) and digital rapid prototyping template-aided dental restoration (observation group). After implantation, the X-axis, Y-axis and Z-axis errors of the two groups were measured, and the patients were folowed up for 12 months.
RESULTS AND CONCLUSION:After implantation, the errors of X, Y and Z axes in the observation group was significantly lower than those in the control group (P< 0.05); at 24 hours after implantation, patients in the two groups showed good appearance of the anterior tooth with no dental prosthesis loosing. Imaging examinations in the two groups showed no obvious bone resorption. These findings indicate that the aid therapy using digital rapid prototyping can obtain satisfactory therapeutic effect on anterior tooth restoration in the maxilary esthetic zone and precisely localize the dental implantation site.
2.Implantation accuracy is not affected by head and angle deviation following computer aided design guide for repair of maxillary posterior tooth defects

BACKGROUND:Maxilary posterior tooth defects can be repaired using the dental implant. However, implantation accuracy is not ensured by traditional plaster models used as implant template, which leads to the incidence of complications,andaffectsthe repair effects.
OBJECTIVE:To study application effects of surgical guide plates designed using the computer aided design (CAD)/computer aided manufacturing (CAM) technology for the repair of maxilary posterior tooth defects.
METHODS:Implant treatment was performed in 30 patients with maxilary posterior tooth defects. Surgical guide plate was made using the CAD/CAM technology after CT scan of the jaw bone. The repair was conducted with guideplateassistant. The deviation between the virtual and actual implant position was measured after implantation. The clinical repair effects and periodontal tissues were observed during the 12-month folow-up.
RESULTS AND CONCLUSION:Thirty-sixCAD/CAM surgical guide plates in 30 patients werestabilized after implantation. Thirty-six implants were emplaced precisely with the assist of the surgical guide plates. There was no any problem during the process of implantation. The deviation of the head between the virtual and actual implant position was very smal, which did not affect the accuracy of implant placement. Before and 6 and 12 months after the repair, gingival crevicular fluid, tumor necrosis factor-α, and sulcus bleeding index were not obviously changed in the patients. Our results suggest that the implant head deviation and angle deviation appearduring the repair process of maxilary posterior tooth defects using CAD/CAM surgical guide for dental implants;however, the implantation accuracy is not affected. The repair effect is ideal and periodontal tissue is in good status after implantation.
3.Targis/composite fiberisused to repairyoung permanent tooth defects

BACKGROUND:Targis/composite fiber is a new type of non-metalic repair material that has good hardness and strength, but it is rarely reported on its application in the repair of young permanent tooth defects.
OBJECTIVE:To investigate the clinical effect of Targis/composite fiber in repairing young permanent tooth defects.
METHODS:Sixty cases of young permanent teeth defects 10-17 years of age were enroled, including 27 males and 33 females. Al cases were subjected to Targis/composite fiber repairing. After 36 months of folow-up, alkaline phosphatase level, probing depth, gingival sulcus bleeding index, tooth mobility, plaque index, gingival crevicular fluid volume and aspartate aminotransferase level in patients were detected; the integrity and edge of dental prosthesis and the color matching.
RESULTS AND CONCLUSION:After 36 months, the alkaline phosphatase level in the patients had no significant changes (P> 0.05), but the probing depth, gingival sulcus bleeding index, tooth mobility, plaque index, gingival crevicular fluid volume and aspartate aminot ransferase level were decreased significantly compared with those before repair (P< 0.05). Additionaly, there were two cases of incomplete dental prosthesis, six of color mismatching, andtwo of unsealed prosthesis edge. These findings indicate that the Targis/composite fiber for repairing young permanent tooth defects has good histocom patibility maintains periodontal health in good condition, is a kind of ideal repair materials.
5.Relationship between Serum Levels of N-Terminal pro Brain Natriuretic Peptide and Creatine Kinase Myocardial,Cardiac Troponin I in Children with Viral Myocarditis

Objective To investigate the association between serum N-terminal pro brain natriuretic peptide(NT-ProBNP) and creatine kinase myocardial(CK-MB),cardiac troponin I(cTnI) in children who were diagnosed as viral myocarditis(VMC),and to explore the clinical significance.Methods Fifty children with VMC were selected as VMC group who were further divided into cTnI(+)group and cTnI(-)group.Fifteen healthy children were selected as healthy control group.The levels of myocardial enzyme of the 2 groups were tested by automatic biochemical analyzer and level of NT-proBNP was detected by specific NT-proBNP enzymelink immunosorbent assay(ELISA) kit.SPSS 13.0 software was used to analyze the data.Results 1.There were 23 cases with positive cTnI and 27 cases with negative cTnI.The levels of NT-proBNP in VMC group including cTnI positive and cTnI negative were significantly higher than those in healthy control group (P0.05).2.In the group with positive cTnI,the level of NT-proBNP was correlated with cTnI.The coefficient of determination was 0.17.3.In the group with negative cTnI,the level of NT-proBNP was correlated with CK-MB.The coefficient of determination was 0.34.NT-proBNP presented a increasing trend during interval of 25-100 U?L-1 and 175-255 U?L-1 of CK-MB.Conclusions The levels in the serum NT-proBNP in VMC children were higher than those in healthy children.Serum detection NT-proBNP may be applied as a new approach in diagnosis of VMC in children.

9.Clinical analysis of 10 children with Wegener granulomatosis

Objective To investigate the characteristics of Wegener granulomatosis in children and to improve its recognition. Methods Ten pediatric patients with Wegener granulomatosis were studied retrospectively by clinical manifestations, serum examination, pathological data, treatment, etc. Results All children had clinical evidence of upper respiratory tract and lungs involvement; kidney lesion occurred in 6 cases(6/10); joints, skin, eyes and nervous system were also involved with different degree, cANCA (PR3) showed positive in 8 cases; Pathological examination (1 kidney sample, 2 nasal mucosa samples, 2 skin samples) showed granulomatous vasculitis and crescents were found in renal biopsy; 7 cases were treated with corticosteroid plus cyclophosphamide, 1 case was treated with corticosteroid plus methotrexate, and improvement were seen in all of them. Conclusions Wegener granulomatosis in children have diverse clinical manifestations and clinical diagnosis is difficult. The upper and lower respiratory tract and kidney are most commonly involved organs. ANCA inspection is characteristic. Treatment with corticosteroid and immunosuppressive can achieve good outcome.
10.Comparison of intramedullary nail and retrograde intramedullary nail combined with hollow lag screw fixation for treatment of ipsilateral femoral neck and shaft fractures

Objective To compare the effect of proximal femoral intramedullary nail fixation and femoral retrograde intramedullary nail combined with hollow lag screw fixation in the treatment of ipsilateral femoral neck and shaft fractures.Methods Data of 42 patients with ipsilateral femoral neck and shaft fractures who were admitted in our hospital from January 2007 to January 2016 were retrospectively analyzed.According to different fixation methods,the patients were divided into two groups:single intramedullary nail group (SIN group) (20 cases,treated with single proximal femoral intramedullary nail,15 males and 5 females with an average age of 39.4);combined internal fixation group (CIF group) (22 cases,treated with femoral distal intramedullary nail combined with femoral neck hollow lag screw,17 males and 5 females with an average age of 42.2 years).In the SIN group,4 cases were type A,9 cases type B,7 cases type C according to AO classification.And 14 cases belonged to Ⅰ or Ⅱ (stable fracture),6 cases of type Ⅲ or Ⅳ (unstable fracture) according to Garden classification.In the CIF group,3 cases were type A,10 cases type B,9 cases type C according to AO classification.And 16 cases belonged to type Ⅰ or type Ⅱ,6 cases type Ⅲ or Ⅳ according to Garden classification.The incision length,operation time,intraoperative blood loss,fracture healing time,postoperative weight time,stability of internal fixation,hip function score,pain in the knee and postoperative complication rate were compared between the two groups.Results There was no significant difference in gender,age and fracture classification between the two groups.The operation time (75.0±10.2 min),intraoperative blood loss (150.6±80.4 ml),hip Harris score excellent rate (80%) of incidence of knee pain in postoperative 3 months (10.0%) of the SIN group were significantly lower than those of the CIF group (105.2±18.4 min,180.0±56.8 ml,86.4% and 31.8%).The length of incision (8.3±1.4 cm) in the SIN group was significantly longer than that in the CIF group (4.0±0.6 cm).There was no significant difference in fracture healing time (SIN group 20.O±4.0 weeks,CIF group 19.6±4.2 weeks) and postoperative weight time (SIN group 8.2±4.0 weeks,CIF group 8.0±4.2 weeks) between the two groups.The main complications of the two groups were delayed union (3 cases of SIN group and 1 case of CIF group),nonunion of fracture (1 case of SIN group) and,coxa vara (1 case of SIN group).The complication of SIN group (30%) was significantly higher than that of CIF group (4.5%),which occurred mainly in unstable femoral neck fracture cases.Conclusion Both proximal femoral intramedullary nail fixation and femoral retrograde intramedullary nail combined with hollow lag screw can effectively treat femoral ipsilateral femoral neck and shaft fracture.Femoral retrograde intramedullary nail combined with hollow lga screw has more therapeutic advantages for unstable femoral neck fractures (Garden Ⅲ and Ⅳ).
