1.MRI characteristic of proximal femur bone marrow edema syndrome.

OBJECTIVETo study the MRI features of proximal femur bone marrow edema syndrome for further improve the understanding of the disease.
METHODSMRI imaging of 10 patients with proximal femur bone marrow edema syndrome was retrospectively reviewed,including 6 males and 4 females with an average age of 41.5 years old ranging from 36 to 57. The courses of diseases ranged from 1 week to 3 months. Among them, 9 cases had clinical manifestations of sudden hip pain, 7 cases had limited ability of walking and hip movement;all patients had no obvious injury history, non of the female patients was pregnant. All patients were followed up from 3 to 12 months, the following-up were topped after MRI when the symptoms disappeared for 3 months.
RESULTSThe MRI demonstrated diffuse bone marrow edema involving the femoral head, neck and the inter-trochanteric region, 13 hips of 10 patients with bone marrow edema included 6 cases in grade 1, 5 cases in grade 2,2 cases in grade 3; 9 hips with hip hydrarthrosis included 6 hips in grade I ,1 hip in grade II, 2 hips in grade III. After treatment for 3 to 12 months the hip symptoms of the patients disappeared and MRI images were normal.
CONCLUSIONMRI is useful in defining the location and extent of proximal femur bone marrow edema syndrome.

2.Selection and clinical application of anti-hypertensive drugs in children

Hypertension in children is defined as systolic blood pressure and/ or diastolic blood pressure≥95th percentile for age,gender and height,on at least 3 occasions. Persistent hypertension despite non - pharmacologic measures should be treated with anti - hypertensive drugs. Angiotensin - converting enzyme inhibitors,angiotensin receptor blockers,calcium channel blockers,β - blockers and diuretics are safe,effective,and well tolerated in children. This article describes the selection and clinical application of anti - hypertensive drugs in children.
3.Clinical observation of Montmorillonite combined with Compound Eosinophil-Lactobacillus Tablet in treatment of pneumonia infants of antibiotic associated diarrhea

Objective To discuss the clinical observation of Montmorillonite combined with Compound Eosinophil-Lactobacillus Tablet in the treatment of pneumonia infants of antibiotic associated diarrhea (AAD).Methods Eighty pneumonia infants with antibiotic associated diarrhea were selected,they were divided into two groups according to the random number table method.The observation group (41 cases) was given Montmorillonite.The control group (39 cases) was given Montmorillonite and Compound Eosinophil-Lactobacillus.The efficacy of Montmorillonite combined with compound Eosinophil-Lactobacillus Tablet in the treatment of pneumonia infants of antibiotic associated diarrhea was evaluated by efficacy,clinical symptom,immune function changes and adverse reactions during the treatment period.Results After treatment,the effective rate of observation group was higher than that of the control group (P < 0.05).After treatment,the fever time,correcting dehydration time,and antidiarrheal time of observation group was shorter than that of the control group (P < 0.05).After treatment,the serum IgG,IgA,and T lymphocyte subsets of CD3+,CD4+ levels were significantly increased in the observation group (P < 0.05).There was no statistical significance on CD8+ before and after treatment.In contrast,there were no statistical significance on the serum immunoglobulin and T lymphocyte subsets before and after treatment in the control group.During the treatment period,no adverse reaction occurred in two groups.Conclusion In summary,Montmorillonite combined with Compound Eosinophil-Lactobacillus Tablet had a good effect on AAD.They could quickly improve the symptoms of diarrhea and immunoglobulin levels in children.It was worthy of clinical use.
5.Application of micro transesophageal echocardiography (micro-TEE) in neonat and infant cardiac surgery

Objective To evaluate the value of phased array probe of transesophageal echocardiography miniature (micro-TEE) in the neonatal and infant cardiac surgery intraoperation.Methods Micro-TEE probe was used in children with weight ≤5 kg undergoing cardiac surgery to detect the atrial and ventricular cavity size,the left and right ventricular outflow tract diameter were detected by transthoracic echocardiography(TTE) 3-7 d after operation,and compared with the measured value immediately after the TEE.Results Before and after intubation,airway index not statistically significant (P > 0.05).Preoperative TEE and TTE inspection results were consistent with intraoperative diagnosis.Postoperative TEE measuring intracardiac structural data (each room,chamber size and left and right ventricular outflow tract diameter) and postoperative TTE measurement value representing the difference was not statistically significant (P > 0.05).Conclusions Micro TEE intraoperative guardianship of low birth weight infants is safe and feasible,and can supplement and improve the preoperative diagnosis,and can accurately and timely evaluate postoperative curative effect.
6.Pulsed wave Doppler ultrasonic manifestations of acute experimental incomplete testicular torsion

Objective To explore the pulsed wave Doppler (PWD) manifestations of acute experimental incomplete testicular torsion. Methods Eight healthy dogs were surgically modeled to obtain unilateral acute testis torsion from 180° to 630°, respectively. The peak systole velocity (PSV) and resistance index (RI) of intratesticular artery, capsular artery and arteria spermatica interna were measured with PWD before torsion, 2 h, 4 h and 6 h after torsion. Testes were examined pathologically after the experiment. Results PSV and RI of capsular artery and intratesticular artery decreased gradually with the extension of torsional time (P<0.05). PSV and RI of superior, torsional and inferior segments of arteria spermatica interna varied greatly and showed no strong regularity (P>0.05) . The pathological results of testes showed no distinct changes or just mild interstitial hyperemia and edema. Conclusion The decrease of PSV and RI of capsular artery and intratesticular artery, especially RI have referential value in the judgement of acute experimental incomplete testicular torsion, whereas changes of PSV and RI of superior, torsional and inferior segments of arteria spermatica interna are not reliable.
7.Relationship between renal function and diabetic osteoporosis in the diabetics

BACKGROUND: Most of the literatures only reported that there is a great difference between diabetics who have a high 24-hour urinary albumin excretion rate and those without renal damage, but there is no obvious difference between cases of microalbuminuria and those without renal damage.OBJECTIVE: To probe into the relationship between diabetic nephropathy and osteoporosis.DESIGN: A case-control study.SETTING: Department of Endocrinology, West China Hospital of Sichuan University.PARTICIPANTS: According to the diagnostic standard set by the American Diabetes Association in 1997 (fasting blood glucose ≥ 7 mmol/L,postprandial blood glucose ≥ 11.1 mmol/L), 96 diabetic patients were selected, including 56 males < 60 years old and 40 females who had not entered the menopausal period, excluding the influence of age and osteoporosis caused by menopause. The average age of the subjects was (48.7±10.5)years, their duration were from one month to twenty-one years with the aver age of (7.85±2.56) years, and their general information had no significant differences.METHODS: According to the urine albumine excretion rate and renal function, the patients were divided into four groups: normal albuminuria group (n=48), microalbuminuria group (n=28), macroalbuminuria group (n=15), renal failure group (n=5). The bone mineral densities of lumbar spines (L2-4), femoral neck, Ward's triangle and trochanter were detected with Dual energy X-ray absorptiometry, and then the fasting blood glucose,2-hour postprandial blood glucose, glycosylated hemoglobin A, alkaline phosphatase, calcium, phosphorus, blood urea nitrogen, creatinine and urine albumine excretion rate were compared between the patients with and without osteoporosis.MAIN OUTCOME MEASURES: The fasting blood glucose, 2-hour postprandial blood glucose, glycosylated hemoglobin A, alkaline phosphatase, calcium, phosphorus, blood urea nitrogen, creatinine and urine albumine excretion rate, as well as the bone mineral density, were observed in all the patients.RESULTS: The L2-3 bone mineral density in the macroalbuminuria group was significantly different from that in the microalbuminuria group (P < 0.05). The proximal femur bone mineral density in the microalbuminuria group was significantly different from that in the normal albuminuria group (P < 0.05). The bone mineral densities of proximal femur and lumbar spine in the renal failure group were significantly decreased as compared with those in the other groups (P < 0.01). The disease course,glycosylated hemoglobin A, alkaline phosphatase and body mass index were significantly different between the patients with and without osteoporosis (P < 0.05).CONCLUSION: The incidence rate of osteoporosis is increased with the aggravation of nephropathy, and diabetic nephropathy may be closely correlated with the decrease of bone mineral density and the occurrence of osteoporosis.
8.The killing effect of NY-ESO-1-sensitzed dendritic cells-induced cellular toxic lymphocyte on human retinoblastoma cell

Background Cell immunologic therapy for retinoblastoma(RB)is becoming a hot research topic.Cancer-testis antigen is a human immunogenic protein and is used to treat some tumors.However,its effect on RB has not been investigated.Objective The present study was to discuss the antigen specific anti-tumor effect of cytotoxic T lymphocytes(CTL)induced by the cancer-testis antigen,NY-ESO-1-sensitized dendritic cells(DCs),on human RB.Methods PCR was performed to amplify target gene fragments from the NY-ESO-1 plasmids,and then the target gene fragments were digested with the restriction enzymes SalI and EcoRI.Harvested fragments were inserted into the pDC316 plasmid to construct the recombinant plasmid pDC316/NY-ESO-1.The expression of NY-ESO-1 protein in human RB cells strain,HXO-RB44,was detected by immunofluorescence and Western blot.Monocytes were isolated from 60 ml of peripheral blood from a healthy donor using Ficoll density-gradient centrifugation with a cell density of 1 × 107/ml.DCs isolated from blood were stimulated with recombinant human granulocyte-macrophage colony stimulating factor(rhGM-CSF)and recombinant human interleukin-4(rhIL-4).The recombinant plasmid pDC316/NY-ESO-1 was transfected into DCs and the DCs were co-cultured with T lymphocytes.The resultant CTL were used as effector cells.The growth of the CTL was detected by MTT assay.The CTL were then added into the growth medium used for culturing HXO-RB44 cells and the vitality of the HXO-RB44 cells was assayed by MTT assay.Results The sequence of the cloned DNA fragment of the recombinant plasmid pDC316/NY-ESO-1 was conforms with the sequence of the NY-ESO-1 gene.The expression of the NY-ESO-1 protein in HXO-RB44 cells was tested by immunofluorescence and Western blot.DCs were successfully induced with rhIL-4 and rhGM-CSF from PBMC.The recombinant expression plasmid pDC316/NY-ESO-1 was successfully transferred into DCs.These DCs had high expression of surface molecules such as HLA-DR(42.1%),CD80(54.2%),CD83(39.7%)and CD86 (94.8%).The CTL that was induced by DCs-sensitized with NY-ESO-1 specifically killed HXO-RB44 cells.CTL induced by the sensitized DCs had a stronger cytotoxic effect against HXO-RB44 cells compared with un-sensitized DCs and CTL un-induced with DCs,as shown by MTT asssay(P<0.05).The anti-tumor activity was highest when the ratio of effector to target was 75∶1(P<0.05).Conclusions DCs transfected by the recombinant plasmid pDC316/NY-ESO-1 can induce the proliferation of allogenic CTLs,which showed a specific anti-tumor effect against HXO-RB44 cells.These results present a new type of immunotherapy for the treatment of RB.
9.On-column Refolding of EC-SOD Inclusion Bodies, Purification and Stability Study

EC-SOD inclusion bodies was refolded on column using metal (Ni) affinity chromatography, based on the metal-binding property of His-tag. The effect of protein amount, urea removal speed and temperature on refolding was observed. We compared the different efficiency purified with Ni-sepharose and Heprain-sepharose affinity chromatography, and studied the stability of the refolded proteins. The results indicate that the inclusion bodies can be renatured with Ni-sepharose affinity chromatography. The increase of the protein amount and urea removal rate , the lower of the renaturation efficiency. Higher temperature was benefit to protein renaturation. Both the Ni-sepharose and Heprain-sepharose affinity column can be used to purified the refolded proteins, but purified by Heprain-sepharose affinity column the protein had higher activity. The activity of renatured protein was stable in 10 ℃~50℃,when pH10 its stability was lower significantly. In denaturating solution the stability of renatured protein was low.
10.Expression of defensins in colon tissues of ulcerative colitis and its implication

Objective To determine the level of defensins(HNP_(1-3))and interleukin-8(IL-8)in colon tissue of patients with ulcerative colitis(UC)and to explore the relationship between HNP_(1-3)and IL-8 and the severity of disease.Methods The level of HNP_(1-3)and IL-8 in colon mucosa of UC patients was detected by immunohistochemistry method.Results The levels of HNP_(1-3)and IL-8 in colon mucosa of UC patients were much higher than that of normal controls.The expression of HNP_(1-3)and IL 8 of UC patients was higher in involved mueosa than in corresponding normal mucosa.The expression of HNP_(1-3) and IL-8 was related to the extent of lesion and the degree of disease.The expression of HNP_(1-3)was significantly correlated with that of IL-8(r_s=0.957,P<0.01).Conclusions The induction of HNP_(1-3) and IL-8 was involved in process of inflammation and development of UC.Regulating mechanisms of HNP_(1-3)expression might be related with the induction of IL-8.
