2.Doppler hemorrhoid artery ligation combined with lauromacrogal sclerosis injection in treatment of hemorrhoids

ObjectiveTo study the clinical value of minimally invasive treatment of the doppler-guided hemorrhoid artery ligation with lauromacrogal sclerosis injection in treatment of hemorrhoids.MethodsA retrospective analysis was made on treatment of 32 cases with hemorrhoids using doppler-guided hemorrhoid artery ligation combined with lauromacrogal sclerosis injection.ResultsAccording to the branch of the Chinese Medical Association by the Surgery the clinical diagnosis and Treatment Guidelines of Hemorrhoids (draft),the Ⅰ degree hemorrhoid was found in 1 case,Ⅱ degree hemorrhoids in 18 cases,Ⅲ degree hemorrhoids in 13 cases.Recovery was found in 32 cases.The longest follow-up time was 30 weeks.No relapse was found. ConclusionsDoppler-guided hemorrhoid artery ligation combined with lauromacrogal case sclerosis injection is a low aggressive minimally invasive method to treat hemorrhoid and has small trauma,definite effect,less pain,fewer complications,and short course and other advantages,but the longterm outcome needs further observation.

Objective To study the expression of telomerase and its possible clinical significance in patients with condyloma acuminatum,bowenoid papulosis,Bowen's disease and vulvar squamous cell car- cinoma.Methods The human telomerase reverse transcriptase (hTERT) was detected by immunohisto- chemistry technique in all patients.Histopathological analysis was done in patients with bowenoid papulosis or Bowen's disease.Results There was a significant difference in the expression of hTERT among normal tissue,condyloma acuminatum and squamous carcinoma,and the expression rate increased successively from normal control to squamous carcinoma.In bowenoid papulosis,the expression of bTERT was significantly higher as compared to that in condyloma acuminatum,significantly lower as compared to that in squamous carcinoma,and was as the same as that in Bowen's disease.Histopathologically,the degree of cell atypia was significantly higher in Bowen's disease than that in bowenoid papulosis.Conclusions The expression of telomerase increases successively from normal skin,benign tumors to malignant carcinomas,which sug- gests that the expression of telomerase may play an important role in both cell proliferation and immortaliza- tion.
5.Forty Children with Vasovagal Syncope

Objective To explore the difference of clinical features and laboratory indexs with different vasovagal responses in(head-up) tilt test(HUT) in children with vasovagal syncope(VVS).Methods Forty consecutive children diagnosed with VVS by HUT were analyzed.The vasovagal responses were divided into vasodepressor,cardioinhibitory and mixed patterns.The distribution and different clinical features and laboratory indexs of different response patterns in the VVS were studied.Results The most common vasovagal response was vasodepressor.There was no significant difference in their clinical features,including the motivation,presymptom,frequency and duration of syncope,baseline heart rate and blood pressure,and serum electrolytes among the children with different vasovagal respon-ses.Conclusion Vosodepressor response is the most common hemodynamic pattern in vasovagal syncope in children.
6. Meta-analysis of bone metabolism indicators in patients with primary osteoporosis under different forms of exercises

BACKGROUND: Physical exercise can significantly reduce bone mass loss, relieve pain and improve bone metabolism in osteoporosis patients, but there is no evidence-based evidence. OBJECTIVE: To investigate the effect of different physical exercises on the treatment of primary osteoporosis. METHODS: Randomized controlled clinical trials regarding the therapeutic effect of physical exercise on primary osteoporosis were screened. The physical exercise group was subjected to physical exercise, and the control group had no regular exercise during the test. The main outcome measures included bone mineral density of lumbar spine L2-L4, visual analog scale score, bone metabolism index (osteocalcin, total type 1 procollagen amino terminal peptide, urine pyridinium/creatinine, blood calcium, blood phosphorus). The included outcome indicators were meta-analyzed using the Review Manager 5.3 software. RESULTS AND CONCLUSION: A total of 25 randomized controlled trials were included. Meta-analysis results showed that physical exercise could effectively improve the bone mineral density of L2-L4 segments in primary osteoporosis patients (mean difference=0.06, 95% confidence interval [0.04-0.08], P < 0.000 01, I2=89%). Subgroup analysis results revealed significant differences in the control group and five-animal exercise & Yi-Jin-Jing group, setting-up exercise group, composite exercise group, and incremental exercise group compared with control group (P < 0.05), while there was no significant difference between other exercise groups and control group (P > 0.05). Physical exercise significantly reduced the pain as determined by the visual analog scale in osteoporosis patients (mean difference=-0.93, 95% confidence interval [-1.08 to -0.79], P < 0.000 01, I2=83%). Exercise intervention could improve serum osteocalcin, total type 1 procollagen amino terminal peptide and blood phosphorus levels, and reduce urine pyridinium/creatinine and serum calcium levels. However, there was no significant difference between exercise groups and control group (P > 0.05). The results of Egger’s and Begg’s tests indicated that publication bias of the included studies was at a low level. All these findings indicate that physical exercise has significant interventional effects on bone mineral density and pain in patients with primary osteoporosis.
7.Changes of Intraerythrocytic Calcium,Serum Cardiac Troponin I and Their Relationships with Heart Function in Infants with Pneumonia Complicated with Heart Failure

Objective To evaluate the relationship of intraerythrocytic calcium(Ca~(2+)),serum cardiac troponin I(cTnI) and contractile and diastolic dysfunction in infants with pneumonia complicated with heart failure(HF),and discuss the role of Ca~(2+) and cTnI.(Met)-hods One hundred and thirty-three infants with pneumonia complicated with HF and 30 healthy children were studied.Intraerythrocytic calcium,serum cTnI and ejection fraction(EF),fractional shortening(FS) and E/A were measured by color Doppler ultrasoundraphy.Results Intraerythrocytic calcium,serum cTnI and EF,FS and E/A in two groups had difference.With the disease deteriorating,the values of Ca~(2+) and cTnI increased,and EF,FS,E/A decreased gradually.There were negative relations between Ca~(2+),cTnI and EF,FS,E/A.Conclusions Ca~(2+),cTnI are concerned with the development of HF.In clinical experiments,the contractile function and diastolic function of heart can be judged by the levels of intraerythrocytic calcium and serum cTnI.

Objective: To explore the pathogenesis of oral lichen planus (OLP) by examining the expressions of hypoxia-inducible factor-la and RTP801 in OLP. Methods: To determine the expression levels of HIF-la and RTP801 in 24 OLP tissues and 12 normal oral mucosa tissues by real-time fluorescence quantitative PCR. Results; The expression of HIF-1α in OLP was higher than that in normal oral mucosa(P <0.05), while the expression of RTP801 in OLP was lower than that in normal oral mucosa( P <0.05). Conclusion; The abnormal expressions of HIF-la and RTP801 may play an important role in the pathogenesis of oral lichen planus. HIF-1α might be a potential molecular target for the treatment of OLP.
