1.An investigation of sleep disturbance and related factors in rheumatoid arthritis patients

Objective To explore the characteristics of sleep disturbance and its related factors in patients with rheumatoid arthritis (RA).Methods A total of 71 patients with RA in Department of Rheumatology Huaxi Hospital have completed the following questionnaires, including Pittsburgh sleeping quality index (PSQI), visual analogue scale (VAS), disease activity score in 28 joints (DAS28), health assessment questionnaire (HAQ), hospital anxiety and depression scale (HADS), fatigue severity scale (FSS) and a self-designed general status questionnaire.Results The prevalence of sleep disturbance was 42.3% (30/71) in rheumatoid arthritis patients (68.4%).The scores of DAS28, VAS, PSQI, HAQ, FSS and HADS in patients with sleep disturbance were significantly higher than those in patients with good sleep, which were respectively 3.90 ± 1.12 vs 2.92 ± 1.92, (5.03 ± 2.63) scores vs (2.41 ± 1.84) scores, (10.87 ± 2.42) scores vs (4.29 ± 1.85) scores, 3.0 (0.0, 7.0) scores vs 2.0 (0.5,4.0) scores, (39.17 ±14.02) scores vs (29.63 ± 16.12) scores, (14.50 ±7.77) scores vs (9.49 ±6.57) scores (P < 0.05 in all scales).According to the results of Pearson correlation analysis, PSQI had significantly positive correlation with DAS28 (r =0.462, P < 0.01), VAS (r =0.556, P < 0.01), HAQ (r =0.360, P < 0.01), FSS(r =0.420, P < 0.01) and HADS (r =0.447, P < 0.01) respectively.The logistic regression analysis indicated that VAS was a predictor for poor sleep quality (P < 0.01).The patients receiving biological agents had significantly (P < 0.05) lower scores of DAS28 (2.86±1.39 vs 3.52 ± 1.1 0), PSQI [(5.90 ± 4.24) scores vs (8.53 ± 3.78) scores], VAS(2.15 ± 2.30 vs 4.05 ± 2.46), HAQ [0.0 (0.0, 2.0) scores vs 3.0(0.0,6.0) scores] compared to those taking oral drugs.Conclusion High prevalence of sleep disturbance in patients with RA is noted, which indirectly influences the activity of disease, quality of life, depression, fatigue and other physical and mental health.Biological agents can partly improve the sleep disturbance and functional status.

3.Study of sex in clinic epidemiology of gastric cancer

Objective To discuss the influence of sex in gastric cancer via the epidemiology and clinical data analysis of gastric cancer patients. Methods The clinical data of 242 gastric cancer patients was collected from Shanxi provincial tumor hospital between 1997 to 1998. Every patient filled in gastric cancer epidemiology form by professional-staff direction, which included personal characteristics, food habits, farmalial history of cancer and other informations. Results The average age of male gastric cancer patients was 6 years older than the females'. Male ulcer type gastric cancer patients were more than the females' (120 vs 38, P < 0.01). There was no different in cell types, differentiation, tumor stage and the clinical pathological features of lymph node metastasis. Male alcoholic drinkers, tea drinkers and smokers were more than the females' . There was no different in sex in tumor familial history and dietary habits of water, well-done food, sour food, vegetables, fruits and amount of radiation of meats, eggs and milk. Conclusion This research reveals the characteristics of sex in the epidemiology of gastric cancer. It is very important and significant not only in the research of gastric cancer etiology, but also in the prevention and intervention of gastric cancer.
4.Accuracy of loss of resistance technique for location of fascia iliaca compartment : evaluation using ultrasound technique

Objective The evaluate the accuracy of loss of resistance technique for location of fascia iliaca compartment using ultrasound technique.Methods One hundred and seventy-five patients undergoing hip surgery were enrolled in the study.A line was drawn on the skin from the pubic tubercle to the anterior superior iliac spine and divided in three equal parts.The site of puncture was marked 2 cm distal to the point at which the lateral met the middle third of the line.The needle was inserted until a loss of resistance was felt twice (fascia lata and fascia iliaca),at which point 0.5 % ropivacaine 30 ml was infused.Correct puncture was identified by visualized spread of local anesthetic solution under the guidance of ultrasound technique.The effective sensory block was recorded within 20 min after administration.Results The accuracy of loss of resistance method for locating the fascia iliac compartment was 56.6 % using ultrasound technique.There were 80 patients in whom fault site of puncture occurred:25 cases in the superficial layer of the fascia iliac and 51 cases in the iliopsoas muscle.The rate of effective block was 98.0 % for correct location and 82.9 % for fault location.Conclusion Loss of resistance technique cannot accurately locate the fascia iliac compartment.
5.Expression and clinical significance of MCL-1 and FBW7 proteins in breast cancer polyploid induced by spindle poisons

Objective To investigate the expression and clinical significance of myeloid cell leukemia-1 (MCL-1) and F-box and WD repeat domain-containing 7 (FBW7) in breast cancer polyploid induced by spindle poisons. Methods (1) Nocodazole spindle poison was used to treat breast cancer cell MDA-MB-231. The morphological changes of cells were ob?served under microscope, and cells were harvested in 0, 6, 12, 24, 48 and 72 h. The cell cycle and DNA-ploidy changes were examined by flow cytometry. The expressions of FBW7 and MCL-1 proteins were detected by Western blot assay. (2) A multikinase inhibitor (Sorafenib) with Nocodazole or Taxol was used to treat MDA-MB-231 cells. MCL-1 protein expression was detected by Western blot assay after 48 h treatment. The cell cycle and DNA-ploidy changes were examined by flow cy?tometry after 48 h treatment. MTT method was used to observe cell proliferation after 48 and 72 h treatment. Results (1)Af?ter treatment by Nocodazole, polyploid characteristics of large cell size and nucleus were appeared. The percentages of octa?ploid were (0.8±0.2)%, (8.5±2.3)%, (7.8±2.0)%, (9.9±0.9)%, (28.2±0.8)%and (35.1±4.9)%after 0, 6, 12, 24, 48 and 72 h treatment, showing the increasing trend in turn (P<0.001). The number of polyploidy (tetraploid and octaploid) cells was as high as (97.6±0.7)%after 48 h treatment. The expression level of FBW7 protein was decreased significantly but the expres?sion of MCL-1 protein was increased significantly after 48 h treatment. (2) After 48 h treatment, the expression level of MCL-1 protein, polyploidy percentage and cell proliferation decreased significantly in Nocodazole+Sorafenib group and Taxol+Sorafenib group compared with those of Nocodazole group and Taxol group (P<0.05). Conclusion The lower expression of FBW7 protein and over-expression of MCL-1 protein are correlated with the formation of breast cancer polyploidy. Sorafenib can reduce polyploid tumor cells by inhibiting MCL-1 protein expression.
6.Effect of HP-β-CD on Solubility of 1,8-Oxido-p-menthane in Volatile Oil of Magnoliae Flos and Study on the Extraction Technology of Xinzhi Nasal Drops

OBJECTIVE:To study the effect of HP-β-CD on solubility of 1,8-oxido-p-menthane in volatile oil of magnoliae flos,and optimize the extraction technology of Xinzhi nasal drops. METHODS:Steam distillation method was adopted to extract the volatile oil in magnoliae flos. Using the content of 1,8-oxido-p-menthane in volatile oil of magnoliae flos as index,HPLC was adopted to determine the effects of HP-β-CD with different concentration on solubility of 1,8-oxido-p-menthane in volatile oil of magnoliae flos. Using the comprehensive scores of imperatorin and extract yield as indexes,orthogonal test was used to optimize the amount of ethanol,volume fraction and extraction time in extraction technology of Xinzhi nasal drops. Verification test was car-ried out. RESULTS:50% HP-β-CD aqueous solution can improve the solubility of 1,8-oxido-p-menthane to 7.6 times. The optimal extraction technology of Xinzhi nasal drops was as follow as 10-fold 80% ethanol,extracting twice,1 h every time. In verification test,the average content of imperatorin was 0.078%(RSD=2.01%,n=3),and extract yield was 10.80%(RSD=1.85%,n=3). CONCLUSIONS:HP-β-CD shows good solubilization effect on the 1,8-oxido-p-menthane in volatile oil of magnoliae flos,and the optimized extration technology for Xinzhi nasal drops is feasible.
7.Study on quality standard of Catechu and its extract

AIM: To study the quality standard on catechu and its extract. METHODS: HPLC was used to determine the contents of (+) Catechin and (-) Epicatechin in catechu and its extract; UV spectrophotometry was applied to determine the contents of active tannic acid in catechu and its extract. RESULT: The contents of catechin and epicatechin are 19.4 and 4.36g/100g in catechu, respectively, and catechin and epicatechin are 48.39 and 3.94g/100g in its extract respectively. CONCLUSION: The method is accurate and reproducible, providing the basis for quality control of catechu and its extract.
8.Anterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients.

OBJECTIVETo investigate the clinical effects of anterior cervical intervertebral space decompression under microscope in treating cervical spondylotic myelopathy in elderly patients.
METHODSFrom June 2009 to March 2012, 43 patients with cervical spondylotic myelopathy were treated with anterior cervical intervertebral space decompression and intervertebral fusion under microscope. There were 26 males and 17 females, aged from 60 to 72 years old with an average of (64.9±3.7) years. Japanese Orthopaedic Association System (JOA) score was from 7 to 12 points with an average of (9.5±1.8) points before operation. The function of nerves was assessed before and after operation according to JOA.
RESULTSAll patients were followed up from 10 to 18 months with an average of (14.7±1.6) months. Postoperative JOA score was (13.81±1.44) points (ranged, 10 to 16), had significantly higher than preoperative (P<0.01). According to the rate of the improved JOA score, 9 cases got excellent results, 26 good, 7 fair, 1 poor.
CONCLUSIONAnterior cervical intervertebral space decompression under microscope for cervical spondylotic myelopathy in elderly patients is safe and effective.

10.Localization of p53(301-393) mutant and its effect on mitosis.

OBJECTIVETo observe the localization of p53(301-393)(residues 301-393) in p53 positive/negative cells and its effect on cell mitosis.
METHODSThe protein expression of p53-GFP and p53(301-393)-GFP was checked by immunoblotting after transfection. Immunofluorescence staining was performed to detect the localization of wide type and mutant in Hela cells (p53 positive) and H1299 cells (p53 negative). The cell morphology of H1299 cells transfected of p53(301-393)-GFP and the cells in mitotic phase were observed. Cell cycle was analyzed by flow cytometry and p53 protein level in HeLa cells was evaluated by Western blot after transfection of p53-GFP and p53(301-393)-GFP.
RESULTSThe protein expression of p53-GFP and p53(301-393)-GFP was verified, p53-GFP was about 90 kMr and p53(301-393)-GFP about 40 kMr. Immunofluorescence microscopy demonstrated that both proteins were diffusely located in the nuclei of HeLa cells and H1299 cells. But different from the p53-GFP, the p53(301-393)-GFP was distributed in the nucleolus of HeLa cells. After transfection of the two plasmids, mitosis was inhibited in H1299 cells and some cells underwent apoptosis. G2/M progression of HeLa cells could be blocked by transfection of p53(301-393)-GFP, but endogenous p53 protein level was not changed.
CONCLUSIONp53(301-393)has a different localization in the p53 positive and p53 negative cells and could inhibit mitosis and cause the cell cycle arrest in G2/M.
