1.Analysis on the Use of Non-steroidal Anti-inflammatory Drugs in Two Hospitals in Tongling City During 2004~2006

OBJECTIVE:To evaluate the utilization status and the developmental trend of non-steroidal anti-inflammatory drugs(NSAIDs) in our city.METHODS:NSAIDs used in two hospitals in Anhui Tongling city during 2004~2006 were analyzed statistically in respect of drug varieties,DDDs and consumption sum.RESULTS:The DDDs of NSAIDs increased,up 7.07% year on year. The consumption of NSAIDs increased by 4.15% in 2005,but decreased by 4.94% in 2006.New type NSAIDs had a great increase in consumption sum.CONCLUSION:The utilization structure of NSAIDs in our city is expected to remain stable in the coming several years,but great attention should be paid to the ADR of NSAIDs.
2.Detection of platelet activation marker and blood clotting activity in patients with hypertension and its clinical significance

Objective To investigate the changes of molecule markers of pre-thrombotic state,expression of CD62P,concentration of plasma fibrinogen(Fib),prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT) in the patients with essential hypertension for providing theoretical evidence for diagnosis and therapy of thrombus complication in early clinical stage.Methods Fifty-three patients with hypertension(grade Ⅰ 17 cases,grade Ⅱ 16 cases,grade Ⅲ 20 cases)were selected and twenty-eight people with medical examination in good health were used as control group.The expressions of CD62P and CD61 were measured by flow cytometry.The concentration of Fib and the levels of PT,APTT,TT were measured by Class coagulation method.Results The expression of CD62P and the concentration of plasma Fib in essential hypertension group were higher than those in control groups(P0.05).Conclusion The essential hypertensive patients are in pre-thrombotic state,the activation of platelet and blood clotting activity increase.Molecular markers should be measured in essential hypertensive patients for prevention and treatment of thrombotic disease.
3.Subchronic toxicity study of silver nanoparticles on medaka

Objective To study the sub chronic toxicity of silver nanoparticles on medaka.Methods Adult fish were divided into silve nenopartides and control group.Animals were collected on 14 days after exposure, and some toxicological endpoints such as death rate, tissue distributed of silver irons, oxidative stress and histopathological damage were measured.Results There were significant difference in death rate of medaka treated with silver nanoparticles and control group.Compared with the control group, the content of silver in gill, intestine and liver of medaka treated with silver nanoparticles were increased significantly.Compared with control group, the activity of LDH in liver and SOD in liver and gill were significantly decreased(P<0.01).The content of MDA in liver of medaka treated with silver nanoparticles was significantly increased(P<0.01).The liver and gill of mekada treated with silver nanoparticles were damaged, compared with control group.Conclusion Nano silver has a certain subchronic toxicity to aquatic life.
4.Comparison of endoscopic and open thyroidectomy

Objective To investigate advantages and disadvantages of endoscopic thyroidectomy.Methods A total of 61 consecutive patients with thyroid diseases from December 2003 to August 2004 in this hospital were freely divided into two groups: the Endoscopic Group(30 patients) and the Open Group(31 patients).The operating time,blood loss,hospital stay,postoperative pain assessment,jugular sensation 3 months after operation, swallowing function,and cosmetic outcomes between the two groups were compared.Results The operating time in the Open Group was significantly shorter than that in the Endoscopic Group(78?34 min vs 126?53 min;t=4.125,P=0.000).The intraoperative blood loss was less in the Endoscopic Group(20?13 ml) than in the Open Group(34?19 ml)(t=-3.313,P=0.002).The postoperative pain in the Endoscopic Group was slighter than that in the Open Group 24 and 48 hours after operation(t=3.255,P=0.002;t=-2.598,P=0.012).Three months after operation,jugular hypesthesia and discomfort at swallowing occurred in 1 and 2 patients in the Endoscopic Group,respectively,whereas jugular hypesthesia or paresthesia occurred in 11 patients(?~2=10.594,P=0.001) and discomfort at swallowing happened in 9 patients((?~2=5.773),P=0.016) in the Open Group.All patients in the Endoscopic Group were satisfied with the cosmetic results,but 19 patients in the Open Group complained of unsatisfactory cosmetic outcomes(?~2=52.141,P=0.000).Conclusions Endoscopic thyroidectomy gives little blood loss,mild postoperative pain,and good cosmetic results,being a feasible and safe procedure.
6.Effect of low frequency stimulation and rehabilitation training on stroke patients

ObjectiveTo investigate the effect of low frequency stimulation on motor function of stroke patients.Methods50 stroke patients were randomly divided into control group and treatment group with 25 cases in each group. Patients of control group received neuromedical and Bobath technical treatment; cases of treatment group received low frequency stimulation besides the above two methods. All patients were assessed respectively with the Fugl-Meyer Assessment and Clinical Nerve Function Impaired Scale at the admission and on 20 days after treatment.ResultsThe motor functions of both groups were improved (P<0.05). But the recovery of treatment group was superior to the control group (P<0.05).ConclusionLow frequency stimulation can promote the stroke patients' motor function recovery.
7.High Spinal Cord Injury Characterized by Flaccid Paralysis

ObjectiveTo investigate the characteristics of patients with flaccid paralysis after high spinal cord injury. Methods1014 cases with traumatic spinal cord injury were investigated. The patients with flaccid paralysis after high spinal cord injury (spinal fractures above the level of T10) were analyzed. Results6 patients were ananlyzed, including 5 males and 1 female, mean of age was (42±12). The neurological injury involved C7 to T8, and the fractures involved T3 to T10. 3 cases had the neurological deterioration upward at least 3 spinal segments after operation compared with the fractures. One case accomplicated with severe pain in the chest had the subacute progressive ascending myelopathy up to C7 level. MRI showed extensive atrophy of thoracic spinal cord 6 months later in 5 cases. ConclusionThe incidence of flaccid paralysis after high spinal cord injury was rare. It presents the extensive thoracic spinal cord atrophy, and the causes and mechanisms are not clear.

9.Sleeve gastrectomy with ileal interposition and duodenojejunal bypass for the treatment of nonobese Type 2 diabetes mellitus

Objective To evaluate sleeve gastrectomy with ileal interposition and duodenojejunal bypass for the treatment of nonobese Type 2 diabetes mellitus.Methods Forty one patients of nonobese Type 2 diabetes mellitus underwent sleeve gastrectomy with ileal interposition and duodenojejunal bypass.Fasting glucose ( FPG ),glycosylated hemoglobin ( HbAlc ),fasting insulin and C-peptide,triglycerides (TG),high density lipoprotein(HDL),low density lipeprotein(LDL) were measured preoperatively and on postoperative first,3rd,6th month.Results Mean postoperative follow-up was 9.6 months (range 6-21 months).95% patients achieved adequate glycemic control (HbAlc < 7% ) without antidiabetic medication.Fasting glycemia decreased from ( 9.7 ± 0.4 ) mmol/L to ( 6.2 ± 0.3 ) mmol/L ( P < 0.01 ).Glycosylated hemoglobin decreased from 8.1% ± 1.4% to 5.8% ± 0.6% ( P < 0.01 ).2-hour postprandial blood glucose decreased from ( 13.6 ± 0.7 ) mmol/L to ( 10.6 ± 0.2 ) mmol/L ( P < 0.01 ).Insulin resistance (Homa-R) decreased from 4.8 ± 1.3 to 1.2 ±0.4 (P <0.01 ).Fasting C-peptide increased from ( 3.3 ± 1.7 ) ng/ml to (4.9 ± 0.2 ) ng/ml ( P < 0.01 ).Fasting insulin increased from ( 10.2 + 1.4 ) mlu/L to (15.6±0.7) mlu/L(P<0.01 ).Triglycerides (TG) decreased from (3.1 ±0.5) mmol/L to (1.9 ±0.4) mmol/L ( P < 0.01 ).High density lipoprotein (HDL) increased from ( 1.2 ±± 0.2 ) mmol/L to ( 1.9 ±0.8 ) mmol/L( P < 0.01 ).Low density lipoprotein (LDL) decreased from (3.5 ± 0.3 ) mmol/L to (2.4 ±0.6) mmol/L (P <0.01 ).Hypertension was controlled in 3/7 cases.Microalbuminuria resolved in 78% patients.Retinopathy was improved in 53% cases.Conclusions Sleeve gastrectomy with ileal interposition duodenojejunal bypass is effective for treatment of nonobese type 2 diabetes mellitus as showed by 6 month's follow-up.
10.Total caudate lobectomy via anterior hepatic transection

Total caudate lobectomy via anterior hepatic transection is still a new technique to resect the tumor in the caudate lobe,which is mastered only by few surgeons.The procedure was successfully performed on a 21-year old patient with focal nodular hyperplasia in caudate lobe.The right and left lobes were first mobilized,then the short hepatic veins were dissected to detach the caudate lobe from the retrohepatic vena cava.Then the liver was split anteriorly and the partial middle lobe was resected.With this process,the tumor was in the sight and we dissected it from the liver parenchyma.The inflow blood was occluded 3 times with a period of 29,27 and 27 minutes,respectively,with an interval of 5 minutes.The total blood loss during operation was 1000 ml.The patient recovered quickly without any complications.The technique for caudate lobectomy via anterior hepatic transection can improve the success rate and safety of caudate lobectomy and deserve clinical consideration.
