1.Clinical study of Zhenwu decoction combined with Linggui-Zhugan decoction combined with western medicine in the treatment of senile heart failure

Objective To observe the clinical effect of Zhenwu decoction combined with Linggui-Zhugan decoction on senile patients with heart failure. Methods A total of 123 patients with heart failure who met the inclusion criteria were randomly divided into 3 groups by random number table method, each group of 41 cases. The patients in the control group were treated with routine western medicine, Zhenwu decoction group was treated with Zhenwu decoction on the basis of the control group treatment, and the combined decoction group was treated with Zhenwu and Linggui-Zhugan decoction on the basis of the control group. The treatment last 4 weeks as 1 course, 2 courses in total. The TCM syndrome score, left ventricular ejection fraction, brain natriuretic peptide level, nt-probnp level and TGF-?1 level were observed, and the clinical effect was evaluated. Results After 8 weeks treatment, Palpitation (0.68 ± 0.53 vs. 0.79 ± 0.64, 1.12 ± 0.70, F=7.207), chest distress (0.66 ± 0.52 vs. 0.79 ± 0.60, 1.36 ± 0.70, F=6.367), edema (0.32 ± 0.31 vs. 0.43 ± 0.41, 0.52 ± 0.37, F=7.054), wheezing (0.62 ± 0.55 vs. 0.82 ± 0.69, 0.88 ± 0.68, F=6.634), fatigue (0.73 ± 0.47 vs. 0.94 ± 0.62, 1.49 ± 0.58, F=5.832), spontaneous sweat (0.46 ± 0.39 vs. 0.89 ± 0.62, 0.99 ± 0.79, F=7.207), insomnia (0.42 ± 0.39 vs. 0.74 ± 0.50, 0.79 ± 0.58, F=6.672) in the combination group were significantly lowere than those in the other two groups (P<0.05). The TGF-?1 (102.75 ± 15.79 ng/L vs. 121.57 ± 19.25 ng/L, 184.35 ± 23.74 ng/L, F=160.776) in the combination group was significantly lowere than that in the other two groups (P<0.01). The total effective rate was 81.8% (27/33) in the combination group, 78.1% (25/32) in the Zhenwu group, and 62.16% (23/37) in the control group. The the difference of total effective rates among three groups were statistically significant (χ2=7.089, P=0.037 ). Conclusions The Zhenwu decoction and Linggui-Zhugan decoction combined with the routine therapy can improve the symptoms and signs of elderly patients with heart failure, and reduce the serum TGF-1 level.
2.Clinical observation on fuzhong (supporting the middle-jiao) manipulation of tuina for infantile anorexia.

OBJECTIVETo observe clinical therapeutic effect of fuzhong (supporting the middle-jiao) manipulation of tuina for infantile anorexia.
METHODSEighty-one children with anorexia were treated with fuzhong manipulation, including abdominal kneading, acupressure at Zhongwan (CV 12), Lanmen (Extra), Tianshu (ST 25), Zusanli (ST 36), Sanyinjiao (SP 6), spine pinching, warm moxibustion and warm scraping with supporting-yang cupping, three times a week, six times as a session. Onset time, therapeutic effect and TCM symptom scores before and after treatment were observed after two sessions and differences of therapeutic effect among different patterns were compared.
RESULTSSeventy-two children had the effect in one session while 41 children had the effect within half of a session. 38 cases were clinically cured, 15 cases were markedly effective, 23 cases were effective and 5 cases were failed. The total effective rate was 93.8%. Compared with before treatment, TCM symptom scores, including appetite, food intake, complexion and defecation, were all obviously improved after treatment (all P < 0.001). The clinical cured rate was 45.2% (19/42) in the pattern of dysfunction of spleen in transportation, 45.8% (11/24) in the pattern of qi deficiency of spleen and stomach, 50.0% (4/8) in the pattern of yin deficiency of spleen and stomach, 57.1% (4/7) in the pattern of liver sufficiency and spleen insufficiency, which had no obvious differences among different patterns (all P > 0.05).
CONCLUSIONThe fuzhong manipulation of tuina has quick onset time and satisfied clinical therapeutic effect, which could obviously improve symptoms and is fit for different patterns of infantile anorexia.

3.Treatment of Recurrent Oral Ulceration (Yin Deficiency Fire Excess Type) by Qianjin Kouchuang Jiawei Granule: a Clinical Observation.

OBJECTIVETo observe the curative effect of Qianjin Kouchuang Jiawei Granule (QKJG) on recurrent oral ulceration [yin deficiency fire excess type (YDFET)].
METHODSTotally 120 patients who suffered from recurrent oral ulceration (YDFET) were randomly assigned to two groups, the experiment group and the control group, 60 in each group. Patients in the experiment group took QKJG, 20 g each time, twice per day, while those in the control group took Kouyanqing Granule (KG) , 20 g each time, twice per day. Fourteen days consisted of one therapeutic course, two for all. Scores for patients' symptoms and signs (ulcer area, exudation, hyperaemia, edema, the number of ulceration, burning sensation, and pain degrees) were assessed before treatment, at day 3 and 7 after treatment. Short-term efficacy was assessed by visual analogue scale (VAS). The total paralysis time and the total number of ulceration at month 12 after treatment were taken as judgment for long-term efficacy. Results Compared with before treatment in the same group, symptoms and signs were obviously improved at day 3 and 7 after treatment in the two groups (P < 0.05). Compared with the control group at day 3 after treatment, the improvement of edema, exudation, pain degree, and burning sensation was more obvious in the experiment group (P < 0.05, P < 0.01). The improvement of edema, pain degree, and burning sensation at day 7 after treatment was more obvious in the experiment group than in the control group (P < 0.05). As for short-term efficacy, the total effective rate was 86.67% (52/60 cases) in the experiment group and 83.33% (50/60 cases) in the control group, with no statistical difference between the two groups (P > 0.05). As for long-term efficacy, the total effective rate was 90.00% (54/60 cases) in the experiment group, significantly higher than that of the control group with statistical difference [81.67% (49/60 cases), P < 0.05]. At month 12 after treatment, the total number of ulceration was reduced and the paralysis time of ulcer attack prolonged in the experiment group, with statistical difference when compared with the control group (P < 0.05).
CONCLUSIONQKJG showed better long-term efficacy than that of KG in treating recurrent oral ulceration (YDFET).

4.Effect of cyclic stretch on the proliferation of C2C12 myoblasts

BACKGROUND:Mechanical stimulation triggers a muscle tissue remodeling, during which, myoblast process is a multi-stage development process. Many signals in extracellular matrix are involved in myoblast process, and mechanical signals are considered as important external factors for muscle formation and regeneration.
Numerous studies concerning cyclic tensile stress effects on myoblast proliferation and apoptosis have been reported, but the specific mechanism underlying the mechanical stimulation in muscle formation is stil unclear. OBJECTIVE:To study the effects of cyclic stretch on proliferation of C2C12 myoblasts and its mechanism.
METHODS:C2C12 myoblasts were cultured in vitro under 10%cyclic stretch for 6, 12 and 24 hours.
RESULTS AND CONCLUSION:The results of 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay showed that with the time of cyclic stretch, the proliferation of C2C12 myoblasts and the rate of S-phase fraction were gradual y increased and peaked at 12 hours (P<0.05), and then decreased. Western blot assay showed that the expression of nuclear transcription factorκB protein was decreased with time, which was inversely correlated to the proliferation of C2C12 myoblasts, but increased at 24 hours under the cyclic stretch. These findings indicate that the cyclic stretch can induce C2C12 myoblasts proliferation and alter the cellcycle. Nuclear transcription factorκB may be involved in this process.
5.Functional and molecular MR imaging in evaluating outcomes of tumor therapy

The evaluation of outcomes of cancer therapy in clinic are largely based on volumetric and morphological evidences.These evidences,however,are very limited in assessing the early effects of therapy.Functional and molecular MR imaging allows co-registration of functional/molecular-based information with high-resolution anatomical detail within the same imaging modality,which makes it possible for assessing the early effects of tumor therapy.
6.Enrichment process of scoparone from Yinchenhao Decoction with macroporous resin

Object To study the optimal conditions and parameters for the enrichment process of scoparone in Yinchenhao Decoction (YCHD) with macroporous resin. Methods With the enrichment degree of scoparone in YCHD as an index, the optimal conditions of the enrichment process were investigated. Results The optimal conditions were that the aqueous extract of YCHD was taken into the macroporous resin column, and kept for 30 min, then the resin was washed by five times of distilled water to get rid of impurity and three — four times of 70% ethanol to elute scoparone. Conclusion The process is feasible to enrich effective ingredient scoparone from YCHD.
7.Detection of lymphocyte subsets in the application of the infectious diseases

Objective Investigate the different changes of host cellular immunity in patients with different infection by compa-ring peripheral blood lymphocyte subsets.Methods Flow cytometry was used to detect the peripheral blood lymphocyte subset a-mong 96 patients including 31 bacterial infection cases,13 fungal infection cases and 62 virus infection cases.Results Compared with control group the ratio of CD3 + T cell was increased in virus group,and the ratio of CD4 + T was decreased in fungi and virus group and the fungus is lower than the virus group,while the ratio of CD8 + T cell was increased in the two groups,and fungi group is higher than the virus,CD4 +/CD8 + ratio in the two groups were reduced.Three groups of NK cells (of CD16 + CD56 + )ratio was reduced to some extent,and bacteria group was lower than that of other two groups.The ratio of B cell was increased in Bacteria group and it was more obvious in the G+ bacteria.NK cells in G+ bacteria and G- bacteria group were relatively lower than control group,but the difference between the two groups was not significance.In virus infection cases,the ratio of CD3 + T in HBV group was higher than that of dengue fever group.And the ratio of CD3 + and NK cells were increased in the two groups.The ratio of CD4 + was decreased in dengue fever group.Conclusion Detection of lymphocyte subset can objectively reflect and help to under-stand the state of the immune function of patients with infectious diseases,providing laboratory basis for the diagnosis and preven-tion in order to reduce the infectious risk and side effect.
9.Temperature variation at the external root surface during Nd: YAG laser irradiation in the root canal in vitro.

OBJECTIVETo assess the temperature variation of the root surface using Nd: YAG laser irradiation in the root canal with different power and to evaluate the safety of laser application on the periodontal region.
METHODSThirty extracted human teeth with single-roots were collected. The teeth were cross-sectioned in the cervical portion, standardizing the roots at a 12-mm length. The roots were used as specimen. The roots were radiographed in the buccal-lingual direction to measure the thickness of the proximal walls, by means of a digital radiographic system. The specimens were divided into three groups according to the laser potency (1.5, 3.0, and 4.5 W). Each group was subdivided into two subgroups according to laser frequency (15 and 30 Hz). With the Nd: YAG laser irradiation for 20 s, the temperature variation of the root surface was monitored by thermocouples located at different parts of the root external wall and recorded by digital thermometers.
RESULTSThe groups irradiated with 4.5 W presented the greatest temperature variation (above 10°C), followed by 3.0 and 1.5 W. The temperatures were statistically different (P < 0.01). The groups irradiated in the same potency, regardless of whether 15 Hz or 30 Hz was used, presented with no statistical difference (P > 0.05). The apical half of the root presented statistically higher temperature rises than the cervical half of the root (P < 0.01).
CONCLUSIONThe temperature variation of the root surface was associated with laser power, irradiation time, and the thickness of dentin. Application of Nd: YAG laser in the root at 1.5 W for 20 s can safely be used in endodontic treatment.

10.Quantitative MR measures in patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy and their relationships with clinical scales

Objective To analyze white matter hyperintensities(WMH)and brain volumes quantitatively in patients with Cerebral autosomal dominant arteriopathy with subcortical infarcts and leucoencephalopathy(CADASIL)on conventional MRI and correlate them with clinical parameters.Methods Fifteen patients with CADASIL confirmed by pathological investigation underwent conventional MRI examinations.Involvement of centrum semiovale,posterior limbs of the internal capsule,corpus callosum,external capsules and white matter of the temporal poles was determined by an experienced radiologist.Normalized brain volume(NBV)and the percentage of WMH to brain volume were calculated with softwares and they were correlated with NIHSS and MMSE statistically.Results Involvement of white matter in the brains of patients with CADASIL included:semiovale centrum in 13 cases,white matter of the temporal poles in 10 cases,external capsules in 8 cases,posterior limbs of the internal capsule in 5 cases,corpus callosum in 4 cases.The percentage of WMH to brain volume was(5.7±1.4)%,and the NBV was(1602±58)×103mm3.Spearman test showed a significant relationship between age and NBV(r=-0.555,P<0.05).There was a significant relationship between NBV and NIHSS(r=-0.624.P<0.05).The percentage of WMH to brain volume correlated significantly with NIHSS and MMSE(r=0.522,P<0.05;r=-0.679,P<0.01).Conclusions The white matter hvperintensities and brain volume in patients with CADASIL can be assessed quantitatively,which showed correlation with severity of the clinical scale.The development of WMH may refleet the degree of cognitive impairment in CADASIL.
