1.A report about the cusing factors in contrast nephropathy

Objective To explore the causing factors of radiocontrast-induced nephropathy (RCIN) after coronary angiography and the rule of hydratation.Methods 279 cases of coronary angiography were retrospectively analyzed,and the clinical features,basic renal function and volume of intra-venous fluid received on operation day between the patients with and without RCIN were compared.Results The total mortality rates of radiocontrast-induced nephropathy were 6.81%(19/279);in patients with hypertension,the RCIN rates were 8.65%(16/185);in patients with diabetic,the RCIN rates were 12.1%(12/99);in patients with preexisting renal dysfunction,the RCIN rate was 0%(0/16);the multifactor Logistic analysis showed that the factors causing RCIN were age,diabetic,the volume of intra-venous fluid,however,the factors such as gender,smoking,hypertension,coronary heart disease,chronic renal failure were irrelative with RCIN.Conclusion The incidence of RCIN in people over 60 years old is much higher.Such condition may be partly cansed by the decreasing renal reserve capacity.Sufficient hydration is an effective way to decrease RCIN.
2.Verification and application of standard method for detecting serum fluoride

Objective Standard method of testing serum fluoride was tested and verified by certified reference material CRM of free fluo ride component in cattle serum (GBW 09143~09144).The method was suited for fluor ide research to assess its practical value.Methods ISE-standard curve,linear relationship,linear range,sensitive and precision were disired result by CRM.400 serum of fluoride in rats were measured.Results The linear ranges of the method were 0.02~5.0 mg/L,r=0.9999,b=57.8,and test lower limit was 0.013 mg/L.The test value was equal to standard value.Conclusions The standard method is accurate,sensitive,convenient and small sample is used;and it is easy to spread.Thus,it has been of practical value.
3.Investigation of present situation and analysis of positive practice environments of doctors in public hospital in China

Objective: This study aims to investigate the positive practice environments evaluation of doctors in public hospital and provide suggestions for Chinese health care reform from the perspective of health service providers.Methods: Using self-designed questionnaire, a national cross-sectional survey was performed in 77 public hospitals across seven provinces/ metropolis, involving a total of 3564 doctors.Positive practice environment includes organizational management and doctor-patient two-dimension relationships.Results: The doctor''s mean score of positive practice environment evaluation was 18.02±4.86 (out of 40).Doctors had a higher evaluation (score) of organizational management than doctor-patient relationship.In the organizational management dimension, the personal advice got the lowest scores(2.24±1.04);and in doctor-patient relations, evasion of medical risk got the lowest scores(1.59±0.81).There was a difference in the positive practice scores of physicians in different regions, and the scores of doctors from the western China were the lowest (P<0.001.Doctors from western medicine hospitals had lower evaluation of positive practice environments than ones from Chinese Traditional Medicine hospitals(P=0.002.Conclusion: As per the analysis conducted in this investigation, the current medical practice environment of doctors in China is not a promising one.Doctor-patient relationship is worse than organizational management.Hospitals should adopt a participatory decision-making model in management and establish a broadband performance based compensation to motivate junior doctors.Besides, government should establish reasonable compensation incentive mechanism, with the aim of promoting doctor-patient trust.
4.Antihypertensive effect of indapamide combined with enalapril on spontaneously hypertensive rats

Objective To observe the antihypertensive effect of indapamide combined with enalapril on spontaneously hypertensive rats(SHRs).Methods Forty SHRs were randomly divided into 4 groups: control,indapamide,enalapril,and indapamide+enalapril(n=10 in each group).Medicine in varied doses was given to rats by intragastric administration.Variations of weight,heart rate and blood pressure were measured. Results Varied doses of medicine did not exert significant effects to the weight and heart rate of SHRs during and after the administration.In indapamide+enalapril group,the pressure of SHRs was significantly decreased with varied doses compared to that before the administration(P

Objective To explore the effect of transforming growth factor β1 (TGF-β1) on epigenetic histone lysine acetylation in the plasminogen activator inhibitor 1 (PAI-1) promoter and transcribe regions in glomerular mesangial cells (GMCs).Methods Chromatin immunoprecipitation assay and real-time quantitative PCR were used to detect Histone3K9 acetylation (H3K9Ac) in the PAI-1 promoter and transcribe regions induced by TGF-β1 and high glucose.Immunoprecipitation was also used to see the cooperation of Smad3,CBP and Sp1 proteins.Results In the four target regions of PAI-1 promoter,TGF-β1 treatment enhanced H3K9Ac at P1,P2 and P3 in GMCs (P < 0.05),but no change was seen in the P4 region which was far from the transcription starting site.TGF-β1 obviously induced H3K9Ac in the T1 transcribe region of PAI-1 instead of T2 (P < 0.05).High glucose increased PAI-1 mRNA expression and H3K9Ac around P1 promoter region (P< 0.05).TGF-β1 neutralizing antibody abrogated high glucose-induced H3K9Ac at PAI-1 promoter (P < 0.01).TGF-β1 treatment could recruit Smad3 and CBP protein binding to the PAI-1 promoter regions (P1,P2,P3),and induce their cooperation in GMCs,which were responsing to TGF-β1 associated H3K9Ac.Conclusion TGF-β1 can induce H3K9Ac in the promoter and transcribe regions of PAI-1,promote Smad3 recruition and cooperation with Sp1 and CBP,which are associated with PAI-1 gene's regulation in GMCs.

7.Expression of serum IL-6 and IL-10 in sepsis patients and their impacts on immune function

Objective To study the expression of serum interleukin (IL )-6 and IL-10 in sepsis patients ,and their correlations with immune function .Methods A total of 56 patients with sepsis were divided into three groups ,including 18 patients in sepsis group ,21 patients in severe sepsis group ,17 patients in septic shock group .All the patients were also divided into survivor and death group according to whether they survived within 2 weeks .Other 30 healthy persons were selected in the control group .Serum IL-6 and IL-10 ,CD4+ /CD8+ were detected and compared .Results Serum IL-6 and IL-10 levels in the three sepsis groups were significantly higher than those in control group ,and with the severity aggravating ,these indicators increased .The level of serum IL-6 in sepsis patients was significantly reduced at the 3rd day ,while there was no difference on the serum IL-10 in severe sepsis group and septic shock group compared with that at admission(P>0 .05) .Compared with survivor group ,serum IL-6 and IL-10 levels on both admission and the 3rd in death groups increased significantly (P<0 .05) .CD4+ ,CD8+ ,CD4+ /CD8+ were negative related with serum IL-6 and IL-10 in both severe sepsis and septic shock group (P<0 .05) .Conclusion The expression of IL-6 and IL-10 in patients with sepsis are over-expressive ,and closely related with disease activity and immune function .
8.In vitro study on paired immunoglobulin-like receptor B regulating growth of retinal ganglion cells

Objective To determine the effect of paired immunoglobulin-like receptor B (PirB) on growth of retinal ganglion cells (RGCs) in vitro.Methods Expression of PirB in RGCs isolated from wild-type C57 BJ/6 mice was measured by immunofluorescence.Real-time PCR and Western blot were performed to detect PirB expressions in RGCs cultured for 1,3,5,7 and 9 days.Primary cultured RGCs were left untreated as controls,transfected with lentiviral-delivered PirB RNAi as Group A,lentiviral-delivered NgR RNAi as Group B,lentiviral-delivered PirB RNAi plus ciliary neurotrophic factor (CNTF) as Group C,lentiviral-delivered NgR plus CNTF as Group D,and only CNTF as Group E.Growth and development of RGCs were evaluated by MTT assay and morphological analysis.Results Immunofluorescence confirmed expression of PirB in RGCs,with its mRNA and protein levels changing from increase at first to decrease later with time,up to peak at days 5 and 7.PirB and NgR were respectively down-regulated in Groups A and B.Viability of RGCs was improved in Group A compared with control group [(40.2 ± 5.3) μm vs (29.1 ± 3.8) μm at day 3 ; (72.2 ± 4.2) μm vs (52.3 ± 8.2) μm at day 5,both P < 0.05].By contrast,no significant difference was found in viability of RGCs among Groups B,D and E when compared to control group (P > 0.05).Growth of RGCs between Groups A and C revealed insignificant difference (P > 0.05).Conclusion Knockdown of PirB is beneficial for the growth of RGCs,suggesting a novel method to treat optic nerve injury.

10.Surveillance of antimicrobial resistance in gram-negative bacilli isolated from lower respiratory tract of patients in Respiratory Intensive Care Unit for 5 consecutive years

Objective To investigate the changing pattern of antimicrobial resistance among gram-negative bacilli isolated from respiratory intensive care unit (RICU) for rational use of antimicrobial agents.Methods Antimicrobial susceptibility of 1 047 isolates of gram-negative bacilli from 2000 to 2004 was tested by disk diffusion method.WHONET 5.3 software was used to analyze the data.Results The most common pathogens were Pseudomonas aeruginosa (42.9%),Stenotrophomonas malto- philia (17.1%),Acinetobacter baurnannii (10.0%) and Klebsiella pneumoniae (6.5%).The susceptibility rate of P.aerug- inosa was relatively higher to ceftazidime (50%-74%),amikacin (33.3%-81.0%),piperacillin-tazobactam (30.4%-64.6%) and cefoperazone-sulbactam (33.5%-47.5%),while the susceptibility to imipenem decreased.The susceptibility rate of S. maltophilia was relatively higher to cefoperazone-sulbactam (47.2%-78.6%) and ticarcillin-clavulanic acid(28.3%-86.6%). More than 90% of Acinetobacter baumannii isolates were susceptible to imipenem.The susceptibility rates of K.pneumoniae to imipenem and cefepime were 92.9%-100% and 55.6%-80.0%,respectively.The susceptibility rate to piperacillin-tazobac- tam decreased from 58.3% to 21.7%.The prevalence of extended-spectrum?-lactamases (ESBLs) in K.pneumoniae increased from 11.1% in 2002 to 47.8% in 2004.Conclusions Most pathogens show significant resistance to the most commonly used an- tibiotics.It is very important to select antibiotics for the treatment of infections in ICU based on the results of susceptibility.
