3. Optimization for prescription of Baoxieling Hydrogel Patch using Box-Behnken test design and research on its transdermal permeability in vitro

Objective: To optimize the prescription of Baoxieling Hydrogel Patch (BHP) using Box-Behnken test design and to investigate its transdermal absorption properties in vitro. Methods: Taking the comprehensive scores of the early adhesion, uniformity, ductility, consistence, skin adhesive ability, repeated exposing paste, and residue as response values, Box-Behnken test design was used to optimize the amounts of sodium polyacrylate NP 800, aluminum glycinate, and fillers, and to validate the optimal formulation. The percutaneous permeation of cinnamaldehyde, eugenol, evodiamine, and rutaecarpine in the optimal formulation was studied by in vitro transdermal delivery experiment with Franz diffusion cells and their contents were determined by HPLC. Results: The optimal ratio of the prescription was as follows: sodium polyacrylate NP 800-aluminum glycinate-fillers (0.82:0.02:1.56). The foremost factors were fillers and aluminum glycinate. Its transdermal absorption met zero order dynamic process. Conclusion: The optimal prescription has uniform paste, suitable consistence, easy ductility, moderate adhesion, and perfect transdermal effect. It could provide the foundation for the development of new prescription of Baoxieling.

5.Dynamic assessment of tear-film stability-associated objective vision in mild or moderate dry eyes based on Optical Quality Analysis System parameters

Background The progenesis and development of dry eye is closely associated with the stability of tear-film.Dry eyes often show the visual disturbance.Therefore,to evaluate the stability of tear-film based on visual quality and explore the relevant dynamic vision change in dry eyes is of important significance.Objective The aim of this study was to evaluate the dynamic changes of objective scattering index (OSI),modulation transfer function cut off (MTF cut off),objective visual acuity (OVA) with tear-film in the patients with mild or moderate dry eye disease using Optical Quality Analysis System.Methods A clinical diagnosis test was performed.Forty-two eyes of 42 consecutive patients with mild or moderate dry eyes were included in Tianjin Eye Hospital from December 2013 to May 2014 based on ‘ The Clinical Diagnosis and Treatment Recommendations for Dry Eye from Consensus of Expert Advice in 2013’,and 29 normal eyes of age-and gender-matched 29 healthy volunteers were simultaneously included as the healthy control group.The dynamic retinal images under non-blinking condition during 15 seconds were recorded by OQAS-Ⅱ system,and the OSI,MTF cut off and OVA values at 0,5,10 and 15 seconds were obtained,and the differences of OSI,MTF cut off and OVA between 0 second and 15 seconds were calculated (△OSI,△MTF cut off,△OVA).The dynamic changes of △OSI,△MTF cut off and △OVA with time were evaluated.The sensitivity and specificity of △OSI,△MTF cut off and △OVA for the differential diagnosis of dry eyes were analyzed by receiver operating characteristic curve (ROC curve).Results The OSI values at 5,10 and 15 seconds in the mild or moderate dry eye group were significantly higher than those at 0 second or those in the healthy control group (all at P<0.05),and the MTF cut off values and OVA values at 5,10 and 15 seconds in the mild or moderate dry eye group were significantly lower than those at 0 second or those in the healthy control group (all at P<0.05).The △OSI,△MTF cut off and △OVA values were 1.03±1.53,(-8.37±9.64)c/d and-0.30±0.32 in the mild or moderate dry eye group,which were significantly higher than 0.06± 0.16,(-0.37 ± 5.27) c/d and-0.03 ± 0.18 in the healthy control group (t=4.10,-5.19,-5.28,all at P<0.05).The area under ROC curve was 0.90,0.79 and 0.78 in △OSI,△MTF cut off and △OVA,respectively.The sensitivity and specificity of △OSI was 78.6% and 93.1% respectively at the diagnostic cut off of 0.3.Conclusions The optical quality parameters related with tear-film stability are gradually reduced with extending of time in mild or moderate dry eyes,but they are stable in healthy eyes.The detecting outcomes by OQAS-Ⅱ system can reflect the dynamic status of tear-film,and this method is of important clinical value for the differential diagnosis and optical quality evaluation in dry eyes.

Objective To investigate the effect of the cerebral infarction in rats and the role of matrix metalloproteinases-2 (MMP-2), matrix metalloproteinases-9 (MMP-9), and tissue inhibitor of metalloproteinases-3 (TIMP-3) in the pathogenesis. Method Sixty-six rats were randomly divided into 3 groups: blank group (n= 6), control group (n=30), model group (n=30). The rat model was established by carotid injection of auto blood thrombus, the control group was given the operation without injection. Each group was subdivided into 24 hours, 48 hours, 3 days, 5 days, 7 days after operative intervention. At serial intervals, the animals in each group were sacrificed and brain tissue samples were collected to determine MMP-2, MMP-9, TIMP-3 expression by immunohistoehemistry. The changes of behaviors were observed and the histopathological changes were investigated by hematoxylin and eosin stain. Results The experimental group had more obvious behavior change than other groups. Objective To explore the function of autonomic nerves system in children with congenital heart disease (CHD) combined with paroxysmal supraventricular tachycardia (PSVT). Methods Fifty children having PSVT and no CHD (PSVT group), 30 children with both PSVT and CHD (CHD group), and 50 cases of healthy children (control group) were selected. The difference of 24-hour heart rate variability (HRV) among three groups was analyzed retrospectively. Results There were statistical differences in the long-range time domain HRV indexes, including SDNN, SDANN, SDNN Index, pNN 50 , and RMSSD among three groups (F=80.32-?263.18, P all?0.01). The SDNN, pNN50, and RMSSD in PSVT group were significantly decreased compared with control group (P all?0 . 05 ), and the SDANN and SDNN Index in PSVT group were decreased but the difference was not statistically significant compared with control group (P all?>?0.05). The SDNN, SDANN, SDNN Index, pNN50, and RMSSD were signiifcantly decreased in CHD group compared with PSVT group (P all?0 . 05 ). Conclusions Children with PSVT have the imbalanced autonomic nerves system which mainly manifests as decreased vagal function, and, if combined with CHD, have the increase of sympathetic nerve tension.
8.Comparative research on color Doppler ultrasonography and MRI in the diagnosis of thyroid carcinoma

ObjectiveTo compare the performance of color Doppler utrasonography (CDUS) alone,magnetic resonance imagine (MRI) alone, and the combination of the two in the diagnosis of thyroid carcinona (TC). MethodsCDUS and MRI results of 54 focuses of carcinoma in 45 cases diagnosed as TC by postoperative pathological results were reviewed. The sensitivity of CDUS, MRI, and the combination of the two in the diagnosis of TC was compared. ResultsThe sensitivity of CDUS and MRI was 77.78% (42/54)and 81.48%(44/54) respectively. The difference had no statistical significance (P > 0.05 ). The specificity of CDUS and MRI was 88.17% and 92.47% respectively, the positive predictive value was 79.25% and 86.27% respectively,and the negative predictive value was 87.23% and 89.58% respectively. The sensitivity of the combination of CDUS and MRI was 94.44% (51/54). The difference between the combination and CDUS or MRI alone had statistical significance( P <0.05). ConclusionsCDUS has higher detection rate than MRI in microcalcification of TC focus. However, MRI can show more clearly of the peripheral invasion and cervical lymph node metastasis.The combination of the two can improve the accuracy of TC diagnosis.
9.Effects of Dangshen extract saponin on apoptosis and the mechanism in ischemia-reperfusion injury of renal grafts

Objective To investigate the effects of Dangshen (DS) extract saponins alleviating apoptosis in ischemia-reperfusion injury (I/R) of renal grafts and the mechanism. Methods The I/R injury model in SD rats was established after kidney transplantation. The SD rats were randomly divided into the following 3 groups (n = 20): sham operation group, I/R model group, DS saponin intervention group. Blood urea nitrogen (Bun) and creatinine (Scr) levels were determined at the 24th h after operation; apoptosis index (AI) was detected by using TUNEL method; the expression of Bcl2 and bax mRNA was detected by RT-PCR. Results As compared with the sham operation group,the blood Bun and Scr levels were significantly increased in the I/R model and DS saponin intervention groups (P<0. 05). As compared with the DS saponin intervention group, the blood Bun and Scr levels were significantly increased in the I/R group (P<0.05). The AI was significantly increased in the I/R model group and DS saponin intervention group. After DS saponin intervention, the AI was decreased from 40. 28 % in the I/R model group to 28. 45 % in the DS saponin intervention group (P<0. 05). The expression of Bcl-2 mRNA in DS saponin intervention group and I/R model group was significantly decreased as compared with the sham operation group (P<0. 05), but that of Bax mRNA was significantly increased in the I/R model group and DS saponin intervention group as compared with the sham operation group (P<0. 05). After DS saponin intervention, the expression of Bcl-2mRNA was increased from 0. 25 in the I/R model group to 0. 391 (P<0. 05), and that of Bax mRNA was decreased from 0. 565 in the I/R model group to 0. 473 (P<0. 05). Conclusion DS extract saponin could significantly alleviate apoptosis in the I/R injury of renal grafts possibly by up-regulating the expression of Bcl-2 mRNA and down-regulating the expression of Bax gene.
10.Cytological Study in vitro on Co-delivery of siRNA and Paclitaxel within Solid Lipid Nanoparticles to Overcome Multidrug Resistance in Tumors.

Multidrug resistance (MDR) remains the major obstacle to the success of clinical cancer chemotherapy. P-glycoprotein (P-gp), encoded by the MDR1, is an important part with complex mechanisms associated with the MDR. In order to overcome the MDR of tumors, we in the present experimental design incorporated small interfering RNA (siRNA) targeting MDR1 gene and anticancer drug paclitaxel (PTX) into the solid lipid nanoparticles (SLNs) to achieve the combinational therapeutic effects of genetherapy and chemotherapy. In this study, siRNA-PTX-SLNs were successfully prepared. The cytotoxicity of blank SLNs and siRNA-PTX-SLNs in MCF-7 cells and MCF-7/ADR cells were detected by MTT; and the uptake efficiency of PTX in MCF-7/ADR cells were detected via HPLC method; quantitative real-time PCR and flow cytometry were performed to investigate the silencing effect of siRNA-PTX- SLNs on MDR1 gene in MCF-7/ADR cells. The results showed that PTX loaded SLNs could significantly inhibit the growth of tumor cells, and more importantly, the MDR tumor cells treated with siRNA-PTX-SLNs showed the lowest viability. HPLC study showed that SLNs could enhance the cellular uptake for PTX. Meanwhile, siRNA delivered by SLNs significantly decreased the P-gp expression in MDR tumor cells, thus increased the cellular accumulation of rhodamine123 as a P-gp substrate. In conclusion, the MDR1 gene could be silenced by siRNA-PTX-SLNs, which could promote the growth inhibition efficiency of PTX on tumor cells, leading to synergetic effect on MDR tumor therapy.
