1.Mandible plasma cell myeloma:A case report

A case of mandible plasma cell myeloma is reported.Plasma cell myeloma is a monoclonal neoplastic proliferation of plasma cells of bone-marrow derivation,usually multi-centric but jaw bones are rarely involved.The treatment requires precise pathological diagno-sis.The prognosis of solitary jaw bone lesions can be good after treatment.
2.Effect of exercise training on tumor necrosis factor-alpha and adiponectin expressions in insulin resistance rats

Objective To investigate the effects of exercise training intervention on the expressions of TNF-α and adiponectin in circulation and tissues of high-fat/high-sucrose diet-induced insulin resistance rats. Methods 29 S-D rats were random divided into 2 groups: Control group (9 rats) and high-fat/high-sucrose diet group (20 rats). After fed for 6 weeks, 18 rats with insulin resistance were random divided into 2 groups: model group ( n = 9) and exercise group ( n = 9). After 6 weeks intervention, serum TNF-α and adiponectin concentration were measured by radioimmunity assay and ELISA respectively, while TNF-α and adiponectin mRNA expressions in liver and skeletal muscle were measured by RT-PCR. Results Fasting plasma glucose(FPG) and fasting serum insulin(FINS) levels increased significantly and insulin sensitivity index(ISI) decreased significantly in rats of model group than those in control group(7.49 ± 1.13 vs 5.06±0.38, 33.57 ±4.87 vs 13.61±2.94, -5.51±0.16 vs -4.21 ±0.22, all P <0.05). Serum TNF-α concentration was significantly higher than those in control group, while serum adiponectin concentration was significantly lower. (3.03 ± 0. 50 vs 2. 39 ± 0. 44, 0. 77 ± 0. 09 vs 0. 86 ± 0. 08, all P < 0. 05 ).Expressions of TNF-α mRNA in liver and skeletal muscle increased significantly and adiponectin mRNA expression significantly decreased in rats of model group compared to those in control group (0. 66 ± 0. 19 vs 0. 05 ± 0. 03, 1.15 ± 0. 20 vs 0. 25 ± 0. 10, 0. 25 ± 0. 10 vs 0. 85 ± 0. 13, all P < 0. 01 ). Fasting plasma glucose(FPG) and fasting serum insulin(FINS) levels decreased significantly and insulin sensitivity index (ISI) increased significantly in rats of exercise group than those in model group(5.77 ± 1.17 vs 7.49 ±1.13, 25.69 ±4.27 vs 33.57 ±4. 87, -5. 10 ±0.31 vs -5.51 ±0. 16, all P <0.05) ;Serum TNF-α concentration was significantly lower and serum adiponectin concentration was significantly higher in rats of exercise group than those in model group ( 2.40 ± 0. 59 vs 3.03 ± 0. 50, 0. 86 ± 0. 10 vs 0. 77 ± 0. 09, all P < 0. 05); Expressions of TNF-α mRNA in liver and skeletal muscle decreased significantly and adiponectin mRNA expression increased significantly in rats of exercise group compared to those in model group (0. 21±0. 10 vs 0.66±0. 19, 0.49 ±0. 17 vs 1.15 ±0.20, 0.97 ±0.20 vs 0. 25 ±0. 10, all P <0.01). Conclusion Exercise training can significantly improve insulin resistance, which may be through modulating the expressions of TNF-α and adiponectin.
3.Assessment of the left ventricular early diastolic synchrony of cardiac resynchronization therapy by real time three-dimensional echocardiographic volume-time curves in patients with dilated cardiomyopathy

Objective To evaluate the diastolic function and relationship between diastolic function and early diastolic synchrony in patients with dilated cardiomyopathy (DCM) by real-time three-dimensional echocardiography (RT-3DE) volume-time curves (VTC) after cardiac resynchronization therapy (CRT).Methods Thirty-nine patients with DCM were enrolled by RT-3DE VTC before and 1 week,6 months,12 months after CRT,draw the left ventricular (LV) 16,12,6 segments LV diastolic early volume standard deviation of the time (Tedv-SD),and with the R-R interval normalized as early diastolic unsynchronized index (DDI) ;draw end-systolic volume (LVESV),LV end-diastolic volume (LVEDV),LV ejection fraction (LVEF) ;and calculate the diastolic peak filling rate(PFR),the ratio of early diastolic volume and enddiastolic volume(EDVearly/EDV).Results LVEDV,LVESV had no significant improvement.Compared with the before and 1 week after CRT,but the improvement was statistically significant after 6 months and 12 months (P <0.05,P <0.01); LVEF after 1 week,6 months and 12 months were statistically significant (P <0.05) ;Compared with before,PFR after 6 months was significantly increased (P <0.05),EDVearly/EDV at 12 months after CRT was significantly reduced (P < 0.01); There was a significantly shortened in each segment (Tedv-SD)/R-R 1 week after CRT (P <0.01),but the parameters had no obvious improvement later.Correlation analysis:△ DDI and △ EDVearly/EDV reduction was significant positive correlation (r =0.52,P <0.01),△DDI and PFR has negative correlation (r =-0.40,P < 0.05),△ EDVearly/EDV and △PFR also had a good relationship (r =-0.56,P <0.01).Conclusions The LV synchrony and diastolic function were improved after CRT in patients with DCM; PFR,EDVearly/EDV can be used as evaluation of left ventricular diastolic function effectively targets.
4.Risk factors of overweight or obesity and their relationship with dyslipidemia among aerospace professional staff

Objective To investigate the prevalence and risk factors of overweight/obesity and their relationship with dyslipidemia among aerospace professional staff,to provide evidence for more effective health management.Methods Nine hundred eighty aerospace professional staff who took health checkup at our center in 2012 were required to complete a self-report life style questionnaire.Height,body weight and blood lipid profiles of the participants were then measured.Logistic regression was used for data analysis.Results A total of 776 valid questionnaires were finally received.The prevalence of overweight or obesity of the participants were 34.4% and 10.1%,respectively.Standardized overweight rate was 36.9% or 36.3%,and standardized obesity rate was 12.3% or 11.8%,according to Chinese adults' level in 2002 or Beijingers' figure in 2006.Univariate logistic regression showed that age (odds ratio (OR) 2.050,1.666 and 2.164; 95% confidence interval (CI) 1.490 to 2.820,1.048 to 2.648 and 1.164 to 4.020),gender (OR =0.296,95 % CI 0.211 to 0.416),marital status (OR =2.178,95 % CI 1.591 to 2.980),cigarette smoking (OR =2.571 and 1.889; 95% CI 1.061 to 6.229 and 1.341 to 2.660) and alcohol consumption (OR =1.547 and 1.796; 95% CI0.570 to 4.195 and 1.319 to 2.447) were risk factors of overweight and obesity.In multivariate logistic regression,age (OR =1.562,1.775 and 2.206; 95% CI 1.029 to 2.372,0.943 to 3.341 and 0.977 to 4.978),female (OR =0.311,95% CI0.206 to 0.469),marital status (OR =1.591,95% CI 1.045 to 2.421),meat intake (OR=0.663,95% CI0.437 to 1.007) and fried food (OR =0.604,95% CI 0.420 to 0.870) were risk factors of overweight/obesity.The risk of developing hypercholesterolemia,hypertriglyceridemia,low high-density lipoprotein hypercholesterolemia,high lowdensity lipoprotein hypercholesterolemia and hypercholesterolemia were significantly increased in overweight/obese adults.Conclusion Higher prevalence of overweight/obese may alarm the need for aerospace professional staff to initiate body weight control and risk factor prevention,especially for male and married adults.
5.Exploration on path linkage and compatibility of migrant workers' medical insurance system in China

Migrant workers incur the challenges of linkage and compatibility of medical insurance systems for their migration across regions and change of identities. To identify a medical insurance system adaptable to their needs, the paper recommends the following: Take their medical insurance into the national account of medical insurance; build a linkage mechanism between cities and villages, and that between cities as well; make the medical insurance system more compatible and portable, in order to safeguard their right of health.
6.Clinic evaluation of interventional treatment for renal artery aneurysm

Objective To evaluate the safety and efficacy of interventional procedure for treating the renal artery aneurysm (RAA).Methods From Jan 2009 to Apr 2014,17 patients,who were diagnosed as RAA and accepted the interventional therapy,were reviewed in our hospital.The mean age in those patients,including 7 males and 10 females,was (46.4±10.3) years old (range from 20 to 67 years old).The related symptoms included backache in 4 cases,abdominal pain in 4 cases,intermittent hematuria in 2 cases,chyluria in one case,oligouria in one case.9 cases were diagnosed as multiple RAA and 8 cases were confirmed as signle cases.In 17 cases,31 aneurysms were found,including 26 true aneurysms,5 pseudoaneurysums,17 sacculated aneurysms,4 spindle-like aneurysms,4 irregular shape aneurysms,4 parenchyma aneurysm and 2 dissecting aneurysm.8 aneurysms located in the main renal artery,19 aneurysms located in the branch of renal artery,4 aneurysums located in the renal parenchyma.Intracavitary coil embolization was used in 4 patients.We carried out parent artery embolization in 3 patients.A combination of the former techniques was performed in 6 cases.Covered stent placement was operated in one case.Combination of the intracavitary coil embolization and nude stent placement were performed in 2 patients.We used two techniques in one patient with multiple artery aneurysms in both sides.Results The interventional treatment of RAAs succeed at the first operation in 16 of 17 patients.17 cases were followed-up from 3 to 53 months (mean 23 months).No severe complications or death cases occurred in this study.Urine occult blood in 3 patients turned to negative after one week.Primary symptoms such as gross hematuria,abdominal pain,lumbodorsalgia,fever vanished or obviously eased after a month.Laboratory tests showed that normal level in SCr,BUN,routine urinalysis 3 months,6 months and 1 year later.No tendency of stent and coil stent shifting was found in 16 patients and the parent arteries were patent in 8 cases,with reexamination bv ultrasonic or computed tomography angiography (CTA).Conclusions Interventional techniques are minimally-invasive,safe and effective methods for treating the RAAs.
7.Effects of faf1 gene knockout by CRISPR/Cas9 on zebrafish cartilage and sarcomere development

Objective To determine the effect of knocking down zebrafish faf1 gene by CRISPR/Cas9 editing technique.Methods gRNA was designed and prepared for the faf1 gene of zebrafish,and gRNA was mixed with Cas9 mRNA by microinjection into zebrafish single cell embryos.The mutant F0 generation zebrafish was screened out by enzyme digestion and gene sequencing.The mutant F0 was genetically outcrossed with the wild-type zebrafish to get the F1 heterozygous zebrafish,and the genotype of zebrafish was detected by microscopic observation.Results The faf1 gRNA and Cas9 mRNA were successfully prepared.The gRNA (gRNA6) located in the exon 6 of faf1 could shift the faf1 gene into frameshift mutations.The mutation type MU1 was screened out and the somatic cytochrome deposition delay was observed in this heterozygous zebrafish.At 4 d post fertilization (dpf),there were sarcomeric dysplasia and head shrinkage,increased hyoid angle and other craniofacial cartilage deformities.And the zebrafish died at 8 ~9 dpf.Conclusion CRISPR/Cas9 knocking out thefaf1 gene produces a new phenotype for zebrafish,with delayed pigment deposition and nodule-like change in tail muscle section.
8.Effects of L-arginine on the hypothermic perfusion and on preservation of isolated pancreas in canines

Objective To study the effects of L-arginine on the reperfusion injury of hypothermic perfusion and on preservation of isolated pancreas(ISP) in canines . Methods After segmental pancreatic transplantation(SPTS) model were set up,the animals were randomly divided into 3 groups:(1)ISP irregulated and preserved with Euro Colln Solution(ECS)plus L-arginine(L-arg group);(2)IPS irregulated and preserved with ECS plus N-nitro-L-arginine methy ester(L-NAME group);and(3)IPS irregulated and preserved with ECS plus normal saline(Control group).The IPS were irregulated and preserved at 0-4℃ for 24h.Serum concentration of amylase and lipase were detected. NO,NOS and myeloperoxidase (MPO) and expression of iNOS mRNA in the pancreas graft were measured. Histological examination was performed. 10.Survey of thyroid nodules in people taking health physical examination

Objective To investigate the prevalence rate of thyroid nodules as well as related influencing factors. Methods In this clinical study, 8,912 healthy people were included, involving medical staff, teachers, electric enterprise staff and civil authorities from January to December in 2014 . The prevalence rate of thyroid nodules and related influencing factors were investigated . Results (1) The prevalence rate of thyroid nodules was 40.4%, 34.9%for male and 46.7%for female. The prevalence of female was higher than that of male (P<0.001);(2) The prevalence rate of thyroid nodules showed a trend of increase with age (P<0.01);(3) The prevalence rate of thyroid nodules in medical staff and teachers was significantly higher than others (P<0.001);(4) The difference was statistically significant in BMI, blood pressure, blood lipid, blood glucose in thyroid nodule group and non thyroid nodules group (P<0.001). All indexes mentioned above of thyroid nodule group was much higher than those of non thyroid nodules group. (5) The prevalence rate of breast hyperplasia of females under the age of fifty years was significantly higher than those of non thyroid nodule group (P<0.001). Conclusion The prevalence rate of thyroid nodules is high. It is related to gender, age, occupation and estrogen. Effective measures should be taken to change people's bad habits, blood sugar control and blood pressure, reduce BMI and work tension in order to reduce the incidence of thyroid nodules.
