1.Effect of basic fibroblast growth factor on the gene expression of decorin in periodontal ligament cells

OBJECTIVE:Studies have confirmed that basic fibroblast growth factor stimulating periodontal ligament cells can promote the proliferation of human periodontal ligament cells.so as to facilitate the reconstruction of lost periodontal tissues.By use of different concentrations of basic fibroblast growth factor,the in vitro cultured normal human periodontal ligament cells were stimulated to observe the decorin gene expression in the periodontal ligament cells.METHODS:The periodontal ligament cells were isolated and cultured using trypsin digestion method.The third generation of cells at logarithmic growth phase were preserved in DMEM containing 10% DMSO and 20% FBS frozen crvopreservation fluid,then moved into liquid nitrogen preservation at the second day Immunohistochemistry was used to identify anti-vimentin and cytokeratin staining The sixth generation of human periodontal ligament cells were divided into experimental group and control group.The experimental group was cultured in DMEM culture medium containing basic fibroblast growth factor at the concentration of 0.1,1,10 μg/L under standard conditions for 24 hours;control group was cultured with DMEM culture medium under standard conditions for 24 hours.The intracellular decorin gene expression was determined using RT-PCR.RESULTS:By microscopic observation,the cells in the control group had no significant changes,and cells proliferated in the experimental group,before stimulation the cells at the bottom of the bottle became denser than those in sparse regions.Adding basic fibroblast growth factor into periodontal ligament cells,could significantly decrease the mRNA expression of decorin,along with the increase of basic fibroblast growth factor concentration,the inhibition effect gradually weakened and became the strongest in the 0.1 μg/L but the weakest at 10 μg/L.CONCLUSION:The basic fibroblast growth factor stimulation can promote the proliferation of periodontal ligament cells and inhibit the synthesis of decorin in a dose-dependent manner,0 1 μg/L induces the strongest inhibitory effect.
2.Impact of palliative radiotherapy to the prognosis of patients with stage Ⅳ non-small cell lung cancer

Objective To investigate the impact of palliative radiotherapy to the prognosis of patients with stage Ⅳ non-small cell lung cancer (NSCLC).Methods Two hundred and forty-four cases of stage Ⅳ NSCLC were collected from our hospital.The study group (n =129) received the treatment of primary tumor and metastases palliative radiotherapy after chemotherapy.The control group (n =115) received only chemotherapy.Followed-up and compared 1-year and 2-year survival rate of the patients,and explored the factors affecting palliative radiotherapy.Results The 129 patients in the study group were followed up for 2 years,and 16 patients survived and 110 patients died.Three patients were lost to follow-up.The median survival time was 8 months.1-year and 2-year survival rates was 29.46% (38/129) and 12.40% (16/129) respectively.In the control group,7 patients survived,and 106 patients died.Two patients were lost to follow up.The median survival time was 6 months.1-year and 2-year survival rates was 11.30% (13/115) and 6.09% (7/115)respectively.Survival rates between the two groups were significantly different (x2 =8.451,P =0.014)Univariate regression analysis found that factors affecting the prognosis of stage Ⅳ NSCLC were KPS score (x2 =5.057,P =0.031),occurrence of brain metastases (x2 =4.781,P =0.029),number of organs of metastasis (x2 =6.341,P =0.010),and the primary stove radiotherapy dose (x2 =5.893,P =0.015) ; Cox regression model analysis showed that the number of organs of metastasis (HR =1.719,95% CI 为 1.172-3.126,P =0.008),the primary focal radiotherapy dose (HR =1.560,95% CI 为 1.082-2.761,P =0.022) were independent prognostic factors.Conclusion Palliative radiotherapy plays a role of prolonging survival time in the treatment of Ⅳ NSCLC.In palliative radiotherapy,we should pay attention to the control of the primary tumor and metastases radiotherapy dose,therefore prolonging the survival time of patients.
3.Diagnostic value of combined detection of inflammatory indicators in bloodstream infection with different pathogenic bacteria

Objective To investigate the diagnostic value of combined detection of procalcitonin (PCT), C-reactive protein (CRP), white blood cell (WBC), and neutrophil percentage (NEU%) in bloodstream infection with gram-positive coccus(G+), gram-negative bacillus (G-)and fungus.Methods Detection results of positive blood culture of 389 patients in a hospital between January 2014 and December 2015 were analyzed retrospectively, according to the results of blood culture, patients were divided into G+ coccus, G-bacillus and fungal bloodstream infection groups, inflammatory indicators of different groups of patients were compared.Results Mann-Whitney U test revealed that PCT level of G-infection group was higher than that of G+ and fungal infection group (comparison between G-infection group and G+ infection group : Z=-2.68,P<0.01;comparison between G-infection group and fungal infection group: Z=-2.46,P<0.05).If PCT≥0.5 ng/mL, CRP≥5.0 mg/L, NEU%≥70% and WBC≥10×109/L were as the cut-off point, statistical analysis revealed the positive rate of PCT in G-infection group was higher than that in G+ and fungal infection group(comparison between G-infection group and G+ infection group:χ2=5.94,P<0.05;comparison between G-infection group and fungal infection group:χ2=7.721,P<0.01);the positive rate of CRP in G-infection group was higher than that in G+ infection group (χ2=5.03,P<0.05).Binary logistic regression was adopted to analyze the efficacy of four indicators for the differentiation of bloodstream infection caused by G+ coccus, G-bacillus, and fungus, only PCT had significant difference in the identification of bloodstream infection caused by G-bacillus, G+ coccus and fungus(P<0.01).Conclusion PCT has high accuracy in differentiating G-bacillus, G+ coccus, and fungus of blood culture, dynamic monitoring of PCT combined with detection results of CRP, WBC, and NEU%, patient's condition can be judged rapidly, and antimicrobial agents can be used rationally, so the mortality of patients with bloodstream infection can be reduced.

6.Relationship between MAT1 expression and clinicopathological features of pancreatic cancers

AIM: To investigate the expression of MAT1 protein in pancreatic cancers and the relationship between MAT1 and clinicopathological features of pancreatic cancers. METHODS: 94 surgical specimens, including 70 pancreatic cancers, 10 pancreatic benign tumors, 14 chronic pancreatitis and 10 autopsy normal pancreas tissues, were analyzed immunohistochemically, and then MAT1 expression and clinicopathological features were compared. RESULTS: MAT1 was expressed mainly in the cancer cells,and also in the fibroblasts, where it was localized within the cytoplasm and nuclear envelope. MAT1 expression was found in 75.7% (53/70) of the cancers, but not detected or weakly expressed in control tissues. 8.Effects of sevoflurane on blood pressure in patients with different ages

Objective To investigate the effect of sevoflurane on blood pressure in patients with different ages and the optimal concen-tration of sevoflurane. Methods 60 patients underwent selective LC operation with conventional induction and sevoflurane maintenance were divided into three groups:the youth group (34 patients), the middle age group (20 patients) and the older age group (6 patients). The SBP, DBP, MAP, CETsev, number of patients, age of patients before induction and 10 min(T1), 20 min(T2), 30 min (T3), 60 min (T4) after induction were recorded. Results Fluctuate of blood pressure were in the normal range after anesthesia maintenance, and MAP were fluctuated within the range of ± 20% before induction. The age under different CETsev were of no significant difference in each time point (P>0. 05). The age composition ratio in different CETsev were of no significant difference in each time point (P>0. 05). As the time of anesthesia extended, the number of patients under 0% ~1%CETsev decreased from 35 to 11; the number of patients under 1% ~2%CETsev increased from 10 to 20;and the number of patients under 2% ~3%CETsev maintened in 14 approximately. Conclusion The effect of sevoflurane on blood pressure of different ages is approximate. 2% ~3% CETsev is the most optimal concentration during the main-tenance of anesthesia.
10.Efficacy and safety of ticagrelor plus cilostazol in the treatment of patients with low body weight after percutaneous coronary intervention

Objective To explore the efficacy and safety of ticagrelor plus cilostazol of different dosage in the treatment of low-weight patients after PCI.Methods A total of 148 consecutive ACS patients (body weight ≤ 65 kg) past PCI and with aspirin intolerance were enrolled and randomly divided into four groups.Patients given cilostazol 50mg twice daily plus clopidogrel 75 mg daily were named as the CC50 mg group.Patients in the CC100 mg group were given cilostazol 100 mg twice daily plus clopidogrel 75 mg daily.The TCS0 mg group were given cilostazol 50 mg twice daily plus standard ticagrelor 90mg twice daily and the TC100 mg group were given cilostazol 50 mg twice daily plus standard ticagrelor 90 mg twice daily.All patients were followed up clinically for 6 months.The clinical endpoints were MACEs and bleeding events.Platelet aggregation at 7 and 30 days after treatment the incidence of clinical endpoints during followup were compared between the four groups.Results Patients in the TC100mg group had the lowest platelet aggregation rates tested on both the 7th and 30th day after treatment among all the 4 groups.After 6 months of follow up,the MACEs rate was not significantly different between the four groups (P =0.930).Bleeding events rates in the TC100 mg group the highest among the 4 but without groups significant differences.Conclusions In ACS patients with low body weight ≤ 65 kg) past PCI and with aspirin intolerance,cilostazol 50mg twice daily plus ticagrelor is a safe and efficacious therapeutic regimen.
