Objective: It is difficult to repair large-area Ⅲ?burn wound in children. Here we introduce the advantages of the back skin donor site in repairing Ⅲ?burn wound in children. Methods: This clinical study included 15 children with Ⅲ?burn, ranging in age from 18 months to 11 years(mean 7.6 years),with the Ⅲ?burn wound area averaging at 12.6% of the total body surface. The back skin donor site was used to repair the wound. The functional site, such as the joint, was preferentially considered if the skin graft was not enough for all the wounds. Results: Most of the skin grafts survived and the donor sites on the back healed very well. The patients were followed up for six months to three years, which revealed that the skin grafts remained soft without dysfunction and the scar hyperplasia was mild on the donor site. Conclusion: The back donor site supplies larger-area skin with good quality. The donor site heals fast. After healing, patients could sleep on back and compress the scar so as to prevent scar hyperplasia, and the scar does not affect the patient's appearance. With all these advantages, the back donor site is a good choice for repairing Ⅲ?burn wound in children.

Objective To investigate the feasibility and the clinical effect of bronchial arterial chemoembolization with ADM-Lipiodol mixture in the treatment of brochogenic carcinoma. Methods Thirty-three cases of bronchogenic carcinoma undertaken bronchial arterial chemoembolization, including 20 cases of adenocarcinoma, 8 of squamous cell carcinoma, 1 of small cell carcinoma and 4 of undifferentiated carcinoma. The clinical staging consisted of 5 cases in II, 13 in III A, 11 in IIIB and 4 in IV. Using Seldinger's technique and selective bronchial arterial catheterization (5 cases of super-selective catheterization). After perfusion of 80 - 100 mg CDDP and 10 mg MMC, embolization with a mixture of 30 mg ADM and 2 - 10 mg Lipidol was performed. Another 3 cases were undergone surgery within 1 week after bronchial arterial chemoembolization. Results The 33 cases of bronchial arterial chemoembolization, showed 2 cases of complete response(CR), 21 of partial response (PR), 9 of stable (S) and 1 of processes (P). The pathology revealed massive necrosis of tumor cells after bronchial arterial chemoembolization. No severe complications such as spinal injury occurred. Conclusions Bronchial arterial chemoembolization with ADM-Lipiodol mixture for the treatment of brochogenic carcinoma is good short-term effectiveness. The procedure should be carried out carefully on the basis of fine digital subtraction angiography.

Objective To analyze the effects of puerperium pelvic floor muscle training on pelvic floor muscle strength and its clinical significance. Methods One hundred postpartum women were included, with full-term singleton pregnancies and with complete follow-up records from Obstetrics Department of Peking University First Hospital between March 1, 2013 and October 31, 2013. Women with vaginal birth and cesarean birth commenced pelvic floor muscle training twice a day from 24 and 72 h after delivery, respectively. According to the different training frequencies, the subjects were divided into three groups: never-training group, occasional-training group (<6 times per week) and regular-training group (≥6 times per week). All patients received pelvic floor muscle strength measurement 6-8 weeks after parturition. And the strength of type Ⅰ and type Ⅱ pelvic floor muscle fiber was divided into 0, Ⅰ, Ⅱ, Ⅲ, Ⅳ and Ⅴgrade. The abnormal strength of typeⅠand typeⅡpelvic floor muscle fiber standed for the grades lower thanⅢ. We compared the general conditions, delivery modes and abnormal ratio of type Ⅰ and type Ⅱ pelvic floor muscle fiber among the three groups, analyzed the relativity between the level of pelvic floor muscle fiber strength and pelvic floor muscle training frequency, and analyzed the influential factors of pelvic floor muscle fiber strength. Statistical analyses were performed by one-way ANOVA, rank-sum test, Pearson χ2 test, Kruskal-Wallis nonparametric test, Nemenyi test, Spearman rank correlation analysis and multivariate Logistic regression analysis. Results There was no statistical difference in age, gestation at delivery, parity, body mass index before delivery, neonatal birth weight and delivery mode among the never-training group (21 cases), occasional-training group (30 cases) and regular-training group (49 cases) (all P > 0.05). There was no statistical difference in labor time of first, second and total stage and episiotomy rate among the vaginal birth cases of the three groups. The abnormal ratio of type Ⅰ muscle fiber strength among the three groups was 100% (21/21), 77% (23/30) and 6% (3/49), respectively, while that of type Ⅱ muscle fiber strength was 100% (21/21), 53% (16/30) and 20% (10/49), respectively. And there were significant statistical differences among the three groups (F=119.16 and 77.84, both P<0.01). Spearman rank correlation analysis indicated that the level of typeⅠand typeⅡmuscle fiber strength had a significant positive correlation with pelvic floor muscle training frequency (r=0.88 and 0.79, both P<0.01). Multivariate Logistic regression analysis showed that pelvic floor muscle training was an influential factor for pelvic floor muscle strength level (95%CI:0.000-0.193, P<0.01). Conclusions Puerperium pelvic floor muscle training can help enhance the tension force of pelvic floor muscle, might benefiting the postpartum recovery of pelvic floor muscle function.

Chinese Pharmacological Bulletin 2009;25(12):1655-1659
Aim To investigate the proliferation of HSCs stimulated by exogenous TGF-β1 (transforming growth factor betal),observe the effect of TFB(total flavonoids of Bidens Bipinnata L.)on smad2/7,typeⅠcollagen mRNA and protein expression of HSCs and study the protective effect and molecular mechanism of TFB on hepatic fibrosis. Methods HSCs were isolated with collagenase Ⅳ perfusion in situ and density gradient centrifugation. The effect of TFB on cell proliferation was observed by MTT colormetric assay. The auto-secretion of TGF-β1 and synthesis of type Ⅰ collagen were measured by enzyme-linked immuneadsordent assay (ELISA).Moreover, the expression of smad2/7, typeⅠcollagen mRNA and protein was measured by semi-quantitative RT-PCR and Western blot methods respectively. Results TFB could markedly inhibit the proliferation of HSCs of liver fibrosis rats stimulated by TGF-β1 and production of TGF-β1 and type Ⅰ collagen. In addition,TFB treatment could significantly down-regulate smad2 and type Ⅰ collagen mRNA expression and up-regulated smad7 mRNA expression of HSCs Smad2 protein expression of HSCs stimulated by TGF-β1 was also down-regulated by TFB.Conclusion TFB has the protective effect against hepatic fibrosis by inhibiting the activation of TGF-β1 signaling pathway and suppressing the HSC proliferation.
