1.Application on improved small incision extracapsular cataract extraction in the sight-regaining program

AIM: To investigate the clinic efficacy and operation safety of improved small incision extracapsular cataract extraction in the sight-regaining program.
METHODS: Totally 82 cases ( 82 eyes ) included in the sight-regaining program of the cataract patients were underwent small incision extracapsular cataract extraction conbined with improved technology ( anterior capsule staining, continuous curvilinear capsulorhexis, auxiliary incision ) , postoperative visual recovery and operative complications were observed.
RESULTS: Postoperative visual acuity: uncorrected visual acuity when out of hospital: ≥0. 3 were 67 cases (82%);1wk after dismissed from hospital:≥0. 3 were 76 cases (93%), admist which ≥0. 8 were 45 cases (55%). There is no serious complication such as posterior capsule rupture, zonular dialysis, etc. in the operation.CONCLUSION: Combined use of improved technology can increase operation safety of small incision extracapsular cataract extraction.
2.Establishment of the Talent Training Mode for Pharmaceutical Specialty in Retail Pharmacies Based on the Modern Apprenticeship

OBJECTIVE:To establish the talent training mode for pharmaceutical specialty in retail pharmacies based on mod-ern apprenticeship,and improve the teaching quality of pharmacy major in vocational colleges. METHODS:According to the basic content of talent training program,the contents including the talent training goal orientation,the recruitment mode,the teaching plan formulation,the curriculum system development,the teaching arrangement,the length of schooling and the certificate acquisi-tion were studied to analyze the feasibility of carrying out the modern apprenticeship of pharmaceutical specialty in retail pharma-cies. RESULTS:Pharmaceutical talents should master the necessary medicine theory,knowledge of relevant laws,regulations and stores management;the trained people could be an enterprise recruitment of employees,enrolled high school students or the two-screening-students;the practical skills training should be highlighted,both sides of school and enterprise should work together to develop the teaching plan formulation;and the curriculum development committee was established to determine the level of knowledge and ability that the retail pharmacies must know and to give the formation of the course;double subject education,dou-ble tutor teaching,work and study alternation were used in the teaching implementation;and flexible educational system was de-signed. CONCLUSIONS:The establishment of talent training mode for pharmaceutical specialty in retail pharmacies based on mod-ern apprenticeship can improve the teaching quality of pharmacy major in vocational colleges. However,the establishment of course system should consider the sustainable development of students,and cautiously set targeted courses based on the characteris-tics of talents requirements in retail pharmacies.
3.Intervention of Tert-butyl Hydroperoxide-Induced Injury in Human Umbilical Cord Derived Mesenchymal Stem Cells with Ginkgo Biloba Extract

Objective To observe the intervention effect of Ginkgo biloba extract EGb761 on tert-butyl hydroperoxide ( t-BHP)-induced injury in human umbilical cord-derived mesenchymal stem cells ( hUC-MSCs) . Methods Proliferation of hUC-MSCs after primary culture was detected by methyl thiazolyl tetrazolium (MTT) assay, and then the optimal concentrations of t-BHP and EGb761 for oxidative stress injured MSC model were screened. Cell apoptosis was assessed by flow cytometry after Annexin V-fluorescein isothiocyanatel propidium iodide ( Annexin V-FITC/PI) staining. Content of malondialdehyde ( MDA) and superoxide dismutase (SOD) activity in hUC-MSCs were evaluated, and the expression levels of p53 and p21Cip1/Waf1 were analyzed by real-time fluorescence PCR. Results Pretreatment with 10~200 mg/L of EGb761 for 3 hours reduced the sensitivity of hUC-MSCs to t-BHP ( 100 μmol/L) induced proliferation inhibition, while EGb761 over 100 mg/L had no significant effect on enhancing the protection of hUC-MSCs . EGb761 at 100 mg/L prohibited hUC-MSCs apoptosis and MDA accumulation in hUC-MSCs induced by 100μmol/L of t-BHP acting for 6 hours, maintained the enzymatic activity of SOD, and decreased the expression of p53 and p21Cip1/Waf1 in hUC-MSCs with t-BHP-induced injury. Conclusion EGb761 is capable of protecting hUC-MSCs against oxidative stress injury, and its mechanism is probably related with the modulation of p53/p21 signal pathway.
4.Construction of compound membrane with corneal stromal cells and collagen-chitosan and its biocompatibility

Objective To investigate the construction of compound membrane with corneal stromal cells and collagen-chitosan by tissue engineering technique and its biocompatibility.Methods Rabbit and human corneal stromal cells were separated and seeded into collagen-chitosan membrane.The compound membrane was transplanted into rabbit corneal stroma.Then the growth condition of keratocytes,the effect on normal keratocytes and degradation of compound membrane were detected by corneal confocal microscope,anterior OCT and histological and immunohistochemical methods ex vivo 1,2,4 weeks after grafting.Results The rabbit and human corneal stromal cells grown well in collagen-chitosan scaffold.The compound membrane degradated gradually after grafting.There was no necrosis and dissolvation.Corneal epithelium,stroma and endothelial cells were all normal.Conclusion Collagen-chitosan can be used as a biological scaffold for construction of corneal stroma.Corneal confocal microscopy and anterior OCT are new methods to observe the biological activity of constructed corneal stroma.
5.Efifcacy of thalidomide combined with VAD in treatment of Elderly multiple myeloma and its adverse reactions

Objective To investigate the efifcacy of thalidomide combined with VAD in treatment of elderly multiple myeloma(MM). Methods 76 cases of elderly MM were randomly divided into two groups, control group (n=38) were treated with VAD alone while the observation group (n=38) were treated with thalidomide and VAD. Clinical efifcacy and adverse reactions in two groups were compared. Results The total effective rate in observation group was 84.21%, which was signiifcantly higher than 55.26%in control group (P<0.05). After treatment, the M protein, myeloma cells andβ2-microglobulin in observation group were (20.77±4.15)×10-2μg/mL, (12.84±2.85)×10-2μg/mL and (2.48±0.53)μg/mL, which were signiifcantly lower than before treatment and control group(P<0.05). Hemoglobin in observation group was (108.83±5.81) g/L, which was signiifcantly higher than before treatment and control group(P<0.05). The adverse reactions were nausea, vomiting, drowsiness, constipation, infection and rash and so on in two groups after treatment, and there was no signiifcant difference between two groups. Conclusion Thalidomide combined with VAD regimen has a better effect in treatment of elderly multiple myeloma than single VAD, has less adverse reactions and well tolerated.

7.Comparison of dexmedetomidine and propofol for stereotactic brain surgery in patients with intractable psychosis

ObjectiveTo compare the effects of dexmedetomidine and propofol for stereotactic brain surgery in patients with intractable psychosis.MethodsThirty male patients with intractable psychosis,aged 22-33 yr,weighing 60-90 kg,scheduled for stereotactic surgery,were randomized to receive either propofol (group P,n =15) or dexmedetomidine (group D,n =15).Anesthesia was induced with iv injection of midazolam 0.05-0.10 mg/kg and fentany 1-2 μg/kg in the two groups,and in addition,dexmedetomidine was infused at 0.3-0.7μg· kg- 1 · h- 1 after a loading dose of 1 μg/kg (duration of infusion > 10 min) and propofol 1-2 mg/kg was injected intravenously before endotracheal intubation in group D and propofol 2-3 mg/kg was injected intravenously and then propofol was infused at a rate of 3-4 mg· kg- 1 · h- 1 in group P.Orotracheal intubation was performed under the guidance of direct laryngoscope.The patients kept spontaneous breathing.The adverse events such as body movement,bucking,apnea,adverse cardiac events and hypoxemia were recorded during location.ResultsThe incidence of body movement,bucking,apnea,tachycardia,hypotension and hypoxemia was significandy lower,while the incidence of bradycardia was significantly higher in group D than in group P ( P < 0.01 ).There was no significant difference in the incidence of hypertension between the two groups (P > 0.05).ConclusionDexmedetomidine provides better anesthesia,exerts less effect on the respiratory and circulatory functions and is safer than propofol for stereotactic surgery in patients with intractable psychosis.

9.Design and implementation of a military disease prevention and control website

Objective To establish a military disease prevention and control website to achieve dissemination of disease control information , resource sharing , business communication in the whole army , and popularize relevant knowledge of disease prevention and control promotion among the troops .Methods Based on the content management system and meta-data repository platform,B/S system architecture, JSP,MySQL and other programming techniques and tools were used to complete the system development .Results The military disease prevention and control website we designed has been for-mally launched ,which was able to implement a set of functions such as content distribution , full-text retrieval ,classification and aggregation of knowledge , online consultation .Conclusion This website plays a positive role in enhancing military prevention and control ,popularizing relevant laws and regulations ,meeting the health needs of the army soldiers and impro-ving the ability of disease prevention in grass-roots units.
10.A Control Study of N-acetylcysteine Therapy for Chronic Obstructive Pulmonary Disease

Objective NAC therapy was used to observe the effect of TNF-?,hs-CRP,PaO2,PaCO2and lung function in patients with COPD,so as to evaluate the therapeutic effect. Methods 65 patients with COPD were recruited and the patients were randomly divided into 2 treatment groups:control group (n=31) and experimental group (n=34). Patients in the two groups received the conventional treatment of COPD in unchanged dosage for the duration of the study except stopping using antibotic drug after infection was resolved. NAC tablets were taken orally at a dose of 600mg once a day in experimental group. After 8 weeks treatment,we appraised lung function and the plasma level of TNF-?,hs-CRP,PaO2 and PaCO2. Results Compared with normal controls,the plasma levels of TNF-? and hs-CRP increased significantly.
