1.Quantitative study of right ventricular function in the normal second and third trimester fetuses by M-mode tricuspid annular displacement

Objective To quantitatively study right ventricular function in the normal second and third trimester fetuses by M-mode tricuspid annular displacement(TAD).Methods TAD was measured using conventional M-mode echocardiography on 161 normal second and third trimester fetuses with gestation age (GA) between 19-38 weeks,meanwhile multiple parameters for evaluating right ventricular function were obtained using pulsed Doppler echocardiography (PW) and myocardial Dopple tissue imaging (DTI).The correlation between TAD and other parameters were analyzed using SPSS 17.0.Results In normal second and third trimester fetuses,the TAD was (9.38 ± 1.71)mm with a range of 5.79-13.90 mm,and was increased with the growth of GA.TAD was correlated with GA,E,A,Em,Am and Sm significantly (r =0.759,0.547,0.320,0.497,0.483 and 0.598 respectively,all P <0.001).TAD was not correlated with HR(P >0.05).TAD showed differences between the second trimester fetuses and the third trimester fetuses (P < 0.05).Conclusions In normal second and third trimester fetuses,the TAD is increased with the growth of GA,and has good correlation with GA,E,A,Em,Am and Sm respectively,and may become a new promising modality to evaluate function of RV simply and accurately.The technique will be propitious to use in hospitals (without DTI) because of simplicity of operator and lower requirement on the technology and equipment precision.
2.A pilot study of forepart rehabilitation in acute stroke patients at different time point

Objective To observe the effect of early rehabilitation treatment at different time point on the motor function and activity of daily living (ADL) and emotion in acute stroke patients. Methods 120 patients with acute stroke were randomly assigned to three reha-bilitation groups and one control group according to the disease course (3 days, 5 days, 8 days when the disease is steady). Each group con-sisted of 30 eases. The patients in each group were treated with the regular medication therapy, rehabilitation groups were treated with com-prehensive rehabilitation treatment including Bobath technique therapy. Neurological deficit, motor function, balance function, ADL, emo-tion of all the patients were assessed before and 21 days after rehabilitation intervention. Results After treatment, the motor function, bal-ance function, ADL and emotion of each rehabilitation group were improved. The patients'rehabilitation scores in FMA, Fugl-Meyer balance function, MBI and HRSD got much better than that in control group. The difference was statistic significant. The effect of rehabilitation treat-ment did not show difference among rehabilitation groups. Conclusion Early rehabilitation treatment for acute stroke does benefit for stroke patients. Effect does not show vary with the start time of rehabilitation treatment within 8 days after acute stroke.
3.A comparative split-face study of intense pulsed light versus 595-nm pulsed dye laser in the treatment of post-acne erythema

Objective To evaluate and compare the clinical efficacy and safety of intense pulsed light (IPL) versus 595-nm pulsed dye laser (PDL) for the treatment of post-acne erythema.Methods A randomized split-face clinical trial was conducted.A total of 20 patients with post-acne erythema were enrolled,and randomized to receive treatment with IPL on one half of the face and 595-nm PDL on the other facial side once every 4 weeks for 3 sessions.Digital photographs were taken using the VISIA,and erythema index was recorded before each treatment and one month after the last treatment.The severity of bilateral facial erythema was evaluated based on a 4-point grading scale before the first treatment and after the last treatment.Pain scores and adverse reactions were recorded using a visual analogue scale (VAS) after each treatment,and a patient satisfaction survey was conducted by questionnaire at the last follow-up.Results The mean erythema index on the IPL side before and after treatment was 472.25 ± 86.02 and 357.15 ±82.71 respectively,and that on the PDL side before and after treatment was 476.40 ± 74.25 and 360.05 ± 64.83 respectively.Repeated measures analysis of variance (ANOVA) showed that the erythema indices on both treated sides significantly decreased over time (F =197.666,P < 0.001),and the efficacy of IPL was better than that of PDL (F =1 173.909,P < 0.001).Erythema severity grades on the IPL side as well as on the PDL side significantly differed between before and after treatment (Z =28.735,31.450,respectively,both P < 0.001).As VAS showed,the pain score on the PDL side was significantly lower than that on the IPL side (t =2.468,P < 0.05).Among the 20 patients,17 and 15 assessed their improvement as good or excellent after PDL and IPL treatment respectively,but there was no significant difference between the two groups (Z =2.696,P > 0.05).The adverse reactions included erythema,burning sensation,tense sensation,blistering and hyperpigmentation on IPL-treated side,and erythema and purpuric reactions on the PDL-treated side,which all disappeared in a few hours to several days.Conclusions Both IPL and 595-nm PDL are effective and safe for the treatment of post-acne erythema,and are worthy of clinical promotion and application.IPL shows superiority in the efficacy,but elicits higher pain sensation compared with PDL.
4.Anti-tumor effect of panax notoginsenosides combined with cyclophosphamide metronomic chemotherapy

Objective To observe the anti-tumor effect, anti-angiogenic effect and immune regulation of panax notoginsenosides (PNS) combined with low-dose cyclophosphamide (CTX) metronomic chemotherapy.Methods The model of B16-BL6 melanoma spontaneous metastasis was established by hypodermic injection of B16 cells in the palmula in mice. All mice were randomly divided into the model group, high-dose CTX group (MTD group), low-dose CTX group (metronomic group), PNS group and PNS combined with low-dose CTX group (combined group). The tumor-inhibition rate and organ indexes of thymus and spleen were observed. The content of interleukin-2 (IL-2) was determined by ELISA, peripheral blood WBC was counted, and the tumor microvessel density (MVD) and the expressions of vascular endothelial growth factor (VEGF) and chemokine receptor-4 (CXCR-4) were detected by applying immunohistochemical staining. The nodules of lung metastasis were counted.Results In the metronomic group, PNS group and combined group the tumor grew slowly and the tumor-inhibition effect was permanent and stable. The tumor-inhibition rate was 66.15% in the combined group with a better tumor-inhibition effect (P<0.01). The toxicity and side effects were decreased in the metronomic group compared with those in the MTD group. The all indexes of immunity were improved in the combined group with a significant difference (P<0.01) compared with those in the MTD group and reached the level in the model group. MVD and the expressions of VEGF and CRCX4 were decreased (P<0.05), and the nodules of lung metastasis were reduced in the combined group.Conclusion PNS combined with low-dose CTX shows a synergistic anti-angiogenic effect, inhibits tumor metastasis with a permanent tumor-inhibition effect and lower toxicity and side effects, and improves the whole body function and immune level.
5.High dose chemotherapy combined with peripheral blood stem cell transplantation (PBSCT) in the treatment of patients with small cell lung cancer

Objective To investigate the therapeutic effect and safety of PBSCT combined with high dose chemotherapy in the treatment of small cell lung cancer(SCLC). Methods From March, 1994 to June, 1999, high dose chemotherapy combined with PBSCT was carried out in 20 patients with SCLC. Eight patients were in CR status while twelve patients were for salvaging therapy. Allo-PBSCT was performed in 1 patient and Auto-PBSCT in 19 patients. After mobilization of stem cell, mononeuclear cell (MNC) was collected for ≥3.0×108/kg, CD34+ cell≥2.0×106/kg. The high dose chemotherapy included carboplatin 350?mg/m2×3 days, VP-16 120?mg/m2×5 days, and E-ADM 40?mg/m2×2 days. Results Encouraging results were achieved in 8 patients who were in CR status. Seven of the eight patients survived after Auto-PBSCT. One patients survived more than 1 year, 4 cases over 2 years, and 3 cases over 3 years. Of the 3 patients, 2 survived more than 4 years. In the group of salvaging therapy, response was good, however there was no long-term survival and median remission time was only 3.5 months. Conclusion High dose chemotherapy combined with PBSCT in the treatment of SCLC with CR status is feasible and safe, although the side effects are severe too. It is worth further studying on clinical practice.
6.Effects of solvent environment on the structure of hepatitis B surface antigen (HBsAg).

As a virus-like particle, hepatitis B surface antigen (HBsAg) was the primary component of hepatitis B vaccine. HBsAg was maintained by the non-covalent interaction of proteins and lipids. The intact structure of HBsAg particle was vital to its function. However, there was no report about the effects of solvent environment on HBsAg structure. In this paper, we studied the effects of temperature, pH, ionic type and salt concentration on HBsAg structure. The results showed that HBsAg was stable at normal temperature, but began to denature above 60 degrees C. The aggregation of HBsAg at pH 3.0 and 4.0 was nearly irreversible, but partly reversible at pH 5.0. The influence of ionic type on HBsAg was generally in accordance with Hofmeister sequence, except that SO4(2-) caused more aggregation than F-. HBsAg aggregates started to be visible in 0.4 mol/L (NH4)2SO4, and the extent of aggregation increased with the salt concentration. Therefore, caution must be taken when using (NH4)2SO4 in the hydrophobic chromatography purification of HBsAg.

7.Application of repetitive transcranial magnetic stimulation in the diagnosis and treatment of alcohol addicts

Alcohol dependence as a chronic disease with high relapse rates,seriously affects individ-ual health and causes related social problems. However,the treatment of alcohol addiction currently lacks ef-fective interventions to reduce craving and prevent relapse. Repetitive transcranial magnetic stimulation ( rTMS) is an emerging intervention in the field of addiction. This paper reviews the researches to introduce the working principle of TMS and its effects on brain functions and neurotransmitters of alcohol dependent pa-tients. Studies have shown that rTMS has a certain effect on reducing alcohol craving,but the existing re-search protocols and their results are not exactly same,and the best parameters need to be clarified. Future research can try to explore optimized treatment options,and find biological efficacy indicators to provide more evidence for further clinical application.
8.Expression of angiogenin in COS-7 cells and analysis of its biological activity.

This study was purposed to investigate the angiogenin (ANG) expression in COS-7 cells and its biological activity. The gene of angiogenin was obtained from mononuclear cells of peripheral blood by using RT-PCR and inserted into eukaryotic expression vector of pcDNA3.1. After being transfected into COS-7 cells, the recombinant ANG was identified by Western blot assay. The function of promoting proliferation of ANG to ECV304 cells was detected by MTT method, and its activity of vascularization was analyzed by chick embryo chorioallantois treated by the culture supernatant after transfection with pcDNA3.1-ang. The result showed that recombinant ANG was expressed in COS-7 cells after transfection for 24 to 36 hours. It could specifically react with monoclonal antibody against ANG. The recombinant ANG could obviously promote the proliferation of ECV304 cells. In contrast with the control group, the culture supernatant of pcDNA3.1-ang transfected group could stimulate the angiogenesis in embryo chorioallantois. It is concluded that the ang transiently expresses in COS-7 cells, and its expression product obviously stimulates the cell proliferation and angiogenesis.

9.Clinical translational research of chimeric antigen receptor-T (CAR-T) cells for the treatment of relapsed and refractory B-cell lymphoma/leukemia.

B-cell lymphoma and leukemia are the most common subtypes of malignant lymphomas. Relapse and refractory to multiple therapy are the main reasons of treatment failure. As the classical anti-tumor methods, surgery, radiation, chemotherapy and palliative therapy have cured lots of cancer patients. However, each year many patients still died of different kinds of hard-to-treat cancers. Although the ratio of complete remission of B-cell lymphoma/leukemia patients particularly with CD20 positive mature B cell malignancies has been largely increased after the application of Rituximab in clinic, nearly 20%-40% patients still died due to relapse and refractory to the treatment. During last five years, the development of chimeric antigen receptor-T (CAR-T) cells, especially CD19 CAR-T cells, which can recognize CD19 specifically expressed on B cells and have been demonstrated to be significantly effective to relapsed and refractory B cell lymphoma/leukemia in clinical trials, has gradually attracted extensively concerning from researchers and clinicians. Many medical institutions all over the world (besides in China) have registered the clinical trials for B-cell lymphoma/leukemia patients by use of CAR-T cells. In this review, we summarize the developmental history, the main ongoing clinical trials and proved potential adverse affects of CD19 CAR-T cells for the treatment of patients with B-cell lymphoma/leukemia.

10.Application of the Oxford classification of IgA nephropathy to predict renal outcome.

OBJECTIVETo validate the value of the Oxford classification of IgA nephropathy in predicting the renal outcome in Chinese population.
METHODSRetrospective study was done in patients with IgA nephropathy. All slides were re-assessed according to the Oxford classification of IgA nephropathy. The primary end point is doubling serum creatinine, or a 50% reduction in estimated glomerular filtration rate (eGFR), or end-stage renal disease. Pathologic predictors for the progression to the end point were determined by univariate and multivariate Cox regression.
RESULTSTotally 533 patients were enrolled in the study. During the follow-up (median: 39 months; range: 12-263 months), 5.07% of the patients reached the end point. While tubular atrophy and interstitial fibrosis and arterial/ arteriolar lesion were associated with the endpoint in univariate analysis, only the T score was predictive of the renal outcome in multivariate Cox regression. Combination of the patho- logic lesions had no impact on renal outcome.
CONCLUSIONAccording to the Oxford classification of IgA nephropathy, the degree of tubulointerstitial fibrosis is the only feature independently predictive of renal outcome.
