1.Surgical treatment for Mayo II B comminuted fracture of the olecranon.

OBJECTIVETo study clinical effects of surgery for the treatment of Mayo II B comminuted fracture in ulna olecranon.
METHODSFrom May 2008 to March 2015, a total of 37 patients with Mayo II B comminuted fracture in ulua olecranon were treated, including 20 males and 17 females, ranging in age from 40 to 65 years old ,with an average of 53 years old. All the patients were treated with open reduction and internal fixation within 4 to 7 days after injuries. All the patients had pain and functional disorder uf elbow joint. The X-ray and CT examination showed ulna olecranon comminuted fracture of Mayo II B. Postoperative complications were observed ,and Broberg-Morrey criteria was used tu evaluate therapeutic effects.
RESULTSAll the patients were followed up ,and the duraiton ranged from 9 to 30 months ,with a mean of 15 months. Two patients had surface infection around incision ,and were healed by changing dressings. No other complications occurred such as needle slipping to stimulate skin ,screw loosening and wire broken. One patient had slight uneveness of joint surface without obvious functional disorder. According to Broberg-Morrey elbow fracture curative effect criteria, 11 paients got an excellent result, 24 good and 2 fair,and the total score was 87.0 ± 7.3.
CONCLUSIONFor the Mayo II B comminuted fracture in ulna olecranon, preoperative preparation, intraoperative restoring of the articular surface smooth and reasonable internal fixation, and postoperative rehabilitation actively, can obtain satisfactory clinical effects.

2.Study on Living Related Liver Transplantation for Treating Wilson′s Disease in Children

Objective To explore the liver pathology from live related liver transplatation(LRLT) of Wilson′s disease(WD) in children,and evaluatethe indication of LRLT.Methods The sample of this study,including the donater and patient,came from the LRLT. It was observed with HE,MASSON,Timm′s and Rubeanic staining.Results With HE and MASSON stainning,hepatocyte showed degeneration,fiber hyperplasic and false lobule formed;Timm′s and Rubeanic stainning showed typical black deposit of granules and conglomerations.Liver pathology revealed that liver cirrhosis appeared,and it was diffrentent from Child Pugh.Conclusions It is of great significance that the liver pathology is useful for the treatment of WD.If liver pathology supportes,the indication of LRLT can be measurably broadened.
3.Participation of Clinical Pharmacists in the Treatment of One Case of Renal Abscess with Iron Deficiency Anemia

Objective: To make out the optimal drug treatment regimen through the participation of clinical pharmacists in the treatment of one case of child with renal abscess complicated with iron deficiency anemia.Methods: Clinical pharmacists participated in the design of treatment program for the child, including the choices of drugs, dosage and route, and the treatment of adverse reactions, in order to make out the individualized medication.Results: By selecting the sensitive antibiotics, the renal abscess caused by Escherichia coli was cured successfully.Conclusion: Clinical pharmacists participating in clinical consultation can further optimize the treatment plan, and play an active role in the treatment of infected patients.
4.Studies on genetic relationship of Dioscorea.

Based on the results of the morphologic studies on genus Dioscorea, the paper summarized the entire chemical constituent that isolated from this genus and analyzed it with the methods of chemotaxonomy. The rules of the chemical constituent and pharmacodynamic effects were analyzed. Seventeen species which belong to Sect. Stenophora Uline of Dioscorea contain steroidal sapogenin. Other species with different main components such as polysaccharide and tannin have have different effects. This chemotaxonomic view point will conduce to establish a phylogeny of the genus Dioscorea.

5.Operation for the treatment of radial head fracture with collapse of anterior articular surface.

OBJECTIVETo evaluate clinical outcomes of fixation for the treatment of radial head fracture with collapse of anterior articular surface.
METHODSFrom March 2006 to January 2013,17 patients with radial head fractures with collapse of anterior articular surface were analysed. According to the Mason classification, there were 12 cases with Mason type II fractures and 5 cases with Mason type III fractures. All the patients were treated with open reduction through posterolateral entrance of elbow joint and Herbert or titanium cannulated screw internal fixation.
RESULTSAll the patients were followed up, and the duration ranged from 6 to 18 months, with a mean of 11.3 months. According to the Broberg and Morrey score system, 2 patients got an excellent result, 12 good and 3 fair. There were no complications such as infection of elbow joint, nerve injury, non-union, traumatic osteoarthritis, heterotopic ossification and elbow instability. However, the postoperative activity range of elbow in the injuried side was less than that in the normal side.
CONCLUSIONRadial head fracture with collapse of anterior articular surface is easily misdiagnosed, and it can be treated with open reduction and internal fixation through posterolateral entrance.

6.A cross-sectional survey of the knowledge on chronic obstructive pulmonary disease in physicians of tertiary hospitals in Northern China

To investigate physicians' knowledge about chronic obstructive pulmonary disease (COPD) in tertiary hospitals in northeast China.Physicians from 77 tertiary hospitals in northeast China were surveyed with a questionnaire,which included questions such as risk factors,symptoms,exacerbations,comorbidities and diagnostic criteria of COPD.Besides cigarette smoking,air pollution and pulmonary infections,only 22.5% (40/178) physicians recognized that the biomass fuels may induce COPD.Totally 59.0% (105/178) physicians recognized the importance of spirometry to the diagnosis of COPD.Besides dyspnea,cough,sputum production,wheezing and chest tightness,only 23.7% (42/177)of physicians considered that limitation of activity was an important symptom of COPD.65.5% (116/177)physicians believed that recurrent lung infections was one of the most important comorbidities of COPD.However,less than 30% [20.9% (37/177)-28.8% (51/177)] physicians were aware of the other important comorbidities.The physicians of tertiary hospitals in northeast China need to be systematically educated on COPD to meet the new guideline.
7.Effect of blood activating stasis removing method on prognosis of intracerebral hemorrhage patients.

OBJECTIVETo observe the clinical effect and safety of blood activating stasis removing method (BASRM) on intracerebral hemorrhage patients.
METHODSA multi-center, prospective, randomized, open, controlled and blinded endpoint design was adopted. Totally 228 intracerebral hemorrhage patients were assigned to the treatment group and the control group, 114 in each group by center randomized method. All patients received basic treatment of Western internal medicine. Patients in the treatment group received intravenous infusion with Xingnaojing Injection (XI) from the 1st day of grouping, 20 mL per day for 14 days. Then they took or were nasally fed with Chinese medical granules (by syndrome typing as complicated with wind syndrome, fire syndrome, and phlegm syndrome) for 21 days. Finally they took Naoxueshu Oral Liquid (NOL), 10 mL each time, 3 times per day till the 3rd month of incidence. Patients' disability degree, activities of daily living, neurological impairment, the effective rate, physiologic functions, mental status, social relationship, and degree of treatment satisfaction were assessed using Modified Rankin Scale (MRS), Barthel index (BI), National Institute of Health Stroke Scale (NIHSS), and patient reported outcome (PRO). Head CT was performed to evaluate the absorption of hematoma at the 1st and 7th day of grouping. The safety was also assessed.
RESULTSTotally 108 patients in the treatment group and 112 patients in the control group completed the trial. There was no statistical difference in the total effective rate between the two groups after 3 months of treatment (P>0.05). The MRS score was obviously lower in the treatment group than in the control group (P<0.01) at month 3 after attack (P<0.01). There was statistical difference in the difference between pre-post hematoma volume between the two groups after 7-day treatment (P<0.05). The NIHSS score of two groups at the 7th, 14th, 21st day, and 3rd month decreased significantly (P<0.05). Compared with the control group, the decremenit of NIHSS score decreased more obviously in the treatment group at day 7, 21, and 3rd month (P<0.05). Compared with the control group, the BI increased (P<0.01); physiologic fupctions, social relationship, treatment satisfaction and total score in PRO scale were all lower in the treatment group than in the control group (P<0.05, P<0.01). The incidence of adverse events occurred in 7 cases (6.14%) in the treatment group and 5 cases (4.39%) in the control group, with no statistical difference (P>0.05).
CONCLUSIONBASRM could lower the deformity rate of intracerebral hemorrhage patients at month 3, effectively promote hematoma absorption within 7 days, improve neurologic impairment, and elevate living abilities at month 3 of onset.

8.Effects of immunosuppressor against kidney disease in treatment of 21 patients with cytomegalovirus pneumonia

Objective To study the clinical feature,treatment,and prognosis of the cytomegalovirus (CMV)pneumonia patients treated with immunosuppressor against kidney disease.Mlethod The patients received immunosuppressor against kidney disease in The First Affiliated Hospital of Sun Yat-sen University from June 1999 to December 2006.CMV antigen of leucocyte in the peripheral blood and/or bronchoalveolar lavage fluid of these patients were detected with immunocytochemical methods,and 21 patients were found suffering from CMV pneumonia.The 21 patients were introvenously injected with ganciclovir 5~10 mg/(kg?d),and the immunosuppressive agent treatment suspended.Their clinical feature and prognosis were retrospectively analyzed. Results The 21 patients received corticosteroids before CMV pneumonia contracted,of them,13 patients had been intensively treated with Methyllprednisolone with mean total dose(3.2?0.6)g.Of them,15 had been treated with cyclophosphamide with mean total dose(3.8?1.3)g.The median time from the beginning of using immunosuppressor to the onset of CMV pneumonia was 25(13~92)days.All patients had fever,cough, shortness of breath and X-ray showed interstitial pneumonia,of them,19 patients developed hypoxemia,and 11 patients' CMV antigen was positive in the leucocyte from bronchial lavage fluid.The result showed 9 patients survived and 12 died.The average duration of treatment with ganciclovir was(26.2?6.3)days. CMV pneumonia is a serious complication in patients who were treated with immunosuppressor against kidney disease.The mortality is high.Ganciclovir is a medicine of choice to treat CMV pneumonia.

In recent years, artificial intelligence (AI) technology has advanced rapidly and has been widely applied in various fields such as medicine and pharmacy, accelerating the drug development process. Focusing on the application of AI in the discovery and optimization of lead compounds, this review provides a detailed introduction to AI-assisted virtual screening and molecular generation methods for discovering lead compounds, while particularly highlighting the cases of AI-drived drugs into clinical trials. Additionally, we briefly outline the application of AI basic algorithm models in quantitative structure-activity relationship (QSAR) and drug repurposing, offering insights for AI-based drug discovery.
10.Effects of sodium butyrate on proliferation and apoptosis in rat mesangial cells

Objective To investigate the effect of sodium butyrate on apoptosis of rat mesangial cells and the regulation of phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin(PI3K/AKT/mTOR)signal pathway,and analyze the possible mechanism.Methods Primary cultured,isolated and purified rat mesangial cells.The cells were randomly divided into blank group,control group[10 μg·L-1 epidermal growth factor(EGF)],low dose experimental group(10μg·L-1 EGF+0.5 mmol·L-1 sodium butyrate),medium dose experimental group(10 μg·L-1 EGF+1.0 mmol·L-1 sodium butyrate)and high dose experimental group 10 μg·L-1 EGF+2.0 mmol·L-1sodium butyrate).Cell survival rate was detected by cell counting kit-8(CCK-8)method,cell cycle was detected by propyl iodide(PI)staining,cell apoptosis rate was detected by flow cytometry,and the expression of PI3K/AKT/mTOR channel-related proteins was detected by Western blot.Results After 24 h of treatment,the optical density of blank group,control group and low,medium,high dose experimental group were 0.36±0.03,0.66 ±0.03,0.57±0.05,0.47±0.02,0.41±0.01;which at 48 h of treatment were 0.55±0.03,0.83±0.04,0.68 ±0.03,0.65±0.02,0.60±0.02,respectively.The results showed that 10 μg·L-1 EGF significantly stimulated the proliferation and activation of mesangial cells(P<0.05),and the proliferation of mesangial cells was significantly inhibited after treatment with different concentrations of sodium butyrate for 24 and 48 h(P<0.05).PI staining showed that the G1 phase blocking rates of blank group,control group and low,medium,high dose experimental groups were(53.37±0.43)%,(46.84±0.67)%,(57.81±0.48)%,(62.77±0.77)%,(67.57±0.71)%,respectively.The results showed that sodium butyrate could induce cell cycle arrest in G1 phase in a concentration-dependent manner(P<0.01).The apoptosis rates of blank group,control group and low,medium,high dose experimental groups were(4.43±0.48)%,(2.45±0.31)%,(6.16±0.33)%,(8.25±0.40)%and(12.12±0.41)%,the differences were statistically significant between control group and low,medium,high dose experimental groups(all P<0.01).Conclusion Sodium butyrate can effectively inhibit the proliferation and induce apoptosis of rat mesangial cells,and its mechanism may be related to the inhibition of PI3K/AKT/mTOR signaling pathway.
