1.Application of different nutritional support modes in early stage for severe mechanical ventilation patients

Objective To explore three nutritional support mode in early stage on curative effect and outcome in the severe patients with mechanical ventilation and provide references for the clinic.Methods A total of 90 patients with mechanical ventilation from February 2009 to December 2011 were randomly divided into group A,B and C with 30 cases in every group,and were recevied parenteral nutrition(PN),enteral nutrition(EN)and combined enteral and parenteral nutrition(CEPN),respectively.The nutrition indexes,the immune indexes and the level of plasma endotoxin were mensurated in the 1st day and the 7th day after mechanical ventilation.The complications and the condition changes were carefully observed.Results Among 90 cases,57 cases were successfully detached from the respirator,at early stage,25 cases died,32 cases had complicatinns.All the observation indexes including nitrogen balance,albumin,IgA,IgG,the level of plasma endotoxi,the cases with complication,and the cases detached from respirator were recorded in the 7th day after mechanical ventilation,group A(PN):(2.3±1.2)g/d,(30.6±2.7),(1.4±0.5),(7.8±2.1) g/L,(37.1 ±6.3) pg/ml,17 cases,13 cases; group B(EN):(4.2 ±0.5) g/d,(33.5 ±1.8),(2.5 ±0.3),(13.6 ±1.5) g/L,(49.7 ±7.3) pg/ml,9 cases,21 cases,group C(CEPN):(5.8 ±0.8) g/d,(35.8 ±1.7),(2.5 ± 0.2),(13.9±1.7) g/L,(50.3 ±7.1) pg/ml,23 cases,6 cases.The differences between all the observation indexes in group B and C were superior to group A,which showed statistically significant difference(P <0.05).The difference of nutrition indexes and the incidence of complication was statistically significant between group C and group B (P < 0.05 ).Conclusions EN combined with PN is an reasonable and effective nutrition support mode for patients with mechanical ventilation. It can improve the curative effects,and reduce the incidence of complication.
2.The clinical study of early systematic nursing intervention on the prevention of venous thrombus of low limbs in ICU patients

Objective To explore the clinical meaning of early systematic nursing intervention on the prevention of low limbs venous thrombus in ICU patients.Methods Totals of 120 patients in ICU were randomly divided into two groups by entering order number,odd numbers were experimental group(60 cases)and double numbers were control group(60 cases).Experimental group received early systematic nursing intervention while control group received ICU conventional nursing.All patients were measured the lower limb diameter per day,and were received colour Doppler examination to tested their femoral venous blood velocity in first,seventh and fourteen day of ICU,and their femoral venous blood velocity and incidence of low limbs venous thrombus were compared.Results No difference of femoral venous blood velocity in two groups was founded before intervention (P >0.05).After intervention,femoral venous blood peak velocity and average velocity of seventh and fourteen day in experimental group was(52.8 ± 9.6),(52.6 ± 8.8)and(34.2 ± 6.9),(35.3 ± 7.2),respectively,and higher than(41.3 ±7.8),(42.5 ±7.6)and(22.5 ±7.1),(23.7±6.6)in control group,differences between two groups were statistically significant(t =2.724,3.125,2.722,3.114 ;P < 0.05); the incidence of low limbs venous thrombus in experimental group was lower than that of control group,and the difference was statistically significant(x2 =4.904,P < 0.05).Conclusions Adopting early systematic nursing intervention can improve the femoral venous blood velocity,and effectively prevent and reduce the incidence of low limbs venous thrombus of ICU patients.
3.Application of the healthcare failure mode and effect analysis in the security management of neonatal BCG vaccination

Objective To explore the effect of the Healthcare Failure Mode and Effect Analysis (HFMEA) in the security management of newborn Bacillus Calmette-Guerin (BCG) vaccination.Methods One thousand neonates born in 2010 were selected as the experiment group who received the BCG vaccination under the management with the HFMEA.One thousand neonates born in 2009 were served as the control group who received routine BCG vaccination after birth.We performed the hazard analysis and decision making tree by making the effort to find the potential failure mode,reason and effect during the vaccination procedure and formulate and take the steps for top priority issue.Results After the application of the HFMEA into the management,the index of the risk priority number (RPN) ≥8 points in the high risk process could decrease to the average≤6 points by means of controlled measures in the procedure of BCG vaccination.The incidence rate of adverse reaction for the BCG vaccination could decrease to 0.0% from 0.6% ( x2 =4.2,P < 0.05 ).Conclusions We can reduce the risk of adverse reaction and promote the rate of safety in BCG vaccination by mean of the management of BCG vaccination procedure with the HFMEA.
4.The expression and effect of LINGO-1 during the differentiation of spinal cord derived neural stem cells

BACKGROUND: It is of great significance to explore the expression and effect of LINGO-1 in the differentiation of spinal cord derived neural stem cells (SpNSCs) for regulating neural stem cell differentiation and repairing spinal cord injury. OBJECTIVE: To investigate the expression features and biological effects of LINGO-1 in the differentiation of SpNSCs. METHODS: SpNSCs were isolated from the rat spinal cord and cultured in vitro. The expression characteristics of LINGO-1 was observed through double immunofluorescence staining of LINGO-1 and Nestin (neural stem cells), β-Tubulin III (neurons), GFAP (astrocytes) and O4 (oligodendrocyte) at 0-5 days of differentiation. SpNSCs isolated from the rat spinal cord were cultured in vitro and divided into siRNA group and control group. The siRNA group was transfected with LINGO-1 shRNA lentiviral vector to down-regulate the expression of LINGO-1, and the control group was transfected with Scramble-shRNA lentiviral vector. The growth of neurites was detected by immunofluorescence staining at 5 days after transfection.RESULTS AND CONCLUSION: The SpNSCs could differentiate into neurons, astrocytes and oligodendrocytes. LINGO-1 was expressed in SpNSCs, neurons and oligodendrocytes, but not in astrocytes. The neurite length of the siRNA group was significantly longer than that of the control group (P < 0.05). In summary, the SpNSCs have the potential of multi-directional differentiation, and LINGO-1 has a negative effect on the neurite growth.
