1.Research review about inflammation promoting neoplasm metastasis

In the tumor microenvironment, inflammatory cells regulate intracellular signaling transduction mediated by the corresponding nuclear transcription factor by releasing inflammatory factors ,which leads to transformation of epithelial cells to mesenchymalcellsand eventually promote tumor metastasis. Because of the intimate relationship between inflammation and tumor metastasis , it would be helpful for further enhancement of effectiveness of tumor therapy if we could better understand both the biological character of tumor and the related inflammatory factors.
2.Effect of caloric restriction therapy on mood and behavior

It has been practiced for thousands years to see the relationship between caloric restriction and health.Many experimental and clinical trials confirmed the success of this approach in extending lifespan and decreasing the risk of chronic diseases.However,besides the benefit for the body,it may also give an effort to mood and behavioral improvement.For nowadays,it has been proved that mental assessment is important and necessary before and during caloric restriction therapy.
3.Clinical significance of aberrant hepatic artery arising from superior mesenteric artery in liver cancer and gastric cancer operations

Objective To analyze the clinical significance of aberrant hepatic artery which originated from superior mesenteric artery in liver cancer and gastric cancer operations.Method Four hundred liver cancer patients who underwent digital subtraction angiography (DSA) and multislice spiral computed tomography angiography (MSCTA) and 86 gastric cancer patients who underwent MSCTA between June 2008 and June 2010 and operated at the First Affiliated Hospital of Guangxi Medical University were included in this study.Preoperatively,the origins of the aberrant hepatic arteries from superior mesenteric arteries were detected by medical imaging.Postoperatively,immunohistochemistry of lymphoid tissues around the aberrant hepatic arteries of the gastric cancer patients were performed using recombinant human cytokeratin 20 (CK20) and carcino-embryonic antigen (CEA) to determine the incidence of lymph node metastases around the aberrant arteries.Results Among 486 patients,49 patients with liver cancer and 14 patients with gastric cancer had an aberrant hepatic artery which originated from the superior mesenteric artery.The rate was 12.96% (63/486).The hepatic artery ran in front of the pancreas in 2 patients (3.17%) and behind the pancreas in 61 patients (96.83%).Immunohistochemical analyses of CK20 and CEA were negative which revealed no metastases in the lymphoid tissues surrounding the aberrant arteries.Conclusions Aberrant hepatic artery originated from the superior mesenteric artery can be classified into the pre-pancreas type and postpancreas type.The majority of aberrant hepatic artery belonged to the post-pancreas type.The clinical significance of aberrant hepatic artery is that the hepatic hilar lymph nodes should be dissected in liver cancer and gastric cancer operations.
4.Verification and application of standard method for detecting serum fluoride

Objective Standard method of testing serum fluoride was tested and verified by certified reference material CRM of free fluo ride component in cattle serum (GBW 09143~09144).The method was suited for fluor ide research to assess its practical value.Methods ISE-standard curve,linear relationship,linear range,sensitive and precision were disired result by CRM.400 serum of fluoride in rats were measured.Results The linear ranges of the method were 0.02~5.0 mg/L,r=0.9999,b=57.8,and test lower limit was 0.013 mg/L.The test value was equal to standard value.Conclusions The standard method is accurate,sensitive,convenient and small sample is used;and it is easy to spread.Thus,it has been of practical value.
5.Clinical effects of tolterodine with pelvic floor electrical stimulation in the treatnent of female OAB patients

Objective To investigate whether tolterodine combined with pelvic floor electrical stimulation is more effective than tolterodine alone in the treatment of wonen with overactive bladder(OAB) and the underlying mechanism.Methods Seventy-three female patients with OAB were recruited from our hospital during Oct.2009 to Dec.2010 and prospectively studied,of whom 39 cases were given tolterodine(2 mg each time,twice daily for4 weeks)alone while the other 34 cases were given tolterodine combined with pelvic floor electrical stimulation for the treatment of OAB.Data on urgency,incontinence,micturition frequency,nocturia episodes and voided volume were collected before and after 4 weeks' treatment using a week micturition diary.Results The differences of changes of OAB symptoms between the 2 groups had statistical significance (P < 0.05).Patients in the tolterodine combined with pelvic floor electrical stimulation group reported treatment benefit than the tolterodine group in the micturitions and the volume voided per micturition(P < 0.05).There were no difference in the number of urgency episodes per 24 hours,incontinence episodes and nocturnal episodes (P > 0.05).Conclusion A combination of toterodine with pelvic floor electrical stimulation could significantly improve the OAB symptoms,and is a potential therapy for female patients with OAB.
7.Analysis of orthopaedic teaching under transfer theory

Contents in orthopaedic are independent relatively and finding out the internal relations during them is helpful to improving the effect of orthopaedic teaching.Transfer theory is important tools for clinical teaching practice,and finding out the common characteristics between the orthopaedic chapters is primary for the theory.This research focuses on the following fields:fractures,nerve injury,infection,tumor and deformity.After the common characteristics between these chapters were analyzed and discussed,we concluded that the transfer theory is helpful in orthopaedic teaching practice,especially for students' comprehension and memory,but still we should avoid some negative effects in teaching process.
8.The medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular for the flatfoot related with accessory navicular

Objective To investigate the clinical outcomes of the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular for the flatfoot related with accessory navicular.Methods From March 2009 to October 2011,13 patients (16 feet) with flatfoot related with accessory navicular received treatment by the medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular.There were 4 males and 9 females,with an average age of 41.3 years (range,18-64 years).All patients had obvious valgus calcaneus,the angle of which was 11.3°± 1.4°.According to AOFAS ankle-hindfoot scores,the arch height,calcaneus inclination angle (CI),talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the lateral weight-bearing radiograph of foot,and the talocalcaneal angle (TC),talar first metatarsal angle (TMT) on the AP view of the weight-bearing radiograph of foot,and the heel valgus alignment on axial radiographs of the hindfoot were measured on the X-ray film.Results All patients were followed up for 12 to 31 months,with the average of 16.8 months.Eleven patients (13 feet) felt no pain 6 months after operation,while 2(3 feet) felt pain after long walking.There was no complication,including infection,nerve injury,un-union,and so on.The average AOFAS ankle-hindfoot score improved from 56.4-±6.4 preoperatively to 88.1±2.8 at the last follow-up.Radio graphically,all parameters were statistically significant between pre-operation and the last follow-up,including the arch height,CI,TC,TMT modifying from 3.8±0.3 mm,9.5°±1.1°,47.3°±2.5°,17.6°±1.6° to 12.0±1.1 mm,20.1°±1.5°,32.3°±2.5°,6.8°±1.0° respectively on the lateral weight-bearing view; TC improving from 39.5°±2.3° to 26.2°±2.0°and TMT improving from 15.2°±1.7° to 6.3°±1.0° on the AP weight-bearing view.Conclusion The medial displacement calcaneal osteotomy with reconstruction of posterior tibial tendon insertion on navicular is a good choice for the treatment of flatfoot related with accessory navicular with excellent clinical outcomes.
9.A study on the hepatic artery lymph node micrometastases of distal gastric cancer

ObjectiveTo study the dissecting necessity of lymph node around normal and abnormal hepatic artery in distal gastric cancer undergoing D2 lymphadenectomy.MethodsSixty gastric cancer patients receiving distal D2 lymphadenectomy by the same surgeon between June 2008 to June 2010 at the Department of Gastrointestinal Surgery, First Affiliated Hospital of the Guangxi Medical University were included in this study. The lymph adipose tissue around the anatomically normal and aberrant hepatic artery was carefully dissected, and the lymph nodes sent for recombinant human cytokeratin 20 (CK20) and carcino-embryonicantigen( CEA )micrometastasisimmunohistochemistry.ResultsWiththe micrometastasis immunohistochemistry of CK20 and CEA, we found the metastasise rate of lymph node around the normal hepatic artery was 27%.Patient age, tumor size, Borrmann type, TNM staging were correlated with the lymph node metastase. There were 7 cases with abnormal hepatic artery originating from the superior mesenteric artery. The hepatic artery ran in front of the pancreas in 1 case and behind the pancreas in 6 cases. We found there are no metastases in the lymph adipose tissue surrounding the abnormal artery.ConclusionsCK20,CEA are suitable immunohistochemical targets for estimating the lymph node micrometastasis. In distal gastric cancer age at 60 or older years, tumor larger than 3 cm and Borrmann Ⅲ-Ⅳ type were risk factors for metastasis of lymph nodes around normal hepatic artery, while aberrant hepatic arteries originating from the superior mesenteric artery are much less likely to have positive lymph nodes in D2 lymphadenectomy.
10.Construction of a micro-vascular network model in vitro by microfluidic technology for hemodynamic investigations

Objective The micro-vascular element plays a key role in the delivery of nutrients and the regulation of hemodynamic behavior, however, research is often hindered by ethical , economic and technological issues .Therefore, construction of a micro-vascular network in vitro will help to study the related pathological and physiological behavior in microvessels.Methods In this study, a micro-vascular element model with features of a micro-vascular network in vivo was designed based on the network structure of retinal arterioles .A micro-vascular network model in vitro, characterized by network asymmetry and the presence of both bifurcation-and side-branches , was developed by soft lithography technology . The developed microdevice allowed for the quantification of the cell -depletion layer ( CDL) thickness and hematocrit ( Ht) distribution within the microchannel networks .Results and Conclusion The study showed the potential of the developed in vitro model in revealing key hemodynamic features which have been detected for microvascular elements in vivo, including the relationships between CDL thickness , Ht and red blood cell distribution .The present study provides a new strategy and a technology for studying hemodynamics and microvascular system diseases in vitro.
