2.Effective analysis of lobectomy by single utility port video-assisted thoracoscopic surgery and video-assisted mini-thoracotomy for treating early-stage non-small cell lung cancer

Objective To compare the clinical effects of lobectomy by single utility port video-assisted thoracoscopic surgery (SP-VATS) and video-assisted mini-thoracotomy(VAMT) for treating early-stage non-small cell lung cancer(NSCLC) .Methods 286 patients with early-stage NSCLC in our hospital from October 2010 to October 2012 were randomly divided into the SP-VATS group(n=150) and the VAMT group(n=136) ,and received lobectomy and lymph node dissection by SP-VATS and VAMT re-spectively .The operative time ,intraoperative blood loss ,chest drainage duration ,postoperative total drainage volume ,lymph node dissection number ,postoperative complications and postoperative pain were compared between the two groups .Results The two groups were smoothly performed the operation .There was no perioperative death .The operative time ,lymph node dissection num-ber and postoperative complications showed no statistically significant difference between the two groups (P>0 .05) .However ,intr-aoperative blood loss ,postoperative total drainage volume ,chest drainage duration and postoperative pain scores (postoperative 1 -3 d) in the SP-VATS group were less than those in the VAMT group ,the differences showed statistical significance (P<0 .05) . The incidence of complications in the two groups showed no statistically significant difference (P>0 .05) .Conclusion SP-VATS lo-bectomy for treating NSCLC is safe and reliable with less injury and rapid postoperative recovery compared with VAMT .SP-VATS lobectomy may be as a preferred surgical mode for early-stage NSCLC .
3.Effects of Tiangui menopause capsules on rat ovarian granulosa cell apoptosis

Aim To observe effects of Seabuckthorn fatty acids on old rat ovarian granulosa cell apoptosis and expression of bcl-2 and fas,then discuss the adjustive effect of Seabuckthorn fatty acids on ovarian function of menopause.Methods 22 months old rats were randomly divided into 5 groups,estrogen control group,old control group,Seabuckthorn fatty acids group of high dose,middle dose and low dose.In addition,one young control group was made up of 3 months old rats.Rat ovarian granulosa cell apoptosis was detected by TUNEL.The expression of protein bcl-2 and protein fas were detected by immunohistochemistry,and bcl-2 mRNA and fas mRNA by in situ hybridization(ISH).Grey analyse was done using Q550CW image analyse system.Results Compared with the old rats control group,Seabuckthorn fatty acids of high dose,middle dose and low dose can differently restrain rat ovarian granulosa cell apoptosis,enhance the expression of protein bcl-2 and bcl-2 mRNA,weakened the expression of protein fas and fas mRNA.Conclusion Seabuckthorn fatty acids restrains ovarian granulosa cell apoptosis through adjusting the expression of bcl-2 and fas.Sequentially,it adjusts estrogen level and treats menopause syndrome.
4.The regulation of estradiol on growth dynamics of human LECs affected by increasing telomerase activity

Background Human LECs can express telomerase activity,which participates in the formation of cataract.It is reported that estrogen can increase the expression of telomerase activity in human endometrial cancer and breast cancer cells and play an important role in promoting proliferation and anti-apoptosis,but whether estrogen exerts its role on human LECs is still unclear.Objective This study aimed to investigate whether β-estradiol (β-E2) can increase the telomerase activity of human LECs and the influence of β-E2 on proliferation and apoptosis of human LECs.Method Human LECs line was cultured and passaged in vitro,and 1×10-6 mol/L β-E2 was added in the medium for 0,6,12,24 and 48 hours,and reverse transcription PCR was used to determine the optimal time of the expression of human telomerase reverse transcriptase (hTERT) mRNA in the cells.Cultured cells were divided into five groups.The cells in the blank contol group were cultured in the routin medium.Ethanol of 0.1% was added in the solvent control group,and 1 × 10-8,1 × 10-7 or 1 × 10-6 mol/L β-E2 was added in the medium in different contents of β-E2 groups,respectively.The relative expression level of hTERT mRNA in different groups was detected by reverse transcription PCR.Telomere repeat amplification protocol (TRAP)-ELISA was employed to determine the telomerase activity.The proliferative value of the cells was assayed by cell counting kit-8,and the apoptosis rate of the cells was examined by Hoechst33258 staining.Results The optimal time of β-E2 to rise the expression of hTERT mRNA (absorbance) was at 24 hours under the 1×10-6 mol/L.The relative expression levels of hTERT mRNA in the cells were 0.477±0.015,0.712±0.013 and 0.914±0.031 in the 1 ×10-8,1 ×10-7 and 1 ×10-6 mol/L β-E2 group,which were signifincatly higher than 0.428±0.010 in the blank control group and 0.426±0.010 in the solvent control group (all at P<0.05).The telomerase activity values (absorbance) were 0.711 ±0.015,0.941±0.010 and 1.249±0.047 in the 1×10-8,1×10-7and 1×10-6 mol/L β-E2 group,which were higher than 0.535±0.013 in the blank control group and 0.543 ±0.013 in the solvent control group (all at P =0.000).The proliferantive values of the cells (absorbance) were significantly raised in the 1 × 10-8 mol/L β-E2 group compared with l × 10-7 and 1 × 10-6 mol/L β-E2 group (both at P =0.000),and no significnant difference was found in the proliferetive values between the blank control group and the solvent control group (P =0.718,0.856).The apoptosis rates of the cells in the 1 × 10-8,1 × 10-7 and 1 × 10-6 mol/L β-E2 group were lower than those in the the blank control group and the solvent control group (all at P=0.000),and there was no significant difference between the blank control group and the solvent control group (P =0.777).No obvious correlation was found between the HLECs preliferative values and hTERT mRNA expression levels or telomerase activity values (r=-0.299,P=0.278;r=-0.157,P=0.576).However,significantly negative correlations were seen between apoptosis rates and hTERT mRNA expression levels or telomerase activity values (r =-0.975,P=0.000;r=-0.981,P=0.000).Conclusions β-E2can increase the activity of telomerase in human LECs,and high dose of β-E2can inhibit apoptosis,but it dose not promote proliferation.
5.Preparation and immunogenicity identification of artificial antigen for luteoloside.

Lonicerae Japonicae Flos was one of the most widely used traditional Chinese medicine for its special biological activities. The content of luteoloside, one of its major compounds, was an important standard for the quantity control of Lonicerae Japonicae Flos. The major method used for the detection of luteoloside was instrumental analysis. Compared with the ELISA method, instrumental analysis was time-consuming, complex pretreatment and low-throughout. Thus, it was significantly important to develop an enzyme-linked immunosorbent assay (ELISA) for luteoloside analysis. Here, the conjugates of luteoloside-bovine (LG-BSA) and luteoloside-ovalbumin (LG-OVA) were produced as the immunogen and coating antigen by the carbodiimide ( CDI) method, respectively. The conjugation ratio of carrier protein and the hapten in the conjugate were determined by UV-Vis spectrophotometry (UV). LG-BSA conjugate was used to immunize Bal b/c mice to produce antiserum. The titer and specificity of antiserum were detected by ELISA. The conjugation ratio of hapten and carries protein were 3. 7: 1 (LG-BSA) and 1. 0: 1 (LG-OVA). The antiserum titer was higher than 2 000 with the linear range of 18.4-4 852.4 μg x L(-1), R2 = 0.988 4 and IC50 = 298.7 μg x L(-1). The result showed that the conjugate antigen LG-BSA was synthesized successfully and the mice can produce specific antiserum injected with artificial antigen.

As the manual muscle testing is not sensitive enough to reflect the change of muscle strength, more and more people begin to use isokinetic dynamometry and handheld dynamometer for quantitative tests of muscle strength measurement. This paper briefly introduced the history of clinical approaches of muscle strength measurement, compared the advantages and disadvantages of various quantitative mus-cle strength testing, on which the specific techniques of clinical muscle strength measurement were discussed.
7.Retrospective Analysis of Thromboembolism Prophylaxis in 115 Patients with Spinal Cord Injury

Objective To retrospectively analyze the lower extremity deep vein thrombosis prevention in patients with spinal cord injury (SCI). Methods A total of 115 SCI patients in our department from April to May, 2015 were included. The clinical symptoms, lower limb deep vein ultrasonic testing, laboratory examination were collected to analyze the occurrence, prevention measures, the thrombus location and management of deep venous thrombosis (DVT) in lower limbs. Results Forty-three patients had thromboprophylaxis in other hospitals before admission, and 105 patients in our department after admission, in which, nine cases were with clinical symptoms in other hospitals and three cases in our department. No pulmonary embolism occurred in them. There was no significant difference in most laboratory index-es between patients with DVT and without DVT in lower limbs (P>0.05). Five patients were with DVT in lower limbs in 43 patients who had thromboprophylaxis, and four cases in 72 patients who did not have thromboprophylaxis. No relationship was found between thrombo-prophylaxis and DVT in lower limbs (χ2=0.663, P=0.415). Five patients were with DVT in lower limbs in 53 patients with complete SCI, and four cases in 59 patients with incomplete SCI. No relationship was found between the severity of SCI and DVT in lower limbs in other hospitals (χ2=0.028, P=0.867). Conclusion DVT in lower limbs could be also occurred in patients who accepted thromboprophylaxis. Labo-ratory indexes are inadequate for the prediction and diagnosis specificity of DVT in lower limbs.

Study design Survey of expert opinion, feedback and final consensus. Objective To describe the development and the variables included in the International Spinal Cord Injury (SCI) Spinal Interventions and Surgical Procedures Basic Data set. Setting International working group. Methods A committee of experts was established to select and define data elements. The data set was then disseminated to the appropriate committees and organizations for comments. All suggested revisions were considered and both the International Spinal Cord Society and the American Spinal Injury Association endorsed the final version. Results The data set consists of nine variables: (1) Intervention/ Procedure Date and start time (2) Non-surgical bed rest and external immobilization, (3) Spinal intervention-closed manipulation and/or reduction of spinal elements, (4) Surgical procedure-approach, (5) Date and time of the completion of the intervention or surgical closure; (6) Surgical procedure-open reduction, (7) Surgical procedure-direct decompression of neural elements, and (8 and 9) Surgical procedure- stabilization and fusion (spinal segment number and level). All variables are coded using numbers or characters. Each spinal intervention and procedure is coded (variables 1 through 7) and the spinal segment level is described (variables 8 and 9). Sample clinical cases were developed to illustrate how to complete it. Conclusion The International SCI Spinal Interventions and Surgical Procedures Basic Data Set was developed to facilitate comparisons of spinal interventions and surgical procedures among studies, centers and countries.
10.Therapeutic effects of comprehensive surgery for recurrent patellar instability in adults

Objective To analyze the clinical effect of medial patellofemoral ligament(MPFL)reconstruction combined with the com-prehensive surgery of anteromedial tibial tubercle transfer and distal shift in the treatment of recurrent patellar instability.Methods The clin-ical data of 60 patients(60 affected knees)with recurrent patellar instability who were admitted to the orthopedics department of Mianyang central hospital in Sichuan Province from April 2012 to September 2016 were retrospectively analyzed.All the patients were examined with knee arthroscopy,lower extremity X-ray,CT and MRI,and they were treated with MPFL reconstruction combined with anteromedial tibial tu -bercle transfer and distal shift comprehensive surgery.All patients were followed up for at least 1 year after operation.The physical indexes, imaging examination indexes and evaluation results of knee function were analyzed before operation and at the last follow -up.Results The follow-up of 60 patients showed that the results of fear test were negative,results of patellar tilt test were symmetrical,and the Q angle re-turned to normal.The activities of flexion and extension were significantly improved,and there was no fractures or dislocation of the patella. Imaging examination showed that the patellofemoral joint was well positioned.At the last follow-up,the patellofemoral congruence angle,de-gree of patellar out-shift and lateral patellar angle of the 60 patients were significantly decreased,and the tibial tuberosity-trochlear groove dis-tance(TT-TG)was also significantly decreased,the differences were significant(P<0.05),and they basically returned to the normal range. At the last follow-up,the Lysholm score and Kujala score were significantly higher than those before operation,the differences were significant (P<0.05).Conclusion The clinical effect of MPFL reconstruction combined with anteromedial tibial tubercle transfer and distal shift com -prehensive surgery is satisfying in the treatment of recurrent patellar instability,which has great significance for relieving symptoms and resto-ring the function of the knee.
