2.Doppler hemorrhoid artery ligation combined with lauromacrogal sclerosis injection in treatment of hemorrhoids

ObjectiveTo study the clinical value of minimally invasive treatment of the doppler-guided hemorrhoid artery ligation with lauromacrogal sclerosis injection in treatment of hemorrhoids.MethodsA retrospective analysis was made on treatment of 32 cases with hemorrhoids using doppler-guided hemorrhoid artery ligation combined with lauromacrogal sclerosis injection.ResultsAccording to the branch of the Chinese Medical Association by the Surgery the clinical diagnosis and Treatment Guidelines of Hemorrhoids (draft),the Ⅰ degree hemorrhoid was found in 1 case,Ⅱ degree hemorrhoids in 18 cases,Ⅲ degree hemorrhoids in 13 cases.Recovery was found in 32 cases.The longest follow-up time was 30 weeks.No relapse was found. ConclusionsDoppler-guided hemorrhoid artery ligation combined with lauromacrogal case sclerosis injection is a low aggressive minimally invasive method to treat hemorrhoid and has small trauma,definite effect,less pain,fewer complications,and short course and other advantages,but the longterm outcome needs further observation.

Objective To explore the difference of clinical features and laboratory indexs with different vasovagal responses in(head-up) tilt test(HUT) in children with vasovagal syncope(VVS).Methods Forty consecutive children diagnosed with VVS by HUT were analyzed.The vasovagal responses were divided into vasodepressor,cardioinhibitory and mixed patterns.The distribution and different clinical features and laboratory indexs of different response patterns in the VVS were studied.Results The most common vasovagal response was vasodepressor.There was no significant difference in their clinical features,including the motivation,presymptom,frequency and duration of syncope,baseline heart rate and blood pressure,and serum electrolytes among the children with different vasovagal respon-ses.Conclusion Vosodepressor response is the most common hemodynamic pattern in vasovagal syncope in children.

Objective To study the expression of telomerase and its possible clinical significance in patients with condyloma acuminatum,bowenoid papulosis,Bowen's disease and vulvar squamous cell car- cinoma.Methods The human telomerase reverse transcriptase (hTERT) was detected by immunohisto- chemistry technique in all patients.Histopathological analysis was done in patients with bowenoid papulosis or Bowen's disease.Results There was a significant difference in the expression of hTERT among normal tissue,condyloma acuminatum and squamous carcinoma,and the expression rate increased successively from normal control to squamous carcinoma.In bowenoid papulosis,the expression of bTERT was significantly higher as compared to that in condyloma acuminatum,significantly lower as compared to that in squamous carcinoma,and was as the same as that in Bowen's disease.Histopathologically,the degree of cell atypia was significantly higher in Bowen's disease than that in bowenoid papulosis.Conclusions The expression of telomerase increases successively from normal skin,benign tumors to malignant carcinomas,which sug- gests that the expression of telomerase may play an important role in both cell proliferation and immortaliza- tion.
6.Effects of tranexamic acid on hidden blood loss after total knee arthroplasty in animal models

BACKGROUND:Hidden blood loss leads to severe anemia after orthopedic joint arthroplasty.Blood transfusion is needed.Use of tranexamic acid can significantly decrease perioperative blood loss,reduce blood transfusion rate,however,mechanism of action of tranexamic acid on hidden blood loss and its relevance are not clear.OBJECTIVE:By establishing the rabbit models of total knee prosthesis,this study was designed to observe thevariation of early preoperative blood loss with tranexamic acid,and to study the effects of tranexamic acid on hidden blood loss and its influence on deep venous thrombosis of lower limb.METHODS: We designed the rabbit knee prosthesis ourselves.A total of 20 adult New Zealand rabbits were randomly divided into two groups,each group with 10 rabbits.Knee arthroplasty on the same side was performed.Experimental group received intravenous injection of 0.5 g 100 mL tranexamic acid at 15 minutes before replacement.Control group was given 100 mL sodium chloride at the same time.Blood was colected from al animals through ear vein at 1 day before replacement,3,6,24,48,72 hours and 7 days after replacement.Blood routine examination and blood coagulation function test were conducted.The content of plasma D-Dimer was determined.Simultaneously,the operation time and intraoperative bleeding amount were recorded.After replacement,lower limb venous ultrasonography was performed.RESULTS AND CONCLUSION: No significant difference in body weight,hemoglobin,hematocrit,and D-Dimer levels was detected between the two groups before replacement (P>0.05).Significant differences in operation time and intraoperative dominant blood loss were detectable between the two groups (P<0.05).Significant differences in hemoglobin and hematocrit were detected at 24 hours after replacement (P< 0.05),and these were better in the experimental group than in the control group.No significant difference in D-Dimer was observed between the two groups after replacement (P> 0.05).Operation time and intraoperative dominant blood loss were significantly associated with hemoglobin and hematocrit at 24 hours after replacement (P<0.05).The changes in hemoglobin and hematocrit at 72 hours and 7 days after replacement were not apparently correlated with intraoperative D-Dimer changes.Obvious thrombus was not seen by lower limb venous ultrasonography after replacement.These results indicated that the use of tranexamic acid before total knee arthroplasty apparently reduced the intraoperative dominant blood loss,shortened operation time,and noticeably decreased hidden blood loss at 1 day after replacement,which was caused by the increased fibrinolysis with surgical stress,and did not apparently impact other hidden blood loss induced by other mechanism.Simultaneously,tranexamic acid is safe and reliable,and does not increase the risk of perioperative thrombus.

Free fatty acids generated from the metabolites of fat have been demonstrated to stimulate the neutrophils to produce reactive oxygen species ( ROS) such as hydrogen peroxide and chlorous peroxide .It has shown that free fatty acid is closely related with myocardial, vascular endothelium, liver, lung and other tissue damage.The mechanism of erythrocyte injury is oxidation of mem-brane polyunsaturated fatty acids and cytosolic hemoglobin by ROS .High levels of free fatty acids can increase the formation of active reaction molecular ROS , thus initiating the oxidative stress reaction .Active molecules can oxalate and damage the DNA , protein, lipid and other molecules of RBC .It has revealed the relationship between free fatty acids and erythrocytes injury , which provided a new di-rection in the research of oxidative anemia .This article reviews the advances in the research of this subject in recent years .

Objective To investigate the protective effect of topiramate(TPM) on neuronal apoptosis in rats with acute seizures.Methods We treated the PTZ-induced seizure rats with TPM at 80mg/(kg?d)(high-dose group) and 40mg/(kg?d)(middle-dose group) or physiological saline(control group) for 2 weeks.Neuronal apoptosis in CA_1 and CA_3 regions in hippocampus was identified by terminal deoxynucletidyl transferase-mediated dUTP-biotin in situ nick end labeling(TUNEL) assay.Results Two weeks following seizures,TUNEL-positive neurons were detected in CA_1 and CA_3 regions each group.The numbers of TUNEL-positive neurons in CA_1 and CA_3 of control group were(35.83?)4.58 and(36.83?)3.83,(23.50?)2.81 and(25.50?)3.72 of high-dose TPM group,(31.52?)3.43 and(32.35?)4.69 of middle-dose TPM group.There was a very significant difference between high-dose TPM group and control group(all(P)0.05).Conclusion High dose administration of TPM after experimental status epilepticus may attenuate seizure-induced hippocampal neuronal injury.
10.Study of Kanglaite-induced apoptosis on human pancreatic cancer cells by cDNA microarray

Objective To assess the expression patterns of apoptosis-related genes in human pancreatic cancer cells (Patu-8988)induced by Kanglaite (KLT), a novel anti-cancer botanical product. Methods The apoptosis of Patu-8988 cells was determined by flow cytometry with Annexin V-FITC and propidium iodide staining. A human apoptosis microarray containing 96 cDNA fragments was used to detect gene expressions, and followed by Western blot analysis to confirm changes in expression of selected gene products. Results KLT induced apoptosis of Patu-8988 cells in a time-dependent manner. cDNA microarray analysis identified 5 down-regulated genes and 12 up-regulated genes (more than three fold) in the first 24 h as a consequence of treatment. Western blot performed for P53, Bcl-2, Bax and Caspase-3 were consistent with the microarray results. The enhanced activity of Caspases-3 was evidently verified by the cleavage of the 89?103 form of poly ADP-ribose polymerase. Conclusion KLT could alter the expression profile of apoptosis- related genes in human pancreatic Patu-8988 cancer cells, which provides valuable insight into the tentative signaling pathways mediating the outcome of KLT-induced apoptosis.
