1.Advances in Studies on Serum Biomarkers and Susceptibility Genes in Differential Diagnosis of Inflammatory Bowel Disease

Inflammatory bowel disease (IBD)includes Crohn’s disease (CD)and ulcerative colitis (UC).The differential diagnosis between CD and UC mainly depends on clinical symptoms,endoscopy,pathological biopsy,laboratory and imaging examinations.In recent years,studies with a variety of IBD-related biomarkers develop rapidly because of its non-invasiveness,simple and easily acceptable.With the development of genome-wide association study (GWAS),great progress has been achieved in studies of gene mutations and susceptibility genes related with CD and UC,which provides new approach for diagnosis of the disease.This article reviewed the advances in studies on serum biomarkers and susceptibility genes in differential diagnosis of IBD.

Objective To explore the optimal protocol of lower-extremity contrast-enhanced MRA (CE-MRA) in the evaluation of diabetic foot.Methods Twenty eight healthy volunteers were scanned by CE-MRA in crus twice with parellel imaging factor (PIF) of 3 or 4.The signal-to-noise ratio (SNR),contrast-to-noise ratio (CNR) and image quality of popliteal artery,posterior tibial artery,anterior tibial artery and peroneal artery were compared.Twenty patients with diabetic foot underwent CE-MRA by both of protocol 1 and 2 in leg,crus and foot.Protocol 1 was the traditional Care-bolus protocol and protocol 2 was the optimized K-space center filling delay-time protocol.The difference of two protocols in venous aliasing and in display of femoral artery,popliteal artery,posterior tibial artery,anterior tibial artery,peroneal artery,dorsalis pedis artery,medial plantar artery and lateral plantar artery were compared.The SNR,CNR of two different PIF sequences were compared by paired t test,and the display of artery of crus was compared by Wilcoxon.The display of vessels and venous aliasing of 2 protocols of diabetic foot patients were compared by Wilcoxon.Results In the images of healthy volunteers with PIF of 3,the SNR were 267±84,174±51,147±42;and the CNR were 232 ±83,139±51,108±39 at popliteal artery,posterior tibial artery and peroneal artery.However,in the images with PIF of 4,the SNR were 239±73,157±53,132±35;and CNR were 206±71,124±50,103±33,respectively.Both the SNR and CNR were higher in the former than in the latter(t values were 2.31 to 4.11,P<0.05).There was no significant difference in the vessel display between the different PIF volunteers (P>0.05).In the protocol 1 of patients with diabetic foot,the display of popliteal artery,posterior tibial artery,anterior tibial artery,peroneal artery,dorsalis pedis artery,medial plantar artery and lateral plantar artery,the venous aliasing in crus and foot were 3.40±0.82,2.70±0.80,2.50±1.00,2.20±0.77,2.30±0.92,2.15± 1.04,1.45±0.60,2.20± 1.01,2.20± 1.06.And in the protocol 2,they were 3.85±0.37,3.55± 0.69,3.30±0.92,2.90±0.79,3.30±0.92,3.25±0.79,1.95±1.10,3.70±0.47,3.65±0.49,respectively(P<0.05).All of these parameters of protocol 2 were superior to protocol 1.Conclusion Using a higher PIF properly,setting the personalized K-space center filling delay-time can contribute to improving the image quality of whole lower-extremity MRA in patients with diabetic foot.
4.Value of serum GSTP1 gene quantitative methylation analysis for early diagnosis of hepatocellular carcinoma

Objective To establish a real-time fluorescence quantitative methylation assay to investigate the methylation status of GSH-sulphur-transferase P1(GSTP1) gene promoter region in hepatocellular carcinoma(HCC) and to investigate whether which can be used as the early diagnostic indicator of HCC .Methods Ninety-five serum samples were collected from 40 patients with HCC ,30 patients with liver cirrhosis and 25 individuals with healthy physical examination as controls .The methylation level of GSTP1 gene in these serum samples were quantitatively determined by using the real-time fluorescence quantitative methylated spe-cific PCR technique .The receiver-operation characteristic(ROC) curves were adopted to evaluate its diagnostic value for HCC .Re-sults The methylation quantitative level of GSTP1 gene in HCC serum was significantly higher than that in the healthy controls (P<0 .05) .The ROC curve analysis demonstrated that the methylation quantitative analysis of GSTP1 gene could efficiently distin-guish HCC and cirrhosis from healthy controls (AUC=0 .8641) .With the methylation rate of 2% as the critical value for diagno-sing HCC ,its diagnostic specificity was 87 .5% ,the sensitivity was 69 .6% ;the combination detection of serum GSTP1 gene methy-lation and serum AFP could increase the detection rate of HCC to 75% .Conclusion The real-time fluorescence quantitative methyl-ation assay can accurately quantify the methylation level of serum GSTP1 gene ,which has certain application value for the early di-agnosis of HCC .
5.Stress ulcer survey of anti-ulcerative drug use in patients undergoing arthroscopic operation

Objective To investigate anti-ulcerative drug use in patients undergoing arthroscopic operation against stress ulcer. Methosds A total of 297 patients undergoing arthroscopic operation during Jan. to Aug. 2013 in our hospital were recruited and retrospectively analyzed with Excellspreadsheet. Results Among 297 patients in the study, ninety-four patients (31.6%) used AUD before operation, most (94.9%) administered by intravenous, and drug utilization index was 1.35. Ninety-eight point nine patients used proton pump inhibitors (PPIs) in which Lansoprazole was prescribed most. Conclusion This study highlights the overuse of PPIs in patients undergoing arthroscopic operation.
6.Up-regulation of leptin in adipocytes exposed to high glucose and its effect in peritoneal angiogenesis

Objective By simulating a high-glucose condition of peritoneal dialysis (PD)fluid,to explore the effect of high glucose on the expression of leptin and its relationship with peritoneal angiogenesis.Methods Adipocytes differentiated from 3T3-L1 were divided into high glucose group (139 mmol/l glucose) and high mannitol group.Leptin levels in supernatant collected at 0 h,12 h,24 h and 48 h were measured by ELISA.Endothelial cells (ECs) were respectively cultured with normal glouse,high glucose,high mannitol condition,supernatants of adipocyte induced by normal glouse,high glucose and high mannitol,high glucose supernatants+leptin antibody,and high mannitol supernatants + leptin antibody.Tubular structure formation and migration of ECs were detected.Results Adipocytes exposed to high glucose for 48 h produced more leptin as compared with control group,high mannitol group,12 h-high glucose group and 24 h-high glucose group (all P < 0.05).Compared with ECs in normal group,ECs in high glucose had less tubular structure formation and increased migration (all P < 0.01).Compared with those of ECs in high glucose,the tubular structure formation and the migration of ECs in adipocyte supernatants induced by high glucose had increased (all P < 0.01),and these effects were reduced by leptin antibody (all P < 0.01).Conclusion There is an up-regulation of leptin in adipocytes exposed to high glucose,which may be an alternative way to prevent peritoneal angiogenesis.
7.Ocular manifestations of intraventricular trigonal meningioma and the associated optic radiation injury

Abstract? AIM: To analyze the ocular manifestations of meningiomas in the trigone of the lateral ventricle, discuss the relevant factors of visual impairment in these patients and things need attention clinically.?METHODS:Retrospectively study on the clinical data of 90 eyes in 45 patients diagnosed of trigonal meningiomas treated at Beijing Tian Tan Hospital from October 2011 to October 2015.Preoperative examinations including visual acuity, optic disc findings, visual field, size of tumors and other change in MRI were analyzed.?RESULTS: Patients'age was 12-68 years old ( mean 41.7 ±13.7 years ). Male/female ratio was 1 ∶4.6. Decreased visual acuity occurred in 18 eyes.Optic disc edema was found in 24 eyes and optic disc pale in 6 eyes. Fourty -seven eyes had visual field defect, mostly homonymous hemianopia or defect. The maximum diameter of tumors was 2.1-9.6cm (4.8±1.7cm).Range of tumor volume was 3.02-193.2cm3(48.3±47.8cm3).A positive correlation of preoperative visual field defect was found with tumor volume, tumor maximum diameter, and brain midline shift respectively. While the preoperative visual field defect was not found any correlation with age, gender, course, and the enlargement of the ventricle and the edema of the tissue around the tumors. After Mann-Whitney U test, the differences on tumor volume, the maximum diameter of tumors, and brain midline shift between the two groups were significant.? CONCLUSION: Patients with trigonal meningiomas often have ocular signs and symptoms.The major reason of visual field defect is the damage of optic radiation around the tumor.The specific position of injured optic radiation determines the type and extent of visual field defect.Both ophthalmologist and neurosurgeon should pay attention to tumors nearby posterior visual pathway. Doing neurophthalmology examinations for these patients and realizing the position between the tumor and posterior visual pathway will be helpful and necessary in surgical planning.
8.Prevention and Treatment for Pressure Ulcer (review)

Pressure ulcer is a kind of cutaneous and subcutaneous exelcosis and necrosis as suffered long-term pressure. Pressure ulcer mainly related with age, long-term in bed, chronic diseases, lower resistibility, vasculitis and less activities, as well as long-term pressing, poor blood circulation and untidy body environment. Pressure ulcer can be classified into 4 stages with severity. The risk to pressure ulcer should be evaluated, mainly with Braden score. Prevention of pressure ulcer included keeping clean, avoiding long-term pressure and find-ing potential pressure ulcer as soon as possible, based on improving the system situation and dealing with original diseases. For the patients suffered from pressure ulcer, it was important to decompress, debride the wound, keep humidity, and apply some new dressing with external drugs and vacuum sealing draining (VSD) and good psychological nurse and communication.
9.Electrochemiluminescence Behavior of [ Ru(bpy) 3 ] [4-(Clph) 4 B] 2/N,N,N',N'-Tetrakis(propyl)-pentanediamine Nanopaticles Modified Electrode

A new organic-soluble co-reactant of N,N,N’,N’-tetrakis(propyl)-pentanediamine (TPPD) was synthesized and co-immobilized with tris ( 2, 2’-bipyridine ) ruthenium ditetrakis ( 4-chlorophenyl ) borate ([Ru(bpy)3][4-(Clph)4B]2) on an indium tin oxide (ITO) electrode for electrochemiluminescent (ECL) study. The hydrophobicity of TPPD prevented its leakage from the electrode surface, thus the long-term stability of the ECL electrode was enhanced. The TPPD/[Ru(bpy)3][4-(Clph)4B]2 modified-ITO electrode exhibited strong, stable and reproducible ECL signal. The ECL signal could be quenched efficiently by phenol with a quenching efficiency of 95. 4% in the presence of 0. 1 mmol/L phenol, demonstrating the potential of the modified electrode in determination of phenolic compounds. The transient-state of the electrogenerated chemiluminescence reaction was also investigated, which revealed that the TPPD/[Ru(bpy)3][4-(Clph)4B]2 modified-ITO electrode had a longer lifetime than conventional TPA-Ru(bpy)2+3 co-reactant system.
10.Analysis of Sodium Intake via Drugs Used by Hypertensive Inpatients

OBJECTIVE:To investigate sodium intake via drug used by hypertensive inpatients during hospitalization. METH-ODS:290 inpatients with hypertension were collected from vasculocardiology department of our hospital between Jan. and Oct. 2013. The types of sodium-containing drugs and sodium-intake during hospitalization were analyzed statistically. RESULTS:275 pa-tients with hypertension were prescribed 38 sodium-containing drugs(94.8%);82.7% were administered by iv gtt with a average drug treatment course of 6.7 days(average hospitalization days of 6.8 days). Solvent containing sodium such as 0.9% Sodium chlo-ride injection and Creatine phosphate sodium for injection were the most sodium donors. 43.3% patients’sodium intake exceeded upper limit of 2 g/day recommended by WHO. CONCLUSIONS:Hypertensive inpatients are prescribed too much sodium-contain-ing drugs,and less intravenous solutions and less adjuvant medicines should be took to decrease sodium intake. Only when prospec-tive benefit are greater than risk,the patients can be prescribed sodium-containing drug prescription.At the same time,manufacturers should annotate the content of sodium in package inserts,which is convenient for physicians to choose drugs.
