1.Clinical Study on Simple Obesity Treated with Abdomen Acupuncture

Objective: To observe the clinical effects of simple obesity treated with abdomen acupuncture. Methods: Sixty patients with simple obesity were allocated to abdomen acupuncture group and body acupuncture group based on the randomized and controlled principle, 30 patients in each group. Fifteen treatments made up one course. The cases were treated for 2 courses. The clinical effects were evaluated by body mass index (BMI) and the declining of body height. Result: The change of BMI and clinical effects were of no significant difference between two groups after 1 course, but there were significant differences after two courses. Conclusion: The clinical effect of abdomen acupuncture treating simple obesity is better, and the effect is persistent and stable.
2.Effects of ginsenoside Rg1 on apoptosis in spinal dorsal horn in development of morphine tolerance in rats with arthritis

ObjectiveTo investigate the effects of ginsenoside Rg1 on apoptosis in spinal dorsal horn in development of morphine tolerance in rats with arthritis.MethodsTwenty-four healthy male SD rats weighing 280-320 g in which intrathecal(IT) catheters were succcessfully implanted without complication were randomized into 4 groups ( n =6 each):group normal saline ( group C) ; group morphine ( group M ) ; group ginsenoside Rg1 (group G) and group morphine + ginsenoside Rg1 (group MG).Arthritis was induced with complete Freund adjuivant injected into the ankle joint of right hind limb according to the method described by Butler et al in all 24 animals.Chronic morphine tolerance was induced by IT morphine 10 μg twice a day for 7 consecutive days in groups M and MG.Ginsenoside Rg1 100 μg was given IT once a day for 7 consecutive days in groups G and MG.Paw withdrawal threshold to mechanical stimulation with yon Frey filaments (MWT) was measured before induction of arthritis (T1,baseline),and IT drug administration (T2) and at day 1,3,5,7 (T3-6) after IT administration.The rats were sacrificed after last MWT measurement and their lumbar segments of the spinal cord ( L3-5 ) were removed for detection of apoptosis in the spinal dorsal horn by TUNEL.ResultsMWT was significantly decreased after induction of arthritis at T2-6 compared with the baseline value before arthritis at T1 in all 4 groups.In group M IT morphine significantly increased MWT at T3.4 compared with the baseline at T2 but the MWT was decreasing at T5.6 indicative of development of morphine tolerance.In group MG,addition of ginsenoside Rgl to IT morphine significantly attenuate the decrease in MWT at T5.6.The number of apoptotic cells in spinal dorsal horn was significantly higher in groups M and MG than in group C,but was significantly lower in group MG than in group M.Concluson Ginsenoside Rg1 can prevent the development of morphine tolerance in rats with arthritis by inhibiting apoptosis in spinal dorsal horn.

Recent studies have revealed that tumor suppressor PTEN does not only closely relate to the development and differentiation of brain but also get extensively involved into the pathophysiological processes of ischemic brain injury.Through research on the mechanisms of molecular regulation mediated by PTEN in ischemic brain injury will provide new perspectives for the treatment of ischemic brain injury.
5.IMPROVING BIOAVAILABILITY OF NAFTOPIDIL BY USING BIOADHESION IN DOGS

AIM To prepare naftopidil bioadhesive sustained-release capsule and study their pharmacokinetics and relative bioavailability in the dog. METHODS Bioadhesive polymers such as hydroxypropyl methylcellulse (HPMC) and Carbopol 934 (CP 934) were used in capsule prescriptions. Naftopidil capsule and two formulations of bioadhesive sustained-release capsules (I and II) were given to five healthy male dogs in a cross-over test. The naftopidil concentrations in plasma were determined by a newly developed HPLC method and the pharmacokinetic parameters as well as the relative bioavailability were measured. RESULTS The C0→∞, Cmax and Tmax of naftopidil capsule was (3728±573) h*ng*mL-1, (697±94) ng*mL-1 and (1.2±0.5) h. These parameters of bioadhesive sustained-release capsule I and II, respectively, were (5518±391) h*ng*mL-1 and (5636±427) h*ng*mL-1; (468±61) ng*mL-1 and (512±72) ng*mL-1; both (4.0±0.7) h. Results from statistics showed that there were significant difference between bioadhesive formulations and the non-bioadhesive one in C0→∞, Cmax and Tmax. The bioadhesive formulations and the non-bioadhesive one were not bioequivalent, the relative bioavailability of the two bioadhesive sustained-release capsules were respectively 150%±14% and 154%±23% when compared with the non-bioadhesive capsule. CONCLUSION It is much improving bioavailability of naftopidil by using bioadhesion.
6.Mesangial cells proliferation induced by high glucose is regulated by macrophage cytokine resistin via activating p38MAPK signal pathway

Objective To investigate the effects of resistin on mesangial cells proliferation induced by high glucose and subsequent change of p38MAPK signal pathway. Methods Human macrophrages were cultured and treated with adenovirus encoding for resistin (Ad-resistin) for 48 h and were then co-cultured with human mesangial cells stimulated by high glucose for another 48 h. Mesangial ceils were harvested and their proliferation was measured by 3H-TdR. Activator protein 1 (AP-1) was examined by immunocytochemistry and laminin of excellular matrix was observed with immuofluorescence. Protein levels of p38MAPK and TGF-β1 were measured by Western blot. Smad2 phosphatase activity was aslo detected by Western blot. Results The mRNA and protein levels of resistin were significantly higher in Ad-resistin treated macrophages than those in Ad treated cells (P<0.01). Over-expression of resistin up-regulated p38MAPK protein levels of human mesangial cells(P<0.05). Resistin also promoted the proliferation of mesangial cells (P<0.01) and the synthesis of laminin stimulated by high glucose. The expression of TGF-β1 and phosphorylation of Smad2 were up-regulated in the mesangial cells (P< 0.05). Conclusion Macrophage cytokine resistin may promote mesangial cells proliferation and abnormal accumulation of excellular matrix stimulated by high glucose via activating p38MAPK signal passway.
7.Protection System Against and Its Effect Occupational Exposure to Hepatitis B Virus

OBJECTIVE To understand the condition of occupational exposure in hospital,and analyze the tendency during last four years.METHODS Occupational exposure in medical personnel were investivated through questionnaireand reviwed the survey report of quantitative detection of the five serological markers of hepatitis B,in the workers exposed in HBV.RESULTS The rate of occupational exposure had declined year by year during 2005-2008,but the result was opposite for HBV.CONCLUSIONS It′s very effective to decrease occupational exposure by strenghening standardization and putting professional protective knowledge in routine work.After setting up and developing protective system against occupational exposure,it′s more prompt in updating information,protective treatment and follow-up test in serum.It shows that this system is helpful to normalize medical personnel action of occupational exposure.In fact,vaccination is an effective way to avoid medical personnel of infecting HBV.
8.The effect of ultrasound in the diagnosis and treatment of rheumatoid arthritis

Objective To explore the effect of ultrasound in the diagnosis and treatment of rheumatoid arthritis .Methods though early diagnose of RA by ultrasound ,DAS28 and MRI′s result to measure the accuracy of ultrasound test .then though the ul‐trasound index at different time point in the treatment of RA patients ,we knew the changes of the joint ,and provide treatment plan and prognosis .Results ultrasound had high accuracy rate in the diagnoses of RA(P< 0 .05) ,and there were correlation among ul‐trasound diagnoses accuracy and DAS28 and MRI score(r= 0 .859 ,P< 0 .05) ;at the same time ,it provided accurate changes of the joint during the treatment ,and provided basis for treatment .Conclusion Ultrasound has manifest advantageous in diagnose RA .It could be used as a method in early diagnose RA and evaluate the effect on RA′s treatment .
9.Comparative Study of Conventional Vascular Ultrasound and Superb Micro Vascular Imaging for Diagnosing Carotid Artery Stenosis in Relevant Patients

Objective: To assess the values of conventional vascular ultrasound (US) and superb micro vascular imaging (SMI) for diagnosing carotid artery stenosis in relevant patients. Methods: A total of 37 patients of extra cranial carotid stenosis (with 70 blood vessels) treated in our hospital from 2014-08 to 2015-03 were retrospectively studied. Digital subtraction angiography (DAS) examination was used as golden standard, the diagnostic efifcacies for carotid artery stenosis by US and by SMI were compared. Results: The accuracy, sensitivity and specificity for diagnosing carotid stenosis by US were 81.42%, 83.33% and 80.95%; by SMI were 91.43%, 92.16% and 89.47% respectively. Conclusion: US and SMI showed good agreement for diagnosing carotid artery stenosis, while SMI was superior to US for accurately assess the degree of carotid stenosis, it might be used as a more reliable method for evaluating carotid plaque and stenosis in relevant patients.
10.Applying a discriminant model based on combination of pediatric critical illness score,D - dimer and procalci-tonin in early predicting the severity of pediatric infection

Objective To establish a discriminant method based on clinical and laboratory data and common examinations for early predicting the severity of pediatric infection. Methods Consecutive hospitalized patients diag-nosed as septic shock were included who were admitted between June 2014 and May 2015 retrospectively. Gender (male - female ratio:1. 25∶ 1. 00)and age(1 month to 6 years old)were matched in all of 18 patients with septic shock,and 27 patients diagnosed as systemic inflammatory response syndrome(SIRS),sepsis and severe sepsis on ad-mission were included respectively in order of sequential admission number during the same period. Additional 36 gen-der - and age - matched children with common infection(non - SIRS)were enrolled as controls. The clinical and labo-ratory examination data of all the included patients were collected and then the pediatric critical illness scores(PCIS) were made according to the worst condition within 24 hours of hospitalization. The parameters correlated with the severi-ty of infection were evaluated by rank correlation and Logistic regression analysis. The discriminant models were estab-lished based on κth - nearest - neighbor analysis and evaluated with clinical diagnosis by interrater agreement test. Results Except for platelet count,the other indexes including PCIS,neutrophil count,C - reactive protein,procalcito-nin(PCT),international normalized ratio of prothrombin time,activated partial thromboplastin time,thrombin time,fi-brinogen,fibrin/ fibrinogen degradation product(FDP)and D - dimer(D - D)all had differences among groups with varying infection severity(all P ﹤ 0. 001). The Spearman's coefficient ρ of PCIS,PCT,D - D and FDP correlated to in-fection severity were - 0. 837,0. 680,0. 679 and 0. 648,respectively(all P ﹤ 0. 001). Multivariate cumulative odds Lo-gistic regression analysis showed PCIS,D - D and PCT were related to infection severity(all P ﹤ 0. 05). The total error rate of discriminant models based on 3 - index combination(Mahalanobis transformation,k = 2)was 0. 091 that was lower than any models based on 2 - index combination or single - index. Using the discriminant model based on three -index combination,the infection severity of 26 patients admitted during June 2015 were predicted with a high interrater a-greement(weighted Kappa coefficient:0. 670,P ﹤ 0. 001)compared to clinical diagnosis. Conclusion The discriminant model based on combination of PCIS,D - D and PCT could assist predicting the severity of pediatric infection earlier.
