2.The value of transcutaneous oxygen saturation in early diagnosis of newborns with critical congenital heart disease

Objective To study the value and significance of transcutaneous oxygen saturation monitoring from different parts in early diagnosis of neonatal critical congenital heart disease. Methods The transcutaneous oxygen saturation in right upper extremities and right lower extremities of neonates born in our hospital from July 2010 to July 2015 were detected at 2 hours to 24 hours after birth. If the transcutaneous oxygen saturation ≤ 0. 95 or difference between upper and lower extremities≥0. 03, color doppler ultrasound echocardiography were done. Results There were 24 385 cases of newborns in our study. 1 680 newborns with abnormal transcutaneous oxygen saturation ( ≤0. 95 or difference between upper and lower extremities ≥0. 03) were examined with color doppler ultrasound echocardiography. 204 cases of congenital heart disease (12. 1% ) contain 9 cases of critical congenital heart disease ( 0. 5% ) were detected. The critical congenital heart disease included complete transposition of great artery ( n = 3), tetralogy of Fallot ( n = 4), artery malformation ( n = 1), and pulmonary atresia with right ventricular dysplasia ( n = 1 ) . Conclusions Transcutaneous oxygen saturation monitoring from different parts has a high value for early detection of critical congenital heart disease in the newborn. It is recommended for clinical use in grass-roots hospitals.
3.Effects of Exogenous Methionine on Arsenical Distribution in Female Mice Exposed to Sodium Arsenite through Drinking Water

Objective To explore the effects of exogenous methionine on arsenical distribution in liver,blood and brain of mice exposed to sodium arsenite through drinking water. Methods The female Kunming mice were randomly divided into control group, the alone arsenic exposure group , the low level methionine intervention group, the moderate level methionine intervention group and the high level methionine intervention group, eight mice in each group . The mice in the experimental groups were exposed to sodium arsenite through drinking water at 50 mg/L arsenic for four consecutive weeks. And at the fourth week the 5 groups were treated intraperitoneally with saline solution (control and As group),100 mg/kg b.w,200 mg/kg b.w or 400 mg/kg b.w methionine,respectively . Twenty-four hours after cessation of methionine administration,mice were anaesthetized and rapidly dissected. The samples of blood,liver and brain were removed immediately for arsenic species analysis. Levels of inorganic arsenic (iAs),monomethylarsonic acid (MMA) and dimethylarsenic acid (DMA) were determined by HG-AAS method. Then total arsenic speciation( TAs), primary methylation ratio( PMR)and secondary methylation ratio( SMR)in each tissue were calculated. The informatization of traditional Chinese medicine resources is the basis of modern medicine. With a spatial attribute traditional Chinese medicine resources could be carried out for in-depth spatial analysis, data mining and traditional Chinese medicine resources regional industrial layout. In this paper, we took the data of Glycyrrhiza uralensis in the third national Chinese medicine resources survey as the experimental data, described the principles and structure of traditional Chinese medicine resources spatial information database. We also described the establishment of analysis model with the help of this spatial database.

Objective To explore the clinical significance of urinary leukotriene E4(LTE4) in children with respiratory syncytial virus(RSV) bronchiolitis.Methods Forty infants with bronchiolitis were enrolled for the identification of RSV-Ag from the nasopharyngeal secretions.Forty cases who were found to be positive of RSV were put into RSV bronchiolitis group.According to the severity of illness,the infants in RSV bronchiolitis group were separated into 2 subgroups:mild illness group (24 cases) and moderate illness group (16 cases).Thirty healthy children were chosen as healthy control group.Urine of the patients in RSV bronchiolitis group was collected in acute phase (on the 1st day) and recovery phase (on the 7th day after the admission).Urine of children in control group was collected .Enzyme-linked immunosorbent assay was used to assay the level of urinary LTE4.Results The levels of LTE4 in healthy control group (M,Q) were 1.28 pmol?mmol-1,1.72 pmol?mmol-1,while the levels of LTE4 in acute phase in RSV bronchiolitis group (M,Q were 30.52 pmol?mmol-1,23.56 pmol?mmol-1,and the levels of LTE4 in recovery phase in RSV bronchiolitis group (M,Q) were 3.98 pmol?mmol-1,3.43 pmol?mmol-1.Urinary LTE4 levels in acute phase in RSV bronchiolitis group were significantly higher than those in healthy control group (P

Objective To investigate the effect ofglycesylated hemoglobin(GHbA,c) on tissue-type plasminngen activator (t-PA)and level of serum plasminogen activator inhibitor-1 (PAI-1) in patients with early diabetic nephropathy (DN).MethodNinety patients with early DN from April 2004 to May 2005 were divided into 3 groups according to the level of GHbA1c,which was respectively less than 7% (group A),between 7% and 9% (group B),and more than 9% (greup C).Thirty healthy adults were chosen as control group.The levels of serum GHbA1c,t-PA and PAI-1 were detected on empty stomach in the morning.Results The level of serum t-PA was lower,the activity of PAI-1 was higher in groups of DN than those in control group (P<0.05 or<0.01),and those were changed with the level of GHbA,c.There were significant differences between group C and group A[t-PA:(0.14± 0.06),(0.28± 0.11) U/ml; PAI-1 (3.25 ±1.01),(1.90q± 1.09) U/ml](P<0.05).Conclusions The fibrinolytic system exists unbalance in patients with early DN.Continuous hyperglycemia in patients with early DN make unbalanced fibrinolytic system more serious.Controlling the level of GHbA1c strictly can play an important role in delaying DN progress.
7.Expression and significance of estrogen receptor alpha in renal tissue of IgA nephropathy

Objective To investigate the expression and significance of estrogen receptor alpha in renal tissue of immunoglobulin A (IgA) nephropathy.Methods Fifty renal tissue samples including forty five cases of IgA nephropathy and five cases of normal expression were collected.The expression of estrogen receptor αt (ERα) was detected by immunohistochemistry method,and its relationship with clinical parameters and the degree of glomerular damage were analyzed.Results ERα was located in the glomerular and renal tubules.With the severity of the lesion,the expression of ERα in renal tissue of IgA nephropathy decreased gradually (P =0.001) and its expression was associated with estimated glomerular filtration rate (eGFR),serum creatinine (Scr),and pathologic grade of IgA nephropathy (r =0.876,-0.818,and -0.736,P < 0.05),The expression of ERα was significantly decreased in hypertensive patients with IgA nephropathy,and the difference was statistically significant compared with that in non-hypertensive group (P =0.011).Conclusions The expression of ERα in renal tissue of IgA nephropathy was significantly decreased and there was a correlation between the degree of renal tissue damage,suggesting that ERα might play a role in the development of renal disease.
8.Surgical treatment for pulmonary carcinoid tumors: a single-center analysis of 62 patients

ObjectiveTo evaluate prognostic factors and the surgical results of pulmonary carcinoid tumors.Methods We retrospectively reviewed the medical records of 62 patients who were diagnosed as pulmonary carcinoid tumors between January 2000 and October 2010 at Department of Thoracic Surgery,Shanghai Chest Hospital.The following information was available for each of the 62 patients:age,sex,pathological type,and TNM stage.ResultsThere were no operative death.The 3-year and 5-year survival rates after surgery were 92.1% and 77.8%,respectively.Of the 62 patients,42 were diagnosed as typical carcinoid tumor,and among them,4 patients (8.3%) had lymph node metastases.Their 3-year and 5-year survival rates were 97.8% and 94.7%,respectively.The remaining 20 patients were diagnosed as atypical carcinoid tumor,and among them,6 patients (37.5%) had lymph node metastases.Their 3-year and 5-year survival rates were 84.4% and 58.8%,which were statistically significant compared with typical carcinoid tumor( P =0.0047 ).There was significant difference in survival rate between the patients with lymph node metastases and the patients without lymph node metastases (P =0.0048).CondusionThe main risk factors affecting survival rate of those patients who were diagnosed as pulmonary carcinoid tumors were pathological types and lymph node metastases.

10.Effects of mistral-air inflatable blanket on neonatal blood coagulation function after surgery for general anesthesia

Objective To evaluate the value of mistral-air inflatable blanket in neonatal temperature,postoperative complications,and the influence of the blood coagulation function after surgery undergoing general anesthesia.Methods A total of 60 newborns with laparotomy operation undergoing general anesthesia were randomly divided into two groups,the control group and the observation group,30 persons each group.The newborns were given mistral-air inflatable blanket in the observation group,and conventional methods in the control group,two groups were transferred to Postanesthesia Care Unit by incubator at 37℃.The introperative nasopharyngeal temprature,HR,MAP and SpO2 were recorded before induction (T0),30 minutes during operation (T1),extubated instantly (T2),15 minutes during PACU (T3),30 min during PACU (T4).The extubation time and the incidence of apnoea and hyoxemia were recorded after operation.PT,APTT,TT and Fib were tested before induction and extubated instantly.Results The temperature of intraoperative and postoperative period were higher in the observation group than the control group,the difference was significant(P < 0.05).The heart rates of intraoperative and postoperative period is significantly higher in the control group than the observation group,the difference was significant(P < 0.05);the mean arterial pressure were no significant statistical difference(P > 0.05).The observation group extubation time were less than the control group,the difference was significant (P < 0.05).There was no significant difference about the incidence of hypoxemia and apnea in two groups(P > 0.05).The PT,APTT,TT and Fib in the observation group were better than the control group,the difference between the groups was significant(P < 0.05).Conclusion Mistral-air inflatable blanket can effectively prevent the decrease of the body temperature during operation and PACU,reduce the exduction time and postoperative complications,and protect blood coagulation function.
