Crohn's disease is one of the main types of inflammatory bowel disease,some patients with Crohn's disease may be beyond the control of the medical complications,finally had to accept surgical treatment.This paper reviews the Crohng disease patients of surgical indications,gastrointestinal complications related to the classification,risk factors,prevention and treatment countermeasure research,designed for Crohn's disease indications for surgery decision and postoperative gastrointestinal complications in the diagnosis,perioperative treatment and prevention to provide some guidance and help.
2.Nucleic acid aptamers and their applications in the diagnosis and treatment of tumor

Nucleic acid aptamers are ligands with high specificity and affinity for their targets, which are screened from large oligonucleotide pools by the SELEX technology. Their characteristics are superior to antibodies in many aspects, which make them important in the research of recognition of molecules. They can be used to determine the levels and inhibit the bioactivity of their targets. There're broad application prospects for aptamers in the diagnosis and treatment of tumor.
3.Methylation of RASSF2A gene promoter in cervical cancer

Objective To investigate the methylation status of the promoter of RASSF2A gene in the tissues of cervical cancer and its impacts in tumorigenesis.Methods Methylation-specific PCR (MSP) and RT-PCR techniques were applied to detect methylation status of RASSF2A gene promoter region and RASSF2A expression in tissues of 62 cervical cancer and 30 normal cervical specimens,respectively.Results Methylation frequency of RASSF2A gene in cancer tissue was higher than that in normal tissue (53.2 % vs 13.3 %,P < 0.05).The mRNA expression of RASSF2A in cancer tissue were less than those in normal tissue significantly (0.254 3 ±0.102 7 vs 0.623 4 ± 0.063 8,t =0.932,P < 0.05).The mRNA expressed of RASSF2A gene was lower in methylated promoter group than that in unmethylated promoter group in cancer tissue samples significantly (0.118 2 ± 0.035 2 vs 0.301 2 ± 0.076 4,t =0.481,P < 0.05).Conclusion Hypermethylation of RASSF2A gene in promoter region,which lead to the loss of RASSF2A gene expression,might play an important role in tumorigenesis of cervical cancer.
4.Clinical research on radiofrequency ablation combined with splenectomy in 53 patients with small hepatocellular carcinoma associated with hypersplenism

Objective To explore the efficacy of radiofrequency ablation (RFA) combined with splenectomy in patients with small hepatocellular carcinoma (sHCC) associated with hypersplenism.Metheds The data of 100 patients with sHCC associated with hypersplenism who received RFA or hepatectomy combined with splenectomy were analyzed retrospectively.The patients were divided into the observation group and the control group based on the intraoperative approach.Fifty-three patients who received RFA and splenectomy were in the observation group,and the remaining 47 patients who received hepatectomy and splenectomy were in the control group.Multiple intraoperative and postoperative factors were compared between the two groups.Results There were significant differences between the two groups in warm ischemia time,operation time,intraoperative blood loss,hospital stay,and amount of blood transfusion (P < 0.05).The postoperative complication rate of the observation group (7.6%,4/53) was significantly lower than the control group (44.7 %,21/47) (P < 0.05).There were no significant differences in 1-,3-,and 5-year survival (respectively,100.0%,75.5%,and 67.9% vs 97.9%,76.6%,and 68.1%) and in disease free survival (96.2%,57.5%,and 41.7% vs 93.5%,58.3%,and 43.8% respectively) between the two groups (P > 0.05).Conclusion RFA combined with splenectomy can be considered as an alternative treatment for patients with sHCC associated with hypersplenism.
7.A contrast on pathology and imaging in senile dementia of the Alzheimer type

Objective To explore the correlation of pathological changes and imaging through a control study on pathology and imaging of 4 cases of senile dementia of the Alzheimer type (SDAT).Methods By HE and Gallyas silver dyeing,four immunohistochemical methods were carried out The loss of neurons cells of all lobes of cerebral cortex,cingulate gyrus,amygdaloid nucleus,hippocampal gyrus,parahippocampal gyrus,meynert basal nucleus,substantia nigra,locus ceruleus,dorsal nucleus of vagus nerve,senile plaques (SP) and the changes of neurofibrillary tangles (NFT) were observed in detail The control study of pathology and imaging was carried out by comparison of CT in different phases before and after death of the patient.Results The degeneration of temporal lobe was marked: (1) The degeneration of middle frontal gyrus of cortex of frontal lobe was the most evident; (2) The degeneration of long gyrus was more evident than that of the short gyrus as the cortex of insular lobe was concerned; (3) The degeneration posterior part of cingulated gyrus was more evident than that of the anterior part.Conclusion The pathological findings of SDAT mentioned above are basically in conformity with the changes of imaging.
8.Effect of acupuncture on the ultramicrostructure of the injured lumbar nerve and inflammatory medium of rats

BACKGROUND: Lumbar spinal nerve root compression causes the damage of neural function and abnormal neural action of lumbar spinal nerve root, which is related not only to mechanical injury of the compressed but also to inflammatory reaction.OBJECTIVE: To establish the animal model of lumbar nerve root compression and investigate the effect of acupuncture on the ultramicrostructure of the injured lumbar nerve and inflammatory medium of rats.DESIGN: A randomized controlled trial.SETTING: Department of Acupuncture & Moxibustion, Shanghai Sixth People's Hospital Affiliated to Shanghai Jiaotong University.MATERIALS: The study was conducted in the Experimental Animal Center of Shanghai Sixth People's Hospital Affiliated to Shanghai Jiaotong University from January 2001 to January 2004. Totally 90 healthy male Wistar rats were randomly divided into model group and model + electroacupuncture group with 45 rats in each.METHODS: Clominorex was injected intraperitoneally for anesthesia.With interspace of spinous process of L5-6 as the center, the special silica gel slice was placed where the right side of L5 nerve root ran out of the dura mater sac, stabilized, sutured layer by layer, and covered with the aseptic gauze. Modeling group: No special disposal was made and the rats were fed regularly after modeling. Modeling + electro-acupuncture group:One day after modeling, a filiform needle of 28 gauge and 3.3 cm (1 inch)was used to puncture Huantiao and Yanglingquan points of the right hindlimb and Jiaji point of the right side of the oppressed nerve root. The oppressed injured lumbar nerve tissue was disposed after 15 days of modeling and 30 daysof modeling + electro-acupuncture, and observed with the optic microscope and electron microscope. The content of inflammatory medium such as prostaglandin E2 and5-hydroxy tryptamine was measured.MAIN OUTCOME MEASURES: [1] Results of damaged lumbar nerve root tissues under the electron microscope and light microscope; [2] content of inflammatory medium such as prostaglandin E2 and 5-hydroxy tryptamine.RESULTS: All the 90 rats entered the final analysis. [1] Hyperemia around the nervous tissues, edema and nerve root demyelination of electroacupuncture 15 days (model + electro-acupuncture group) were relieved.The structure of Schwann cells was normal. Massive inflammatory cells gathered around the nervous tissues in 30-day model group (model group).There was severe demyelination of nerve root tissues, and the gap between the nervous tissues widened and red blood cells leaked out. Demyelination of electro-acupuncture 30-day group (model + electro-acupuncture group)became mild and the edema in nerves was alleviated. [2] The content of prostaglandin E2 in the locally oppressed rat lumbar nerve tissues of 15-day and 30-day model + electro-acupuncture groups was obviously lower than that in 15-day and 30-day model groups [(61.17±9.77), (76.65±7.39) ng/L;(56.05±7.98), (71.04±6.99) ng/L, P < 0.01], while the content of 5-hydroxy tryptamine in the locally oppressed rat lumbar nerve tissues of 15-day and 30-day model + electro-acupuncture groups was obviously lower than that in 15-day and 30-day model groups [(1 413.4±194.5), (2 587.7±214.7) ng/L;(780.4±133.0), (1 355.0±147.7) ng/L, P < 0.01].CONCLUSION: Electro-acupuncture can alleviate inflammatory reaction around the injured lumbar nerves and improve the ultramicrostructure of lumbar nerve root. It has the effect of decreasing the content of prostaglandin E2 and 5-hydroxy tryptamine in oppressed lumbar nerve root tissues and partially inhibiting inflammatory reaction.
9.EFFECTS OF GENISTEIN ON TUMOR-ASSOCIATED GENE EXPRESSION IN MCF-7 CELLS

Objective: To characterize the effects of genistein (GS) on tumor-associated gene expression in MCF-7 cells and to explore the mechanisms. Method: MCF-7 cells were treated with GS (75?10-6 mol/L) or with vehicle (0.1% ethanol, EtOH) for 72 h. The cells were collected and total RNA was extracted, marked by two different fluorescence dyes (Cy3 and Cy5) using reverse transcriptional reaction, respectively. The derived cRNA was hybridized to human tumor-associated microarrays. Data were processed using the Scan Array 3000 and ontological analysis was performed using the Ima Gene 3.0 software. Results: At 72 h, 18 transcripts were downregulated compared to EtOH vehicle control by greater than 2- fold and 4 were upregulated by less than 0.5-fold. The predominant ontological groupings were oncogenes, cell proliferation, apoptosis, and estrogen-regulation genes. Conclusion: GS treatment is associated with significant changes in gene expression in several functional categories in MCF-7 cells, and this might account for the role of genistein in prevention of breast cancer.
10.THE MECHANISMS OF PROLIFERATION AND APOPTOSIS IN MCF-7 CELLS BY DIETARY ESTROGENS

Objective: This study was designed to investigate the molecular mechanisms of proliferation and apoptosis by genistein and zearalenone through regulation of mRNA and protein expression of PCNA, bcl-2 and bax in breast cancer MCF-7 cells. Methods: The cells were maintained in DMEM medium with 10% fetal bovine serum. Five days before the beginning of experiments, the cells were seeded in phenol red-free DMEM medium containing 5% charcoal dextran–treated FBS. The cells were harvested and seeded in 6-well culture plates or in 75 ml flacks. After various concentrations of genistein and zearalenone treatments for 72 h, the cells were harvested and mRNA and protein expression of proliferation cell nuclear antigen(PCNA), bcl-2 and bax were detected by reverse transcription-polymerase chain reaction and immunohistochemistry, respectively. Results: At the concentration of 75 ?mol/L, GS could significantly down-regulate bcl-2 and PCNA mRNA expression and up-regulate bax mRNA expression, and zearalenone indicated an opposite result. These results were further confirmed by following immunohistochemistry. Conclusion:PCNA, bcl-2 and bax pathway might be involved in cell proliferation and apoptosis events regulated by dietary estrogens genistein and zearalenone in breast cancer MCF-7 cells.
