1.Effect of irbesartan hydrochlorothiazide combined with candesartan cilexetil in the treatment of elderly patients with hypertension and its protective effect on cardiac function

Objective To investigate the effects of irbesartan hydrochlorothiazide combined with candesartan cilexetil in the treatment of elderly patients with hypertension and its protective effect on cardiac function.Methods 180 elderly patients with hypertension were selected,and they were randomly divided into observation group and control group according to the digital table,90 cases in each group.The observation group was given irbesartan hydrochlorothiazide combined with candesartan treatment,the control group was given candesartan.The heart function improvement after treatment,clinical efficacy and patients in hospitalization,complications,critical events and new death of the two groups were compared.Results After treatment,the heart rate,EVDD,SBP and other indicators of the two groups declined,which of the observation group[(56.14 ± 6.15)mm,(112.12 ± 20.12)mmHg,(70.45 ± 8.69/min)]were lower than the control group[(60.12 ± 6.78) mm(119.45 ± 21.45) mmHg,(82.12 ± 9.12/ min)],there were significant differences between the two groups (t1 =4.124,P =0.000;t2 =2.364,P =0.019;t3 =8.788,P =0.000).The total effective rate of the observation group was 93.33%,which was higher than 74.44% of the control group,there was significant difference between the two groups (x2 =11.879,P =0.000).Conclusion Irbesartan hydrochlorothiazide combined with candesartan in the treatment of elderly hypertensive patients can effectively improve the heart function of patients,has significant effect.
2.Evaluation on the effect of Ubenimex tablets on the treatment of patients with triple-negative breast cancer undergoing chemotherapy

Objective To evaluate the immune function and the degree of bone marrow suppression of ubenimex tablets on the treatment of patients with triple-negative breast cancer underwent chemotherapy.Methods108 cases in Jiaxing hospital of traditional Chinese medicine were randomly divided into the control group and the experimental group, 54 cases in each group.The two group were all treated with AC-T chemotherapy (pirarubicin+ cyclophosphamide + docetaxel).At this basis, the experimental group were given ubenimex.Flow cytometry was used to detect the changes of T lymphocyte subsets, natural killer cell (NK cell) immune function and peripheral blood cell parameters in the two groups before and after the treatment and 6 months after the treatment.ResultsCompared to the control group, the T lymphocyte subsets and NK cell changes in the experimental group before and after chemotherapy were significantly smaller (P<0.05).The T lymphocyte subsets and NK cells in the experimental group were more accurate than those in the control group (P<0.05).The peripheral blood cells in the experimental group were significantly smaller than those in the control group (P<0.05).Compared with the control group, the whole blood cells increased significantly in the experimental group 6 months after chemotherapy(P<0.05).ConclusionIt can help to reduce the effect of chemotherapeutic drugs on the immune function and bone marrow suppression which ubenimex tablets was used on the treatment patients with triple-negative breast cancer c.Patients can be successfully complete the chemotherapy process, and the long-term and short-term adverse reactions were less, prognosis is good, clinical promotion value is significant.
4.Effect of low-level 84 disinfection solution on reproductive physiology of mice

Objective To study the effect of low-level 84 disinfection solution on reproductive physiology of mice . Methods 72 KM mice were randomly divided into three groups :0.5%84 disinfection solution group ( group A) , 2.0%84 disinfection solution group (group B) and saline solution group (group C).12 males and 12 females in each group. Corresponding medicine was given once a day by intragastric gavage for 9 weeks.The estrous cycle of female mice was observed by exfoliative cytology of vagina .After 9 weeks, The sperm abnormality test was performed , Serum samples were taken for sex hormone test by ELISA, and the histological change of testis (or ovary), heart, liver, lung and kidney were observed.Results Compared with group C , No significant difference was observed in the weight variation and serum sex hormone level in group A and B (P >0.05).The difference of the estrous cycle of female mice was not significant (P >0.05).No histopathological abnormality was observed .Compared with group C, There was no significant difference in the rate of sperm abnormality in group A ( P >0.05 ) , but the rate in group B was significantly increased ( P <0.05 ) . Conclusion 0.5% 84 disinfection solution has no significant effect on reproductive physiology of mice , The concentration of disinfection solution must be strictly controlled when used .
5.Current Situation of TCM Human Resources Distribution and Equity Research in China

Objective To discuss existing problems in talent team construction; To provide evidence for the government to enact proper strategies based on the analysis of the current situation of TCM practitioners (assistants) distribution in China from quantity, quality and distribution equity. Methods The research is studied through conducting a descriptive analysis of TCM practitioners (assistants) from quantity, education, job titles, ages, and other basic conditions and changes. Gini coefficient was used to evaluate the distribution of TCM practitioners (assistants). Results The quantity of TCM practitioners (assistants) increased by 53.7% from 2010 to 2015. Concerning the quality of personnel, the ratio of the personnel with bachelors degree or above increased from 38.6% to 44.2%, and a slight increase was observed with the personnel with senior title. In terms of age structure, the proportion of the age of TCM practitioners (assistants) under 25 years old, 35 to 59 years old decreased, and the proportion of the age of TCM practitioners (assistants) above 60 years old, 25 to 34 years old increased. From the perspective of equity, there is a difference between the provincial-level administration of human resources distribution. The Gini coefficient as per population distribution was around 0.1, which indicated good equity. From the geographical area configuration, the Gini coefficient was approximately 0.58, which showed high level of disparity. Conclusion There are problems of insufficient quantity, low quality and distribution disparity among regions in TCM practitioners (assistant) in China.

Based on the investigation of a hospital's grants during 2004-2005 and the authorized during 2002-2005 (385 in total), we tried to find the trends and bias of clinical sciences at present. We analysed on the types of grants (clinical or basic), research objects, experimental methods, discipline involved, modes of cooperation, and background of applicants. A close connection between research and clinical trial, interdisciplinary study, patient-centered practice and international competitions are becoming more and more important. To meet the needs of clinical research, policies and devotions such as technical platforms are needed.

8.Role of PI3K/Akt Signaling Pathway in the TCP Wear Particles-induced Calvarial Osteolysis in Mice Model

Objective To explore the role of the PI3K/Akt signaling pathway in the calvarial osteolysis induced by TCP wear particles in mice model.Methods Thirty-six male ICR mice were randomly divided into a sham group (n=12),TCP group (n=12)and a LY294002-treated group (n=12).A murine calvarial model of osteolysis was established through implanting 30 mg of TCP particles onto the surface of bilateral parietal bones following the removal of the periosteum.On the second postoperative day,LY294002 (5 mg·kg-1)was locally injected to the calvarium under the periosteum three times a week;mice in the sham group received local injection of normal saline (N.S.)in the calvarium,and the injection time was consistent with that of LY294002.Two weeks later,the calvaria and periostea were obtained after the mice were executed.The calvarial osteolysis,bone mineral density (BMD)and bone mineral content(BMC)were analyzed using Micro-CT,Hematoxylin-Eosin (HE)staining was conducted to observe the inflammatrory response and formation of osteoclasts.Real-time PCR was applied to detect the mRNA level of tartrate-resistant acid phosphatase (TRAP),the marker of osteoclasts formation,cathepsin K (CstK),receptor activator for nuclear factor-κB kigand (RANKL)and c-Fos.The release of tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6)and IL-1β were measured using enzyme-linked immumsorbent assay (ELISA).Results Micro-CT and histological analysis indicated that LY294002,the specific inhibitor of PI3K,significantly prevented TCP wear particles-induced osteolysis and osteoclastogenesis,and increased BMD and BMC in the calvaria of mice.Real-time PCR data revealed LY294002 significantly suppressed the increase in mRNA level of osteoclastogenic genes such as TRAP,CstK,RANKL and c-Fos in the calvaria of TCP wear particles-implanted group.ELISA assay showed that TCP wear particles-induced release of TNF-α,IL-1β and IL-6 was significantly inhibited by LY294002 treatment.Furthermore,LY294002 significantly attenuated TCP wear particles-triggered activation of Akt,and down-regulated the level of p-AktSer473 and p-AktThr308.Conclusion PI3K/Akt signaling pathway contributes to TCP wear particle-induced osteolysis,and can be developed as a new therapeutic target for the prevention and treatment of bone destruction diseases caused by wear debris.
