1.Clinical result of phacoemulsification after implantation of an apodized diffractive multifocal IOL(Acrysof ReSTOR)

Objective To evaluate the visual outcomes,depth of focus and safety after implantation of the Acrysof ReSTOR and the Acrysof Natural.Design Nonrandomized clinical trial.Participant 53 patients with age-related cataract(61 eyes)were divided into two groups:Acrysof ReSTOR group(MIOL)included 28 patients(31 eyes),and reference group-Acrysof Natural group(SIOL)included 25 patients(30 eyes).Method All patients underwent phacoemulsification and IOL implantation.At 1 week,1 month and 3 months postoperatively,distant and near visual acuities were observed,and depth of focus was measured 3 months postoperatively.Main Outcome Measure Uncorrected distant visual acuity(UCDVA),uncorrected near visual acuity(UCNVA),best corrected distant visual acuity(BCDVA),best corrected near visual acuity(BCNVA),distant corrected near visual acuity(DCNVA),depth of focus.Result At 3 months postoperatively,in MIOL group:UCDVA was 0.90?0.15,UCNVA was 0.70?0.19,DCNVA was 0.73?0.21 and depth of focus was 5.5D(+1.5~-4.0D),while in SIOL group:UCDVA was 0.84?0.14,UCNVA was 0.36?0.10,DCNVA was 0.26?0.08 and depth of focus was 2.5D(+1.0~-1.5D).In MIOL group,UCDVA and UCNVA at 1 month were markedly better than those at 1 week. Conclusion Acrysof ReSTOR can provide excellent outcomes both in distants and near visions and it may reduce the dependence on spectacles in near vision.(Ophthalmol CHN,2006,15:344-347)
3.Observation on the therapeutic effect of kangfuxin liquid topical treatment on open wound of postoperative perianal abscess

Objective To explore the effect of kangfuxin liquid topical treatment on open wound of postoperative perianal abscess.Methods200 cases of patients with open wound abscess surgery in our hospital from February 2014 to September 2016 were randomly divided into control group and experimental group,with 100 cases in each group.The control group was given conventional treatment, the experimental group were treated by kangfuxin liquid.The relevant therapeutic indicators and clinical healing time between two groups were compared.ResultsThe effective rate of the treatment group was 93.0%, which was significantly higher than that of the control group (85.0%), the difference was statistically significant(P<0.05).The granulation growth period of the experimental group was (5.14±1.41) days, and the average healing time was (15.21±2.49) days.Which were significantly shorter than those of the control group[(7.45±1.32) days, (21.16±2.69) days], and the differences were Statistical differences (P<0.05).ConclusionTo improve the treatment effect to a great extent open wound infection of kangfuxin liquid in treatment of perianal abscess after surgery, shorten the healing time, improve the patient's symptoms, with further clinical promotion and application significance.
4.Effects on recovering of corneal wound and postoperative discomfort of different surgical procedures for pterygium

?AIM: To evaluate the effects on recovering of corneal wound and postoperative discomfort of different methods for primary pterygium.?METHODS: Forty-seven cases ( 60 eyes ) of primary pterygium were excised under microscope with limbal epithelial transplantation, with sharp dissection ( 24 cases, 30 eyes, Group A) and blunt dissection (23 cases, 30 eyes, Group B).All cases were followed up for 1d to 1mo.?RESULTS: The recovering of corneal wound was better in Group B on 1st day and 3rd day after surgery.Pain, photophobia and tears, foreign body sensation were more serious in group A on 1st day after surgery with a statistically significant difference (P=0.005,0.015,0.012). Pain, photophobia and tears, foreign body sensation were more serious in Group A on 3rd day after surgery with a statistically significant difference ( P=0.019,0.018, 0.015).There was no statistically significant difference on 1wk and 1mo after surgery (P>0.05).? CONCLUSION: Compared with sharp dissection, primary pterygium excised with blunt dissection can significantly improve recovering of corneal wound and postoperative discomfort.
5.Pathologic study on the perineural invasion in 513 cases of pancreatic cancer

Objectives To investigate the characteristics of neural invasion of pancreatic cancer as well as its relationship with other clinicopathological factors. Methods The neural invasion situation of 491 cases of ductal adenocarcinoma and other 22 pancreatic malignancies, 41 cases of benign tumor of pancreas and 21 cases of chronic pancreatitis was observed under light microscope, and its relationship with other clinicopathological factors was analyzed. Results The rate of neural invasion in ductal adenocarcinoma (74%) was much higher than in other types of pancreatic neoplasm (23% ,P < 0.01). Pancreatic ductal adenocarcinoma cell often invaded through peripheral nerve membrane into inner nerve fiber bundle, sometimes even invaded the whole cross-sectional nerve fiber. But neural invasion was not associated with differentiation of the tumor. The occurrence of chronic inflammation in the para-tumoral pancreas (52%) was also higher than that in other types of malignant (14%) or benign lesions (15% ,P <0.01). Lymphocytes neural invasion rate in pancreatic ductal adenocarcinoma was 65%, which were significantly higher than those in other types of malignant (36%) or benign lesions (22%, P < 0.01). Neural invasion rate in pancreatic ductal adenocarcinoma was associated with paratumoral chronic pancreatic inflammation and lymphocytes neural invasion, but not with lymph node metastasis. Conclusions Neural invasion was characteristic biological behavior in pancreatic ductal adenocarcinoma.
6.Reconstruction of skin and soft tissue defects by pedicle skin flaps

Objective To explore the effect of using pedicle skin flaps for treatment of skin and soft tissue defects.Methods From January 2004 to September 2010,87 patients with skin and soft tissue injury which had been treated by using pedicle skin flaps without vessel anastomosis were retrospectively analyzed,including 61 males and 26 females with an average age of 40 years (range,14-61).The pedicle skin flaps selected were known vessel skin flaps or unknown vessel random skin flaps.The reconstruction treatment increased multiple selections of skin flaps for patients.Skin and soft tissue defects were located at the hand in 21 cases,at the lower extremities in 66.The kinds of skin flaps included adjacent transposition flaps in 50 cases,distal skin flaps in 37.The areas of the skin flaps were 2.0 cm×1.5 cm-18 cm×12 cm.Donor sites were as follows:lower leg in 61 cases,abdomen in 18,and hand in 8.Results Three patients appeared skin flaps hematoma after operation,which were removed immediately with no effect on flaps survival.Pedicle skin flap was compressed due to unstable fixation in 1 case,which made distal flap tissue necrosis approximately 2 cm in width.After dressing change for 6 weeks,the flaps got survival.Thirty seven patients underwent the second stage operation for breaking the perdicle 3 weeks after the first operation.All 87 patients were obtained follow-up for an average of 13 months (range,3-60).The total of 90.80% patients treated with pedicle skin flaps were satisfied with the results.Conclusion Pedicle skin flap surgery can be performed more easily with less limitations of surgical designs.The survival rate of the flap is high,and the treatment of pedicle skin flaps could be well applied for soft tissue defects.

9.Kinematics Modeling and Analysis of Central-driven Robot for Upper Limb Rehabilitation after Stroke.

The present paper proposed a central-driven structure of upper limb rehabilitation robot in order to reduce the volume of the robotic arm in the structure, and also to reduce the influence of motor noise, radiation and other adverse factors on upper limb dysfunction patient. The forward and inverse kinematics equations have been obtained with using the Denavit-Hartenberg (D-H) parameter method. The motion simulation has been done to obtain the angle-time curve of each joint and the position-time curve of handle under setting rehabilitation path by using Solid Works software. Experimental results showed that the rationality with the central-driven structure design had been verified by the fact that the handle could move under setting rehabilitation path. The effectiveness of kinematics equations had been proved, and the error was less than 3° by comparing the angle-time curves obtained from calculation with those from motion simulation.

10.Analysis of clinicopathologic features and prognostic factors of 30 patients with extramamma-ry Paget’ s disease

Purpose To evaluate the clinical and pathological features of extramammary Paget’ s disease ( EMPD) and to investigate the related prognostic factors. Methods The clinical and pathological data of 30 patients with EMPD were reviewed retrospectively. The HE slides in the file were rechecked. Immunohistochemical stains were performed on archival paraffin-embedded sections with En-Vision method in 20 cases. Follow-up data were recorded. The relationship between the clinicopathological parameters and prognosis was statistically analyzed. Results Among the 30 patients, there were 26 males and 4 females, aged from 53 to 88 years with median age 70. Macroscopically, typical presentations of EMPD were eczematoid lesions. Microscopically, Paget’s cells were distributed sin-gly or in groups ( as glandular or nests patterns) within the epidermis. Immunohistochemical stains showed that Paget’ s cells were pos-itive for CK7, and negative for CK5/6 and p63. Follow-up data were available in 23 cases, 8 cases recurred resection, among them, 6 patients had positive resected margin and 4 died. The recurrent rate and mortality between the groups of dermal invasion and no der-mal invasion (P<0. 01 and P<0. 05), the groups of lymphatic metastasis and no lymphatic metastasis (P<0. 01), and the groups of positive and negative resected margin (P<0. 01 and P<0. 05) were statistically different. There were no statistically different with age and extent of lesion (P>0. 05). Conclusions CK7 is a sensitive and specific marker of Paget’s cells. Dermal invasion, lymphatic metastasis and positive resected margin are poor prognostic factors in EMPD.
