Objective To investigate the expression of glucocorticoid receptor(GR), GR? mRNA and GR? mRNA in peripheral blood mononuclear cells(PBMC)of patients with multiple sclerosis(MS) and healthy controls and investigate the relationship between GR protein, GR? mRNA, GR? mRNA and intravenous injection methylprednisolone(IVMP)effect.Methods GR in PBMC was measured by radioligand assays in 20 patients with relapsing-remitting MS(RRMS), 6 with the second progressive MS (SPMS)and 26 healthy controls.Semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR)was used to detect the expression of GR? mRNA and GR? mRNA in PBMC of patients with MS and healthy controls.The effect of IVMP was evaluated by the expanded disability status scale(EDSS). BACKGROUND:Our previous study has already manifested that Chinese medicine Xiaokening can effectively prevent and treat the early diabetic nephropathy. OBJECTIVE:To compare the effect of rhein and archen on the apoptosis of mesangial cells of rats cultured with high glucose. METHODS:Mesangial cells were stimulated by different concentrations of rhein and archen (20, 40 and 80 μmol/L). Morphology of apoptotic cells was observed by hematoxylin-eosin staining. Karyon apoptosis was examined by fluorescence staining of DAPI. cellapoptosis rate was detected by flow cytometry. RESULTS AND CONCLUSION:The outcome of hematoxylin-eosin staining and DAPI staining indicated that the effect of rhein was much stronger than that of archen on the apoptosis of mesangial cells in the rats cultured with high glucose. Comparison of the apoptosis rate also indicated that the rhein had a stronger effect on the earlier and the later stage apoptotic rate of mesangial cells than archen. Both rhein and archen with different concentrations can induce apoptosis of mesangial cells, but the effect of rhein is much stronger.

Objective To analyze influencing factors of physicians' turnover intention at public county hospitals from career stage perspective.Methods Physicians career life was divided into three stages according to professional ranks and titles.The sample was drawn using stratified multistage random methods from public county hospitals in Shandong province and 677 questionnaires were completed,giving a 90.3％ valid response rate.Data were analyzed using multi-group stepwise linear regression.Results The percentage of responders with middle and high-level turnover intention was 23.0％and 6.5％ respectively,and there was no statistically significant difference in turnover intention across three stage subgroups.In physicians at stage 1,the most important factors to predict turnover intention were superiors ' appreciation (r =-0.22,P ＜ 0.05) and payment rationality (r =-0.21,P＜0.05),while in physicians at stage 2,the most important predicting factors were autonomy (r=-0.39,P＜0.05)and learning demand(r=-0.22,P＜0.05),and in physicians at stage 3 were career development(r=-0.31,P＜0.05)and autonomy(r=-0.21,P＜0.05).Conclusion Autonomy and workload are common factors of turnover intention with different predicting power across three career stages.Physicians adjust their career needs to self-actualization with development of career stage,so welltargeted incentives should be taken to stabilize the medical staff of public county hospitals.

It has been suggested that HCN1 is primarily expressed in hippocampus, however little is known about its effects on spatial learning and memory. In the present study, we investigated the effects of non-specific HCN1 blocker CsCl on spatial learning and memory by using Morris water maze and in situ hybridization in mice. The results showed CsCl 160 mg/kg ip for 4 days, and the mean escape latency was 34 s longer than that of normal control (P<0.01). In hippocampal tissues, staining for the HCN1 mRNA was stronger in the DG and CA1 region of the hippocampus (P <0.05, P<0.05, when CsCl-administration group was compared with normal group). Our results suggested that CsCl could significantly affect the spatial learning and memory in mice, and HCN channel is involved in the process of learning and memory.

Objective:To explore the application of scenario simulation and rapid film reading training in the standardization of cervical vertebra DR examination in radiology department.Methods:Using the method of scene simulation in the examination room to take DR films of cervical vertebra, two senior technicians in charge evaluated and guided the deficiencies of junior technicians from the aspects of patient check, exposure conditions, body position placement, film printing, inspection safety and radiation protection. Every morning, DR images of cervical vertebra were randomly selected and read quickly, and the junior technicians should quickly browse the images within one minute and make quality control evaluation. Then the senior technician in charge pointed out the shortcomings of the evaluation and related improvement methods. After that, the technical specification manual of cervical vertebra DR was compiled based on the deficiencies and problems found by each technician, and 200 patients aged 18-55 years, without abnormal movement and no internal fixation before and after the establishment of standardization were randomly selected in group A (before standardization) and group B (after standardization). The image quality and repetition projection rate of group A and group B before and after the standardization were compared. SPSS 25.0 was used to analyze the data.Results:The image quality score of group B (8.83±0.33) was higher than that of group A (8.45±0.30), and the repetition projection rate of group B (2%) was lower than that of group A (9%), with statistical significance ( P<0.05). Conclusion:Through the training mode of rapid film reading and scenario simulation, the standardization of cervical vertebra DR examination technology suitable for the situation of the department has been established, which effectively solves the problem of abstraction in the technical teaching of radiology department. The standardization establishment plays an important role in standardized training, for which, the technicians can quickly grasp the work process of the department and the corresponding examination technology during participating in the standardization establishment and training process.

Objective To investigate the mechanisms of Mieyou Decoction in the treatment of Hp-associated gastritis of spleen-stomach damp-heat syndrome. Methods Seventy BALB/c mice were randomly divided into control group, model group, high concentration of Mieyou Decoction group, low concentrations of Mieyou Decoction group, and gastric triad group, with 14 rats in each group. BALB/c mice of Hp-associated gastritis of spleen-stomach damp-heat syndrome models were established by composite factor. After the success of modeling and continuous 14-day administration, the expressions of TLR2, TLR4 protein, TLR2 mRNA, and TLR4 mRNA were detected by immunohistochemistry and qPCR. ELISA was used to detect the expressions of IL-6 and IL-8 in the serum. Results Compared with control group, the expressions of TLR2, TLR4 protein, mRNA, IL-6, and IL-8 in model group significantly increased (P<0.01);the expressions of TLR2, TLR4 protein, mRNA, IL-6, and IL-8 in high concentration of Mieyou Decoction group, low concentrations of Mieyou Decoction group, gastric triad group were lower than model group, with statistical significance (P<0.01);the expressions of TLR2, TLR4 protein, mRNA, IL-6, and IL-8 in high concentration of Mieyou Decoction group were higher than gastric triad group, but the differences were not statistically significant (P>0.05). Conclusion Mieyou Decoction may play a role in the treatment of Hp-associated gastritis of spleen-stomach damp-heat syndrome through intervention in the expressions of TLR2 and TLR4.
