1.Research progress of biomechanics and finite element analysis of lumbar interspinous devices

BACKGROUND:The lumbar interspinous device, as a kind of non-fusion technology has been extensively applied in the clinic and exerts superiority in biomechanics compared with the traditional fusion technology. With the development of prosthesis design and impanation technology, it reveals a better application prospect. OBJECTIVE:To review the biomechanics and finite element analysis of lumbar interspinous devices. METHODS:The first author retrieved the databases of CNKI, WanFang, PubMed and SpringerLink using the keywords of“lumbar spine, interspinous devices, biomechanics, finite element analysis”in Chinese and English, respectively. Researches related to the biomechanics and finite element analysis of lumbar interspinous devices were included and repeated researches were excluded. A total of 44 literatures were enrol ed for review, including 8 Chinese and 36 English literatures. RESULTS AND CONCLUSION:(1) Biomechanical y, several interpinous devices, which are commonly used in the clinic, can increase the stability of the implanted segment in sagittal alignment by limiting the range of flexion-extension, with no significant change in lateral bending and axial rotation. (2) Few studies analyze the influence of the implant size and placement on the implanted segment and on the adjacent segments. (3) Through the stress nephogram, three-dimension finite element analysis can intuitively analyze the changes of the stress distribution in the intervertebral disc, isthmus and facet joints before and after implantation. Both biomechanical studies and finite element analysis indicate that interspinous devices can share the load of the disc and facet joints, and at the same time, make no effect on the range of motion and stress of the adjacent segment. (4) In conclusion, the short-term biomechanical advantage of the interspinous devices is obvious, but further studies are needed. The finite element analysis can simulate different body physical environment, and can analyze mechanical distribution changes after implantation, which is an effective way to evaluate the mechanical mechanism of the interspinous devices.
2.Significance of Kisspeptin levels in diagnosis and therapeutic evaluation of idiopathic central precocious puberty in girls

Objective To explore the significance and value of Kisspeptin in the diagnosis and therapeutic evaluation for idiopathic central precocious puberty(ICPP) in girls.Methods Twenty-four girls with ICPP and 21 girls with premature thelarche(PT) who were hospitalized in the Children's Hospital of Jiangxi Province from Jun.2012 to Jan.2013 were selected as ICPP group and PT group,and 25 healthy girls were selected as healthy control group.The coagulation vein blood in ICPP girls before treatment and after 6 months treatment and PT girls and healthy girls were collected,and enzyme linked immunosorbent assay was used to detect the plasma Kisspeptin level,and t test was used to analyze the differences among the 4 groups.Results The Kisspeptin level of girls with ICPP [(1.80 ± 0.13) μg/L]was apparently higher than that of PT groups [(1.41 ± 0.10) μg/L] and healthy control group[(1.39 ± 0.13) μg/L],and the differences were statistically significant (t =10.974,14.787,all P =0.000).However,the difference of Kisspeptin between PT group and healthy control group was not statistically significant(t =10.970,P =0.095).In addition,the Kisspeptin level of ICPP girls who undewent 6-month treatment [(1.49 ± 0.15) μg/L] was significantly lower than that before treatment,and the difference was statistically significant (t =10.80,P < 0.05) ;but,compared with PT group and healthy control group,there was no significant difference (t =6.32,P =0.060 ; t =7.44,P =0.214).Conclusions Kisspeptin level is related with pubertal development,and it can be used as an important evidence in ICPP diagnosis and an important parameter in ICPP therapeutic evaluation.
4.Inhibitory effect of LIGHT-Fc gene transfection on human esophageal squamous carcinoma cell line Eca109

Objective To investigate the in vitro inhibitory effect of LIGHT on human esophageal carcinoma cells. Methods LIGHT Fc expression vector was transfected into human esophageal squamous carcinoma cell line, Eca109. The inhibitory effect of LIGHT gene on cell growth was detected by MTT and cell growth curve. The expressions of LT?R and HVEM were detected by RT PCR. Results Expression of LIGHT Fc gene could inhibit Eca109 cell proliferation. The growth curve of Eca109/LIGHT was significantly lower than that of the control group in the culture medium containing 1% FCS. MTT test showed that there was significant difference in cell viability between Eca109/LIGHT and the control group ( P
6.Observation of Pattern Changes of Syndrome in TCM of 258 Cases of Acute Stroke and Therapeutic Effect Evaluation of Small Compound Prescriptions in Stroke Unit with Treatment Based on Syndrome Differentiation

Objective: To investigate into the etiological factor,distribution of syndrome(patterns),pathogenesis features as well as the curative effects of small compound prescriptions of(Traditional) Chinese Medicine(TCM) in the treatment of acute stroke in stroke unit.Methods:(Cases) corresponding to diagnosis standards were randomly divided into control group(102 cases) and treatment group(156 cases).Conventional treatment methods were given to cases in control group.On the base of control group,small compound prescriptions were added to in treatment group,successively 14-21 days.The relation between types and time and TCM scale(TCMS),the NIH stroke scale(NIHSS) and Barthel index(BI) were observed in the two groups.Results: Stroke was age-related,the older,the more stroke patients.Stroke was closely related to hypertension.Ascending hyperactivity of liver yang was main type in the previous three days,this type(decreased) with the time going on.On the contrary,the type of wind phlegm and blood stasis was secondly main type in the previous three days,it increased with the time going on.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation covered 95 per cent of all patients in the previous 21days.In the two groups,the mean scale of TCM and NIH decreased,and BI increased after treatment,this statistical difference could also be seen between the two groups.Conclusion: Excessive types are main types in the acute stage of stroke.Types of ascending hyperactivity of liver yang,wind phlegm and blood stasis as well as phlegm heat and constipation are its main types.Transferring of ascending hyperactivity of liver yang into endogenous wind may be trigger of reaction link of wind,fire,phlegm and blood stasis in TCM theory.Small compound prescriptions of TCM aiming to clearing away liver fire to stop wind,to activate blood for removing blood stasis and phlegm,to removing phlegm and heat from discharge can reduce scale of TCM and NIH,increase BI in the treatment of patients in stroke unit,which provide beneficial proofs for evaluation of effects of TCM.
7.Expression of c-jun in injured tissues and its forensic interest

Oncogene c-jun is a member of jun family,the immediately early genes(IEGs),and belongs to one of the nuclear transcription factors of basic leucine zipper(bZIP)family.Combined with many gene promotors,c-jun is involved in the regulation of gene transcription.Its products play important roles in regulating gene expression,cell proliferation,differentiation and apoptosis.The structure,biological funetion,regulation of c-jun and its roles contributing to tissue damage are reviewed in this article,which may provide understanding for severity of tissue injury and wound age estimation in the field of forensic pathology.
8.Effects of insulin on macrophage phenotype transformation under high glucose condition

Objective · To investigate the effects of insulin on high glucose-cultured humanmononuclear cell line THP-1 and macrophage phenotype transformation in diabetic wounds. Methods · THP-1 cells were cultured with normal (5.6 mmol/L) and high (25 mmol/L) glucose, respectively,stimulated with PMA for differentiation, and induced to M1 macrophages with LPS. After treated with insulin for 6 h, expression changes of M1 type macrophage markers inducible nitric oxide synthase (iNOS), tumor necrosis factor α (TNF-α), and interleukin-1β (IL-1β), as well as M2 type macrophage markers arginase1 (Arg1) and IL-10 were detected using real-time PCR andWestern blotting. High fat diet feeding plus multiple intraperitoneal injections of low dose streptozotocin (STZ) were used to induce type II diabetes rat model. After blood glucose level has been stable for five weeks, two fullthickness skin wounds with the diameter of 1cm were made on the back of DM rats. Wounds were randomly assigned to being treated with insulin (0.2 U insulin /20 μL saline) or saline (20 μL saline) using the random number table. Characteristics of macrophagephenotypes were observed 3, 7, and 25days after wounds were made. Normal rats (n=3) served as controls. Results · After being cultured with high glucose, the mRNA levels of M1 markers iNOS and TNF-α were up-regulated in LPS-induced THP-1 cells, while the mRNA levels of M2 markers Arg1 and IL-10 were down-regulated.Afterbeing treated with insulin for 6 h, mRNA levels of iNOS and TNF-α weredown-regulated, protein levels of iNOS, IL-1β were down-regulated too, while mRNAand protein levels of Arg1 and IL-10 were up-regulated. In addition, the expression level of phosphorylated NF-κB-p65 was significantly increased after high glucose culture and was significantly decreased after insulin intervention. Compared to normal rat skin wounds, the expression of iNOS in macrophages was significantly increased in wounds of diabetic rats. The expression of iNOS in macrophages was high in saline treated wounds 3 and 7 days after the wounds were made and the expression of Arg1 was low 25 days after the wounds were made. In insulin treated wounds, the expression of iNOS started to decrease on day 7 after the wounds were made and the expression of Arg1 was significantly higher than that in saline treated wounds on day 25 after the wounds were made. Conclusion · Insulin can induce macrophage phenotype transformation from M1 to M2 under high glucose condition and the mechanism may be associated with the phosphorylation of NF-κB-p65.
9.Role of Clinical Pharmacists in the Individualized Treatment of Purulent Meningitis

OBJECTIVE:To explore the role of clinical pharmacists participating in the individualized treatment for purulent meningitis. METHODS:Clinical pharmacists participated in the therapy for a patient with purulent meningitis complicated with Staphylococcus aureus infection. According to patient's condition,clinical pharmacists assisted physicians to formulate preliminary therapeutic plan. Reviewing relevant guidelines,domestic and foreign literatures,clinical pharmacists suggested to combine with dexamethasone so as to relieve inflammatory reaction. According to the results of drug sensitivity test,based on vancomycin plasma concentration monitoring and population pharmacokinetics model fitting,clinical pharmacists suggested to reduce the dose of vanco-mycin to 0.5 g,ivgtt,q12 h. The pharmaceutical care were conducted throughout the therapy,including efficacy evaluation of an-ti-infective therapy,ADR monitoring,renal function monitoring,etc. RESULTS:Physicians adopted some suggestions of clinical pharmacists. The disease condition of the patient was recovred,and no ADR related to vancomycin was found. On the 16th day, the patient was discharged from the hospital. CONCLUSIONS:Clinical pharmacists participate in treatment of purulent meningitis, assist physicians to optimize therapy plan based on relevant guideline,literature,etiological examination,blood concentration moni-toring and pharmacokinetics model fitting results. It not only guarantee therapeutic efficacy of anti-infective therapy,but also pre-vent and reduce the occurrence of ADR.
10.Exploration of Community Pharmaceutical Care Talents in Higher Vocational Colleges

OBJECTIVE:To explore the cultivation method for community pharmaceutical care talents in higher vocational colleg-es. METHODS:According to the talent training aims to meet the demand of community pharmaceutical service positions in higher vo-cational colleges,the teaching reform was carried out for talent cultivation mode,course structure system,training base construction and teaching methods. RESULTS:In terms of talent cultivation mode,the work-integrated learning mode was built;in terms of course structure system,“Medicine,Pharmacy,Humanities and Expansion”course module was constructed to further optimize it;in terms of training base construction,the capital investment was increased to improve the school training base,meanwhile,cooperation between schools and enterprises was carried out to strengthen the construction of out-of-school training bases;in terms of teaching methods, PBL,case teaching model and situational teaching method were implemented. CONCLUSIONS:The quality of pharmaceutical talents has been significantly improved by implementing reform of the talent cultivation mode,the course structure system and teaching meth-ods,together with constructing training bases.
