Nowadays,medical students have high pressure in hunting job.We analyzed the background of hunting job for medical students and discussed the corresponding psychological problems.Finally,we provided the countermeasures to solve these problems for medical students to find a job.

Background Conventional corticosteroid therapy after laser epithelial keratomileusis (LASEK) is topical application of 0.1％ fluorometholone eye drops for 4 months,but this therapy increases the ocular hypertension risk and leads to poor compliance in patients.It is necessary to explore a safe and effective brachytherapeutic drug.Objective This study was to compare the clinical outcomes between the brachytherapy of 0.5％ loteprednol etabonate eye drops and the long-term therapy of 0.1％ fluorometholone eye drops after LASEK.Methods Clinical data were retrospectively analyzed from 208 right eyes in 208 mild and moderate myopia patients who received LASEK at Peking University Third Hospital from 2009 to 2013.The eye drops of 0.5％ loteprednol etabonate was used 6 times daily since operation day until postoperative day 5,and 4 times daily for a week,and then tapered for three consecutive weeks in 104 eyes of the loteprednol group after LASEK,and 0.1％ fluorometholone eye drops was applied topically 4 times daily for initial one month and then tapered for three consecutive months in another 104 eyes in the fluorometholone group after LASEK.Corneal epithelial healing time was compared between the two groups by recording the wearing duration of corneal contact lens.Noncontact intraocular pressure (lOP) was detected and corrected 5 days,2 weeks,1 month and 3,6 months after LASEK.Haze was scored based on the criteria of Cheng under the slit lamp microscope and apparently optometry was performed 1 month and 3,6 months after LASEK.Results The number of eyes removed corneal contact lens in 5,6 and 7 days after LASEK was not significantly different between the loteprednol group and the fluorometholone group (x2 =1.180,P =0.554).No considerable differences were seen in the eye numbers with the equivalent spherical lens (absolute value) ≤0.5 D in 1 month,3months and 6 months after LASEK (Z=-1.028,P=0.304;Z=-0.398,P=0.691;Z=-0.445,P=0.656).In the fifth days after surgery,the median corrected IOP was 13.52 mmHg and 13.12 mmHg in the loteprednol group and the fluorometholone group,respectively,showing a significant difference between them (Z =-1.985,P =0.047),but no significant differences were found in IOP in 2 weeks,1 month and 3 months,6 months after surgery between the two groups (all at P＞0.05).IOP elevated (postoperative IOP-preoperative IOP ≥ 5 mmHg) after LASEK in 5 eyes in the loteprednol group and 4 eyes in the fluorometholone group (x2 =0.116,P =0.733).In 1 month after surgery,the number of eyes in different grades of haze was significantly decreased in the loteprednol group compared with the fluorometholone group (x2=13.506,P=0.009),however,there was no significant differences in haze grading in postoperative 3 months and 6 months between the two groups (x2 =2.199,P =0.699;x2 =1.154,P =0.562).Conclusions Brachytherapy of 0.5％ loteprednol etabonate eye drops appears to have a similar effect in antiinflammation and inhibition of haze to the long-term therapy of 0.1％ fluorometholone eye drops after LASEK.Furthermore,topical application of 0.5％ loteprednol etabonate eye drops dose not increase the risk of ocular hypertension.

[Objective]To study the clinical characteristics of osteochondrosis of lumbar posterior vertebral edge(OLPV) and evaluate the efficiency of surgical treatment. [Method]From August 2000 to August 2003,41 cases of OLPV patients were treated,all cases underwent decompression of posterior approach and selective resection of the compressive things.Furthermore,1 case was treated with trans-vertebral pedicle internal fixation and inter-transverse process bone graft because of excessive decompression and preoperative lumbar instability.[Result]Low back and leg pain disappeared or obviously being relieved in all cases,and the results of histological analysis suggested the diagnosis of OLPV in all cases.Forty-one cases were followed from 5 to 8 years,average 6.8 years.No lumbar instability or spondylolisthesis was found in any case.Six cases suffered slight low back pain occasionally without obvious impaired quality of life.The preoperative JOA score was 14.73?2.06,when followed by 3 months,the score changed into 27.14?4.13,there was statistical significance between them(P

Objective:To investigate the development of vestibular system function in 7 to 14 years old normal boys and the ADHD boys;Methods:The present study involved two groups of subjects:148 ADHD boys(age range 7 years to 14 years) diagnosed with DSM-IV diagnostic criteria and 148 normal boys with the same age.Each group was divided into 4 clusters according to two years gap.All subjects were tested for the vestibular system function by the Vestibular Function Test System 2000/ ENG-V600 with the eye movement index.Results:From 7 to 14 years,the development of vestibular system function in normal boys had the characters as the description below.The optokinetic system function exhibited more development from seven years in the normal group.From 7 to 10 years,the time delay of saccade and anti-saccade in normal boys was significantly shorter(In the normal controls,the delay time of left saccade was 149?66 ms in 7-8 years old boys,108?64 ms in 9-10 years old boys,117?72 ms in 9-10 years old boys,the delay time of left anti-saccade was 178?127ms in 7-8 years old boys,101?88 ms in 9-10 years old boys,P

Objective To investigate the effects and clinical value of neoadjuvant chemotherapy in patients with locally advanced cervical cancer.Methods 102 cases with locally advanced cervical cancer treated were randomly divided into observation group and control group,the observation group received the surgery after the neoadjuvant chemotherapy,and the control group was given a direct surgical treatment.Operative time,blood loss and postoperative pathologic results were observed,and chemotherapy and toxicity evaluation were made for the observation group.Results The chemotherapy effective rate of the observation group was 74.51％,and the toxicity could be tolerated;the operation time and blood loss of the observation group were significantly lower than those of the control group(all P ＜ 0.05) ;surgical trauma and postoperative complications' occurrence rate of the observation group were significantly lower than those of the control group (all P ＜ 0.05) ; pelvic lymph node metastasis and vaginal cutting edge of the residues of the observation group were significantly lower than those of the control group (all P ＜ 0.05).Conclusion For locally advanced cervical cancer,NACT has a positive significance in reducing the tumor volume,low-er tumor stage,and at the same time reducing the difficulty of operation,surgical trauma,postoperative complications,and preventing recurrence and metastasis,which can be used as a better adjuvant treatment program for locally advanced cervical cancer.

Objective To determine the clinical features of Klebsiella pneumoniae pneumonia and antimicrobial resistance of Klebsiella pneumoniae, providing the basis for rational use of antibiotics. Methods Eighty-six strains of Klebsiella pneumoniae were isolated from the sputum specimen which collected from the inpatients in our hospital from Dec 2008 to Dec 2010,which were identified with Bio Merieum Vitek 2. A total of 86 children were enrolled in the study, which were divided into hospital infection/colonization group (n = 68) and community infection group (n = 18). Clinical information were recorded. Extended spectum β-lactamase (ESBLs) and antibiotic susceptibility of Klebsiella pneumoniae were determined. Results Klebsiella pneumoniae pneumonia in children mainly occured in infants, especially in infants with basic diseases.Clinical manifestations of Klebsiella pneumoniae pneumonia had no much difference from common pneumonia. The detection rate of ESBLs was high(76. 7%). ESBLs were found in 64 strains of the hospital infection/colonization group (94. 1% ,64/68) ,while in 2 strains of the community infection group (11. 1% ,2/18).There was a significant difference between the two groups (P ＜0. 01). The drug resistance of hospital infection/colonization group was significantly higher than that of community infection group. The strains of hospital infection/colonization group were sensitive with cefotan, piperacillin/tazobactam, carberpenem, amikacin and ciprofioxacin only, while were highly resistant to penicillins, cephalosporins and monoamine antibiotics.The average hospital stay of the hospital infection/colonization group was (15.0 ± 7. 1) d, while the other was (8. 2 ± 3. 8) d. There was a very significant difference between the two groups in average length of stay (P＜0.01). Conclusion Klebsieila pneumoniae pneumonia in children mainly occurs in infants with basic diseases. The symptoms are untypical. Klebsiella pneurmoniae multi-drug resistance is a serious problem,which must be paid attention to. We should make a difference between the two groups when antibiotics were prescribed.

OBJECTIVE:To prepare nattokinase immunoliposomes and its lyophilized product and to evaluate its pharmaceutical quality. METHODS: Nattokinse liposomes were prepared by film dispersion method, then the immunoliposomes were prepared by carbodiimide method coupled anti-human fibrin D-dimer monoclone antibody SZ-63 and lyophilized and evaluated about its pharmaceutical quality. RESULTS: Lyophilized immunoliposomes were multilamellar vesicles with its mean diameter, Zeta potentials at 251.6 nm, —34.6 mV, respectively. The lyophilized product of nattokinase immunoliposomes exhibited good appearance and satisfactory redispersibility, with its loss on drying at 14.30% and critical relative humidity at 45.66%. CONCLUSION: Nattokinase immunoliposomes and its lyophilized product have been prepared successfully.

Objective To retrospectively analyze the pathogenic bacterial distribution and drug resistance in the patients with u-rinary tract infection in our hospital to provide a basis for clinically rational use of antibacterial drugs .Methods The midstream u-rine of the patients with urinary tract infection in our hospital during 2013-2015 was performed the bacterial culture .The bacterial identification and drug susceptibility test were performed by using the Vitek-2 Compact automatic microorganism identification in-strumen .The data were statistically analyzed by adopting the WHONET 5 .6 software .Results Among 11130 urine culture sam-ples ,1676 strains of pathogenic bacteria were cultured with the detection rate of 15 .1% ;among them ,1332 strains were Gram-negative bacteria and accounted for 79 .5% ,275 strains were Gram-positive bacteria and accounted for 16 .4% and 69 strains were fungi and accounted for 4 .1% .The top 3 pathogenic bacteria of detection rate were in turn Escherichia coli ,Klebsiella pneumoniae and Enterococcus faecalis .Six hundreds and eleven strains of ESBLs bacteria were detected ,in which Escherichia coli ,Klebsiella pneumoniae accounted for 88 .5% and 10 .2% respectively .Escherichia coli had higher sensitivity to carbapenems ,fosfomycin ,ami-kacin ,etc .(>95 .0% ) .Klebsiella pneumoniae had higher sensitivity to carbapenems (>90 .0% ) .No vancomycin ,teicoplanin and linezolid resistant Enterococcus faecalis was detected .One strain of vancomycin resistant enterococcus faecium was detected .These pathogens mainly derived from the urology surgery department ,the constituent ratio of female was higher than that of male .Conclu-sion The pathogens of urinary tract infection in this hospital is dominated by Gram-negative bacteria ,Escherichia coli is mainly pathogenic bacterium .Multidrug resistant species and number are continuously increased .Clinicians should rationally and correctly use the antibacterial drugs based on the drug sensitivity test results for improving the effect of clinical anti-infection treatment .
