Clinical guidelines, coming into being with the development of evidence-based medicine, are of great significance in reducing medical malpractice and regulating medical behaviors. However, more effective and intelligent measures should be taken for their development and application due to their low utilization and poor compliance of doctors in their implementation. The emergence and structure of clinical guidelines were elaborated with related problems summarized. The knowledge expression models of digital clinical guidelines were analyzed and compared with the challenges faced by the current knowledge expression models of clinical guidelines pointed out.

Objective To analyze on the efficacy and toxicity of MAAE (MIT ,Ara-C,AMSA,VP16)regimens on treating refractory and relapsed acute leukemia in adult patients. Methods MAAE (MIT, Ara-C,AMSA, VP16) regimen, which consisted of MIT 10mg/d (d1~3), intravenousdrop, Ara-C 200mg/d (d1~7),intravenousdrop, AMSA 75 mg/d (d1~3), intravenousdrop VP16 100 mg/d(d1~4), intravenousdrop were used to treat 25 cases of adults with refractory and relapsed acute leukemia (AL). G-CSF 5 μg/kg were used every day when WBC<0.5×109/L. Results In the 25 cases with refractory and relapsed acute leukemia, 14 cases (56 %) reached complete remission, 5 patients(20 %) reached partial remission, the total effective rate was 76 %. Conclusion MAAE regimen was a very effective alternative treatment for CR induction in adult patients with refractory and relapsed AL and low toxicity.

In chronic kidney disease (CKD), inflammatory responses during the progression of renal tissue and tissue injury related causes its progression to end-state renal disease. Among them, nuclear factor (nuclear factor, NF)-kappaB signaling pathway by regulating the corresponding nuclear expression of target gene transcription, as well as affecting the synthesis of inflammatory mediators, induction of inflammation lead to kidney damage and renal fibrosis. Some single herbs and their extracts (such as Astragali Radix, Scutellariae Radix, Ginkgo Folium) and some traditional Chinese medicine (such as Danggui Buxue decoction, Qilian decoction) can reduce the inflammatory damage induced by renal tissue NF-kappaB signaling pathway and delay the progression of CKD.

AIM: To observe the effects of sodium restricted or supplemented on atrial natriuretic polypeptide (ANP) and cardiac function of rats with congestive heart failure (CHF).METHODS: CHF rats were divided into three groups with sham operation rats group as control. Radioimmunassay was used to determine the ANP contents of plasma and myocardium, at the same time cardiac function was measured.RESULTS: In sodium restricted group, the plasma sodium and atrial ANP and left ventricular systolic pressure and artery pressure were obviously lower than those in CHF group, while the plasma and ventricular ANP and the right atrial pressure were remarkably higher; In sodium supplemented group, the plasma sodium and arterial pressure had no obvious change compared with the control, the plasma and myocardium ANP and the right atrial pressure had no difference as compared with CHF group, while the left ventricular end diastolic pressure was remarkably lower and the left ventricular systolic pressure were obviously higher. CONCLUSION: Keeping the balance of plasma sodium by an appropriate supplement of sodium intake after CHF might be beneficial to the ANP biological effects and the cardiac function.

Objective To investigate the impact of 1,25(OH)2D3 on histological changes and activation of STAT3 in BLM?induced pulmonary fibrosis mice. Methods 30 male C57BL/6 mice were randomly divided into control group ,BLM group and BLM+VD group. Mice in BLM group and BLM+VD group received intratracheal injection of BLM(3 U/kg). Control group were intratracheally injected equal volume of sterile saline. From the first day after the surgery, mice in BLM+VD group received intraperitoneal injection of VD (5μg/kg·d). After 21 days, H&E and Masson′s trichrome staining were carried out. Aschroft score were used to evaluate histological changes in lungs. IL?6,IL?4 and INF?γin BALF were assessed by Elisa. p?STAT3,α?SMA and Collagen I were detected by western blot (WB) and immunohistochemistry. Results Fibrosis score and level of α?SMA,Collagen I in BLM group were significantly higher than that in control group (P < 0.05). However ,treatment with VD effectively at?tenuated fibrosis (P<0.05). IL?6 and IL?4 increased while INF?γwas decreased in BALF of BLM group (P<0.05). VD could ameliorate these changes. Upregulation and neuclear translocation of p?STAT3 were observed in BLM group, while VD intervention could inhibit phosphorylation of STAT3. Conclusions VD attenuate BLM?induced pulmonary fibrosis and regulate inflammatory cytokines probably by blocking STAT3 activation.

Objective To investigate the operational technique of laparoscopic splenectomy and pericardial devascularization (LSPD) and evaluate the clinical efficiency of this method for the treatment of portal hypertension. Methods With the new understanding of anatomical space around the spleen, the cardia and the fundus, two gaps and two tunnels can be created in LSPD.Retrospective analysis was made on the clinical data of patients who underwent LSPD from Jun 2013 to Mar 2015.The operative time, intraoperative blood loss, postoperative hospital stay, conversion rate and postoperative complication rate were measured. Results A total of 189 cases underwent surgery successfully, including 34 cases of splenomegaly and 21 cases of severe esophageal varices. The operative time was (125 ± 52) min, intraoperative blood loss (58 ± 32) ml,postoperative hospital stay (7.5-2.1) d.There were 4 conversion cases in this study. Conclusions The splenic pedicle and stomach pedicle can be safely dissected with the two gaps and two tunnels principle, which makes LSPD safe and convenient.
