2.The effects of the neotype hand-arm sling on shoulder subluxation and transferring movement in hemiplegic stroke patients

Objective To explore the effects of the neotype hand-arm sling on shoulder subluxation and transferring movement in hemiplegic stroke patients.Methods Thirty hemiplegic stroke survivors with shoulder subluxation and poor elbow flexor muscle tone (<2 on the Modified Ashworth Scale).were selected and randomly divided into three groups.Group 1 was trained in static upright standing,plus 10 min of sit-to-stand training and 20 min of walking training with no support for the hemiplegic,subluxed shoulder.Group 2 did the same training using a bagtype shoulder sling,and group 3 used the neotype hand-arm sling.Index finger palpation was used to evaluate the degree of shoulder subluxation,and the Five Times Sit-to-Stand (FTSST) and Timed Up-and-Go (TUG) times for the three groups were compared.Results In the static standing training,both the bag-type sling and the neotype hand-arm sling could effectively reduce shoulder subluxation.However,after 10 min of sit-to-stand training the neotype hand-arm sling was found to be significantly superior to the bag-type shoulder sling in improving shoulder subluxation,with the latter having no effect on shoulder subluxation.In the 20 rin of walking training,the neotype hand-arm sling provided significantly more effective support than the bag-type shoulder sling,although both could support the shoulders.Those using the neotype sling had shorter FTSST times,though there were no significant differences in the average TUG times.Conclusions The neotype slings can more effectively counter shoulder subluxation in upright stance,especially during sit-to-stand maneuvers and walking.It might have positive effects on transferring ability.

4.Triptolide inhibited the invasion ability of pancreatic cancer cells through TLR4/NF-kB signaling pathway

Objective To investigate the role of TLR4/NF-kB signaling pathway in inhibited invasion ability of pancreatic cancer cells caused by triptolide (TP).Methods PANC1 cells were divided into parental cells group,TP group,lipopolysaccharide (LPS) group and TP + LPS group.50 ng/ml of TP was added in culture medium in TP group,and 1 μg/ml of LPS was added in culture medium in LPS group,while 50 ng/ml of TP was pretreated for 2 h and 1 μg/ml of LPS was added in culture medium in TP + LPS group.All the ceils were cultured for 24 h.The TLR4 and matrix metalloproteinase-9 (MMP-9) mRNA and protein expression were evaluated by real-time PCR and Western blot.The NF-kB activity was determined by dual-luciferase reporter assay system.The invasion ability of pancreatic cancer cells was evaluated by transwell invasion chamberassay.Results The TLR4 mRNA expressions in parental cells group,TP group,LPS group and TP + LPS group were 0.41 ± 0.06,0.46 ± 0.10,0.20 ± 0.04,0.25 ± 0.06 ; the TLR4 protein expressions were 0.55 ±0.06,0.55 ±0.06,0.18 ±0.04,0.13 ±0.00; the activities of NF-kB were 13.0 ±3.0,31.6 ±4.3,7.3 ±1.5 and 10.8 ± 2.1,and the numbers of invasion cell were (56.8 ± 8.6),(104.5 ± 12.8),(32.0 ± 5.7) and (46.8 ± 7.0) ; the MMP-9 mRNA expressions were 0.36 ± 0.05,0.58 ± 0.07,0.18 ± 0.03,0.30 ± 0.004 ;the MMP-9 protein expressions were 0.31 ± 0.04,0.53 ± 0.08,0.11 ± 0.02,0.15 ± 0.00.In LPS group,TLR4 mRNA and protein expressions were not statistic significant when compared with those in parental cells group,but the activities of NF-kB,the numbers of invasion cell,MMP 9 mRNA and protein expressions were statistically increased when compared with those in parental cells group (t =8.654,7.593,6.655,4.982,P <0.01).TLR4 mRNA and protein expressions,activities of NF-kB,the numbers of invasion cell,MMP 9 mRNA and protein expressions in TP group were significantly lower than those in parental cells group (t =-7.609,-9.948,-4.176,-5.915,-8.179,-9.948,P< 0.01).TLR4 mRNA and protein expressions,activities of NF-kB,the numbers of invasion cell,MMP 9 mRNA and protein expressions in TP +LPS group were significantly lower than those in LPS group (t =-4.437,-14.805,-10.506,-9.700,-9.055,-8.932,P< 0.01).Conclusions TP can inhibit pancreatic cancer cell invasion,and the mechanism is related to the inhibition of TLR4/NF-kB signaling pathway and down-regulation of MMP-9 expression.
5.Allogeneic adipose-derived stem cells combined with nano collagen-based bone for repair of ulna bone defects

BACKGROUND:With the continuous development of medical model, high-level treatment and rehabilitation for bone defects are required. Therefore, bone tissue engineering technique based on stem cel culture has become a more important component of bone defect repair.
OBJECTIVE:To explore and analyze the feasibility of alogeneic adipose-derived stem cels combined with nano colagen-based bone for repair of ulna bone defects.
METHODS:Seventy adult female New Zealand rabbits were selected, 10 of which were used to isolate and culture adipose-derived stem cels, another 60 were randomized into scaffold group, scaffold+bone autograft group, scaffold+alogeneic adipose-derived stem cels group, with 20 rabbits in each group. The middle part of the ulna was resected to construct 12-mm bone defect models in the three groups, and then, corresponding materials were implanted. At 6, 9 and 12 weeks after implantation, bone mineral density was detected; at 14 weeks after implantation, the rates of solid fusion rate, non-solid fusion, and false joint were observed.
RESULTS AND CONCLUSION:Compared with the other two groups, the rate of solid fusion was significantly higher and the rates of non-solid fusion and false joint were lower in the scaffold+alogeneic adipose-derived stem cels group (P < 0.05). Compared with the scaffold group, the rate of solid fusion was higher and the rates of non-solid fusion and false joint were significantly lower in the scaffold+bone autograft group (P< 0.05). The bone mineral density increased dramaticaly at 6, 9, 12 weeks after implantation of scaffold+alogeneic adipose-derived stem cels, which was significantly higher than that in the scaffold group and scaffold+bone autograft group (P < 0.05). Compared with the scaffold group, the bone mineral density in the scaffold+bone autograft group was significantly higher (P < 0.05). These findings indicate that alogeneic adipose-derived stem cels combined with nano colagen-based bone has exact fusion effects with a higher bone mineral density.
6.Nano-hydroxyapatite/polyamide 66 as a direct pulp capping agent has effects on histological changes of odontoblasts and microvessels

BACKGROUND:Previous studies have demonstrated that nano-hydroxyapatite/polyamide 66 (nHA/PA66) composites as a root canal filing material possess the basic physical and chemical properties, have antibacterial properties against the infection of medulary cavity, and have no cytotoxicity. However, it is rarely reported as a direct pulp capping agent.
OBJECTIVE: To observe the histological reaction of nHA/PA66 composites as the pulp capping agent.
METHODS:The first and second molars from 12 Wistar rats were selected to prepare class I cavities by exposing the dental pulp. Then, these rats were randomized into three groups and subjected to dental pulp capping using nHA/PA66 composite, nHA and calcium hydroxide, respectively, folowed by filing with glass ionomer cement. At 7 and 30 days after filing, histological changes of odontoblasts and microvessels in the dental pulp were observed using ink perfusion method.
RESULTS AND CONCLUSION: At 7 days after filing, significant changes in dental pulp fibroblast proliferation, predentin thickening, dilation of blood vessels were observed in the nHA/PA66 group; only dental pulp fibroblast proliferation with no predentin thickening and dilation of blood vessels was found in the nHA group; in the calcium hydroxide group, the pulp tissues in the pulp chamber were mostly damaged, and obvious hyperplasia in dental pulp fibroblasts and certain inflammatory cel infiltration with vascular necrosis were observed. At 30 days after filing, predentin thickness was increased further, dental pulp fibroblasts proliferated, the number of odontoblasts increased, and dilation of blood vessels was observed in the nHA/PA66 group; predentin thickening and dilation of blood vessels were obviously visible in the nHA group; pulp necrosis was found on the pulp surface below the pitting pore, odontoblasts grew actively at the pulp junction, with no obvious thickening of the predentin, and there were dense vessels below the necrotic area. These findings indicate that the nHA/PA66 composite can induce the regeneration of dental pulp cels.
7.Effect of SonicFill ultrasonic resin on rat odontoblasts and dental pulp vessels

BACKGROUND:The mechanical properties of SonicFil ultrasonic resin are better than those of the traditional light-cured composite resin, but its effect on odontoblasts and dental pulp vessels is unclear.
OBJECTIVE:To study the effect of SonicFil ultrasonic resin on odontoblasts and dental pulp vessels of rats using histological methods.
METHODS:Twenty-five Wistar rats are selected for experiment. Ninety-six teeth from the first and second molars were selected and randomly divided into SonicFil ultrasound resin group and Filtek Z350 nano resin group with 48 teeth in each group. Experimental teeth in each group were equaly subdivided into 7 days group and 30 days group. Besides, randomly selected five teeth with no intervention served as normal control group. After anesthesia and disinfection, class I cavities on the occlusal surface of the maxilary first and second molars were prepared and filed with SonicFil ultrasound resin and Filtek Z350 nano resin group, respectively. The rats were sacrificed by ink perfusion method, and tooth specimens were taken to observe the changes of odontoblasts and pulp microvessels.
RESULTS AND CONCLUSION: At 7 days after filing, two groups had similar changes in the pulp presenting with mild inflammation. Besides, there were scattered inflammatory cels distributing in the cave pulp tissue, and in most of the specimens, odontoblasts arranged disorderly with little vacuoles and microvascular dilatation in the pulp. At 30 days after filing, in the SonicFil ultrasonic resin group, odontoblasts arranged orderly, inflammatory reaction and microvascular dilatation disappeared basicaly. In contrast, in the Filtek Z350 nano resin group, disordered odontoblasts decreased, inflammatory reaction weakened, as wel as microvascular dilatation became less. These results show that under the experimental condition, both of the SonicFil ultrasonic resin and Filtek SonicFil Z350 nano resin play certain roles in the formation of odontoblasts and dental pulp vessels, but over time al of these effects can automaticaly disappear.

Objective To evaluate the effect of external direct current shock on left atrial(LA) diamension and volumes after cardioversion for atrial fibrillation(AF) and the relation between LA size and function.Methods The initial study population included 68 patients with AF.According to cardioversion patterns, the patients were divided randomly into cardioversion with direct-current shock(36 patients) and cardioversion with drugs(32 patients). 10.Male Hodgkin lymphoma of breast: report of one case and review of literature

Objective To discuss the incidence, clinical features, diagnostic methods, treatment and prognostic factors of Hodgkin lymphoma of breast. Methods The clinical data, diagnosis, treatment and prognosis of one male patient diagnosed as Hodgkin lymphoma of breast were retrospectively analyzed, and literatures were reviewed. Results The 71-year-old male patient had a main physical sign of a local painless lump in breast. The lump increased progressively, associated with generalized lymphadenopathy, skin itching and sweats. Pathologic results showed the diagnosis of classical Hodgkin lymphoma (Ann Arbor stage: ⅢEB). The patient eventually died after 7 months following the combined chemotherapy. Conclusions Hodgkin lymphoma of breast is a rare disease lacking in specific clinical manifestations. It is difficult to clarify the diagnosis before operation, and its definite diagnosis largely depends on pathological and immunohistochemical examination. The combination of chemotherapy with radiotherapy may have a favorable efficacy, with the poor diagnosis.
