1.Periacetabular osteotomy through ilioinguinal approach for the treatment of developmental dysplasia of the hip in middle-aged adults

Objective To investigate the indications and mid-term outcomes of periacetabular osteotomy through ilioinguinal approach for developmental dysplasia of the hip (DDH) in middle-aged (40-50years old) adult patients.Methods Data of 10 adult patients who had undergone periacetabular osteotomies through ilioinguinal approach for DDH from August 2005 to February 2010 were retrospectively analyzed.There were 7 females and 3 males,aged from 40 to 47 years (average,42 years).Bilateral hips were involved in 3 patients,and unilateral hip was involved in 7 patients.Before this hospitalization,1 patient had received Chari osteotomy,and 1 had received hip-shelf procedure.According to the T(o)nnis classification,3 hips were classified as Grade 0,5 as Grade Ⅰ and 2 as Grade Ⅱ.The Shenton line was discontinuous in 6 hips.The average lateral center-edge angle was 3.50°±4.88°; the average anterior center-edge angle was 1.80°±5.07°; the average acetabular index angle was 18.20°±3.01°; the average femoral head extrusion index was 31.30%±4.37%; the average Harris hip score was 67.40±7.75.Changes of the indexes mentioned above were observed during the whole period of follow-up.Results All patients were followed up for 10 to 76 months (average,37 months).The T(o)nnis grade improved in 2 cases,from preoperative Grade Ⅰ and Grade Ⅱ to Grade 0 and Grade Ⅰ at final follow-up,respectively.At final follow-up,the Shenton line was discontinuous in 2 hips; the average lateral center-edge angle improved to 30.40°±3.31°,the average anterior center-edge angle improved to 29.50°±3.03°,the average acetabular index angle decreased to 4.50°±2.55°,the average femoral head extrusion index decreased to 9.90%±4.33%; the average Harris hip score increased to 84.10±4.07.The major complications included 3 cases of lateral femoral cutaneous nerve palsy which recovered in 4 to 6 months,and 1 case of iliac fossa hematoma which disappeared in 2 weeks.Conclusion With more strict surgery indication and more meticulous manipulation,periacetabular osteotomy through ilioinguinal approach can achieve satisfactory results in the treatment of DDH in middle-aged adults,which can increase hip joint congruence,relieve hip symptom and delay progression of osteoarthritis.
3.The relationship between supratentorial cerebrovascular diseases and secondary Wallerian degeneration: a report of 272 cases

Objective To investigate the characteristics of Wallerian degeneration (WD) of secondary pyramidal tract after supratentorial cerebrovascular diseases, and to study the relationship between secondary WD and clinical performance. Methods We retrospectively reviewed and investigated the results of brain CT films of 272 cases suffered from supratentorial cerebral vascular diseases during 2001 till 2004. The number of supratentorial disorders was counted respectively in WD group and non-WD group, the distribution feature of these disorders was analyzed, and the changes in the cerebral peduncles of the midbrain and Activities of Daily Living Scale (ADLs) were determined. The information obtained was analyzed statistically respectively. Results Seventy-four cases of secondary WD were found, the rate of WD outbreak was 27.2%. Objective To observe the effects of high sucrose,high saturated fatty acid and high unsaturated fatty acid(diets) on insulin resistance and endothelium-dependent vasodilation function.Methods Adult Wistar rats were divided into normal control(NC) group,high sucrose(HS) group and high saturated fatty acid(HSF) group,high unsaturated fatty acid(HUF) groups.Insulin sensitivity was tested by hyperinsulinemic-euglucemic clamp after 24 weeks.Acetylcholine-induced(or sodium nitroprussideinduced) relaxation of preconstricted isolated renal arteries was measured by Mulvany myograph.Results GIR was obviously lower in experimental groups than that in NC group.GIR was negatively correlated with triglyceride (TG),free fatty acid(FFA).Acetylcholine-induced relaxation was markedly decreased in all experimental groups compared with that in NC group and the maximal response was decreased 37.4% in HSF group,32.7% in HUF group,27.7% in HS group.Acetylcholine-induced relaxation was enhanced by incubation with L-Arg and decreased incubated with L-NNA,MB in all experimental groups.Vasodilation response was negatively correlated with TG,INS and well positively correlated with NO,GIR.There was significantly negative correlation between FFA andNO.Conclusions: The rats fed high sucrose,high saturated fatty acid and high unsaturated fatty acid diets developed insulin resistance with reduced endothelium-dependent vasodilation function.

?Advances in genome-wide analysis have revealed that up to 90%of the human genome is transcribed.However, only approximately 1% of RNA transcripts encode proteins, and the remaining transcripts are noncoding RNAs.Noncoding RNAs can be roughly divided into small noncoding RNAs (<200nt ) and long noncoding RNAs ( LncRNAs, >200nt ). Small noncoding RNAs include microRNAs, transfer RNAs and small nucleolar RNAs, whereas the long noncoding RNAs comprise ribosomal RNA, natural antisense transcripts, etc. Although the biosynthesis and biological activities of microRNAs are well studied through bioinformatics and active biological molecules analysis, the understanding of LncRNAs on these aspects is still limited.LncRNAs play multiple roles in regulating gene transcription and translation, and epigenetics.Aberrant LncRNAs expression can occur in various pathological processes and significantly related to the pathogenesis or poor prognosis of ophthalmological diseases. In this review, we will focus on the characteristics and regulatory functions of LncRNAs that are commonly associated with ophthalmological diseases.

9.Relationship between normal weight obesity and cardiovascular risk factors

Objective To determine the relationship between normal weight obesity (NOW) and cardiovascular risk factors.MethodsA total of 940 adults who received a health examination in out hospital were recruited in a cross-sectional study,and 407 with a body mass index (BMI) of18.5 to 25 kg/m2were enrolled for further analysis.Body fat percentage ( BF% ) was measured by bioelectrical impedance analysis (BIA),and the subjects were assigned to the NOW group ( BF% ≥25% for male or BF% ≥35%for female) or the control group ( BF% < 25% for male or BF% < 35% for female).Cardiovascular risk factors and their detection rates were compared between the two groups by using independent sample t test and x2 test.The correlationbetweenNOW and cardiovascular risk factors was assessedbylogistic regression.Results The prevalence of NOW in men and women were13.1% and14.9%,respectively.The prevalence of NOW was increased with age ( x2 =6.90,P <0.05 ).Systolic blood pressure (SBP),diastolic blood pressure ( DBP ),total cholesterol ( TC ),triglycerides ( TG ),low-density lipoprotein cholesterol ( LDL-C) and serum uric acid (SUA) were significantly increased in the NOW group (t values were 2.97,2.44,2.54,5.09,2.71and 3.91,respectively; all P < 0.05 ) ; whereas high-density lipoprotein cholesterol ( HDL-C) was significantly decreased in the NOW group (t =-3.90,P < 0.05 ).The prevalence of hypertension,hyperglycemia,high triglyceride,low HDL-C,dyslipidmia and hyperuricemia was increased in the NOW group in comparison with the control group ( x2 values were 6.76,5.58,14.50,11.97,10.97 and 8.76,respectively;allP< 0.05 ).LogisticregressionshowedNOWincreasedtheriskof hypertension,hyperglycemia,dyslipidmia or hyperuricemia by 2.186,2.120,2.088 or 4.175 times.After adjustment for age and gender,the risk for hyperuricemia was decreased to 3.491,but remained statistically significant higher.Conclusions NOW may be correlated with cardiovascular risk factors,and those with NOW could be at higher risk for cardiovascular diseases.
10.Clinical characteristics and homology of Acinetobacter baumannii infection in patients in a respiratory intensive care unit

Objective To study transmission characteristics of Acinetobacter baumannii (A.baumannii )clone and clinical features of infected patients in a respiratory intensive care unit (RICU),so as to guide the effective pre-vention and control of A.baumannii transmission.Methods 40 A.baumannii strains isolated from RICU patients were performed homology analysis by pulsed-field gel electrophoresis (PFGE)and cluster analysis,relationship of strains was determined,antimicrobial resistance and clinical data were analyzed.Results 40 A.baumannii strains were divided into 11 genotypes(A-K),the main epidemic strains were type A,B and E.The resistant rate to imipen-em was the lowest(70.00%),the next was levofloxacin (77.50%).The average age of 40 infected patients was 67 years old,the average length of hospital stay was 41 days,12 patients died because of invalid treatment.There was overlapping hospitalization among patients infected with type A and E strains;among patients infected with type B strain,there was no overlapping hospitalization among the last 6 and first 6 infected patients.Conclusion The main epidemic strains of A.baumannii were type A,B and E,antimicrobial resistant rate is high;the infected patients are with high average age and long length of hospital stay.It is important to reduce the transmission of A.baumannii through rational use of antimicrobial agents,strict aseptic operation,and intensified disinfection and sterilization of hospital environment and medical devices.
