1.Transvenous closure of secundum atrial septal defects with atrial Amplatzer septal ocluder

Objective Our purpose was to report Initial clinical application experience and assess the immediate and short-term effect of transcatheter closure secundum atrial septal defects (ASD) using Amplatzer atrial septal ocluder.Methods Procedures were perfomed under fluoroscopy and transesophageal echocardiography monitoring. Nine patients (3 male, 6 female) underwent attempted transcatheter closure of a ASD using the Amplatzer atrial septal ocluder device with 10F, 11F or 12F long sheath at a mediant age of 33.0?5.2 years (range 8 to 52 years) and weighed 22 kg or more. Other congenital cardiac anomanies which require surgery were excluded. The mediant diameter of ASD at its narrowest segement were mensureted with the balloon catheters was 23.3?6.2 mm (ranger 11 to 30 mm). Systolic pressures of pulmonary arteries were 24.4?5.5 mm Hg (24-46 mm Hg) with catheterization mensuration. After the procedure, TEE were perfomed immediately to find whether there any residual shunt retained. Follow-up evaluation was color flow mapping at 24 h, 1, 3 and 6-months after closure. Results Nine patients had successful device placement. Transesophageal echocardiography showed that 8 patients had complete immediate closure and one had a small residual shunt after the operation, and could get up in the next day. The complication of cerebral throumbus embolism occurred in one woman patient during the procedure. She accepted immediately thrombolysis therapy with urokinase and recovered after several days. Conclusion Transcatheter closure of secondum ASD using Amplatzer atrial septal occluder device is an effctive nonsurgical therapy method. The operation has specialities of simple, safe with a high succes rate of placement and a fine occlusion effect. Further clinical trias are underway.
2.Determination of the Relevant Substances in Tegafur Injection by HPLC

OBJECTIVE: To establish an HPLC method for the determination of the relevant substances in tegafur injection.METHODS: The determination was performed with a Kromasil C18 column with the column temperature maintained at 30 ℃,and the mobile phase was methanol-water(25∶75) at a flow rate of 1.0 mL?min-1.The UV detection wavelength was set at 282 nm and the sample size was 20 ?L.RESULTS: The relevant substances were well-separated from the chief agent,and the contents of the relevant substances in the sample were 2.35%,2.37%,and 2.24%,respectively.CONCLUSION: Compared with TLC,HPLC is more accurate and sensitive,and it is applicable for the determination of the relevant substances in tegafur injection.

6.Risk factors of recurrence of valvular atrial fibrillation after surgical radiofrequency ablation

Objective The purpose of this study was to explore the risk factors of recurrence of atrial fibrillation (AF) after surgical radiofrequency ablation in patients undergoing cardiac valve surgery.Metbods Retrospective analysis was made in 255 cardiac valve patients undergoing concomitant radiofrequency modified Maze operation from April 2010 to December 2012 in Nanjing Drum Tower Hospital.Multivariate logistic regression analysis was used to screen independent risk factors for predicting recurrence of AF.Results Excluding 8 patients died after surgery,11 cases who were junctional rhythm after ablation or received pacemaker implantation and 60 lost case,176 patients were finished follow-up.107 cases maintained stable sinus rhythm (SR),whereas other 69 cases developed AF recurrence.Multivariate logistic regression analysis found larger left atrial size,higher B-type natriuretic peptide,lower preoperative heart rate and postoperative AF rhythm before discharge were independent risk factors for predicting AF recurrence.Conclusion Larger left atrial size,higher B-type natriuretic peptide and lower preoperative heart rate before surgery can impact outcome of surgical radiofrequency ablation in patients undergoing cardiac valve surgery after following 12(4-38)months.They can be used in clinical practice to choose more suitable cardiac valve patients with AF for surgical ablation.Postoperative appearance of AF rhythm before discharge indicated a poor prognosis,which should be interfered with drugs or electrical cardioversion.
7.Spatial-time cluster analysis of distribution of schistosomiasis in Jiangling County

Objective To analyze the spatial-time distribution characteristics of schistosomiasis in Jiangling County ,Hubei Province from 2006 to 2011. Methods The surveillance data of schistosomiasis of Jiangling County from 2006 to 2011 were col-lected,and a spatial database was established. The spatial-time permutation cluster analysis was performed by SaTScan 9.1.1 to detect the spatial-time cluster areas of schistosomiasis,and a risk map was drawn by ArcGIS 10.0. Results Compared to 2006, the infection rates of human,cattle and Oncomelania hupensis snails decreased by 47.67%,93.34%and 52.41%,respectively in 2011. The space-time permutation clustering analysis of the infection rates of human,cattle and snails showed 4,3 and 4 clusters,respectively,and all the clustered areas were distributed in inner embankment areas. Conclusions From 2006 to 2011,the schistosomiasis endemic situation presents a decline trend in Jiangling County. The detected cluster areas are the im-portant areas for schistosomiasis control. The space-time permutation statistics could be used in the analysis of endemic situation of schistosomiasis.
8. Atomic absorption spectrometry for detecting metal element dissolution in angelica dahurica following different extraction procedures

Objective To apply atomic absorption spectrometry (AAS) for studying the differences in metal element dissolution (Cr, Cd. Pb and Cu) from Angelica dahurica following different extraction procedures. Methods The above four elements were extracted by water extraction and ethanol extraction methods. The water-soluble fraction and suspension fraction were separated from decocted solution by microporous filtering film; the water-soluble fraction was further divided into organic fraction and inorganic fraction by macroporous adsorption resin. The total contents of Cr, Cd. Pb and Cu and the amounts in different fractions were determined by wet digestion method and AAS. and morphological analysis was conducted. Results The extraction rates for the four elements were in the range of 25. 14%-49. 62 %. with the sample recovery rates ranging 96. 09%- 100. 43 % and the relative standard deviation (RSD) being≤ 3% (n=6). Dissolution rate of metal elements by traditional water extraction method was low. The extraction rates of Pb and Cu by ethanol extraction method were higher than those of the water extraction method. Both in water decoction and ethanol decoction. the contents of four elements in the suspended species and inorganic species were higher than those in the soluble species and organic species. Cu content in organic species in water decoction was higher than that in ethanol decoction (24. 33% vs 13. 57%). Conclusion The metal element dissolution from Angelica dahurica can be decreased by optimizing the extraction method. which finally leads to reduced toxicity.
9.Effect of Jin’s Three-needle Scalp Electroacupuncture on Cyt-c and Caspase-3 Expressions in the Cerebral Cortex in Intrauterine Distress-induced HIBDRats

ObjectiveTo investigate the effect of jin’s three-needle scalp electroacupuncture on Cyt-c and Caspase-3 expressions in the cerebral cortex in Intrauterine distress-induced HIBD rats and reveal the mechanism of its protective action on the brain. MethodThe abdomen was incised in a female SD rat at 21 days of pregnancy. Bilateral uterine horn blood vessels were tightly clamped with a hemostat for five minutes. An infant rat was then taken out by a cesarean section. A rat model of hypoxic-ischemic brain damage was confirmed by the use of a behavior test and braintissue sections. The rats were randomized into model and acupuncture groups. The model group was not needled. Acupuncture groups 1, 2, 3 and 4 began to be needled at 3, 5, 7 and 14 days after birth, respectively. Acupuncture treatment was given for seven consecutive days. The normal control group was naturally delivered, not used to make a model and not needled. All groups of rats were decapitated to take the brain tissue at 21 daysafter birth. Cyt-c and Caspase-3 expressions in the cerebral cortex were detected.ResultCyt-c optical density value decreased somewhat in the four acupuncture groups; there was a statistically significant difference between acupuncture group 1 or 2 and the modelgroup (P<0.05). Caspase-3 expression was significantly down-regulated in acupuncture group 1; there was a statistically significant difference compared with the model group (P<0.05).ConclusionThe protective effect of acupuncture beginning at 3 days after birthon the brain is related to the down-regulation of Cyt-c and Caspase-3 expressions in intrauterine distress-induced HIBD rats.

To provide a foundation for the development of rapid and specific methods for the diagnosis of rabies virus infection,anti-rabies virus monoclonal antibodies were prepared and rabies virus nucleoprotein and human rabies virus vaccine strain (PV strain) were used as immunogens to immunize 6-8 week old female BALB/c mice.Spleen cells and SP2/0 myeloma cells were fused according to conventional methods:the monoclonal cell strains obtained were selected using the indirect immunofluorescence test; this was followed by preparation of monoclonal antibody ascitic fluid; and finally,systematic identification of subclass,specificity and sensitivity was carried out.Two high potency and specific monoclonal antibodies against rabies virus were obtained and named 3B12 and 4A12,with ascitic fluid titers of 1∶8000 and 1∶10000,respectively.Both belonged to the IgG2a subclass.These strains secrete potent,stable and specific anti-rabies virus monoclonal antibodies,which makes them well suited for the development of rabies diagnosis reagents.
