1.The short-term fusion rate of osteophyte filled cervical cage evaluated by a combination of CT, MR and X-ray film

Objective To observe the short-term fusion rate of using osteophyte filled cage in ante-rior cervical disectomy and fusion. Methods From May 2007 to May 2009, 103 patients receiving anterior cervical disectomy, spine fusion by different surgeons were performed with cages containing exclusively au-tologous osteophyte collected from both anterior and posterior vertebral edges adjacent to the intervertebral space. There are 69 males, 34 females, from 25 to 77 years of age, average 53.6 years. 60 patients received one level fusion. 33 and 10 patients received two-level and three-level fusion respectively. Patients were re-viewed 3 months after operation and fusion was evaluated by X-ray film, reconstructive CT and MR. Results 1. Only 1.9% (2/103) of patients' X-ray films indicate non-fusion (internal fixation failure) while 31.7% (32/ 103) of patients' CT and 30.1% (31/103) of patients' MR indicate non-fusion or being confusing to make a diagnosis. 2. The diagnostic coherence between CT and MR as to evaluate fusion is good (kappa=0.686, P= 0.00). 3. The total fusion rate was 77.7%(80/103) while the highest fusion rate achieved by single surgeon was 90%(18/20). The total one level fusion rate was 80%(48/60) while the highest rate by single surgeon was 88.2%(15/17). The total two-level fusion rate was 78.6%(26/33) while the highest rate by single sur-geon was 90%(9/10). The total three-level fusion rate was 60%(60/100) while the highest rate by single surgeon was 100%(3/3). No statistic significant difference in fusion rate was found between any two of the three groups. Conclusion The method of using osteophyte filled cage can acquire ideal short-term fusion rate in one and two-level anterior cervical disectomy, fusion and plate fixation.
2.Analysis of high-risk factors and clinical characteristics of respiratory distress syndrome in late preterm infants

Objective To compare the characteristics of respiratory distress syndrome(RDS) between early preterm infants and late preterm(LP) infants,and to investigate the high-risk factors and clinical characteristics of RDS in LP infants.Methods A total of 240 premature infants with RDS treated in our hospital from January 2014 to January 2015 were selected and divided into 2 groups according to the gestational age(GA),143 cases in early preterm group (GA <34 weeks) and 97 cases in LP group(GA 34-36 weeks).The basic situation,high risk factors in perinatal period,clinical characteristics,treatment and prognosis in RDS premature infants were retrospectively analyzed.Results The delivery mode in the LP group was mainly dominated by cesarean section,the cesarean section rate reached 83.51%,which was significantly higher than that in the early preterm group (P<0.05).The premature occurrence rate caused by cesarean section because of premonitory uterine rupture was higher (27.84%);the treatment effect of pulmonary surfactant(PS) and noninvasive assisted ventilation therapy in the LP group was inferior to that in the early premature group,moreover the mechanical ventilation rate was higher,the duration of mechanical ventilation was longer;the incidence rates of pulmonary hypertension and intrauterine infection were higher than those in the early preterm group.Conclusion LP infants with RDS are mainly dominated by cesarean section without onset of labor process,intrauterine infection may be its risk factor,PS use is effective,but its effect is more poor than that in the early preterm group,the mechanical ventilation rate in the LP group is higher,but the prognosis is good.
3.The Clinical Value of Pulse Pressure Variation to Monitor the Fluid Responsiveness and Effects of PEEP in Ventilated Patients with Acute Lung Injury

Objective To investigate whether pulse pressure variation (△PP) reflect the effects of PEEP and fluid resuscitation (FR) on hemodynamic effects.Methods Twenty critical patients with acute lung injury was ventilated with volume control (VT =8 mL/kg,Ti/Te = 1: 2) ,and PaCO2 was kept at 35 to 45 mm Hg.PEEP was setted as 5 cm H2O and 15 cm H2O in randomized order.Hemodynamic parameters including cardiac index, pulse pressure, central venous pressure, etc.were monitered by PiCCO system.Measurements were performed after the application of 5 cm H2O PEEP (PEEPs group)and 15 cm H2O PEEP (PEEP15 group) respectively.When the PEEP-induced decrease in cardiac index (CI) was > 10%, measurements were also performed after fluid resuscitation.Results Compared with PEEPs group, CI was decreased significantly in PEEP15 group(P < 0.05), and APP was increased significantly (P < 0.05).In 14 patients whose PEEP-induced decrease in CI was > 10%, fluid resuscitation increased CI from (3.01±0.57)L · min-1· m-1to (3.62±0.68)L · min-1 · m-2(P<0.01),and decreased △PP from (17±3)% to (10±2) % (P < 0.01).PEEP15-induced decrease in CI was correlated negatively with APP on PEEP5 (r =-0.91,P < 0.01) and with the PEEP15-induced increase in △PP (r =-0.79, P < 0.01).FR-induced changes in CI correlated with APP before FR (r = 0.96, P < 0.01) and with the FR-induced decrease in APP (r =-0.95, P < 0.01).Conclusions In ventilated patients with ALI, △PP may be a simple and useful parameter in predicting and assessing the hemodynamic effects of PEEP and FR.
6.Differential diagnosis of the mixed epithelial and stromal tumor of kidney and cystic nephroma with multi-detector CT

Objective To investigate the features of the mixed epithelial and stromal tumor of kidney (MESTK) and cystic nephroma (CN) on multi-detector CT (MDCT), in order to improve the accuracy of preoperative diagnosis and differential diagnosis. Methods Based on blind-retrospective review, CT features of 6 MESTK patients and 6 CN patients were reviewed retrospectively, and compared with pathological results postoperatively. Results All the lesions were single regular multi-cystic masses. Five patients with MESTK were diagnosed as Bosniak Ⅲ and 1 as Bosniak Ⅳ. In contrast, 6 of CN were all diagnosed as Bosniak Ⅱ. Six MESTK lesions were all consisted of irregular mixture of solid and cystic components, and the average thickness of septa was 0.93 cm. Six CN lesions were entirely cystic masses without solid components, and the average thickness of septa was 0.23 cm. After contrast enhancement, the solid components of MESTK and the cystic wall of CN presented mild-to-moderate delayed enhancement. Conclusion Some signs on multi-detector CT can help to differentiate MESTK and CN, especially the presence of solid components of the tumor.
7.Cefminox Used to Treat Bacterial Liver Abscess:Its Clinical Efficacy

OBJECTIVE To observe the clinical efficacy of cefminox for the treatment of bacterial liver abscess. METHODS Totally 118 patients with bacterial liver abscess were treated with cefminox 2 g iv drip 12 h or 8 h for(5-10 d),then with cefminox 1 g in drip 12 h for 21-35 d. RESULTS The total cure rate was 89.4%,the overall efficacy rate was 97.6%,and side effect rate was 2.4%. CONCLUSIONS Cefminox is an effective antibiotic in treating bacterial liver abscess.

Vitamin D is a fat-soluble steroid hormone.Vitamin D receptor(VDR),a type of nuclear receptor,recognized by Vitamin D has many biological functions and the pathway of 1,25 (OH)2D3/VDR plays an important role in human such as regulating calcium phosphate metabolism,glucose metabolism,lipid metabolism and water metabolism.Water channel protein aquaporin(AQP) is a kind of smaller hydrophobic trans-membrahe proteins,expressed in cell membrane of renal tubules,endocrine glands and red blood cells.Aquaporin plays a key role in the regulation of water balance and metabolism.Vitamin D has a positive effect in diabetic nephropathy via inhibiting renin-angiotensin system,while aquaporin is a new biological marker of chronic kidney disease and kidney injury.Latest studies have showed that the expression of aquaporin can be regulated by vitamin D.Therefore,Vitamin D and aquaoprin play an important role in chronic kidney disease and kidney injury.
9.EFFECT OF EXOGENOUS GLUTAMATE ON COMPOUND ACTION POTENTIAL (CAP) AND COCHLEAR MICROPHONICS (CM) OF GUINEA PIG′S COCHLEA

To investigate the changes in CAP and CM recorded from the cochlear round window when glutamate solution in different concentrations(10mmol/L, 5mmol/L, 1mmol/L) was instilled into whole cochlea perilymph. The results demonstrated that there was a dependent correlation between CAP threshold shift and glutamate concentration. CAP threshold shift and amplitude could be markedly influenced by glutamate instillation. However, amplitude of CM was not obviously affected, and the I/O function of CM amplitude always maintained a nonlinear characteristic. The results suggest that glutamate has selective effects between inner hair cells and outer hair cells. High concentration glutamate might cause an increase in intracellular calcium, even as high as to induce toxicity. It could be induced by glutamate when it acts on IHC′s presynaptic autoreceptor in positive feedback manner, or on the afferent neurons, so that the synapsis is directly damaged by glutamate concentration overload in synaptic cleft.
10.Effect of Buzhong Yiqi Decoction medicated serum to the expression of PI3 K、AKT、Survivin in lung adenocarcinoma cell lines A549/DDP

Objective To observe the effect of Buzhong Yiqi Decoction medicated serum on PI3K、AKT、survivin in lung adenocarcinoma A549/DDP cells,and explore its functional mechanism.Methods Buzhong Yiqi Decoction medicated serum of middle dosage (0.576 g/mL)were prepared,and A549/DDP cells were divided into four groups:blank serum group,the best containing medicine serum group and LY294002 group (blockers group), the best containing medicine serum +LY294002 group (combination group).The protein expression of PI3K,AKT and Survivin were detected by Immunocytochemistry and Western blot.Results Compare with blank serum group,the protein expression of PI3K、AKT and Survivin in best containing medicine serum and combination group were significantly lower(P<0.05 ),and the later was more lower,but there was no significant differences between best containing medicine serum and combination group.The expression of survivin in LY294002 group were significantly lower than blank serum group(P<0.05 ).Conclusion Buzhong Yiqi Decoction can reduce the protein expression of PI3K,AKT and surviving,increase the sensitivity of A549/DDP to DDP through PI3K/AKT signal transduction pathway.
