Objective To investigate the effects of glutamine(Gln) and epidermal growth factor(EGF) on small intestinal repair in neonatal rats with necrotizing enterocolitis(NEC).Methods Forty neonatal rats of NEC model were divided into 5 groups:group A(control group),group B(NEC model),group C(NEC+Gln),group D(NEC+EGF+Gln).After NEC model of neonatal rats,all the neonatal rats were returned to their mothers.In the 4~(th) day,all the subjects were put to the death.Intestinal tissue located at the boundary of ileum and cecum was obtained to observe histological changes.Morphological changes were measured under a light microscope,and expression of proliferating cell nuclear antigen(PCNA) as an index of cell proliferation was observed using immunohistochemical staining and image analysis,expression of apoptosis was observed by TUNEL staining and image analysis.Results In group B,the histological lesions ranged was different,and was graded 3 score.In group C,D,the histological lesions ranged was graded 1 score.The number of(PCNA) in group B was lower than that in group A(P

Hypermetabolism syndrome,especially in skeletal muscles,is exceedingly common in septic patients.Many studies suggest that the activity of ubiquitin-proteasome pathway is a vital procedure for hypermetabolism of skeletal muscles of septic patients and its activation can be influenced by a variety of factors,such as proinflammatory cytokine and glucocorticoid,etc.Evidences indicate that insulin has played an important role in opsonizing the activation of ubiquitin-proteasome pathway.This paper reviews the regulation effect of insulin to the hypermetabolism of the skeletal muscles of septic patients through ubiquitin-proteasome pathway.

Objective To evaluate efficacy and clinical feasibility of 125Ⅰ combined with high intensity focused ultrasound(HIFU)therapy for hepatocellular carcinoma.Methods.The study included 10 cases of hepatocellular carcinoma(primary hepatic carcinoma,5 cases.Tumor recurrence of hepatocelluhr carcinoma after hepatecomy,5 cases).All the cases were treated with 125I combined with HIFU.Resultsl The operationWas successfully completed in all the cases.No severe post-operative complication occurred.CT acall found no displacement of 125Ⅰ seeds.CT 8can showed that reduced size of tmnor changed in different degree.The l month,3 month,6 month and l year survival cases were 8,7,5 and 2.During 3 to 18 months follow up,2 cases died within 1 month.Two cases survived more than 22 months.Conclusion Using 125Ⅰ combined with HIFU is safe,minimally invasive and effective for treatment of hepatocellular careinom.

Objective: To observe the clinical efficacy of warm needling plus tuina in treating senile functional constipation. ＜br> Methods: Ninety elderly patients with functional constipation were randomized into two groups. Forty-six patients in the treatment group were intervened by warm needling plus tuina; the other forty-four patients in the control group were intervened bySan Huang tablet. ＜br> Results: In the treatment group, 19 patients were cured, 24 cases showed improvement, 3 cases failed, and the total effective rate was 93.5%; in the control group, 16 patients were cured, 18 cases showed improvement, 10 cases failed, and the total effective rate was 77.3%. There was a significant difference in comparing the therapeutic efficacy (P＜0.05), and the therapeutic efficacy of the treatment group was higher than that of the control group. ＜br> Conclusion: Warm needling plus tuina can produce a higher therapeutic efficacy than takingSan Huang tablet in treating senile functional constipation.

Triple-negative breast cancer (TNBC) is a subgroup of breast cancers defined by a lack of the expressions of estrogen,progesterone and HER2 receptors,with more aggressive biological and clinicopathological characteristics and a close relationship with basal-like and breast cancer susceptibility gene-1 (BRCA1)-related breast cancers.TNBC is insensitive to most available hormonal or standard therapeutic agents,associated with increased risk for distant metastases and with poorer prognosis than other types of breast cancer.A deeper insight into the biology of TNBC may lead to improved therapies and better clinical outcomes of the disease.

ObjectiveTo assess the effect of balloon o f ureter through ureteroscopy for management of ureter stenosis.Met hods42 cases ureter stenosis underwent balloon dilation of ureter,2 2 being male,20 being female.The average age was 46 years.There was 5 ureteropel vic junction stenosis,8 upper ureter stenosis,5 middle part stenosis,24 lower pa rt stenosis.The length of stenosis was from 0.5 cm to 2.0 cm.Result sThe balloon dilating catheter was passed over the ureter stenosis area and was put in proper position in 39 cases.3 cases were failed(7.2%). 37 ca ses successfully underwent balloon dilation of the ureter on one session.All pat ients were followed up for 6 to 24 months(mean 14 months).The degree of hydronep hrosis decreased with no recurrence in 37 cases(88.1%).2 casese failed to gain the effect after the double-J stent was removed(4.7%). There are no serious co mplication found in all patients.ConclusionsBalloon di lation of ureter offers many advantages,such as:good result,repeatable and reduc ing the cost and hospital stay.

BACKGROUND:Human umbilical cord blood contains a large number of hemopoietic stem cels and mesenchymal stem cels. Neonate umbilical cord blood-derived stem cels show milder immunological rejection and lower immunogenicity than peripheral blood- and bone marrow-derived mesenchymal stem cels. OBJECTIVE:To investigate the feasibility of umbilical cord blood-derived CD133 cels for treatment of gestational diabetes, so as to provide novel methods for clinical treatment of gestational diabetes. METHODS:Mouse models of gestational diabetes were established by high-fat and high-sugar diet feeding. Umbilical cord blood-derived CD133 cels were sorted by flow cytometry sorting technique and then transplanted into mouse models of gestational diabetesvia the tail vein. At 7 days after cel transplantation, serum levels of fasting blood glucose, fasting insulin, total cholesterol and triacylglycerol were measured and mouse pancreatic tissue injury and repair was observed by hematoxylin-esoin staining. Mouse insulin resistance index and pancreatic islet function were analyzed using the homeostatic model assessment. RESULTS AND CONCLUSION:Umbilical cord blood-derived CD133 cels could be isolated from umbilical cord blood monocytes using flow cytometry sorting technique, with cel purity of (90.24±2.56)%. At 7 days after cel transplantation, serum levels of fasting blood glucose, fasting insulin, total cholesterol and triacylglycerol were significantly decreased, insulin resistance index was significantly decreased, and pancreatic islet function was significantly improved in mouse models of gestational diabetes (alP < 0.05). In addition, atrophy of pancreatic tissue and infiltration of inflammatory cels were reduced. These results show that umbilical cord blood-derived CD133 cel transplantation can improve the disease condition of gestational diabetes by repairing injured pancreatic tissue, decreasing insulin resistance index and improving pancreatic islet function.

Objective To investigate the clinical significance of acute insulin resistance during surgical sepsis. Methods Forty surgical patients with sepsis in experimental group and twenty patients with similar APACHEⅡ but without sepsis in experimental control group were enrolled in our study. In the experimental group, the patients were categorized to sepsis group and septic shock and MODS(SM)group according to clinical manifestations, and were also grouped according to different causes of sepsis, i.e.severe acute pancreatitis(SAP)group, intestinal fistula group and other diseases group. Determinations of fasting blood glucose, fasting plasma insulin, clinical nutritional parameters, plasma TNF-α, APACHEⅡ, sepsis-related organ failure assessment(SOFA)and severe scores were simultaneously performed at designated time points. Insulin resistance index was calculated using homeostasis model assessment(HOMA)to assess insulin sensitivity of surgical patients with sepsis. Results (1)A significant elevation of lgHOMA-IR levels was found in 84% of tests in sepsis. Peak lgHOMA-IR values increased in all patients.(2)LgHOMA-IR was significantly higher in patients with sepsis than those without sepsis from the first 24 hours(1.22±0.23 vs 0.73±0.21, P＜0.01)to the 7th day(0.46±0.32 vs 0.30±0.13, P＜0.01).(3)There was no significant difference in lgHOMA-IR among SAP, intestinal fistula and other groups at different times(P＞0.05).(4)There was significant difference in lgHOMA-IR between sepsis group and SM group during the whole septic episode(P＜0.01).During the septic episode, there were significant correlation between lgHOMA-IR and APACHEⅡ(r=0.591, P＜0.01), lgHOMA-IR and SOFA(r=0.496, P＜0.01), lgHOMA-IR and SS(r=0.553, P＜0.01), and lgHOMA-IR and TNF-α(r=0.658, P＜0.01).(5)lgHOMA-IR was independently directly correlated with LDL, prealbumin, cholesterol and triglyceride. R2 of the equation was 0.188. Conclusion There usually is insulin resistance in surgical sepsis, regardless the primary diseases. Insulin resistance may simply be a marker reflecting an underlying physiological derangement, implying indicates higher mortality. Among several clinical nutrition parameters, LDL, prealbumin, cholesterol, and triglyceride showed closer relationship with insulin resistance in patients with sepsis.
