1.The influence of dialysis treatment on insulin resistance in patients with end-stage renal disease

Objective To compare the effect of two different dialysis modalities, hemodialysis (HD) and continuous ambulatory peritoneal dialysis (CAPD) on insulin resistance in patients with adult end-stage renal disease (ESRD), and to identify the possible predictive factors for insulin resistance. Methods Fifteen non-diabetic patients with ESRD (ESRD group) were selected. Eight patients were treated with HD (HD group), and 7 patients were treated with CAPD (CAPD group). The insulin inhibition was examined by hyper insulin-euglycemic glucose clamp technique before and after dialysis treatment, and the glucose disposal rate (GDR) was used as an index of insulin sensitivity during the clamp technique. Meanwhile, 8 healthy controls were selected as control group. The biochemical parameters which might be associated with insulin resistance were determined by multiple linear regression. Results The GDR in control group was (9.93 ± 1.33) mg/(kg · min), in ESRD group was (6.44 ± 1.76) mg/(kg·min), and there was statistical difference (P<0.05). The GDR in HD group after treatment was increased from (6.53 ± 1.84) mg/(kg · min) to (9.74 ± 2.88) mg/(kg · min), and there was statistical difference (P<0.01). The GDR in CAPD group after treatment was increased from (6.35 ± 1.65) mg/(kg·min) to (8.18 ± 1.76) mg/(kg·min), and there was statistical difference (P<0.05). Multiple linear regression result showed that the levels of urea nitrogen, hematocrit and bicarbonate were significant predictive factors in insulin resistance (P<0.05). Conclusions CAPD and HD therapy can improve insulin resistance in adult patients with ESRD.
2.Prognosis evaluation of intermittent high-volume on-line hemofiltration and hemodialysis in critically ill patients with acute kidney injury

Objective To compare clinical prognosis between intermittent high-volume predilution on-line hemofiltration (HF) and standard intermittent hemodialysis (HD) in critically ill patients with acute kidney injury (AKI).Methods This was a prospective,randomized,controlled single-centre clinical study.The mortality and recovery rate of kidney function were compared in critically ill adult patients with AKI between HF group (using intermittent high-volume predilution on-line HF,50 cases) and HD group (standard intermittent HD,45 cases).The prognosis of 60 d was followed up,including 60-day all-cause mortality,in-hospital all-cause mortality and recovery of kidney function.Results There were no statistical differences in basic clinical characteristics between 2 groups (P > 0.05).The results of 60-day' follow-up showed,the mortality in HF group was 68.0% (34/50),and in HD group was 82.2% (37/45).There was statistical difference (risk ratio 0.75,95% CI 0.80-1.28,P =0.036).There were no statistical differences in the in-hospital mortality and recovery rate of kidney function (P > 0.05).There was no statistical difference in complication rate between 2 groups (P> 0.05).Conclusion The mortality is significantly lower in critically ill patients with AKI treated with intermittent high-volume predilution on-line HF,compared those treated with standard intermittent HD.

4.Neurolymphomatosis, a case report

Neurolymphomatosis(NL) is characterized by lymphomatous infiltration of the peripheral nervous system. We report a case of neurolymphomatosis(NL) which was confirmed by sural nerve biopsy. Sural nerve specimen from a 49-year-old female patient with weakness of limbs was examined with routine histochemical and immunohistochemistry staining, in which the first antibodies against CD3, CD20, CD45, CD45RO and CD68 were used. Numerous T-lymphoma cells invaded in the adipose tissue of epineurium of sural nerve. The nerve biopsy showed marked axonal degeneration of myelinated fibers. The clinical and histopathologic findings confirmed the diagnosis of neurolymphomatosis.
5.Construction and screening of human AQP1 shRNA expression vectors

Objective To construct and screen effective shRNA expression vectors targeting human AQP1 gene ,and evaluate the interference efficiency of the AQP1 shRNA recombinant plasmids ,thus provide basis for further exploration on the effect and mechanism of AQP1 gene on human breast cancer cells .Methods Four pairs of shRNA sequences targeting human AQP1 gene were designed and synthesized ,and then inserted into the GV115 vector .AQP1 shRNA and control shRNA plasmids were trans‐fected into human breast cancer MCF‐7 cells .The expression of AQP1 mRNA and protein were detected by real time PCR(RT‐PCR) and Western blot to evaluate the interfering efficiency .Results RT‐PCR demonstrated that AQP1 was expressed in human breast cancer MCF‐7 cells .Sequencing showed that the shRNA vectors targeting AQP1 were successfully constructed .48 h after the AQP1 shRNA transfection ,AQP1 mRNA and protein expression levels in MCF‐7 cells were reduced to a significant degree ,and the AQP1 shRNA 4 plasmid vector could inhibit the AQP1 most efficiently .Conclusion The AQP1 shRNA recombinant plasmids vectors were successfully constructed and can significantly inhibit the expression of AQP1 in MCF‐7 human breast cancer cells .
6.Effect of luxS mutation on the oxidative stress of Streptococcus mutans

Objective To investigate the effect of luxS inactivation on the oxidative stress of Streptococcus mutans and perform preliminary analysis of potential mechanism.Methods Strains were grown to mid-logarithmic phase and divided into three groups,one was used as control and inoculated into normal TPY medium,and the other two groups were experimental groups,and there were separately inoculated into TPY containing 58.8 mmol/L hydrogen peroxideor TPY containing 58.8 mmol/L hydrogen peroxide and 0.1 mmol/L 2,2'-dipyridyl.The survival rate of strain was calculated at 0.5,1,and 2 h.All the data were statistically analyzed.Results Compared with the control group,the survival rate of luxS mutation was always higher than standard strain at all pre-determined time inexperimental groups (P<0.05),and compared with experimental group without iron chelator,the survival rate of strains was not raised with the added of iron chelator (P>0.05).Conclusion luxS gene is involved in oxidative stress tolerance of Streptococcus mutans,and the oxidative stress tolerance is not achieved by avoiding the toxic effects of the Fenton reaction
7.Effect of low-dose remifentanil on the systemic artery tension of baby rabbit with septic shock

Objective To investigate the effect and mechanism of low-dose remifentanil(RF)on systemic artery tension of baby rabbit with septic shock.Methods Thirty-six systemic artery strips were prepared from 12 baby rabbits and randomly divided into 6 groups:control group,lipopoly-saccharide(LPS)group,RF group,LPS + RF group,NG-nitro-L-arginine(L-NNA)pretreated group,LPS + L-NNA pretreated group.Each group had six rings(n =6).The effect of low-dose RF on the systemic artery tension of baby rabbit with septic shock was observed with isolated vascular ring technique.The change was also obversed after L-NNA pretreatment.Results In control group:the relaxant rates after low-dose RF at the time point of 5 min,10 min,20 min were(18.48±3.96)%,(23.63±4.42)%,(28.33±3.73)%(P<0.05),compared with NS group.After pretreatment with L-NNA,RF-related relaxant rates of systemic arteries decreased significantly to(8.15 ± 1.01)%,(13.08 ± 1.46)%,(18.54 ±2.94)%(P < 0.05).In LPS group,low dose RF(4 μg/L)did not bring out any response to systemic arteries(P >0.05).The tension was not affected with pretreatment of L-NNA(P > 0.05).Conclusion These results suggest that low dose RF has relaxant effect on systemic arteries of baby rabbits,and NO may be involved.Low dose RF has no relaxant effect on LPS-pretreated systemic arteries of baby rabbits,and it has no association with NO.
8.Investigation and analysis in medication compliance of patients with ankylosing spondylitis

Objective To investigate the current status of medication compliance of patients with anky-losing spondylifis and analyze its related factors. Methods Seventy-four patients with ankylosing spondylitis were investigated about their general conditions,medication compliance and its related factors.The results were analyzed. Results Thirty-nine patients(52.7%) had good medication compliance while the rest 35 cases has poor medication compliance among which 24 patients with drug withdrawal exceeded 1 month.The medication compliance for patients with disease process of lower than 0.5 year,0.5 to 1.0 year,1 to 2 years and above 2 years was 82.0%,60.0%,25.0%and 31.0%.The logistic regression analysis showed that the disease process,dis-ease knowledge and media advertisement were the main influencing factor for medication compliance,while the age,sex,marital status,education background,adverse reaction,satisfaction degree with medication effect, economic conditions and depression were not major influencing factors. Conclusions About half of patients with anky-losing spondylitis had poor medication compliance,especialy with disease process of above 1 year.The main in-fluencing factors include disease process,disease knowledge and media advertisement.

10.Enhanced Effects of BoNT/A DNA Vaccines by Electric Pulses and Bupivacaine

Objective:To determine if suitable electric pulses-mediated DNA and DNA and bupivacaine complexes delivery technologies could enhance effects of botulinum neurotoxin serotype A (BoNT/A) DNA vaccines in mouse model. Methods:Vaccination of mice i.m. with plasmid DNA replicon vaccine pSCARSHc and conventional plasmid DNA vaccine pcDNASHc following electric pulses and with DNA and bupivacaine complexes. AHc-specific group antibody ELISA titers and lymphocyte proliferative responses of mice were detected and IgG1 and IgG2a isotype profiles were assayed. Results:Immune effects of DNA vaccines were enhanced following electric pulses and bupivacaine delivery. Effects of DNA vaccines following electric pulses were better than that of DNA vaccines formulated with bupivacaine,and the combined delivery technology of electric pulses and bupivacaine induced the highest level of specific antibodies and lymphocyte proliferative responses. Plasmid DNA replicon vaccine pSCARSHc induced relatively higher AHc-specific antibodies and lymphocyte proliferative responses in immunized Balb/c mice than conventional plasmid DNA vaccine pcDNASHc in these DNA delivery technologies. And vaccine pSCARSHc induced Th2/Th1-type immune responses with a general bias to Th2-type,and vaccine pcDNASHc induced Th2-type immune responses. Conclusion:Suitable electric pulses-mediated DNA and DNA and bupivacaine complexes delivery technologies could enhance effects of BoNT/A DNA vaccines in mouse model. Therefore,the methods described here potentially provide suitable strategies in developing an efficacious vaccine against botulinum neurotoxin serotype A.
