2.Effects of liver compression on intrahepatic iodine solution dissemination in rats

Objective To study the regulation of iodine solution dissemination through the portal vein duo to compression of liver tissue in SD rat models.Methods SD rats were divided into three groups:liver segment blocking group,non-liver segment blocking group and placebo group.Right anterior segments of liver were blocked by blocking ribbon before the compression of liver tissue in liver segment blocking group,the liver tissue was pressed in 20 times.The procedures were all the same with blocking groups in non-liver segment blocking group except that the liver tissue is free from blocking.Placebo group only was opened and closed the abdomens.The iodine solution was injected into the liver tissue in right anterior segments and iodine ball should be confirmed under the X-ray in every experimental rats.The severity of the iodine predations in the liver was divided into four degree:Degree A indicated the iodine ball was in situ;no predations;Predations were in right anterior segments only in degree B;Degree C was found iodine spots in other liver segments except right anterior segments;Degree D indicated iodine spread to other organs.The iodine balls were pressed under the pressure between 10 and 15 mm Hg.Results Majority of iodine balls(36/45,80%)were formed successfully in liver tissues.Compression of the iodine balls in liver tissues can cause iodine predations in the vessels in liver.The severity of the dissemination was related to the pressure and the range of the pressings.The blocking of the liver segments before the operation can reduce the predations of iodine.Conclusion Pressing of the liver tissue can caused the iodine balls disseminate in vessels,cancer cells or cause thrombus disseminate in the vessels,Which can be reduced by avoiding the compression of the liver or tumom during the operation.

Objective To establish real-time fluorescence quantitative polymerase chain reaction(RFQ-PCR) for measurement of the expression level of a proliferation-inducing ligand(APRIL)and its receptors in children with non-Hodgkin′s lymphoma(NHL).Met-hods Specific primers and TaqMan probes of APRIL and its receptors had been designed,and fluorescence of the PCR products were detected continuously during amplification.According to the standard curves created by plasmid DNA,the expression level of target genes in clinical samples had been determined using software,and these results were presented as the ratios of target genes′ mRNA to ?2 microg-luobulin(?2M)′s mRNA.Results The detection range of RFQ-PCR was between 101-109 ng/L,the coefficient of variation values for both intra-experimental and inter-experimental reproducibility ranged from 1.68% to 5.97% and 6.40% to 10.58%,respectively.The results from 22 samples showed that the expression level of APRIL,B cell maturation antigen(BCMA) and transmembrane activator and calcium modulator and cyclophilin ligand(TACI) in peripheral blood of NHL were significantly higher than those in normal children(P

Objective To measure the levels of human interleukin (IL)-32 in the serum and induced sputum of patients with chronic obstructive pulmonary disease (COPD) and investigate the possible roles of IL-32 in COPD.Methods Sixty patients with acute exacerbation of COPD ( AECOPD),60 patients with stable COPD,and 30 healthy subjects were recruited.The concentrations of IL-8,tumor necrosis factor alpha (TNF-α),and IL-32 in serum and induced sputum were measured by enzyme-linked immunosorbent assay (ELISA).The correlations among IL-32,IL-8,TNF-α,and lung functions were investigated. The data were analyzed using a statistical software package (SPSS 13.0).Variables were compared with one-way ANOVA,and correlations among variables were analyzed using Pearson's correlation coefficient or Spearman's correlation coefficient.Results The serum IL-32 level was significantly higher in AECOPD patients [(175 ± 88) ng/L] than in healthy subjects [ (59 ± 21 ) ng/L] and in stable COPD patients [ (89 ± 34) ng/L] (P < 0.05) ; the serum IL-32 level was also significantly higher in stable COPD patients than in healthy subjects (P < 0.05).The sputum IL-32 level was significantly higher in AECOPD patients [ ( 163 ± 117) ng/L] than in healthy subjects [ ( 75 ± 38 ) ng/L] and stable COPD patients [ ( 108 ± 63 )ng/L] (P <0.05); the sputum IL-32 level was also significantly higher in stable COPD patients than in healthy subjects ( P < 0.05 ).The sputum IL-32 level in AECOPD patients was positively correlated with the sputum IL-8 and TNF-α levels (r =0.49 and 0.53,respectively) (P <0.01 ).The sputum IL-32 level in AECOPD patients was negatively correlated with FEV1 predicted values,FEV1/FVC,and PaO2 (r =-0.44to -0.33) (P < 0.01 ).The serum IL-32 level in AECOPD patients was positively correlated with the serum IL-8 and TNF-o levels (r =0.45 and 0.61,respectively) (P < 0.01 ).The serum IL-32 level in AECOPD patients was negatively correlated with FEV1 predicted values,FEV1/FVC,and PaO2 (r =-0.46to - 0.29) ( P < 0.01 ).Conclusions IL-32 may be involved in the pathogenesis of airway inflammation in COPD.IL-32 may be a useful marker of acute exacerbation of COPD.

Objective To explore the therapeutic effect of electroencephalograph(EEG)biofeedback on clinical symptoms and cognitive function of children with attention deficit hyperactivity disorder(ADHD).Methods Twenty children aged(10.4?2.2)years old who were diagnosed as ADHD according to American Diagnostic and Statistical Manual of mental disorders the 4th ed received EEG biofeedback treatment.The treatment was administered by Autogenic A620 machine,? wave(4-8 Hz)was suppressed and sensory motor rate wave was strengthened(12-15 Hz)at the same time.The treatment was total 40 times.Before and after treatment,Conner's Behavior Rating Scale-parent questionnaire was administered to assess the clinical symptoms,wechsler memory scale(WMS)to assess the long-term memory,short-term memory and instantaneous memory,number cancellation test and stroop test were used to assess the cognitive process speed and selective attention,and continuous performance test(CPT)was administered to assess the attention and impulsiveness.The children in healthy control group who compared with ADHD children by one to one came from the primary or middle school near the hospital,the age difference between ADHD child and healthy control group was within 6 months.All the measurements were administered to the healthy control group.Results After the EEG biofeedback treatment,the score of behavior factor in Conners Scale decreased significantly(P0.05).Conclusions Forty sessions EEG biofeedback can improve clinical symptoms,attention,memory,selective attention and cognitive process speed of ADHD children significantly,some indicators even become normal.
6.Characteristics of the occurrence of silicosis in the workers exposed to uranium dust.

OBJECTIVETo understand the laws and characteristics of silicosis incidence of the workers exposed to the uranium dust from 1956 to 2002.
METHODSThe length of employment, age of occurrence of silicosis, and its duration and death age were compared between the uranium miners and uranium geological prospecting teams, and also between the miners exposed to uranium dust and those exposed to mixed uranium dust by single factor analysis method.
RESULTSWith time going on, the length of employment, age of occurrence of disease and its duration and death age were prolonged respectively in the workers exposed to uranium dust. The length of employment and age of occurrence of disease in uranium geological prospecting teams [(10.15 +/- 5.95) and (40.60 +/- 9.86) years respectively] were shorter than those in uranium miners [(14.23 +/- 8.12) and (41.38 +/- 10.98) years], but the duration (P(50)) and death age were longer [(14.29 years vs 12.52 years), (53.69 +/- 10.04) years vs (51.45 +/- 10.85) years respectively]. The length of employment and age of occurrence of disease in those exposed to uranium dust only [(11.78 +/- 8.06) and (38.04 +/- 9.89) years] were shorter than those exposed to mixed uranium dust [(12.74 +/- 6.29) years, (41.40 +/- 10.67) years], but the duration (P(50)) and death age were longer than the mixed uranium dust ones [(14.59 years vs 13.20 years), (53.93 +/- 10.60) years vs (51.82 +/- 10.20) years].
CONCLUSIONThe difference in the occurrence of silicosis in the workers exposed to uranium dust may be attributed to the physical and chemical character of the uranium dust and the different working circumstance.

7.Effect of EGb and quercetin on culture neonatal rat cardiomyocytes hypertrophy and mechanism.

AIMTo investigate the effect of extract of ginkgo biloba (EGb) and quercetin (Que) on the hypertrophic response induced by angiotensin II (Ang II) in the primary culture of neonatal rat cardiomyocytes and its mechanism.
METHODSTotal protein content of cardiomyocytes was measured by lowry's method. Superoxide dismutase (SOD) activity and malondialdehyde (MDA) content were measured by SOD and MDA assay kits. The expression of phospho-ERK1/2, phospho-JNK and phospho-P38 were detected by Western blot. The expression of c-fos mRNA was checked by RT-PCR.
RESULTS(1) The total protein content and cell size of cardiomyocytes increased significantly after Ang II treatment, EGb and Oue inhibited these effects of Ang II. (2) EGb and Que were able to enhance the SOD activity and reduce the production of MDA. (3) Ang II significantly activated ERK1/2, JNK and P38, only JNK activation was inhibited by Que and DPI but not ERK1/2 and P38 activation. (4) EGb, Que, Captopril and DPI all decreased Ang II-stimulated early response gent c-fos mnRNA expression.
CONCLUSIONEGb and Que could inhibit AngII-induced cardiomyocyte hypertrophy through a ROS-dependent pathway, the effect of Que might be related to the JNK and c-fos cascade.

8.Extract of Ginkgo biloba and quercetin inhibit angiotensin-II induced hypertrophy in cultured neonatal rat cardiomyocytes.

OBJECTIVETo investigate the effect and related mechanisms of extract of ginkgo biloba (EGb) and quercetin (Que) on angiotensin II (AngII) induced hypertrophy in the primary cultured neonatal rat cardiomyocytes.
METHODSPrimary cultured neonatal rat cardiomyocytes were treated with placebo (control), AngII (10(-6) mol/L), AngII + EGb (40 microg/ml), AngII + Que (4 microg/ml), AngII + captopril (10(-5) mol/L) and AngII + DPI [diphenylene iodonium chloride, NADPH inhibitor (10 micromol/L)] respectively. Total protein content of cardiomyocytes, cardiomyocytes size, superoxide dismutase (SOD) activity and malondialdehyde (MDA) content were measured. The expression of phospho-Extracellular-signal regulated kinase 1/2, phospho-c-Jun N-terminal kinase and phospho-P38 were detected by Western blot.
RESULTSThe total protein content and cell size of cardiomyocytes increased significantly in AngII treated cells and these effects could be blocked by EGb and Que. EGb and Que also enhanced the SOD activity and reduced the production of MDA. AngII significantly activated ERK1/2, JNK and P38 expressions and only JNK activation could be inhibited by Que and DPI.
CONCLUSIONEGb and Que can inhibit AngII-induced cardiomyocyte hypertrophy through a ROS-dependent pathway. Que could also block the JNK activation induced by AngII.

9.Changes in serum levels of antinuclear antibody, anti-double-stranded DNA antibody and anti-extractable nuclear antigens antibody before and after anti-tumor necrosis factor-α therapy in psoriatic patients

Objective To investigate changes in serum levels of antinuclear antibody(ANA), anti?double?stranded DNA(dsDNA)antibody and anti?extractable nuclear antigen(ENA)antibody before and after anti?tumor necrosis factor?α(TNF?α)therapy in psoriatic patients. Methods Clinical data obtained from 32 patients with psoriasis were analyzed retrospectively. Of the 32 patients, 13 received intravenous injection of 5 mg/Kg infliximab at week 0, 2, 6 for 3 sessions, then once every 8 weeks(infliximab group), while other 19 received subcutaneous injection of 25 mg etanercept twice every week(etanercept group). The treatments in the 2 groups both lasted more than 3 months. Serum levels of ANA, anti?dsDNA antibody and anti?ENA antibody and changes of clinical symptoms were detected and observed respectively before each treatment in the infliximab group, as well as every 3- 6 months in the etanercept group. The 75%reduction in psoriasis area and severity index(PASI75)and disease activity score of 28 joints(DAS28) were used to evaluate clinical efficacy. Serum levels of ANA, anti?dsDNA antibody and anti?ENA antibody were measured by indirect immunofluorescence(IIF)assay, Western blot analysis combined with enzyme?linked immunosorbent assay(ELISA), and Western blot analysis, respectively. Results After 3?month treatment, the 32 patients achieved clinical remission to different extents. Of 32 patients receiving anti?TNF?αtherapy, 7(21.9%)developed new autoantibodies. Concretely speaking, 4 patients in the infliximab group developed autoantibodies in 8.3 ± 5.1 months, including 3 cases positive for ANA and 3 for anti?ENA antibody. Three patients in the etanercept group developed autoantibodies in 9.0 ± 3.0 months, including 3 cases positive for ANA and 1 for anti?ENA antibody. Conclusion Partial patients with psoriasis may develop autoantibodies after anti?TNF?αtherapy.
10.The clinical observation of ultra-sound guided lumbar plexus and sciatic plexus nerve block combined with general anesthesia on elderly patients undergoing hip replacement surgery

Objective To observe the clinical effects of ultra-sound guided lumbar plexus and sciatic plexus nerve block combined with general anesthesia on elderly patients undergoing hip replace-ment surgery.Methods Eighty elderly patients scheduled for unilateral hip replacement surgery were randomly divided to lumbar plexus and sciatic plexus nerve block combined with general anesthesia group (group N)and general anesthesia group (group G).The airways of all the patients were con-trolled by laryngeal mask airway (LMA)in both groups.The bispectral index (BIS)of patients in both groups was maintained in the range of 45-55.The dosage of sulfentanyl,visual analogue scale (VAS)at 2,4,8 and 24 h after surgery,and the dosage of patient controlled analgesia (PCA)drugs were recorded.The period from end of surgery to extubation,off-bed activity and discharged from hospital,mortality in 30 days after surgery were recorded.Meanwhile,postoperative delirium (POD) and postoperative cognitive dysfunction (POCD),and severe cardiovascular and pulmonary complica-tions were evaluated.Results No patient died in 30 days after surgery in both two groups.One patient suffered from severe pulmonary infection in group G.The dosage of sulfentanyl of group N was less than the value of group G (P <0.05),the periods from end of surgery to extubation,off-bed activity and discharged from hospital of group N were shorter than those in group G (P <0.05 ).The VAS scores and incidence of POD and POCD in group N were lower than those in group G (P < 0.01 ). Conclusion In elderly patients undergoing hip replacement surgery,ultrasound-guided lumbar plexus and sciatic plexus nerve block combined with general anesthesia could help reduce usage of opioids during operation,offer better analgesia effect,shorten the period of off-bed activity and discharged from hospital and reduce incidence of POD and POCD in elderly patients.
