Objective To evaluate the efficacy of interventional therapy for advanced pancreatic carcinoma. Methods 33 cases of advanced pancreatic carcinoma accepted interventional therapy from April 2005 were retrospectively analyzed. All patients were unoperable and accepted one or more times of celiac and superior mesenteric arterial chemotheraputics perfusion with dosage of 2:1. The embolization was further introduced with the addition of liver invasion. The repetition interval was kept at 6 weeks with no severe complications. Results The one with follow-up CT imagings showed obvious decrease of the lesion size, together with release or disappearance of the sensation of abdominal pain and abdominal distention. The life span prolonged with average survival of 13 months, including the longest of 22 months and the life quality improved. Conclusions The interventional therapy could be the first method of choice in the management of advanced pancreatic carcinoma.

AIM:To elucidate the correlation between the expression of aldehyde dehydrogenase 1 (ALDH1)/ATP-binding cassette subfaminly G member 2 ( ABCG2 ) and microvessel density ( MVD ) in epithelial ovarian cancer ( EOC) .METHODS:In 198 specimens of EOC and 60 specimens of ovarian benign epithelial tumor tissues , the protein expression of ALDH1/ABCG2 and CD105 ( microvessel marker ) was detected by immunohistochemical staining .RE-SULTS:The positive rates of ALDH1 and ABCG2 in the EOC were 64.1%and 61.6%, respectively , while the positive rates in benign epithelial tumor tissues were 8.3%and 6.7%, respectively , and there were significant differences between them (P<0.05).In EOC and benign epithelial tumor tissues , the MVD were 22.6 ±9.7 and 5.03 ±3.35, respectively, and the difference was also significant (P<0.05).The expression of ALDH1 and ABCG2 in EOC was significantly related to differentiation, FIGO stage, and abdominal organ and lymph node metastasis (P<0.05).MVD had correlation with dif-ferentiation, FIGO stage, ascite, and abdominal organ and lymph node metastasis (P<0.05).MVD had positive correla-tion with the expression of ALDH1 and ABCG2 (P<0.01).There was also a positive correlation between the expression of ALDH1 and ABCG2 ( P<0.01) .Over-expression of ALDH1/ABCG2 and MVD≥23 were related to the poor prognosis . The survival rates in ALDH1/ABCG2 positive and MVD≥23 groups were significantly lower than those in ALDH 1/ABCG2 negative and MVD<23 groups (P<0.05).The FIGO stage, the expression of ALDH1/ABCG2 and MVD were indepen-dent prognosis factors of EOC ( P<0.05 ) .CONCLUSION: The results suggest that the expression of ALDH 1/ABCG2 and MVD in EOC are related to differentiation , lymph node metastasis , clinical stage and prognosis .Combined detection of these indexes may play an important role in predicting the progression and prognosis of EOC .

Objective To explore the effect of gastrin_(17) on E cadherin/?-catenin complex,a down- stream effector of FAK pathway,in colonic carcinoma cell line CoLo320WT.Methods pCR3.1/GR plasmid expressing gastrin receptor CCK 2R was transfected into colonic carcinoma cell line CoLo320 by Lipofectamine~(TM) 2000 and expressing stably CCK 2R clones was screened by G418.The expression levels of gastrin receptor of Coi.o320 and the transfected cell line Colo320WT were assayed by RT PCR. CoLo320WT cells were treated by 10~(-8)mmol/L gastrin_(17) at distinct time points (0 hr,1 hr,6 hr,12 hr, 24 hr,48 hr),whilst treated by 10_(-6) mmol/L L365,260 (gastrin_(17) receptor blocker) simultaneously for 30 minutes and then treated by gastrin_(17) again for 12 hr.Expression levels of phosphorylated FAK Tyr397 and total FAK in CoLo320WT under gastrin_(17)intervention were detected hy Western blot.Ex- pression levels of E-cadherin and?-catenin complex in TX-100 solution fraction and TX-100 insolution fraction of CoLo320WT cells were detected by coimmunoprecipitation and Western blot.Distribution of E-cadherin and?-catenin in CoLoWT320 were observed by immunocytochemistry. Results Phosphoryla- ted FAK Tyr397 expression in CoLo320WT cells increased in time dependent fashion under gastrin_(17) intervention and peaked at 12 hour after intervention, while decreased by L365,260 inhibition. But gas trine_(17) had no effect on total FAK in CoLo320WT cells.Expresion levels of E-cadherin and?-catenin com- plex in TX-100 solution fraction were decreased apparently, but increased again after L365,260 block- ing.On the contrary, the expression levels of E-cadherin and?-eatenin complex in TX-100 solution frac tion differed from that in TX-100 solution.Cytoehemistry observation had revealed that E-cadherin and?-catenin transferred from cell membranes into cndochylemas, nuclei and cytoskeleton under gastrin_(17) in- tervention. Conclusions Gastrin_(17)affected significantly the distribution of E cadherin/?-catenin complex in CoLo320WT by phosphorating FAK Tyr397 and activating FAK pathway when it bound to its recep- tor CCK-2,therefore promoted invasion and metastasis of CoLo320WT.

Objective To evaluate the protective effect on lung by using continuous pulmonary artery perfusion with oxygenated blood and L-arginine during cardiopulmonary bypass(CPB).Methods Forty five cases received mitral valve replacement were randomly divided into 3 groups and each group involved 15 cases. Group I(control group), patients received routine procedure of CPB. Proup Ⅱ, patients received rcontinuous pulmonary artery perfusion with oxygenated blood. Group Ⅲ,continuous pulmonary artery perfusion with oxygenated blood containing L-arginine (200 mg/kg) (n=15). All cases received routine procedure of CPB and continuously infused from the root of pulmonary artery until releasing aortaoaic clamp. Blood samples were collected from the radial artery respectively at the following time points:after the induction of anaesthesia, 1 hour after opening of aorta, 0, 6, 12, 24 hours after patients being taken back to ICU. ELISA test was used to detected the expression of tmmor necrosis factor-α(TNF-α), interleukin-6 (IL-6) and interleukin-10 (IL-10). Lung tissue samples (1.0 cm ×1.0 cm×1.0 cm) were obtained from right lower lobe. The pathologicl changes of lung tissues were observed under light mi-croscope by using HE staining. Results at each time points, the expression of TNF-α, IL-6 in group Ⅱ and group Ⅲ weresignificantly lower than that in group Ⅰ (P<0.05). The level of TNF-α, IL-6 in group Ⅲ were lower than in group Ⅱ(P<0.05). However, the expression of IL-10 in group Ⅱ and group Ⅲ were higher than in group Ⅰ, and the level of IL-10 in group Ⅲ were higher than that in group Ⅱ(P<0.05). In the group Ⅰ: HE staining showed marked pulmonary interstitial edema, intra-alveolus neutrophilic granulocyte exudation with karyorrhexis. In the group Ⅱ, light capillary vessel hyperaemia and pulmonary interstitial lymphocyte exudation were detected. Nearly normal lung tissue were observed in group Ⅲ. Conclusion Continuous pulmonary artery perfusion with oxygenated blood and L-arginine could inhibit the synthesis of inflammatory factors significantly and increase the releasing of anti-inflammatory factors during CPB. Therefore, it may reduces pulmonary inflammatory reaction and have protective effects on lung tissue.
