AlM: To evaluate the application of phacoemulsification of different nucleus density using ozil and traditional mode.METHODS: A total of 89 eyes (72 patients ) ( visual acuity was of 0. 6 and above after 1mo follow - up) of different nucleus density level (LOCS Ⅱ criteria grade Ⅲ 46 eyes, grade Ⅳ and more 43 eyes ) were randomly assigned into 2 groups: ozil group (group A), grade Ⅲ 22 eyes (torsional energy 80% lP on);grade Ⅳ and more 17 eyes (torsional energy 100% lP on); Traditional mode group(group B), grade Ⅲ 24 eyes (energy 50% ), grade Ⅳ and more 26 eyes (energy 60% ~ 70% ) . All surgeries were performed by the same experienced surgeon,who use the chop to split the nucleus in the application of phacoemulsification. lntraoperative parameters were total equivalent pawer ( TEP ), cumulative dissipated energy ( CDE ) and effective phaco time ( EPT ) and surgical complications. The effectiveness of the two modes in dealing with hard - core cataract phacoemulsification were compared.RESULTS: GradeⅢ nucleus dealing: TEP of ozil group was significantly higher than that of the traditional mode group [(24.58±7.78)% vs (13.84±1.97) %]and EPT of ozil group was significantly lower than that of the traditional mode group (50. 59±14. 73 s vs 60. 19±9. 04 s, P<0. 05). CDE showed no difference between two groups [(13.12±6.03)% vs (13.38±2.85)]. Grade Ⅳ and more nucleus dealing: CDE [( 34. 10 ± 13. 48 )%] and EPT (104. 64±32. 4s) of the ozil group was higher than CDE [(30. 31 ± 13. 48)%] and EPT (93. 01 ± 41. 01s) of the traditional mode group, but there were no difference between two groups. Obstacles in the needle of phacoemulsification surgery: ozil group 4/17, traditional mode group 2/26 (χ2=2. 16, P=0. 14).CONCLUSlON: Bothozil and traditional mode can deal with all kinds of nucleus effectively and safely. Ozil mode is more efficacy and quick deal in gradeⅢnucleus. With the increase of nucleus hardness, the traditional mode still have the advantage of high efficiency and no obstacle to dealing patients with grade Ⅳ and more nucleus. Choose according to different nuclear hardness ultrasonic model can improve the operation efficiency and security.

OBJECTIVE:To establish HPLC fingerprint for the leaves of Camptotheca acuminante in Guizhou.METHODS:HPLC method was performed.The determination was performed on Gemini-NX C18 column with mobile phase consisted of acetonitrile-0.2％ phosphoric acid (gradient elution) at the flow rate of 1.0 mL/min.The detection wavelength was set at 370 ran,and the column temperature maintained at 30 2.The sample size was 10 μtL.Using sorbitol as a reference,HPLC fingerprints of 14 batches of the leaves of C.acuminante were determined.The chromatographic fingerprint was analyzed with Similarity Evaluation System for Chromatographic Fingerprint of TCM (2004 A) in terms of common peak indentification,similarity evaluation and cluster analysis.RESULTS:There were 10 common peaks in HPLC fingerprints for 14 batches of the leaves of C.acuminate.And the similarity of 13 batches of the leaves of C.acuminate was greater than 0.90,and that of another one was less than 0.90.The leaves of C.acuminate were classified into 3 groups.CONCLUSIONS:The established fingerprint can provide reference for identification and quality evaluation of the leaves of C.acuminate.

We describe two cases in which a forceps imprintdeveloped in the AcrySof ReSTOR IOL optic whileinserting these IOLs into the cartridge with straightclamping forceps. In case 1 ,the AcrySof ReSTOR IOL wasexplanted and observed under scanning electronmicroscopy (SEM). The SEM showed that the stepdesign of ReSTOR Multifocal IOL was well maintained. Incase 2, visual acuity, contrast sensitivity and wavefrontmeasurements were performed and no specific changeswere found. Strong evidence does not exist that suggeststhe on-axis forceps imprint can significantly compromisevisual acuity.

OBJECTIVETo study the effects of Citalopram on the mRNA expression of bax and bel-2 in frontal cortical neurons and on cell apoptosis of rats after stress.

METHODSTwenty-four healthy male SD rats were randomly divided into three groups (n = 8). The control group did no receive any treatment, the stress group was subject to stress and given normal saline and experimental group was given Citalopram irrigation stomach after stress. Rats were forced to swim to establish chronic stress model (15 min/d, 4 weeks), bax, bcl-2 mRNA expression were tested by in situ hybridization technique (ISH), TUNEL assay was used to determine cell apoptosis, Nikon image analysis software were used to measure the number of positive cells in each index.

RESULTSCompared with the control group, the stress group showed a larger number of bax mRNA expressing cells( P < 0.01), a smaller number of bcl-2 mRNA expressing cells (P < 0.01), and the staining intensity of positive cells was significantly reduced( P < 0.01). Compared with the stress group, the experiment group showed more reduced number of bax mRNA positive cells( P < 0.01) and significantly increased bcl-2 mRNA positive cells( P < 0.05), a small amount of positive cells were found, compared with that in the stress group, nuclear condensation in the experimental group was reduced significantly and the staining was obviously weaker( P < 0.01).

CONCLUSIONCitalopram significantly antagonizes bax mRNA and potentiatesbcl-2 mRNA protein expression and inhibits apoptosis of rat prefrontal cortical neurons caused by chronic stress, which might be one possible mechanism of Citalopram for prevention and treatment of psychosis caused by chronic stress.

Objective This study aimed to establish a prediction system of lymph node metastasis of gastric cancer patients,to facilitate guiding the treatment of patients with gastric cancer.Method s We analyzed 255 cases of gastric cancer from January 2005 to December 2009 in Beijing Friendship Hospital.They all had surgery or palliative gastrectomy and then examined pathological lymph node metastasis.Their gender,age,preoperative weight loss,anemia,pyloric obstruction,chronic disease history,family history,tumor location,tumor size,higher preoperative CEA,preoperative tumor markers (CEA,AFP,CA199,CA125)elevationed (one or more),preoperative choline esterase,preoperative albumin,preoperative hemoglobin,preoperative platelet and preoperative urinary protein were made Logistic regression analysis.Results The combination of preoperative tumor size,higher tumor markers (CEA,AFP,CA199,CA125) in one or more was possible for lymph node metastasis prediction.The area under ROC curve was about 100%,Showing high discriminant ability.Conclusions The tumor size and preoperative tumor markers (CEA,AFP,CA199,CA125) are important predictive parameters for lymph node metastasis of gastric cancer.Tumor size combined with the preoperative tumor markers (CEA,AFP,CA199,CA125) predicting lymph node metastasis can help us to carry out other work,such as neoadjuvant therapy,etc.

Objective To systematically review the efficacy and adverse events of sevoflurane or propofol combined anesthesia for induction of general anesthesia. Methods We searched the PubMed, OVID, EMBASE,Cochrane Library and Chinese Biomedical Database (CBM) for studies on efficacy of sevoflurane versus propofol for anesthesia induction. The quality of the studies was evaluated by the method recommended by Cochrane Collaboration. Meta-analysis was conducted using the Cochrane Collaboration's RevMan 4.2.8 software. Results Thirteen prospective randomized controlled trials involving 968 patients were included in our meta-analysis. The results showed that the time from onset of induction to loss of consciousness and the time for induction were significantly longer, the incidence of respiratory depression lower in S group than in P group. There were no significant differences in the incidence of adverse events during induction and success rate of inserting LMA at first attempt between the 2 groups. The incidence of adverse events with LMA insertion was significantly lower in S group than in P group. Conclusion Propofol combined anesthesia is suitable for rapid induction of anesthesia, and sevoflurane combined anesthesia is indicated for slow induction of anesthesia in patients with potential respiratory difficulty and for LMA insertion.

Objective To investigate the condition of exercise, medicine compliance and the rehabilitation of patients with arteriosclerosis obliterans (ASO) in lower extremity after the surgery, and also to study the influence of self-efficiency and social support to their rehabilitation. Methods The questionnaire survey was performed in 102 patients who had surgical or interventional therapy of arteriosclerosis obliterans in the lower extremity in our hospital, investigated the general information of the patients, their knowledge of postoperative rehabilitation, medicine compliance, exercise condition, general self-efficiency and social support. Results The postoperative time for the patients with arteriosclerosis obliterans was 2 to 22months, the knowledge rate for postoperative rehabilitation was 7.8%-46.1%.The Morisky score for the patients of taking medicine for the treatment of vascular disease was 2.0,and there was only 6 patients(5.9%)with good medicine compliance. There were 22 patients(21.0%) had completed the ambulation training according to the guidelines, and there were 25 patients(24.5%) had completed the Buerger training. The self-efficiency score of the patients was 2.78 ± 0.76, and the gross score of social support was 32.38 ± 7.89. Spearman correlation analysis shows that the Morisky score for the patients of taking medicine for the treatment of vascular disease was in positive correlation with self-efficiency (r=0.289, P=0.003), but in negative correlation with social support (r=–0.286, P=0.006). Conclusions Most of the patients with arteriosclerosis obliterans have poor medicine compliance, and do not do exercise as required by the guidelines. Patients with high self-efficiency demonstrates medicine noncompliance, however the enhancement of social support will help patients improve their medicine compliance.
