1.Situation,characteristics and challenges of traditional medicine cooperation under multilateral mechanisms

Multilateral mechanisms are important platforms and vehicles for cooperation in traditional medicine.Relying on multilateral platforms,China is able to enhance the international influence of the domestic traditional Chinese medicine and form synergies with other member countries to enhance regional or cross-regional radiation effects in order to promote sustainable development of traditional medicine around the world.This paper focuses on China's participation in major multilateral health cooperation mechanisms,including global initiatives(WHO-led global cooperation framework,the Belt and Road Initiative),regional multilateral mechanisms(ASEAN countries,China-Japan-Korea Health Cooperation Mechanism,Shanghai Cooperation Organization(SCO),The Greater Mekong Subregion(GMS)),and cross-regional multilateral mechanisms(BRICS,Forum On China-Africa Cooperation(FOCAC),G20),and sorts out the cooperation policies and actions in traditional medicine under different mechanisms,to analyze the characteristics and challenges of the existing mechanisms in traditional medicine cooperation,and to provide evidence for China's promotion on multilateral cooperation in traditional medicine.
2.Application of mutated inbred animal models in the experimental teaching of medical genetics

Objective Mutated inbred animal model is introduced to the practical course of genetic diagnosis in the hope that medical students are able to apply what they have learned to clinical cases, based on a deep understanding of principle and technology on gene mutation detection. Methods We integrated DNA extraction, polymerase chain reaction, agarose gel electrophoresis, and gel imaging analysis into a comprehensive experiment and arranged 4-year-programme undergraduates majoring in preclinical medical sciences to conduct it with the purpose of investigating the internal relations between phenotype and genotype in a hairless Uncv mouse model. Subsequently, the questionnaire aimed at evaluating learning effect on the part of students was handed out and their feedbacks were analyzed. Results More than 90% of respondents are satisfied with the general learning effect. Especially, 98. 7% of students support the enhancing effect of the new teaching mode on their research skills and 96% consider the practical course helpful to their problem-solving ability. Conclusions The introduction of mutated inbred animal model to the practical system of molecular diagnostics proves beneficial to boost students' learning effect and scientific research quality. Our practice also provokes thoughts on the further utilization of animal models in teaching system of medical sciences.
3.Study on the role of microRNA-195 in transforming growth factor-β/Smad signal pathway and the fibrosis mechanism

Objective To explore the effect of miRNA-195 (miR-195) on Smad7 expression and rat hepatic stellate cells line (HSC-T6) activation incuced by transforming growth factor-β1 (TGF-β1).Methods HSC-T6 were cultured in vitro,with 5 ng · mL-1 TGF-β1 as an injury factor simulating the rats liver fibrosis models.The cells divided into six groups:control group,model group (TGF-β1 group),miR-195 mimic experimental group,miR-195.mimic NC experimental group,miR-195 inhibitor experimental group and miR-195 inhibitor NC experimental group.After the HSC-T6 treated with 5 ng · mL-1 TGF-β1for 24,48,72 h,the mRNA expression of miR-195,Smad7,and α-SMA was detected by Real-time PCR.The protein expression of Smad7 was detected by Western blot.Results Under the induced by TGF-β1,the expression of miR-195 increased gradually (P < 0.01),the expression of Smad7 showed a decreasing trend (P < 0.05),and the expression of oα-SMA was gradually upregulated (P < 0.01) with time.Compared with the model group,miR-195 mimic could promote activation of HSC-T6 induced by TGF-β1,and increase miR-195,α-SMA mRNA expression (all P <0.01),as well as reduce Smad7 mRNA and protein expression (all P <0.01);while miR-195 inhibitor could inhibit activation of HSC-T6 induced by TGF-β1,decrease miR-195,α-SMA mRNA expression (all P < 0.01),as well as upregulate Smad7 mRNA and protein expression (all P < 0.01).Conclusion miR-195 is involved in pro-activation of HSC-T6 by inhibiting Smad7 expression.

5.Study on prevalence of and influencing factors of mild cognitive impairment among elderly people in communities of Nanning

Objective To investigate the prevalence and influencing factors of mild cognitive impairment (MCI) among elderly people in communities of Nanning, so as to provide scientific basis for the formulation of MCI early intervention measures. Methods A total of sample of 3 000 elderly people aged 60 or above living in Nanning area were sampled out using the method of cluster random sampling from three communities in Nanning. The Beijing version of the montreal cognitive assessment (MOCA-BJ) was administered by face to face interview. Results 833 MCI patients were detected. The standardized prevalence rate of MCI was 27.27% by the age composition of the population in Nanning in 2010. Single factor analysis showed that there were significant differences(all P<0.05)in the prevalence of MCI in different groups by age, education, occupation, exercise time, sleep time, number of reading, community activities and housework. Multivariate analysis showed that old age, primary education level, occupation of farmer/migrant worker and enterprise personnel/worker were the risk factors of MCI, adequate sleep time, moderate exercise time and reading times were the protective factors of MCI (all P<0.05). Conclusions The prevalence rate of MCI is high among the elderly in Nanning. The elderly with advanced age, primary school education, occupation as farmer/peasant-worker and enterprise personnel/workerare at high risk of developing MCI. Effective measures should be taken as soon as possible to prevent the occurrence and development of MCI.
6.Dynamic investigation on the co-infection status of two pathogens in ticks from tourist point in Heilongjiang province

Objective To monitor the co-infection status of Borrelia burgdorferi sensu lato (R.b.s.1) and spotted fever group Rickettsia (SFGR) in tourist areas of Heilongjiang province.Methods Polymerase chain reaction (PCR) was used to detect the 5S-23S rRNA intergenic spacer of B.b.s.1 and ompA of SFGR in ticks,dynamically collected from tourist areas of Heilongjiang province in 2010.Amplification products from positive ticks were sequenced,and phylogenetic analysis was conducted by Mega 5.0 software package.Results 849 ticks were collected from two tourist points,with the dominant ticks in Tiger Mountain and Jingpo Lake were Ixodes persulcatus and Haemaphysalis concinna.Regarding the Ixodes persulcatus from Tiger Mountain,the infection rates of B.b.s.1 and SFGR were 26.15% and 10.05%.The infection rate of SFGR was 13.33% in Haemaphysalis concinna and the B.b.s.1 was tndiscovered in the same ticks from Jingpo Lake.However,the co-infection could only be detected in Ixodes persulcatus of both tourist areas.Surveillance data showed that the major ticks were more likely to be appeared in July at Tiger Mountain and in June at Jingpo Lake.Data from the sequence analysis on B.b.s.1 showed that the B.b.s.1 in tourist areas could be classified into three different genotypes,other than B.garinii and B.afzelii.We first detected B.valaisiana-like group genotype in northeast of China.Results from the sequence analysis of SFGR positive products showed that the two DNA sequences of newly detected agents were completely the same as Rickettsia sp.HL-93 which was detected in Hulin and Rickettsia sp.H820 found in northeast,China.Conclusion The co-infection of B.b.s.1 and SFGR was detected in ticks from the tourist areas of Heilongjiang province,and data from the sequencing of specific fragment showed that various kinds of genotypes existed in this area.However; the rates of co-infectionitis-different according to environment,time and population that contributed to the kinds of and the index of ticks existed in the surveys points,also the infection rate of the ticks was studied.
7.Clinical investigation of 144 patients with drug-induced liver injury and its influencing factors analysis

Objective In order to provide support for the rational use of drugs,we summarize the clinical characteristics of patients with drug-induced liver injury (DILI) and statistics of the allergic drugs.Methods According to the common drug-induced liver injury diagnostic criteria of our country,the clinical data of 144 hospitalized cases in the third Xiangya hospital with DILI were analyzed retrospectively,then,potential pathogenic drugs were summarized.Results A total of 144 patients with DILI were enrolled in this study:male account for 53.47% (77/144 cases),female account for 46.53% (67/144 cases).The average age was (44.32 ± 16.02) years old,and 50.70% (73/144 cases) of the patients were over 40 years old.Liver injury was caused by several kinds of drugs,23.61% (34/144 cases) were traditional Chinese medicine,20.83% (30/144 cases)were immunosuppressive agents,15.97% (23/144 cases) were anti-tuberculosis drugs,7.64% (11/144 cases) were antineoplastic drugs.18.75% (27/144 cases) patients were cured,22.92% (33/144 cases) patients got better while others account for 58.33% (84/144 cases),and the average hospitalization days of patients with DILI were 16 days.Age,gender and duration of the disease had no significant effect on prognosis (P > 0.05),while there were significant differences between liver function indexes and prognosis (P <0.05).Conclusion In this study,the incidence of DILI was higher in elderly people.A variety of drugs were able to cause DILI,including traditional Chinese medicine,immunosuppressive agents and anti-tuberculosis drugs.
8.Risk analysis and control suggestions for suicidal abuse of over-the-counter drug difenidol tablets

Objective To analyze the literature related to diphenidol tablets poisoning,the characteristics of poisoning were summarized to provide reference for controlling the suicidal abuse risk of diphenidol tablets.Methods The global literature on suicide,overdose,poisoning,shock,and death related to difenidol published from January 1,2011 to December 31,2022 was analyzed,including gender,age,dosage,cardiac(blood)concentration,poisoning symptoms,etc.Results Young women were the majority of people with poisoning.The highest proportion of the age group is 11 to 30 years group.Patients who take medication doses greater than 3 000 mg may have a higher risk of death;patients with a heart(blood)concentration greater than 6 μg·mL-1 may have a higher risk of death.Malignant arrhythmia,consciousness disorders,coma,and apnea are common serious adverse events during poisoning.Conclusion It is recommended that the drug regulatory authorities should require the Listing permit holder of difenidol tablets to add the risk and symptoms of poisoning into the instructions.It is suggested that restricting individual consumers from purchasing large amounts of difenidol tablets in the short term.It is recommended that canceling the high-dose sales packaging of difenidol tablets.It is suggested that converting difenidol tablets into prescription drugs,even consider canceling the registration certificate of difenidol tablets.
9.Clinical trial of pancreatic kallidinogenase combined with irbesartan in the treatment of elderly patients with type 2 diabetic nephropathy

Objective To explore the efficacy of pancreatic kallidinogenase enteric-coated tablet combined with irbesartan tablet in the treatment of elderly patients with type 2 diabetic nephropathy(DN).Methods Elderly patients with type 2 DN were selected as the research subjects,and were randomized into control group and treatment group.The control group was given 150 mg of irbesartan tablets once a day,while the treatment group was given 240 U of pancreatic kallidinogenase enteric-coated tablets three times a day on the basis of the control group.The clinical efficacy,renal function indicators[serum creatinine(SCr),blood urea nitrogen(BUN),urinary albumin excretion rate(UAER)],hemodynamic indicators[fibrinogen(FIB),whole blood viscosity,hematocrit(HCT)],microvascular lesion indicators[vascular endothelial growth factor(VEGF),soluble intercellular adhesion molecule-1(sICAM-1)],B-ultrasound detection indicators[maximum aortic flow velocity(Vmax),minimum diastolic flow velocity(Vmin),resistance index(RI)at the renal hilum]before and after treatment and adverse drug reactions were compared between both groups.Results After treatment,the total effective rates in control group and treatment group were 85.42%and 97.92;SCr levels were(90.47±18.14)and(80.28±12.04)μmol·L-1;BUN levels were(7.24±1.34)and(6.54±1.21)mmol·L-1;UAER levels were(36.17±6.07)and(31.04±5.21)μg·min-1;FIB levels were(4.32±0.59)and(3.95±0.48)g·L-1;whole blood viscosity values were(7.38±1.15)and(6.81±0.98)mPa·s;HCT levels were(38.63±7.01)%and(36.17±6.48)%;VEGF levels were(254.18±45.59)and(212.14±40.48)pg·mL-1;human sICAM-1 levels were(336.40±61.57)and(295.30±58.46)pg·L-1;the Vmax of renal artery were(72.58±3.60)and(74.98±3.78)cm·s-1;the Vmin values were(22.48±3.14)and(24.83±3.63)cm·s-1;the RI values were 0.73±0.06 and 0.68±0.07,respectively.There were statistical differences in the above indicators between control group and treatment group(all P<0.05).The total incidence eares of adverse drug reactions in control group and treatment group were 4.17%(2 cases/48 cases)and 8.33%(4 cases/48 cases)respectively(P>0.05).Conclusion Pancreatic kallidinogenase enteric-coated tablet combined with irbesartan tablet can effectively improve the renal function of elderly patients with type 2 DN,improve the blood flow and delay microvascular lesion,and enhance the efficacy,therefore it is safe and effective.

This study adopted the policy text analysis method,review the historical background of the enactment,aimed to comparatively analyze the international pharmaceutical trade policies of the BRICS countries.The main objectives of the BRICS countries'international pharmaceutical trade policies included ensuring stable and accessible drug supply,expanding exports of domestic products and creating a favorable political environment.For these purposes,Brazil,Russia,and South Africa all ensure drug supply through substantial imports.However,they have also taken measures such as compulsory patent licensing and promoting localization of production by foreign companies to reduce import dependence.India,on the other hand,protects its domestic industry by resisting drug imports to ensure drug supply while simultaneously promoting the export of pharmaceutical products.China continually optimizes approval and data monitoring procedures to align with international standards,creating a favorable trade environment and expanding exports.China should further refine its international pharmaceutical trade policies while ensuring the autonomy of domestic drug research and supply,fostering stronger collaboration within BRICS nations and promoting global access to public healthcare products.
