1.Changes and clinical significance of serum human cartilage glycoprotein-39, osteopontin and rheumatoid factor in patients with rheumatoid arthritis

Objective To explore the changes and clinical significance of serum human cartilage glycoprotein-39(HC gP39),osteopontin (OPN) and rheumatoid factor (RF) in patients with rheumatoid arthritis (RA).Methods Serum HC gP39,OPN levels were measured by ELISA in 98 patients with RA and 98 healthy controls.Serum RF was detected by nephelometric immunoassay.Results The serum HC gP39,OPN and RF levels of RA group were significantly higher than the healthy control group (t =20.25,32.71,36.34,all P < 0.01),and serum HC gP39,OPN and RF levels in active phase patients were higher than the inactive phase patients (t =24.22,45.62,50.15,all P <0.01).The levels of serum HC gP39,OPN increased gradually with the increase of the staging severity(F =18.48,12.36,all P <0.05).The serum level of RF was positively correlated with the levels of HC gP39,OPN in patients with RA (r =0.682,0.656,all P < 0.01),the serum level of HC gP39 was positively correlated with the level of OPN (r =0.608,P < 0.01).Conclusion The HC gP39,OPN and RF reflect situation of RA patients and have close relationship with the clinical progression of RA,which can be used as one of important indexes to judge RA activity and different staging.

The Pareto effect generated in use of journal articles covered in Wiley Online Library ( WOL) , a full-text database developed by China Pharmaceutical University, was analyzed, which showed that the number of WOL-covered source journals in ACIE, SSCI, A & HCI, the number of full-text journals that published SCI-covered papers of our university, the key and strong subjects of our university were the leading factors of Pareto effect gener-ated in use of WOL, studies on Pareto effect are thus of importance for the library in its development of literature resources, data information service and subject service.
4.Evaluation of teaching effects of evidence-based medicine in graduate students

Objective To assess the courses of evidence-based medicine among graduate students. Methods A sample of 118 graduate students who took the course were surveyed twice by using the same self-designed questionnaire at the beginning and the end of the course.Pre-post comparison was conducted to measure the impact. Results The students were able to grasp the most of the contents of the course,and reshaped their opinions on evidence-based medicine,with which,a positive impact on their medical practice was expected. Conclusion The course of evidence-based medicine for graduate students has resulted in a positive consequence.
5.Clinical study on high concentrations of sodium hyaluronate eye drops for moderate to severe dry eye

AIM: To observe the efficacy of high concentrations of sodium hyaluronate ( 3g/L SH ) for moderate to severe dry eye.
METHODS: Forty moderate to severe dry eye patients were included in the study according to the diagnosis criteria and randomized into two groups. The patients of the trial group received topical administration of high concentration sodium hyaluronate (3g/L), and those of the control group received sodium hyaluronate ( 1g/L ) plus recombinant human epidermal growth factor. The dry eye symptom scores, ocular surface disease index ( OSDI) scores, tear film break-up time ( BUT) , SchirmerⅠ test and corneal fluorescein staining score were evaluated. All the indexes were compared between the two groups 2wk before and after treatment.
RESULTS: There were no significant differences of the indicators between the two groups before treatment. After 2wk treatment, the differences were statistically significant compared to former except for the SchirmerⅠtest. Compared with the control group, the symptom scores and the OSDI scores were lowered. No significant differences were found in the other indicators between these two groups.
CONCLUSION: Topical usage of highconcentrations of sodium hyaluronate (3g/L) is beneficial for remitting the ocular symptoms in moderate to severe dry eyes, and also improve the quality of life of patients.

8.Comparison of two criteria for evaluating severity of community-acquired pneumonia in China

Objectives To understand current status of the admission and treatment for the patients with community-acquired pneumonia (CAP) in central hospitals of Shanghai area,and to evaluate the severity of patients admitted to the hospital with CAP by the criteria set in the Guidelines for Diagnosis and Treatment for CAP developed by the Chinese Medical Association in 2006 and provide evidence for its popularization and application throughout the country.Methods Medical records of 137 patients with CAP admitted to the hospital from January 1,2005 to September 30,2006 were retrospectively studied and analyzed with SPSS 10.0 software.Chi-square test and ANOVA were used to evaluate the severity of the patients with CAP by the criteria set in the Guidelines and to correlate it with pneumonia severity index (PSI).Statistical analysis was performed for the difference between length of hospitalization,cost,length of intravenous use of antibiotics,the number of risk factors,and fatality during hospitalization between three groups of patients categorized based on the severity criteria in the Guidelines.Results There existed a good relationship between the criteria for severity of CAP by the Guidelines and PSI,with a Pearson's coefficient of correlation of 0.577,P

10.Effect of transforming growth factor-β1 on monocyte Toll-like receptor 4 expression in septic rats

BACKGROUND: Sepsis is a tough problem in critical ill patients. This study aimed to investigate the dynamic changes of monocyte Toll-like receptor (TLR) 4 expression in peripheral blood of septic rats and to determine the effects of transforming growth factor (TGF) -β1 on TLR4 expression. METHODS: Altogether 132 clean level SD rats were randomly divided into a control group (n=12), a sepsis model group (n=60), and a TGF-β1 intervention group (n=60). In the sepsis model group and TGF-β1 intervention group, the rats were subdivided into five groups (2-hour group, 6-hour group, 12-hour group, 24-hour group, and 48-hour group), with 12 rats in each group. Cecal ligation puncture (CLP) was performed in the sepsis model group and TGF-β1 intervention group to establish models of sepsis. The rats in the sepsis model group were injected with 1 mL normal saline at the caudal vein 0.5 hour after the model establishment; the rats in the TGF-β1 intervention group were injected with 20 ng/mL or 250 g TGF-β10.5 hour after the model establishment. Flow cytometry was used to detect the change of monocyte TLR4 in peripheral blood, and enzyme-linked immunosorbent assay (ELISA) was used to detect the change of TNF-α level in peripheral blood. RESULTS: At 6-12 hours after CLP, the monocyte TLR4 in peripheral blood started to decrease, and reached the lowest level at 12 hours. Compared to the control group, the monocyte TLR4 expression at 6 and 12 hours was lowered significantly (P<0.05). Compared to the sepsis model group at 2, 24 and 48 hours after CLP, the monocyte TLR4 expression in the TGF-β1 intervention group decreased dramatically (P<0.05), but there were no differences between the two groups at 6 and 12 hours respectively. Compared to the control group, the concentration of NF-κB in liver tissue increased significantly 6 hours after CLP (P<0.05). After use of TGF-β1, the concentration of NF-κB was decreased significantly but still higher than that of the control group. Compared to the control group, the concentration of TNF-α in peripheral blood was increased significantly at 2-48 hours after CLP (P<0.05). After use of TGF-β1, TNF-α was further increased. CONCLUSION: During sepsis, TGF-β1 can decrease the monocyte TLR4 expression and NF-κB in liver tissue, but facilitate the formation of proinflammatory mediator TNF-α. This finding indicates that TGF-β1 may play a role in promoting inflammatory response during sepsis, but this regulation is not via direct regulation of monocyte TLR4 in peripheral blood.
