1.The value of high field MRI conventional 3 D-TOF sequences and MRA in diagnosis of moyamoya disease

Objective To explore the value of MRI sequence and MRA for diagnosis of moyamoya disease(MMD).Methods Retrospective analysed of 1 8 cases of moyamoya disease clinical datas,1 8 cases were used routine head MRI scan,including T1 WI, T2WI,DWI,transverse and sagittal T2WI sequence FLAIR;MRA three dimensional time of flight(3D-TOF).Results 18 cases of patients with MRA showed bilateral internal carotid artery,anterior cerebral artery distal and proximal middle cerebral artery steno-sis or occlusion of signs.Among them,1 1 cases were found with secondary cerebral infarction,3 cases of cerebral hemorrhage,7 ca-ses of centrum semiovale,corona radiata and periventricular cerebral ischemia,4 cases of cerebral atrophy,8 cases of cerebral soften-ing and gliosis.Conclusion MRI and MRA examination have important value in the diagnosis of moyamoya disease,treatment.
2.Diagnostic value of Wells score and D-Dimer detection in acute pulmonary embolism

Objective To assess the diagnostic value of Wells score and D-Dimer detection in acute pulmonary embolism. Methods Forty-two suspected cases of pulmonary embolism were collected from March 2012 to September 2014 Then Wells score and D-Dimer detection were applied to confirm the disease. Results Of the total, 26 patients were diagnosed by computed tomographic pulmonary arteriography (CTPA). There were 7, 24 and 11 suspected cases which had low, medium and high Wells score respectively. Among them, 1, 15, and 10 cases were diagnosed by CTPA. The diagnosis rates were 14.2% (1/7), 62.5% (15/24) and 90.9%(10/11) respectively. The D-Dimer level in pulmonary embolism group was significantly higher than that in non-embolism group(P < 0.01). The D-Dimer level of cases with medium or high Wells score was higher than those with low Wells score (P < 0.05). Conclusion Wells score combined with D-Dimer is convenient, accurate and safe for predicting acute pulmonary embolism.

4.Repeatability of Ophtha TOP in measuring corneal parameters and its agreement with Pentacam

Background Accurate measurement of corneal refractive parameters plays an important role in the diagnosis of eye diseases,design of refractive surgery and calculation of intraocular lens (IOL) power.Ophtha TOP is a new IOL calculator,so it is necessary to evaluate the repeatability and accuracy of Ophtha TOP in measuring corneal refractive parameters.Objective This study was to evaluate the repeatability of Ophtha TOP in measuring corneal refractive status in normal eyes and determine the agreement of the measuring outcomes between Ophtha TOP and Pentacam in pre-surgery cataract eyes.Methods A reliability evaluation method of diagnosis test was performed from September 2013 to October 2014.Sixty eyes of healthy volunteers aged (30.83 ±8.66) years old were examined in Tianjin Eye Hospital with Ophtha TOP for more than 5 times to evaluate the stability in measuring corneal refractive parameters.Corneal refractive parameters were measured by both Ophtha TOP and Pentacam for more than twice in 30 pre-surgery cataract eyes to assess the agreement with Pentacam.Keratometry and astigmatism parameters were recorded,including the flattest meridian (Kf),steepest meridian (Ks),mean keratometry (Km) and the astigmatism at J0 and J45.Intra-class correlation coefficients (ICC),repeatability (2.77Sw) and coefficient of variation (CoVs) were used to assess the stability of Ophtha TOP,and Bland-Altman graphs were adopted to value the consistency of measuring outcomes between Ophtha TOP and Pentacam.This study complied with Helsinki declaration,and written informed consent was obtained from each patient prior to any medical examination.Results The ICC of Ks,Kf,Km,J0 and J45 by Ophtha TOP were all ≥0.90,and 2.77Sw were all ≤0.36,The CoVs of Kf,Ks,Km by Ophtha TOP were all ≤0.30.The measuring values of Kf,Ks and Km were significantly higher in the Ophtha TOP than those in the Pentacam,with the mean differences (0.18±0.28) D,(0.24±0.29) D and (0.21± 0.26) D,respectively(t =3.48,4.50,4.49,all at P=0.00).The mean difference of the J0 was (0.07±0.21) D and that of the J45 was (-0.02±0.18)D between Ophtha TOP and Pentacam,showing insignificant differences between them (both at P>0.05).Bland-Altman graphs revealed that the 95% limit of agreement (LoA) of Kf,Ks and Km between the two methods was-0.37 D to 0.73 D,-0.33 D to 0.81 D and-0.30 D to 0.72 D,respectively,and that of J0 and J45 was-0.34 D to 0.48 D and-0.37 D to 0.33 D,respectively.Conclusions Ophtha TOP shows an excellent repeatability.Compared with Pentacam,Ophtha TOP presents a good consistence for measurement of the corneal astigmatism and moderate consistence for corneal curvature.

8.Quality of life of main caregivers of patients with Parkinson's disease

Objective To examine the caregivers' burden and quality of life (QOL) of main caregivers of patients with Parkinson's disease by analyzing caregiver and patient-related factors. Methods A total of 53 cases with Parkinson's disease seen from October 2012 to December 2013 in Yantaishan hospital of Yantai City were selected. Assessments: (a) EuroQol-5 questionnaires, including: EQ-5 dimensions (EQ -5d) and visual analogue scale (VAS). (b) Unified Parkinson's Disease Rating Scale. (c) Hoehn-Yahr(H-Y) stage; (d) Parkinson's disease sleep scale (PDSS), 15 items; (e) Hamilton Anxiety Rating Scale (HAMD-14), Hamilton Depression Rating Scale (HAMA-24); (f) Chinese version of Zarit caregiver burden interview. Results Spearman correlation analysis showed that the EQ-5D index and EQ-VAS negatively correlated with HAMD-14 of caregivers of Parkinson diseasepatients (r=- 0.318, - 0.046) (P<0.05), and negatively correlated with H-Y staging (r=-0.592, -0.531), UPDRSI (r=-0.434, -0.316) , UPDRS Ⅱ (r=0.521, 0.513) , UPDRS Ⅲ (r=0.520, 0.534), UPDRS Ⅳ (r=0.564, 0.509), PDSS in patients with Parkinson's disease (r=0.547, 0.490), HAMD-14 (r=0.315, 0.350), HAMA-24 (r=0.413, 0.401) of Parkinson's disease patients (P<0.05). Caregiver burden was related with HAMD-14 (r=0.496), HAMA-24 (r=0.551),H-Y stage (r=0.697), PDSS (r=0.659), UPDRS Ⅰ-Ⅳ (r=0.538, 0.668, 0.696, 0.600), EQ-5D index of Parkinson disease patients (r=0.682), EQ-VAS of Parkinson disease patients (r=0.670) (P<0.05); and positively related with anxiety of caregivers (r=0.275) , and a negative correlation with the caregivers' quality of lives (r=0.665) (P<0.05). Bivariate analyses showed significant correlations between caregiver burden and patients' sleep, scores of Parkinson's motor symptoms, and caregivers' depression. Based on multiple regression analysis, Zarit score proved to be the main predictor of caregivers' QOL. Caregivers' age, total numbers of caregivers and H-Y stage also proved to be a relevant factor. Conclusion Improvement of patient's sleep, motor symptoms anddepression might alleviate caregivers' strain. There is a correlation between caregivers' burden and QOL. QOL of caregivers can be improved through reducing their care-burden.
9.Vascular ultrasound combined with Seldinger technology improves the success rate of peripherally inserted central catheter and reduces potential complications

ObjectiveTo investigate the applicability and superiority of modified Seldinger technique (MST).MethodsTotally 108 patients receiving peripherally inserted central catheter (PICC) through vascular ultrasound combined with MST technology were enrolled as MST group and another 108 patients receiving PICC using traditional direct vision method as control group.The first puncture success rate,PICC succesa rate,catheterization time required,and early complication rate were compared between these two groups.ResultsThe first puncture success rate was 88.89% in MST group and 72.22% in control group (P=0.002) ; the catheterization success rate was 82.41% in MST group and 57.41% in control group ( P =0.000).In the MST group,there were two cases where the catheter within neck was ectopic ( P =0.003 ),two cases experienced bleeding at the puncture sites (P =0.017),three cases had phlebitis (P =0.001 ),and one case suffered from catheter blockage (P=0.010).In the eontrol group,the corresponding numbers were 13,10,18,and 9.However,the time required for PICC showed no significant difference between these two groups (P=0.685).In the MST group,15 cases were catheterized in less than 15 minutes,72 cases between 15 and 25 minutes,and 21 cases for more than 25 minutes.In the control group,12 cases were catheterized in less than 15 minutes,70 cases between 15 and 25 minutes,and 26 cases for more than 25 minutes.ConclusionPICC through MST can be more effective in positioning the targeting blood vessels,increasing the success rate of catheter insertion,reducing various tissue damages,and lowering potential complications.
10.Clinical effects comparison of ropivacaine combined with two doses of sufentanil on labor analgesia

Objective To investigate clinical effects differences of ropivacaine combined with two doses of sufentanil on labor analgesia.Methods300 lying-in women with painless childbirth in the period from January 2016 to December 2016 in department of obstetrics in Deqing hospital of traditional Chinese medicine were chosen and randomly divided into two groups including A group (150 cases) with ropivacaine combined with conventional doses of sufentanil for 7.5μg and B group (150 cases) with ropivacaine combined with low doses of sufentanil for 2.5μg;the block onset time of sensory and motor, arrival time at the maximum block level, modified Bromage score, Apgar score, VAS score at different time points and adverse reaction rate of both groups were compared.ResultsTwo groups of maternal sensory and motor block work time and reach the highest block plane comparative differences had no statistical significance.The modified Bromage score of B group were significant better than A group(P<0.05).The adverse reaction rate of B group were significant lower than A group(P<0.05).ConclusionCompared with conventional dosage sufentanil, luo pp paid in combination with low dose used in epidural sufentanil analgesia can obviously reduce the working time, improve the quality of anesthesia and reduce the incidence of adverse reactions, is suitable for clinicians to choose application.
