1.The correlation analysis of interleukin-1 family genotypes with coronary heart disease in elderly patients

Objective To investigate the correlation of interleukin-1 family genotypes,including interleukin-1 (IL-1α,IL-1β) and interleukin-1 receptor antagonist (IL-1Ra),with coronary heart disease (CHD) and serum lipoprotein level in the elderly.Methods Interleukin-1 family genotypes were detected in 318 elderly controls and 329 elderly CHD patients by polymerase chain reaction and restriction fragment length polymorphisms method.Serum levels of lipoproteins were inspected simultaneously.Results The TT and Ⅰ / Ⅰ or Ⅰ/Ⅳ genotype frequency of IL-1Ra was 90.3% in elderly CHD patients,but 82.4% in controls.Carriers with TT,Ⅰ / Ⅰ or Ⅰ/Ⅳ genotype of IL-1Ra were at an increased risk with an odds ratio of 1.98 in elderly CHD patients as compared with controls (x2=8.55,95% CI:1.25-3.16).The TT and Ⅰ/Ⅰ or Ⅰ/Ⅳ genotype frequency of IL-1Ra was 96.2% in elderly CHD patients with acute coronary syndrome,but 84.8% in elderly CHD patients with stable angina.Carriers with TT,Ⅰ / Ⅰ or Ⅰ/Ⅳ genotype of IL-1Ra were at an increased risk with an odds ratio of 4.54 in acute coronary syndrome group as compared with stable angina group (x2=12.17,95%CI:1.81-11.36).The CT or TT genotype frequency of IL-1α-889 was 22.8% in acute coronary syndrome group,but 7.6 % in stable angina group.Carriers with CT or TT genotype of IL-1α-889 were at an increased risk with an odds ratio of 3.59 as compared with stable angina group (x2 =14.93,95%CI:1.82-7.03).There were no significant differences in levels of serum lipoproteins among the different genotypes (P>0.05).Conclusions In elderly patients with coronary heart disease,IL-1α(-889) CT or TT genotype carriers are at high risk for acute coronary syndrome,but IL-1Ra CC,TC,Ⅰ / Ⅱ or Ⅱ / Ⅱ genotype carriers are at a low risk for CHD or severe CHD.

3.Study on Vancomycin-resistant Enterococci Carried by Intestinal Tract in Different People Groups

512?g/mL; MIC of 16 VIE to vancomycin were 16?g/mL; MIC of 8 VIE to vancomycin were 8?g/mL. It is a risk factor for hospital infection that VRE carriage of inpatients in intestinal tract is high. There is 100% agreement be-tween phenotypes and genotypes in 46 vancomycin resistance enterococci. VRE are multiresistant. Part iso-lates have high homology.

Multiple myeloma is a neoplasm of mature and immature plasma cells, it remains an incurable disease using conventional chemotherapy and increasing aggressive approaches. In recent years, due to the better understanding of myeloma biology, genetics and tumor formation, there are lots of new active drugs or combinational chemotherapy regimens having been developed, such as proteasome inhibitors, immunomodulatory agents etc, they are more effective than conventional chemotherapy. This article summarizes the recent advances with the new options for the treatment of multiple myeloma.

6.Eosinophilic cystitis in children

Objective To discuss the clinical manifestations and CT findings of eosinophilic cystitis in chidren.Methods Nine cases including Six boys and 3 girls,three to 13 years old,mean age of 8.3- year,have symptoms of hematuria,irritative voiding,dysuria and abdominal pain。The eosinophilic cystitis was pathologically proved in 7 patients and eosinophilic granulomatous cystitis in 2 patients,which based on cystoscopic tissue biopsy or surgery retrospectively.Results Local thickened bladder walls or nodular and sessile masses along the bladder dome showed in four cases with eosinophilic granulomatous cystitis,and the diffusely irregularly thickened bladder walls showed on CT scans in the rest 5 cases with eosinophilic cystitis.Conclusion CT findings are helpful to reveal the site,size and other features of eosinophilic cystitis in children.But biopsy of the lesion is necessary to rule out other bladder tumor and selecting the proper management.
7.Effect of early rehabilitation in patients with severe traumatic brain injury

Objective To investigate the clinical effect of early rehabilitation treatment on patients with severe traumatic brain injury (sTBI).Methods Forty sTBI patients were divided into treatment group (n =20) and control group (n =20) according to the random number table.Conventional treatment was performed on all patients including dehydration to decrease intracranial pressure,hemorrhage control,neurotrophic treatment,antiinflammation therapy,and gastric acid control.In addition to these interventions,patients in treatment group received hyperbaric oxygen treatment,median nerve stimulation,fastigial nucleus stimulation,and bedside motor therapy in the early period.Intracranial pressure and partial pressure of brain tissue oxygen (PbtO2) were continuously monitored during the process of treatment.GCS was measured before and 15 days after treatment and single-photon emission computed tomography (SPE-CT) was used to evaluate cerebral perfusion.Results There was no statistical difference between the two groups with respect to GCS in advance of treatment (P > 0.05),but GCS differed between treatment group and control group after treatment [(10.18 ± 3.75) points vs (8.33 ±2.36) points,P <0.05],with substantial improvement in treatment group.Significantly improved cerebral perfusion was seen in treatment group.On day 5 after treatment,intracranial pressure in treatment group lowered significantly compared with that in control group (P < 0.05).On day 6 after treatment,PbtO2 was significantly higher in treatment group than in control group (P < 0.05).Conclusion Early rehabilitation treatment leads to improved outcome and acts a positive effect on nerve function recovery.
8.Finite element model of distal tibial articular surface defect:Biomechanical analysis

BACKGROUND:Finite element analysis has been widely used for the research of bone and joint biomechanics, but the reports about finite element analysis of distal tibial articular surface defect are rare at home and abroad. OBJECTIVE:To establish ankle three-dimensional finite element model, produce distal tibial articular surface defects with different areas, and to simulate the distal tibial articular surface deformation and displacement under the different phases, thus predict the maximum al owable degree of distal tibial articular surface defect and explore the mechanics pathogenesis of ankle traumatic arthritis. METHODS:Continuous tomographic images were obtained by multi-slice spiral CT scan of a normal adult male ankle, and then the images were imported into the Mimics medicine modeling software to generate a entity model;the large general-purpose finite element analysis software ANSYS 13.0 was used for meshing, material property assignment and generating a finite element model. Restricted boundary conditions and simulated ankle distal end axial force, and then the stress distribution and displacement results of distal tibial articular surface in different phases were obtained. RESULTS AND CONCLUSION:The total number of units of the established finite element model of ankle joint was 157 990, and the total number of nodes was 193 801. On three phases, with the increase of the distal tibial defect area, the contact area was gradual y decreased, especial y in plantar flexion with the defect diameter of 13 mm, the change of the area was most obvious;The contact area of the neutral position was largest;with the increase of the distal tibial defect area in the neutral position and dorsiflexion, the peak stress was increased gradual y, and significantly increased after the diameter changed into 11-13 mm;in the neutral position and 10° of dorsiflexion, the peak stress mainly concentrated in the posteromedial and posterolateral quadrant;in 10° of plantar flexion, the change was complex, and when the diameter was 11-13 mm, the peak stress was increased gradual y with the increasing of defect area, when the diameter increased to 13 mm, the peak stress reached maximum. The maximum diameter of distal tibial articular surface defect was considered to be 11-13 mm. The joint function wil be affected when the diameter of distal tibial articular cartilage and bone bed defects was more than 11-13 mm.
9.The biological effects of zoledronic acid in combination with doxorubicin on SGC-7901 cell line of gastric cancer

Objective To explore the possible effect of combined treatment of zoledronic acid and doxorubicin on proliferation,invasion and adhesion of SGC7901 cell line,and potential underlying mechanisms. Methods MTT and Transwell experiments were used respectively to investigate the effect of different concentrations of zoledronic acid and doxorubicin on the proliferation,adhesion and invasion of SGC7901. The mRNA levels of MMP2, TIMP2, MMP9, ICAM1 and CD44 were quantified by real time PCR. Results Gastric cell line SGC-7901 were incubated with zoledronic acid for 48 h in different concentration of 10-6,10-5,10-4,10-3 mol/L,their inhibition rates were 4.98%,12.19%,27.34% and 73.13% separately,which were higher than control group (P<0.01),and enhanced along with the dose increasing and the time extension (P<0.05).Gastric cell line SGC-7901 was incubated with zoledronic acid 10-4 mol /L and doxorubicin 0.5 mg/L,respectively,or in combination for 2 h,4 h,6 h,and thereafter,the detected adhesion rates were 5.03%,24.38% and 59.65% in control group,4.40%,19.77% and 51.22% in zoledronic acid group,4.28%, 18.62% and 51.02% in doxorubicin group,4.02 %,15.02% and 46.95% in combination group. The adhesion rates were reduced after incubation with different medicament concentration (P<0.05).In the study of cell invasion,the cell numbers per high power field were:105.11 ± 10.43,98.37 ± 15.75,96.19 ± 9.24,83.02 ± 19.72 after treatment with zoledronic acid,doxorubicin or their combination at different concentrations. Compared with control, signal medicament had weak effectiveness in invasiveness in SGC-7901 (P> 0.05),whereas in combination group it was significantly decreased (P<0.05).Both zoledronic acid and doxorubicin decreased the mRNA expressions of CD44,MMP2,MMP9,ICAM1 while increased TIMP2 mRNA level (P<0.05or P<0.01) versus control.The effect was more evident with combination treatment(P<0.05 or P<0.01),compared with doxorubicin group. Conclusions Both zoledronic acid and doxorubicin can inhibit the proliferation,adhesion and invasion of SGC7901. Combination treatment has additive effect.It may be related to the down-regulation of mRNA of ICAM1,MMP2,MMP9,CD44 and up-regulation of mRNA of TIMP2.
