1.Practice and thinking of quality management of organ procurement organization

BACKGROUND:With the introduction of related policies on organ procurement and distribution, the organ procurement and standards of the organ procurement have a clear operational criteria, as wel as how to al ocate organs. But the work processes and organizational positioning on organ procurement organization are not enough elaborated and normal. OBJECTIVE:To analyze the experience of Guangzhou General Hospital of Guangzhou Military Command as the organ procurement organization units in quality management, timely review working progress and explore areas for improvement, which provide the corresponding countermeasures for enhancing quality management and establishing the efficient and professional organ procurement organization. METHODS:A computer-based online search in CNKI database and PubMed database was performed with the keywords of“organ donation, organ transplantation, Organ Procurement Organization, quality management”in English and Chinese by screening titles and abstracts. RESULTS AND CONCLUSION:According to relevant laws and regulations, Guangzhou General Hospital of Guangzhou Military Command formulates a standard evaluation standard and sets up the team, thus achieving the reunification of the responsibilities and powers. At the same time, they improve the relevant organ donation,acquisition, preservation, registration, and design different quality management plans according to these different aspects. In addition, In addition, the hospital hold regular coordinator training and make it standardization. Through a series of quality management measures, the hospital organ procurement organization has gradual y regularized and achieved some results. It is recommended to further strengthen scientific research, achieve the unification of efficiency and effectiveness of scientific research;improve the awareness of the hospital leaders, consummate the mechanism of organ procurement;focus on personnel training, improve the professional quality of the organization members;enhance information management, achieve multi-disciplinary col aboration;build quality evaluation system, achieve the whole process of quality management;emphasize on medical ethics, and promote the harmony between the hospital and the patient.
2.Dexmedetomidine protects against LPS-induced ALI in septic mice

Objective To observe the effect of dexmedetomidine on inflammation, oxidative stress and lung injury in lipopolysaccharide( LPS)-induced septic mice.Methods Forty eight male adult BALB/c mice were randomly divided into three groups (n=16): normal control group (Ctrl), sepsis group (Sep), and Dex group.Ae septic mice model was established by LPS 20 mg/kg,and Dex 30μg/kg injected intraperitoneally at 0.5 h after LPS injection.The concentrations of serum IL-6 and IL-10 were detected at 2 h and 6 h after LPS injection while myeloperoxidase ( MPO ) activity and malondialdehyde( MDA) content of lungs were detected and weight dry ratio ( W/D) of lungs was calculated at 6 h after LPS injection.Pathological changes were observed in left lung HE stained with optical microscopy at 6 h after LPS injection. Results Compared with Sep group, the concentrations of serum IL-6 decreased significantly(P <0.05), while the concentrations of IL-10 increased significantly(P<0.05) at 2 h and 6 h after LPS injection in Dex group.MPO activity, MDA content and W/D of lungs decreased significantly(P<0.05) at 6 h after LPS injection in Dex group.The injury to the lung was lightened significantly under optical microscopy in Dex group.Conclusion Dex protects against LPS-induced ALI in septic mice by inhibiting systemic inflammatory response, reducing lung tissue inflammatory infiltration and oxidative stress.
3. Chemical Constituents from the Roots of Scrophularia ningpoensis Hemsl and Their Anti-inflammatory Activities

OBJECTIVE: To study the chemical constituents in the roots of Scrophularia ningpoensis Hemsl. METHODSE: Compounds were isolated by chromatographic techniques and their structures were elucidated by spectral methods. The anti-inflammatory activities of all isolated compounds were evaluated by lipopolysaccharides (LPS)-stimulated BV2 cells model. RESULTS: Eighteen compounds were isolated and identified as 2'''-acetyl angroside C (1), saccatoside (2), 6-O-α-L-(2″-O-feruloyl)rhamnopyranosylcatalpol (3), scrophularioside (4), harprocumbide A (5), 6″-O-β-D-glucopyranosylharpagoside (6), harpagoside (7), 8-O-(p-coumaroyl)-harpagide (8), 8-O-feruloylharpagide (9), 6-O-α-D-galactopyranosylharpagoside (10), 6'-O-cinnamoylharpagide (11), angoroside B (12), angoroside C (13), scrophuloside B1 (14), 2-(3-hydroxy-4-methoxyphenyl)ethyl-O-α-L-arabinopyranosyl-(1→6)-O-α-L-rhamnopyranosyl-(1→3)]-β-D-glucopyranoside (15), darendoside B (16), 6-O-caffeoyl-β-D-fructofuranosyl-(2→1)-α-D-glucopyranoside (17), and sibirioside A (18). CONCLUSION: Compound 1 is new compound, and compounds 2-4, 12, 14, and 17 are isolated from this plant for the first time. Compounds 1, 12, and 13 show significant inhibition against nitric oxide production in LPS-stimulated BV2 cells model.
4.Effects of propofol and etomidate on inflammation and oxidative stress in septic mice

Objective To observe the effects of propofol and etomidate on inflammation and ox-idative stress in septic mice.Methods Sixty-four male adult BALB/c mice were randomly divided into four groups:normal control group (N),sepsis group (S),propofol treatment group (P)and etomid-ate treatment group (E).The septic mice model was established by lipopolysaccharide (LPS,20 mg/kg)intraperitoneal injection,and propofol (60 mg/kg)or etomidate (10 mg/kg)was injected in the abdominal cavity at 0.5 h after LPS injection.Serum interleukin-6 (IL-6)concentrations and in-terleukin-10 (IL-10)concentrations were measured at 2 h and 6 h after LPS injection;malondialde-hyde (MDA)content of lung,liver and kidney tissue was measured at 6 h after LPS injection. Results Compared with group N,serum IL-6 concentrations increased significantly (P <0.05),and IL-10 concentrations decreased significantly (P <0.05)at 2 h and 6 h after LPS injection in group S;MDA content of lung,liver,kidney increased significantly (P <0.05 )at 6 h after LPS injection in group S;Compared with group S,serum IL-6 concentrations decreased significantly (P <0.05),and IL-10 concentrations increased significantly (P < 0.05 )at 2 h and 6 h after LPS injection in both group P and E;MDA content of lung,liver,kidney decreased significantly (P <0.05 )at 6 h after LPS injection in group E,but only MDA content of lung decreased significantly (P <0.05)at 6 h af-ter LPS injection in group P;Compared with group P,serum IL-6 concentrations was significantly lower (P <0.05),and IL-10 concentrations was significantly higher (P <0.05)at 2 h and 6 h after LPS injection in group E;MDA content of lung,liver,kidney was significantly lower (P <0.05)at 6 h after LPS injection in group E.Conclusion Both propofol and etomidate injected in the abdominal cavity can reduce injury of inflammatory and oxidative stress in septic mice induced by LPS,and the effect of etomidate is more significant.
5.Early detection of abnormal left atrial function by speckle-tracking in hypertensive patients

Objective To evaluate the feasibility of speckle-tracking echocardiography in detecting left atrial (LA) function impairment in early hypertensive (HT) patients.Methods Echocardiography was performed in 154 HT patients with LA volume index <28 ml/m2 and 64 age/gender-matched control subjects.LA global longitudinal strain in late diastole (Sa),systole (Ss),and total strain (Stot =Sa + Ss),strain rate in late diastole (SRa),systole (SRs),and early diastole (SRe) were measured using off-line speckle-tracking analyzing software in apical 4 chamber view.Results Both parameters reflecting LA reservoir function [Stot (23.7 ± 6.0) % vs (25.7 ± 5.9) %,p =0.03 and SRs ( 1.2 ± 0.3) s- 1 vs ( 1.3 ±0.3) s-1,P =0.03]and conduit function [Ss (12.0 ± 5.1)% vs (14.0±5.7)%,P =0.02 and SRe (1.0 ± 0.4)s- 1 vs ( 1.2 ± 0.4)s- 1,p <0.001 ]decreased significantly in HT group than control,while LA pump function parameters (Sa and SRa) had no differences.Ss,Stot and SRe correlated significantly with HT and HT duration.Conclusions LA phasic function are impaired in HT patients with normal LA size.Speckle tracking echocardiography is a promising tool for the early detection of LA dysfunction.
6.Mechanical property of different cross-sectional area screws in middle and upper thoracic vertebral pedicle-rib complex

BACKGROUND:Due to the importance of pedicle adjacent structures, once the screw replacement appears a deviation, adjacent structures may be damaged, leading to extremely serious consequences. Although the security of screw placement in thoracic vertebral pedicle-rib complex is significantly greater than that of pedicle screws, the mechanics of the pedicle-rib complex at different cross-sectional areas of the screw are rarely reported.
OBJECTIVE:To observe mechanical property of different cross-sectional area screws in the middle and upper thoracic vertebral pedicle-rib complex.
METHODS:Five specimens of adult cadaveric thoracic spine (T 1-T 10 ) and adjacent rib segment (50-60 mm long) were used. The bone density of specimens was measured using difunctional bone density testing machine, and osteoporotic vertebral body was excluded. The position of the screws was detected with CT images. The maximal withdrawal force of the pedicle screw was measured with biomechanical force test machine.
RESULTS AND CONCLUSION:Thirty-eight specimens at normal bone density were implanted with 25 screws (5.5 mm), 25 screws (6.0 mm) and 26 screws (6.5 mm). Because the pedicle screws destroyed the pedicle-rib complex and perforated the vertebral body, we final y obtained the withdrawal force of 68 screws. The axial withdrawal force of pedicle screws at different diameters was (812.36±147.22) N, (868.64±160.48) N and (946.48±157.58) N, respectively. There were significant differences between the 5.5 mm screws and the 6.5 mm screws (P<0.05). Experimental findings indicate that, the pedicle screws (diameter>5.5 mm) are suitable in the middle and upper thoracic vertebral pedicle-rib complex due to strong internal fixation and clinical requirement.
7.Correlation between polymorphisms of transforming growth factor-β1, tumor necrosis factor-related apoptosis-inducing ligand genes and nodular thyroid disease

Objective To study the correlation between individual gene polymorphisms of transforming growth factor (TGF)-β1 + 869 T/C,tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) + 1525 G/A genes and nodular thyroid disease.Methods From September 2007 to December 2009,a total of 544 patients with nodular thyroid disease diagnosed in the Department of Endocrinology,The First Affiliated Hospital of Baotou Medical College,Inner Mongolia University of Science and Technology were selected,including 136 cases of nodular goiter patients (node group),132 cases of thyroid tumor (adenoma group),146 cases of Graves patients (GD group),and 130 cases of Hashimoto's thyroiditis (HT group).One hundred and thirty-five healthy subjects were enrolled as control group.Two milliliters of fasting venous blood of all subjects were collected.Polymorphisms of the TGF-β1 + 869 T/C and the TRAIL 1525 A/G genes were identified by the polymerase chain reaction-single strand conformation polymorphism (PCR-SSCP) and the restriction fragment length polymorphism (PCR-RFLP) methods.Results TGF-β1 + 869 T/C:The CC genotypes and C allele frequencies of nodular goiter group [47.0%(64/136),63.2%(172/ 272)] were significantly higher than those of normal control group [18.0%(22/135),45.2% (122/270); x2 =30.76,17.79,all P < 0.05].The genotypes and allele frequencies of adenoma group[42.4% (56/132),59.1% (156/264)] were significantly higher than those of the normal control group (x2 =24.40,10.34,all P < 0.05).The risk of population carrying the C allele suffering from nodular goiter was 2.086 times of those carrying the T allele (OR =2.086; 95% CI:1.480-2.943).The risk of population carrying the C allele suffering from adenoma was 1.752 times of those carrying the T allele (OR =1.752,95% CI:1.244-2.469).TRAIL + 1525 G/A:the genotypes and allele frequencies of nodular goiter group [40.4% (55/136),62.9% (171/272)] were significantly higher than those of normal control group [12.0% (16/135),48.5% (131/270); x2 =9.176,11.307,all P < 0.05].The genotypes and allele frequencies of adenoma group[53.3% (70/132),73.1% (193/264)] were significantly higher than those of the normal control group (x2 =9.806,33.82,all P < 0.05).The risk of population carrying the G allele suffering from nodular goiter was 1.796 times of those carrying the A allele (OR =1.796,95% CI:1.275-2.531).The risk of population carrying the G allele suffering from adenoma was 2.884 times of those carrying the A allele (OR =2.884,95% CI:2.009-4.142).Conclusions TGF-β1 + 869 T/C and TRAIL + 1525 G/A gene polymorphisms may be related to the incidence of nodular thyroid diseases; G allele of TRAIL and C allele of TGF-β1 may be predisposing genes of patients with nodular goiter.
8.High altitude hypoxia aggravates chronic inflammation induced by LPS in rats

Objective To investigate the effect of high altitude hypoxia on chronic inflammation in rats.Methods Forty SD rats were randomly divided into 4 groups: control group (Con),chronic inflammation group (CI),high altitude hypoxia group (HH),high altitude hypoxia+chronic inflammation group (HH+CI).Rats in CI group were injected with lipopolysaccharide (LPS) (0.5 mg/kg) through the caudal vein twice a week for 4 weeks.Rats in HH+CI group were treated just as CI group was,but together with HH group rats were settled in a hypoxic environment of 6000 m altitude for three days.Pathological changes in lung tissues were observed by hematoxylin eosin stain.The peripheral white blood cell count and classification were measured.The levels of interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α) in serum and lung tissues were detected by enzyme-linked immunosorbent assay (ELISA).Changes in IL-6 expression in rat lung tissues were observed by Western blotting.Results After LPS and high altitude hypoxia exposure,inflammatory cells infiltration and alveolar capillary expansion were observed in rats' lung tissue.Compared with Con group,not only the peripheral white blood cell count,but also the level of IL-6 and TNF-α in serum and lung tissue increased in CI and HH group(P<0.01).IL-6 expression levels observed by Western blotting were also increased in HH and CI group(P<0.01).High altitude hypoxia and chronic inflammation interacted(P<0.01).The peripheral white blood cell count was higher in HH+CI group than in other groups,and IL-6 and TNF-α expressions in lung tissue were increased(P<0.05).Conclusion An LPS-induced chronic inflammation model in rats is successfully obtained,and high altitude hypoxia could aggravate chronic inflammation.
9.The survey about HIV infections among voluntary blood donors of Shaoguan in the recent ten years

Objective To analyze the voluntary blood donors of Shaoguan city in recent 10 years HIV infection status and distri‐bution characteristics ,and to take effective prevention and control measures ,in order to reduce the risk of HIV infection through blood transfusion .Methods HIV screening test in the early by the reagent of two different manufacturers ,from 2005 to 2014 vol‐untary blood donors specimens of 265 771 doses of blood donation in Shaoguan ,and early screening positive specimens confirmed by Center for Disease Control of Shaoguan .Results The voluntary blood donors of Shaoguan in the 10 years that initial screening test anti‐HIV positive were 310 cases ,and 35 cases confirmed positive;The confirmed positive rate was 1 .317/10 000;The HIV infec‐tion rate in 2012 was highest (2 .867/10 000) and there were 9 cases ,which was significant difference (P<0 .05) .The district of the maximum rate was New Fengxian ,which was 5 .369/10 000 ,and different districts had significant difference (P<0 .05) .The epidemiology survey about HIV infections found that male were 30 cases(85 .71% ) ,which was significantly more than women;By age distribution ,most HIV infections were 18 to 35 years old young adults ,which accounted 26 cases(74 .29% ) .Married and un‐married people infected respectively accounted 28 .57% and 71 .43% .The occupations of HIV infections were scattered with farm‐ers ,staffers ,the service industry ,cadres in the majority .The HIV infection rate with low education was relative higher ,which had some relationship to educational level .The residents and the non‐residents infected respectively accouted 71 .43% and 28 .57% ;Blood donors for the first time of HIV infections were 24 cases (68 .57% ) .Blood donors for the second time of HIV infections were 11 cases (31 .43% );Other virus co‐infection respectively were 2 HBV infections ,1 HCV infection and 3 syphilis infections .The route of transmission was mainly through sexual transmission that was mainly heterosexual transmission ,which was 33 cases (94 .58% ) .Conclusion The voluntary blood donors of Shaoguan with HIV is on the rise ,and give priority to with male youths spread through heterosexual sex .It should be pay high attention to by the government ,society and blood stations ,and strengthen the propaganda work of AIDS prevention and control checks ,and minimize the risk of HIV transmission by blood transfusion in or‐der to guarantee the safety of clinical blood transfusion .
10.Optimization of prokaryotic expression condition and purification of soluble GST-CRH protein

Objective To obtain the recombinant corticotropin releasing hormone (CRH) protein with soluble, high purity protein through optimizing prokaryotic expression condition and purifying glutathione thiol transferase (GST)-CRH protein. Methods To detect the expression of soluble CRH protein through grope of the host strain GST-CRH temperature of induction expression, the host strain concentration (OD600), IPTG concentration and induction time, the purification of GST-CRH was performed by GST-CRH agarose gel. Western Blot assay was used for the expression identification of the target protein. Results The optimal conditions for the induction of CRH protein were determined: temperature of 30 ℃, IPTG induced concentration 0.1 mmol/L, bacteria density (OD600) 0.8, the induction time of 8 hours, purified GST-CRH>95% fusion protein was obtained. Conclusion The optimal expression conditions of GST-CRH are obtained, and the soluble protein of high purity GST-CRH is also obtained.
