BACKGROUND: Patient subjective satisfaction is of great significance for the clinical use of implant denture.OBJECTIVE: To investigate the patient satisfaction with single or bridge crown for repair of consecutive missing posterior teeth in the maxilla.METHODS: We selected 203 patients with consecutive missing of maxillary first and second molar teeth, who had received single (n=99) or bridge (n=104) crown for implant restoration 1 year ago. All patients were surveyed with a subjective satisfaction questionnaire and visual analogue scale.RESULTS AND CONCLUSION: There was no significant difference in sex and age (35-64 years) between the two groups. Food impaction ratio and crown off ratio were higher in the bridge crown group than the single crown group.Noticeably, most patients did not have the habit of using dental floss. The overall satisfaction scores were 88.96% in the single crown group and 85.32% in the bridge crown group, respectively. Four indexes with less than 80 scores in the two groups were that I dare to use the implant teeth to bite hard food, my implant teeth are easily cleaned, it takes me less time to clean my implant teeth than natural teeth, and I think the dental implant price is acceptable. Results from the analysis of variance showed that food impaction influenced chewing effect (P=0.047), made the use of dental floss difficult (P=0.031), made patients discomfort (P=0.038), and the use of dental floss could enhance the cleaning effect on dental implant (P=0.019), and largely reduced brushing bleeding (P=0.029). In conclusion, patients with missing maxillary posterior teeth can obtain higher satisfaction after single crown or bridge crown repair. However, food impaction and crown off become more serious after repair with bridge crown, while the repair with single crown exhibits better performance.

4.Role of Video-assisted Thoracic Surgery in management of Penetrating Thoracoabdominal Injuries

Objective To evaluate the value of video-assisted thoracic surgery(VATS)for penetrating thoracoabdominal injuries.Methods Under general anaesthesia and double-lumen tracheal intubation,VATS was carried out to examine patients with chest injuries.Those who had mild injuries received VATS combined with mini-incision surgery.Electrocoagulation or suturing was employed to control intraoperative bleeding,and then blood clots were removed.Injured diaphragm muscles were repaired,and organs with hemorrhage or ruptures were sutured or resected.The seriously injured cases underwent open surgery and intra-abdominal exploration instead.Results A total of 18 patients received the operations.Among them,15 patients underwent VATS combined with mini-incision surgery(repair of the diaphragm was performed on 15,controlling intercostal arterial bleeding on 8,repair of the lung on 2,wedge resection of the lung on 2,and coagulated hemothorax removal on 2),the other 3 were converted to open surgery for repairing the diaphragm(3),heart(1),or esophagus(1),or lobectomy(2).Abdominal surgery via the thorax were performed on 13 cases,including repair of the liver in 1,lienectomy in 1,gastrorrhaphy in 1,repair of diaphragmatic hernia in 3,and intra-abdominal exploration in 7.Five patients received open surgery(pancreatic neoplasty in 1,repair of the liver in 1,lienectomy in 1,gastrorrhaphy in 2,and repair of the caul and mesenterium in 3).One patient who had multi-organ penetrating injuries died of hemorrhagic shock,circulatory failure,and DIC after open thoracic and abdominal surgeries.The mean operation time in this series was(125?44)minutes(ranged from 45 to 220),and the mean blood loss was(1089?582)ml(500 to 10 000 ml).Twenty-four hours after the operation,the mean volume of chest drainage was(234?75)ml(100 to 350 ml)in the first 24 hours after the operation.The chest drainage tube was withdrawn 2.5(2 to 5)days postoperation.The patients expelled gas in 1 to 4 days(mean,2 days).Totally,12 patients were followed up for 3 to 12 months(mean,6 months),during which none of them had trauma-related complications.Conclusion VATS combined with mini-incision surgery is safe and effective for patients with thoracoabdominal injuries,if the cases were carefully selected.

The migration of endothelial cell induced by fluid shear stress is related to many physiological and pathological responses in organism.The redistribution of membrane receptors and proteins,caused by shear stress,is a vital step for cell migration,followed by chemical reactions and signal transduction.Then cell morphological changes are induced,such as polarity,protrusion,and adhesion,which lead to the migration of endothelial cell at last.The goal of this paper is to review what is known about the mechanical models of endothelial cell migration and the responses of cells to fluid flow.It is helpful for better understanding the intrinsic mechanism of mechano-chemical signal pathway of the migration of endothelial cells induced by fluid shear stress.

Huang Ti's "Nei Ching Su Wen" is the earliest classical work on medical science in ancient China. It summed up the results of observations made in long years through practice by the ancient Chinese labouring people, accumulating thereby lots of valuable knowledge and experience in different lines of medicine.Some two thousand years ago, Chinese medical scientists in the Chou and the Chin Dynasties already conceived the advanced idea of laying special emphasis on prophylactic measures, and began to adopt diet therapy as a means of medical treatment. Principles of preparing complete diet were worked out by them. And in this connection, it is mentioned in the book "Nei Ching Su Wen" that "five kinds of cereals are the means of subsistence, five kinds of cattle provide beneficial food in the form of meat, five kinds of vegetables can be used for food enrichment, and five kinds of fruits may serve as supplement." These principles point out the necessary sorts of food to constitute a complete diet and their respective positions in it. That is to say, cereals including beans and peas are the staple food and animal meat is of secondary importance, with vegetables to enrich and fruits to supplement the first two. A diet thus prepared will not only consist of all the nutritive elements needed by the human body but also represent an appropriate arrangement of the different kinds of nutrients. Such a diet is, no doubt, in accord with the dietetic theory of modern science.
9.Risk factors for acute kidney injury after aortic arch operation under deep hypothermic circulatory arrest

Objective To analyze risk factors for acute kidney injury after aortic arch operation under deep hypothermic circulatory arrest.Methods Between January 2005 and June 2011,549 cases aortic arch replacement under deep hypothermic circulatory arrest were retrospectively analyzed.According to the occurrence of acute kidney injury they were divided into two groups.Univariate and multivariate analysis (multiple logistic regression) were used to identify the risk factors.Results AKI occurred in 102 cases (18.6%) and 27 cases(4.9%) had dialysis.Multiple logistic regression showed that body mess Index(OR =1.072,95% CI:1.006-1.141,P =0.031),serum creatinin (OR =1.011,95% CI:1.006-1.017,P =0.000),cardiopulmonary bypass time(OR =1.006,95 % CI:1.002-1.009,P =0.005) and the peak intraoperative glucose level (OR =1.007,95 % CI:1.002-1.011,P =0.003) were independent risk factors for AKI.Conclusion The higher BMI,serum creatinin level maybe indicate the occurrence of AKI,and AKI maybe can be reduced by controlling CPB time and intraoperative hyperglycemia.
10.Reoperation on aortic disease in patients with previous aortic valve surgery

Objective Retrospectively analyze 47 cases received reoperation with aortic disease after aortic valve replacement to deepen the understanding of aortic valve disease.Methods From January 2003 to June 2012,47 patients with previous aortic valve replacement received aortic root or other aortic operation because of new aortic disease.38 male and 9 female,the interval (6.0 ± 3.8) years. All cases with new aortic disease were diagnosed by cardiac ultrasound and aortic computed tomography.Bentall's procedure were operated on 14 patients,total aortic arch replacement with elephant trunk procedure on 14 patients,aortic root and aortic arch with elephant trunk procedure on 7 patients,ascending aortic replacement on 10patients,total thoracic and abdominal aorta replacement on 2 cases.All patients were followed by clinic interview or telephone.Results Aortic dissection and aneurysmal dilatation were occurred on ascending aorta,each account for 50%,in patients with previous aortic valve replacement because of rheumatic valve disease and bicuspid aortic valve; 3 cases with Marfan syndrome occurred ascending aortic dilatation and 4 cases occurred aortic dissection.Diameter in ascending aorta increased (5.2 + 7.1)mm per year and aortic sinus (3.3 ± 3.1)mm per year.The value of ascending aortic dilatation per year in patients with rheumatic disease was higher than patients with Marfan syndrome(P < 0.05).47 patients were re-operated in fuwai hospital,1 patients died in operating room because aortic dissection seriously involved right coronary artery.7 patients have renal insufficiency after operation and all were cured by hemofiltration; neurological complication occurred in 14 patients including that 7 patients stroked and 7 patients had transient brain dysfunciotn.There were no postoperative spinal cord deficits occurred.All patients were followed up,the mean follow up time were(53.49 +33.79) months.8 cases were died during follow-up and threeyear survival rate was 83%.There were no cases received operation due to aortic disease during follow-up.Conclusion Deepening the understanding of aortic valve disease combine ascending aorta changes,especially pay attention to patients with previous aortic valve replacement because of Marfan syndrome and rheumatic disease during follow-up after first operation,all efforts should decrease the occurrence of aortic adverse events in long term.
