1.Comparison of three anesthetic methods in rabbit bone defect repair

Objective To compare the application effects of pentobarbital sodium ,urethane and midazolam combined with ket-amine/Sumianxin Ⅱ in rabbit bone defect repair .Methods 120 healthy male adult rabbits were randomly divided into three groups according to different anesthetic agents ,40 cases in each group .The group A was intramuscularly injected by pentobarbital sodium , the group B was given urethane by the ear vein and the group C was given midazolam combined with ketamine/and Sumianxin Ⅱ by intramuscular injection .The onset time of anesthesia ,maintenance time ,breathing rate ,heart rate ,complications ,recovery time ,ad-ditional anesthetic agents and postoperative death rate in 3 groups were recorded respectively .Results The onset time of anesthesia in the group B and C was(4 .53 ± 0 .65) min and(3 .56 ± 1 .31) min respectively ,which were significantly shortened compared with the group A(P<0 .05);the maintenance time of anesthesia in the group B was (152 .12 ± 12 .53) min ,which was significantly short-ened compared with the group A and C (P<0 .05);the recovery time in the group C was (31 .05 ± 5 .73) min ,which was significant-ly shortened compared with the group A and B(P<0 .05) ,the propability of additional anesthetic agents and the postoperative mor-tality in the group C were significantly decreased compared with the group A and B (P<0 .05) .Conclusion The combined anesthe-sia of midazolam and ketamine/Sumianxin Ⅱ has definite effect without any observed intraoperative depressions of breathing rate and heart rate and with high safety ,high controllability and less influence on the long term survival of the experimental animals , which is a more ideal anesthesia method of the rabbit bone defect repair .
2.Application of locking plate in elderly patients with proximal humeral fractures

Objective To explore the clinical curative effect of locking plate in elderly patients with proximal humeral fractures. Methods Seventy elderly patients with proximal humeral fractures were selected, and all patients were treated with surgical treatment. Among them, application of locking plate was in 35 cases (locking plate group), and application of ordinary steel plate was in 35 cases (ordinary steel plate group). The operation time, intraoperative blood loss, fracture healing time and curative effect between 2 groups were compared. Results The operation time, intraoperative blood loss and fracture healing time in locking plate group were significantly lower than those in ordinary steel plate group: (61.3 ± 5.6) min vs. (78.3 ± 4.2) min, (190.5 ± 11.5) ml vs. (268.5 ± 21.2) ml and (12.6 ± 1.8) weeks vs. (16.5 ± 1.5) weeks, the excellent and good rate was significantly higher than that in ordinary steel plate group: 94.29% (33/35) vs. 74.98% (26/35), and there were statistical differences (P<0.05). Conclusions The curative effect of locking plate in elderly patients with proximal humeral fractures is accurate. Locking plate can be fixed firmly, and it is conducive to early recovery after surgery.
3.Application of Advanced Simulation Human in Clinical Practice of Medical Nursing

Objective To explore the effect of comprehensive experiments on clinical practice of internal medicine nursing in the patients with advanced simulation human Simman3G.Methods Two grades of nursing students were selected by random cluster sampling and divided into the experimental group and the control group.The control group used the traditional clinical probation method to carry out clinical probation teaching in Department of internal medicine,the experimental group used the advanced simulation human Simman3G to conduct a comprehensive simulation of teaching before clinical probation.After the completion of the internship the self compiled questionnaire was given to two groups of students to investigate their opinion on the clinical probation.Then we compared,the semester examination results between two groups.Results In the experimental group,the grade of internal medicine nursing was higher than the control group at the end of the semester,P < 0.05.The results of questionnaire survey showed that there was no significant difference between the two groups in the promotion of theoretical knowledge and the two dimensions of the survey of nursing work environment.In the training of clinical thinking,improve nursing skill,communication skill,and the learning interest.At the great harvest of 5 dimensions of probation,the experimental group of students' recognition degree was significantly higher than the control group,P < 0.01.Conclusion Before clinical probation of internal medicine nursing,the application of high-end analog teaching can significantly improve the effectiveness of clinical probation.
4.Effects of pacing the right ventricular outflow tract on the cardiac hemodynamic and the ventricular activation sequence

Objectives To compare the hemodynamic effects and ventricular activation sequence associated with the respective pacing of the right ventricular outflow tract (RVOT) and right ventricular apex (RVA). Methods 79 patients with complete atrioventricular block (AVB) or sick sinus syndrome (SSS)to undergo permanent ventricular stimulation either at the right ventricular apex (RVA) (n = 38) or right ventricular outflow tract (RVOT) (n =41) were selected. The left ventricular ejection fraction (LVEF),cardiac index (CI), mitral E/A ratio and QRS duration were recorded before implantation of pacemaker and 6 or 12 months after implantation. Results There were no significant differences in CI , LVEF, E/A between RVOT and RVA pacing(P >0. 05). Compared with normal sinus rhythm, the QRS duration was significantly lengthened when pacing at RVOT or RVA (P <0. 05). RVA pacing was significantly longer than RVOT in QRS duration (P < 0. 05). Conclusions There was no significant difference with the hemodynamic effect between RVOT and RVA pacing in subject with normal heart function. Compared with RVA pacing, RVOT pacing remained relatively normal ventricular activation sequence.

7.Endovenous laser treatment of the varicose veins in lower limbs: A retrospective analysis of 450 cases

Objective To evaluate the efficacy of endovenous laser treatment(EVLT) in the treatment of varicose veins in lower limbs. Methods A total of 450 cases of varicose veins in lower limbs(606 limbs) received EVLT with 940 nm diode laser under a continuous mode from October 2003 to February 2006.All the patients were evaluated with color ultrasonography after the operation.Results The follow-up duration was 2~29 months(mean,16 months).Satisfactory effects were achieved in XXX cases,with varicose veins subsided and sense of swelling and heaviness significantly relieved.In 49 cases accompanying active ulcers before operation,the ulcer healed within 10 days after operation in 44 cases and within 16 days in 5 cases.Five cases experiencing a heat injury were dismissed from hospital on the 7th day after operation.Conclusions EVLT is a minimally invasive operation possessing safety,simple performance,and effectiveness,and without leaving any scar.It is an ideal method in the treatment of varicose veins of lower limbs.

9.Therapeutic effect of intracranial hematoma smash puncture on the aged patient with cerebral hemorrhage

Objective To analyze the advantages of intracrani al hematoma smash puncture (IHSP) in the treatment of cranial hemorrhage for the elderly. Methods A total of 104 cases of cerebral hemorrhage a dmitted in the recent 3 years were collected and divided into 4 grou ps, i.e . group A, elderly with IHSP treatment (n=36), group B, elderly with regular medical treatment (n=33), group C, non-elderly with IHSP treatment (n= 21) and group D, non-elderly with routine therapy (n=14). The clinical and follow-up outcomes were studied and analyzed. Results There w as significant difference in rehemorrhage rate after puncture between grou p A (0/36) and group C (2/21). Significant difference was found in early mortali ty rate in group A (16.67%) and group B (39.40%). The rate of activity of daily living (ADL) 1-2 was significantly higher in group A (58.62%) than group B (5.88 %)(P<0.005) and that of ADL 4-5 was obvious lower in group A (13.79%) than g roup B (47.06%)(P<0.025). Conclusion Intracranial hematoma smash hemorrage is safe and efficient in the treatment of elderly cerebr al hemorrhage, especially in reducing the disability rate.

The treatment plan of facial neuritis in Evidence-based Guidelines of Clinical Practice in Acupuncture and Moxibustion (2011 edition) is discussed, and case information of facial neuritis during the recent five years in department of acupuncture and moxibustion, PLA General Hospital, is retrospectively analyzed. In accordance with anatomy of the facial nerve to form the acupuncture prescription, the detailed diagnosis and treatment method for facial neuritis are introduced. The advantages of the diagnosis and treatment method for facial neuritis are summarized, hoping to establish a more comprehensive, standardized and unified treatment plan.
