Objective: To compare the efficacy of different treatment protocols in treating transient synovitis of the hip (TSH) in children and to optimize the clinical treatment strategy for this condition. Methods: Ninety kids with TSH were divided into a control group, a chiropractic group and a chiropractic plus foot bath group using the random number table method, with 30 cases in each group. The control group was treated with conventional traction; the chiropractic group was given chiropractic treatment based on the control group; the chiropractic plus foot bath group was given Chinese medicine foot bath based on the chiropractic group. Traction and foot bath were conducted once daily while chiropractic was done once every other day, all with 14 d as a treatment course for a total of two courses. Changes in the visual analog scale (VAS) score and range of motion (ROM) of the hip joint in the three groups were observed, and the efficacy was compared. Results: The total effective rate was 93.3％ in the chiropractic plus foot bath group, versus 76.7％ in the chiropractic group and 66.7％ in the control group, and the total effective rate was notably higher in the chiropractic plus foot bath group than in the other two groups (both P<0.05). Respectively after the first and second treatment course, the VAS score decreased significantly in each of the three groups compared with that before treatment (all P<0.01), and the ROM of the hip joint in flexion increased significantly (all P<0.01). After two treatment courses, the VAS score was lower in the chiropractic plus foot bath group than in the other two groups (both P<0.05), and its ROM of the hip joint in flexion was larger than that in the other two groups (both P<0.05). Conclusion: Based on traction, chiropractic plus Chinese medicine foot bath can effectively reduce pain and improve motor function of the hip joint in treating TSH.

This study aims to construct a eukaryotic expression system for envelope gene of Jaagsiekte sheep retrovirus, observes its localization in 293T cells, and investigates the potential in inducing malignant transformation of NIH3T3 cells. By RT-PCR, the full-length cDNA of envelope gene of Jaagsiekte sheep retrovirus (exJSRV-env) was amplified from the extract of naturally infected sheep lung. The clone of target gene was sub-cloned into eukaryotic expression system pEGFP-C1, and validated by PCR, restriction endonuclease, and sequencing. Bioinformatic analysis concerning biological function and cellular localiza tion of exJSRV-env was also performed. The recombinant clone of exJSRV-env was transfected into 293T cells and NIH3T3 cells by Lipofectamine LTX. The expression and celluar localization in 293T cells were validated by confocal microscopy. Soft agar colony formation assay was employed to test the anchorage-independent growth of NIH3T3. DNA sequencing and restriction enzyme digestion with Kpn I and Hind III indicated the correct construction of the recombinant plasmid, which was named pEGFP-C1/exJSRV-env. Amino acid sequence alignment of exJSRV-env with reference sequences found 85%-100% homogeneity. A YRNM motif was discovered at the cytoplasmic tail of envelope gene, which is exclusively found in exogenous viruses. Phylogenetic tree analysis showed that our clone of exJSRV-env clustered closely with pathogenic exogenous Jaagsiekte sheep retroviruses. Fluorescence microscopy indicated typical membrane localization of exJSRV-env protein. NIH3T3 cells transfected with exJSRV-env lost contact inhibition, and acquired colony forming ability in soft agar. This study indicated that envelope protein of Jaagsiekte sheep retrovirus can induce malignant transformation of mouse fibroblast cell NIH3T3. Discoveries of this study provide a basis for further structural and functional research on Jaagsiekte sheep retrovirus envelope protein.

Abstract: Mycobacterium senegalense is one of the major pathogens causing bovine farcy, and reports of its infection in human are rare. Here is a report on a woman who had been taking hormones and immunosuppressants for a long time for SLE and underwent abdominal soft tissue infection with Mycobacterium senegalense after abdominal liposuction, to provide reference for clinical diagnosis and treatment. The patient, female, 32 years old, has a history of SLE for more than 2 years, and currently takes "methylprednisolone, hydroxychloroquine, and mycophenolate mofetil" regularly. Nine months before the patient was admitted to the hospital, she once performed abdominal, waist and buttock liposuction in a medical beauty institution. One month after the operation, several masses gradually appeared on the abdominal wall, accompanied by tenderness, one of the masses had obvious fluctuation on palpation and purulent fluid could be drawn out. The location of the abdominal wall mass was consistent with the insertion site of the liposuction needle. After the onset of the disease, the patient went to the medical beauty institution for puncture of the abdominal wall mass, and 5 mL of purulent fluid was pierced and sent for bacterial culture, and cultured "Mycobacterium Senegalense", after 3 days of treatment with "cephalosporin" antibiotics (specifically unknown), the symptoms did not improve, so she went to the second affiliated hospital of hainan medical college. After completing the relevant examinations during the hospitalization in our hospital, in order to clarify the etiology, another abdominal puncture to extract pus was performed, the mycobacterial culture + identification results: Mycobacterium senegalense. Consistent with the out-of-hospital results, the diagnosis of Mycobacterium senegalense infection was confirmed. After 3 months of treatment with "cefoxitin, azithromycin, amikacin, and levofloxacin", the patient's abdominal wall soft tissue infection was cured. Trauma or invasive procedures can lead to skin, muscle, or bone infection with nontuberculous mycobacteria (NTM), which can manifest as chronic painless nodules that progress to purulent folliculitis and abscesses. NTM infection should be suspected when the patient's wound has been exposed to water, there is a history of surgery, and empirical anti-infection is ineffective. This is the first case of Mycobacterium senegalense infection caused by medical beauty, which tell people that they should be cautious when choosing medical aesthetic projects and medical aesthetic institutions.

Objective To investigate the clinical effect and safety of pidotimod combined with ganciclovir for injection in the treatment of children with infectious mononucleosis.Methods Ninety-four children with infectious mononucleosis were divided into control group and treatment group,each group 49 cases.Control group was treated with ganciclovir for injection 5 mg · kg-1 +5％ glucose injection 100 mL,q12 h,intravenous drip.On the basis of control group,treatment group was treated with pidotimod oral solution 400 mg,bid,orally.All children were treated for 2 weeks.The immune function,cytokines and myocardial enzymes were measured before and after treatment,the symptoms and signs of remission were observed and recorded,the clinical efficacy and adverse drug reactions were compared in two groups.Results After treatment,the clinical efficacy in treatment group and control group were 91.49％ (43 cases /47 cases),74.47％ (35 cases/47 cases),with significant difference (P ＜0.05).After treatment,the serum CD4 ± in treatment group and control group were (34.52 ± 7.24)％ and (29.83±6.65)％,CD8± were(26.27 ±6.36)％ and(31.82 ±6.73)％,CD4±/CD8± were 1.38 ±0.40 and 0.93 ± 0.35,immunoglobulin A (IgA) were (1.21 ± 0.25) and (0.99 ± 0.23) g · L-1,immunoglobulin G (IgG) were (10.03 ± 1.50) and (8.94 ± 1.42) g · L-1,serum tumor necrosis factor-α (TNF-α) were (1.14 ± 0.52) and (1.81 ± 0.63)ng · L-1,serum interleukin-6 (IL-6) were (11.20 ± 3.56) and (14.08 ± 3.75) ng · L-1,all with significant difference (all P ＜ 0.05).The adverse drug reactions were mild nausea,vomiting and other gastrointestinal reactions in treatment group,with the incidence of 10.64％ (5 cases/47 cases).The adverse drug reactions were mild nausea,vomiting and other gastrointestinal reactions and rash in control group,with the incidence of 8.51％ (4 cases/47 cases).There was no significant difference between the two groups (P ＞ 0.05).Conclusion The clinical effect of pidotimod combined with antiviral drugs in the treatment of children with infectious mononucleosis is exact,it can improve the immune function and reduce the myocardial enzymes and inflammatory markers.

Objective To investigate the effects of different deflation methods of endotracheal tube cuff on coughing response and hemodynamics during extubation. Methods Ninety patients undergoing elective surgery for endotracheal intubation under general anesthesia were selected as study subjects and randomly divided into study group and control group, with 45 patients in each group. In the study group, the cuff end of the endotracheal tube cuff was connected to a syringe and a non-liquid pressure gauge through a three-way stopcock. Before extubation, the cuff was aspirated to decrease the cuff pressure at a rate of 3 cmH₂O/s. In the control group, the gas in cuff was rapidly deflated by aspirating all the gas inside with a syringe during extubation. The incidence and severity of coughing response during extubation were recorded in both groups. Mean arterial pressure (MAP) and heart rate (HR) were recorded before induction of general anesthesia (T₀), before cuff deflation (T₁), immediately after cuff deflation (T₂), 1 minute after extubation (T₃), 3 minutes after extubation (T₄), and 5 minutes after extubation (T₅). Adverse events were also recorded in both groups. Results The coughing response during extubation started immediately after cuff deflation in both groups. The incidence and severity of coughing response were lower in the study group than in the control group (P < 0.05). Compared with the control group, values of MAP were lower at T₂ to T₄ and values of HR were lower at T₂ to T₅ in the study group (P < 0.05). The incidence of post-extubation pharyngeal discomfort was 6.67 % in the study group, which was lower than 26.67 % in the control group (P < 0.05). There was no significant difference in the incidence of post-extubation hypoventilation between the two groups (P>0.05). Conclusion The method of slowly reducing the pressure of the endotracheal tube cuff at a constant rate can reduce the incidence and severity of coughing, decrease postoperative extubation complications, and stabilize hemodynamics, with better effects than the method of rapidly aspirating all the gas inside the cuff at once.

OBJECTIVETo explore the current status of HIV infection and HIV-related knowledge, attitude and practice (KAP) among men who have sex with men (MSM) in 14 cities in China.

METHODSA cluster sampling method was administrated and 14 large or medium cities were selected. The respondent-driven sampling (RDS) was also used to conduct a questionnaire survey in 6101 MSM. Blood specimens were collected for HIV and syphilis testing.

RESULTSA total of 6101 MSM were selected and investigated, with an HIV infection rate of 6.3% (384/6101) and a syphilis infection rate of 14.0% (854/6101). The proportion of MSM knowing correct HIV/AIDS knowledge was 59.7% (3644/6101); 6.2% (378/6101) and 11.2% (683/6101) of the respondents ever had buying sex or selling sex in the last six months respectively; the proportion of consistent condom-using each time while with male partners in the last month was 39.2% (2392/6101). The proportion of condom-using while with male partners was 65.2% (3978/6101) in the last time among MSM.

CONCLUSIONA high HIV infection rate was observed among MSM with limited HIV/AIDS awareness and high risk behaviors. It is necessary to initiate a rapid response to prevent HIV spreading in MSM.

The anatomical location of lymphocyte ontogeny in the developing mouse embryo remains controversial. To define the site that can generate lymphocytes de novo, the intraembryonic splanchnopleura (SP) and extraembryonic yolk sac (YS) at 8.5 days postcoitum, when systemic circulation is not established, were investigated. The results indicated that in standard colony forming assay, the cells from both splanchnopleura and yolk sac formed typical myeloerythroid colonies, but their types were distinct. When cocultured with the OP9, the splanchnopleura produced B cells expressing B220, CD19 and surface IgM. Using a three-step culture protocols with the OP9 expressing Delta-like 1 as feeders, the splanchnopleura produced immature T precursor cells (CD44-/CD25+) and more mature single positive T cells (CD4+/CD8-) after 16 days of incubation. However, the yolk sac failed to generate B and T lymphocytes under identical conditions. It is concluded that prior to linked embryonic circulation, the splanchnopleura other than the yolk sac had robust lymphoid potential in vitro. In the future, more reliable evidence from novel model animals will ultimately delineate the embryonic origin of lymphocytes in vivo.

OBJECTIVETo study the HIV infection rate and relevant behavioural characteristics among drug users (DUs) in the 15 cities of China-Gates HIV/AIDS Program.

METHODSBy literature review from the public security sector and expert consultations, the size of drug using population was estimated, questionnaire was used to survey relevant behavioural characteristics of DUs, and blood specimens were collected for HIV, syphilis and HCV testing.

RESULTSIn 2007, the number of registered DUs in the 15 cities was 176 004, while the estimated number was 477 989. DUs mainly used heroin and crystal meth. Of the survey subjects, 41.0% (2685/6155) had correct knowledge about HIV/AIDS, and Hangzhou 97.6% (410/420), Qingdao 85.5% (342/400), Wuhan 69.7% (308/442), Kunming 63.8% (255/400), Shanghai 45.4% (237/522), Xi'an 44.5% (178/400), Nanjing 38.8% (155/400), Beijing 36.5% (237/650), Guangzhou 29.5% (122/413), Haerbin 25.4% (114/447), Haikou 23.8% (96/402), Chongqing 22.4% (90/401), Sanya 20.0% (80/400), Tianjin 16.2% (41/253), Shenyang 9.8% (20/205), and the difference among the cities has statistical significance (χ(2) = 1482.833, P < 0.05). The rate of needle sharing at the last injection was 10.8% (665/6155), and Hangzhou 10.8% (45/420), Chongqing 26.4% (106/401), Guangzhou 2.6% (11/413), Shenyang 31.3% (64/205), Shanghai 0.4% (2/522), Kunming 10.8% (43/400), Beijing 10.6% (69/650), Tianjin 5.4% (14/253), Xi'an 3.3% (13/400), Wuhan 17.9% (79/442), Sanya 20.0% (80/400), Haikou 2.2% (9/402), Haerbin 8.5% (38/447), Nanjing 21.4% (86/400), Qingdao 1.5% (6/400), and the difference among the cities has statistical significance (χ(2) = 483.044, P < 0.05). Only 5.0% (8737/176 004) of DUs and 1.2% of HIV positive DUs participated in needle exchange. The HIV, syphilis and HCV infection rates were respectively 2.76% (155/5620), 3.38% (190/5620) and 32.35% (1818/5620). The HIV infection rates of different city was:Hangzhou 0.4% (1/273), Chongqing 7.5% (30/401), Guangzhou 2.9% (12/413), Shenyang 0.5% (1/205), Shanghai 0.32% (1/316), Kunming 16.25% (65/400), Beijing 2.6% (16/624), Tianjin 2.0% (5/253), Xi'an 1.25% (5/400), Wuhan 0.24% (1/411), Sanya 1.75% (7/400), Haikou 0.2% (1/402), Haerbin 1.1% (5/447), Nanjing 1.1% (3/275), Qingdao 0.5% (2/400), and the difference among the cities has statistical significance (χ(2) = 354.203, P < 0.05).

CONCLUSIONThe HIV prevalence rate of IDUs in 15 cities slowly decreased although the HIV prevalence rate of IDUs in Chongqing and Kunming and other cities had surpassed the international warning level. The risk of sharing needles for HIV infection was dropped, and more attention should be paid to the potential risk of HIV transmission via unsafe sex due to the use of new drugs, such as amphetamine chloride.

AIMTo investigate the pharmacokinetics of mycophenolic acid (MPA), an active metabolite of mycophenolate mofetil (MMF) in Chinese adult liver transplant patients.

METHODSThirty-eight liver transplant patients (male 30, female 8) receiving MMF 1.0 g, twice daily in accordance with the recommended regimen were included in this study. Plasma MPA concentrations were measured by high performance liquid chromatography at 0.5, 1, 1.5, 2, 4, 6, 8, 10 and 12 h after the administration of a single dose. Pharmacokinetic parameters were calculated with 3P97 software.

RESULTSThe plasma MPA concentration-time curve was characterized with an early sharp peak reached at 0.5 - 6.0 h after oral administration. And in some patients there was a small second peak due to enterohepatic circulation of mycophenolic acid glucuronide (MPAG), which underwent deglucuronidation and re-absorption as MPA at 4 to 12 h postdose. The mean peak plasma concentration (C(max)) and area under concentration-time curve (AUC(0-12 h)) were (12 +/- 7) microg x mL(-1) and (44 +/- 16) microg x h x mL(-1), respectively. However, a large variability of pharmacokinetic parameters existed in these patients.

CONCLUSIONIn view of the inter-individual variability of MMF pharmacokinetics, plasma MPA concentration should be monitored routinely after MMF administration for individual patient.

OBJECTIVETo explore the relevant factors of the causes of sexual orientations of gay.

METHODFrom March to June 2013, 350 gays were recruited from one music bar and three bath centers where gays frequently visited in Changsha city, by proportional stratified sampling method. Meanwhile, another 332 males who identify themselves as non-homosexuality were also recruited considering the composition of ages, gender and educational background. Questionnaire survey was conducted to all the subjects, with 300 effective ones reclaimed. The questionnaire included the general demographic information, traits of character, the condition of foster in childhood and information of family members. The differences between the gays and non-homosexuality groups were analyzed to explore the causes of the sexual orientations of gays.

RESULTSThere were statistical significant differences between gays and non- homosexuality group on following indexes (χ(2) was 59.63, 5.90, 16.01, 84.99, 161.57, 77.77, 112.32, 190.84, 30.10 respectively, all of P < 0.05) :had a tender father and an impervious mother, were physically weak, not agile, physically inactive, attentive to details, highly conservative, not adventurous, and radical in childhood, were raised as girls before the age of 18, liked to dress as girls before the age of 18, don't liked to play toy knives and toy guns before the age of 18, suffered from sexual abuse before the age of 18 (e.g. forced to expose private parts or forced to have sex) by adults, had read or watched books or films about homosexual and experienced sexual pleasure from that before the age of 18. The rate of gays on these indexes was separately 62.3% (187/300), 57.7% (173/300) , 62.3% (187/300) , 63.0% (189/300), 67.3% (202/300) , 62.7% (189/300), 68.0% (204/300), 65.0% (195/300) and the rate on these indexes of non-homosexuality group was separately 21.3% (64/300), 28.0% (84/300) , 25.0% (75/300) , 12.7% (38/300), 31.3% (94/300), 17.7% (53/300) , 12.7% (38/300), 42.7% (128/300) . The rate of gays on these factors:the youngest boy in family, had the father or twin brothers who were homosexual or self identified as gay was 62.7% (188/300), 56.0% (168/300) and 62.0% (18/29) respectively; and the rate was 40.7% (122/300), 4.0% (12/300) and 20.0% (2/10), respectively among non-homosexuality group. The difference showed statistical significance (χ(2) was 34.52, 193.14, 5.27 respectively, all of P < 0.05).

CONCLUSIONThe correlative factor of sexual orientation of gays maybe was family relationship, tend and education since childhood, psychological characteristics, sexual experience during puberty.
