1.Research on the applications of wireless analgesic system in anesthesiology department

Objective:The effect of Wireless Analgesia System for the patients with postoperative pain, chronic pain and cancer pain in Anesthesiology Department will be discussed in this article. Methods:With the wireless signal being sent from the analgesia terminal transmitter to the base station of analgesia control, the analgesia pump can be connected with wireless analgesia terminal. Drugs can be delivered persistently or intermittently to keep the plasma concentration in an appropriate level according the vested parameters from the wireless system of analgesic delivering. Results:The information from the patients in the process of analgesia can be processed and saved by the system. According to the information of running and alarm displaying at the monitoring station, the analgesia records can be kept automatically. Conclusion:The satisfaction degrees on analgesia treatment can be improved, and the burden of work for anesthesiologists can be lessened as well, which improve the quality of medical services and promote the development of informatization in anesthesiology department.
2.Placental endoglin expression in women with severe preeclampsia

Objective To investigate the relationship between placental endoglin expression and the pathogenesis of severe preeclampsia (SPE). Methods Forty nine pregnant women with SPE and 40 normal pregnant controls were collected in Peking University First Hospital from January 2006 to January 2008,among which,nine SPE patients complicated with fetal growth restriction (FGR),six with hemolysis,elevated liver enzymes and low platelets syndrome (HELLP syndrome) and 12 with heavy proteinuria. The expression of placental endoglin was detected and semi-quantified by immunohistochemistry.Data were analyzed by independent sample t test. Results Endoglin was presented on cell membranes of placental syncytiotrophoblasts and vascular endothelial cells. The endoglin density of SPE group was higher than that of control group (0.1621± 0.0029 vs 0.1576 ± 0.0038,t=- 6.367,P<0.05).No significant difference in endoglin density was found between FGR group and non-FGR group (0.1611±0.0026 vs 0.1623±0.0029,t=1.107,P>0.05) ; neither did the heavy proteinuria group and non-heavy proteinuria group (0.1611±0.0032 vs 0.1624±0.0027,t=1.350,P>0.05).The endoglin density of HELLP group was lower than that of non-HELLP group (0.1595±0.0032 vs 0.1625±0.0027,t=2.495,P=0.016). Conclusions The elevated placental endoglin expression might contribute to the pathogenesis of SPE.
3.The application of objective structured clinical examination in the graduation practice examination of undergraduate nursing students

Objective To summarize the principle and structure of objective structured clinical examination(OSCE)in the graduation practice examination of undergraduate nursing students.Methods A OSCE model was designed with a case running through the whole process of it by referring to the classic model of OSCE and applied it to the graduation practice examination of undergraduate nursing students of grade 2004.Results 75.0%students believed that the setup of OSCE location was reasonable and 78.1%of them thought that they were used to this kind of examination.100.0%of them considered that this examination model could objectively evaluate the general clinical capacity and at the same time make clear the future purpose of next internship stage. Conclusion The OSCE model could objectively evaluate the general clinical ability of students and truly reflect the existing problems,thus provide benefitable reforrences for the next internship stage.
4.Reflecting on the supervisors' consciously strengthening quality control of the academic degree graduate students' talent education

Improving the education quality of graduate students is the new situation of gradu-ate education reform and development to each university of China. During the overall process of ex-ploring and reforming quality control of graduate student talent cultivation, institute of life sciences in Chongqing Medical University put forward that intensively constructing the supervisors' conscious quality control of talent cultivation was the driving force to realize the quality improvement of talent cultivation. The construction and improvement of supervisors' conscious quality control on graduate education was reflected according to the supervisors' professional quality, postgraduates' thought moral education, and postgraduates' scientific literacy training, respectively. This will lay the foundation for the future comprehensive improvement of postgraduates' academic ethics and effective promotion of the educative reform on graduate education quality.
5.Discussion on quality control of dissertation review and oral defense of medical science degree postgraduates

The dissertation review and defense process of medical science degree graduate stu-dents has become a mere formality because of the manner of postgraduates cultivation, cultural factors and objective factors, resulting in poor quality of degree dissertation. This paper deeply analyzed the reasons for the above problems, and proposed five aspects of adjustment through the reviewer choosing, review process, evaluation scale, evaluation content and evaluation opinions to perfect the eval uation system. It also proposed four aspects of adjustment through strengthen the significance, thesis defense, defense supervision content development process and the detalled reply results to perfect defense system.

7.A possible mechanism of relief of symptoms by transmyocardial laser treatment for angina pectoris

Objective To explore the mechanism of relieving symptoms of angina pectoris with transmyocardial laser treatment. Methods Twenty mongrel dogs were divided randomly into 4 groups (5 dogs each): I. sham-operated negative control animals undergoing thoracotomy and pericardiotomy only; II. ligation of the coronary arteries to create acute myocardial ischemia; III. 40 minutes after production of acute myocardial ischemia, transmyocardial punctures were made with high power CO_2 laser; IV. chemical destruction of cardiac nerves by application of 85% phenol to the epicardium after myocardial ischemia. The myocardium was harvested for examination 2 weeks later. Cardiac sympathetic afferent nerve function and innervation was assessed with immunohistochemical staining of tyrosine hydroxylase (TH), which was the specific enzyme of sympathetic nerves, and computer image analysis was done. Histopathological changes in myocardium were also evaluated with haematoxylin-eosin staining. Results Immunohistochemical analysis showed positive staining in I and II groups but negative or weakly positive in III and IV groups. 8.Pharmacokinetics character of propofol during anhepatic period of pediatric liver transplantation

Objective: To investigate pharmacokinetics of propofol during anhepatic period of orthotopic liver transplantation (OLT) in children. Methods: In 5 children undergoing OLT, 2 mg/kg propofol was injected intravenously at the beginning of induction and anhepatic periods respectively. The plasma concentration of propofol was measured by reverse-phase high performance liquid chromatography(RP-HPLC), and the pharmacokinetics character of propofol between anhepatic and inducing periods were compared. Results: Propofol had wide distribution and high clearance, and its plasma concentration-time curves were fitted to a three-compartment open model. Conclusion: There is no difference of propofol metabolism between anhepatic and inducing periods, suggesting that there must be notable extra-hepatic metabolism when propofol used in children undergoing OLT.
9.Number of Fetal Nucleated Red Blood Cell in Maternal Blood and Placenta in Pregnancies with Intrauterine Growth Retardation

Objective To examine the number of fetal nucleated red blood cell (NRBC) in maternal blood and placenta tissue in fetal growth restriction(FGR) pregnancies. Methods 20 women of 28-36 weeks' gestation at age of 21~30(including 9 FGR pregnancies)were chosen. Fetal cells were isolated from maternal blood with discontinuous density gradient centrifugation. The isolated cells were made smear and counted under the microscope; After delivery, the placenta tissue were made into sections and also counted under the microscope; To determine the origin of the NRBC , the single NRBC was analysed by primer extension preamplification (PEP) and polymerase chain reaction (PCR). Results The number of NRBC in 9 FGR pregnancy women's peripheral blood ranged from 12/7 ml~40/7 ml,(average 22.6/7 ml). The number of NRBC in the control pregnancies ranged from 0/7 ml~10/7 ml, (average 5.4/7 ml). 10.The predictive value of DWI for progression free survival in patients with cervix cancer after neoadjuvant chemotherapy

Objective To evaluate the predictive value of diffusion weighted imaging ( DWI) for progres-sion free survival(PFS)in patients with cervix cancer after neoadjuvant chemotherapy (NACT).Methods Thirty two consecutive patients with pathologically confirmed cervical cancer underwent MRI including DWI before and after NACT.Pathologic results and MR images were reviewed .Univariate analysis was applied by Kaplan -Meier method.The Cox proportional hazard model was used to evaluate prognostic variables for multivariate analysis .Re-ceiver operating characteristic ( ROC) curves were used to find optimal cutoff values of independent prognostic fac -tors for disease progression.Results Of 32 patients,10 had disease progression during follow -up period.In uni-variate analysis,FIGO stage,tumor size,the depth of tumor invasion,lymph node metastasis and ADC changes be-tween before and after neoadjuvant chemotherapy (ΔADC) were significantly associated with the disease progres-sion.In multivariate analysis , FIGO stage and ΔADC were the independent prognostic factors for PFS .Optimal cutoff values that determined by ROC curves analyses were FIGO stage ⅡandΔADC 0.31.Area under the ROC curve(AUC)of FIGO stage and ΔADC were 0.841(sensitivity 90.0%,specificity 68.2%)and 0.864(sensitivity 80.0%,specificity 81.8%),respectively.Conclusion In patients with cervix cancer after NACT ,FIGO stage andΔADC are significant predictive values for PFS .
