The clinical data of 39 cases of idiopathic hyperaldosteronism (IHA) confirmed by operation were analyzed. The indications of operation consisted of (1) unable controlled the patients′ conditions by pharmacotherapy, (2) severe side effects due to high doses of medication and (3) difficulty in distinguishing between adenoma and nodule hyperplasia. The unilateral total adrenalectomy was suggested.

Objective To evaluate Caspofungin for the treatment of fungous infection liver transplant patients. Methods From 2003 to 2008 clinical data of 27 cases of liver transplant patients with fungal infections were reviewed retrospectively. Before 2005 (control group) fungal infections were treated with amphotericin B or Fluconazole in 13 cases. After 2005, the infection was treated with Caspofungin (observation group) in 14 cases. Liver function (AST, ALT, TBIL) and renal function (BUN, Scr) were evaluated at one and two weeks respectively. Result of treatment was evaluated 7 ～ 14 d after the treatment as for the clinical cure rate, with or without acute rejection. Result The liver function of the observation group compared with that of the control group at one week was as AST(t =8. 03 ,P <0. 01), ALT(t =9. 09, P<0.01), TBIL(t =6.01,P<0.01), and at 2 week as AST(t=5.59,P<0.01), ALT(t =6.60,P< 0. 01), TBIL(t = 8.45,P <0. 01). The renal function of the observation group compared with that of the control group after one week as for BUN(t =6. 51 ,P <0. 01), Scr(t =5.66,P <0. 01). At 2 week,as BUN (t =7.61,P <0.01), Scr(t =6.91,P <0.01). The clinical cure rate of the control group was 7/13 (53%), that of the observation group was 12/14(86%) ; Two cases in the observation group and one in the control group had acute rejection which was successfully managed by methylprednisolone pulse therapy. Conclusion Caspofungin is an ideal alternative therapy for fungal infection after fiver transplantation.

Objective To investigate the effects of extractum trametes robiniphila murr combinated with hydroxyurea on proliferation and apoptosis of K562 cells and whether the 2 drugs have synergistic effects,and to detect the antiproliferation and apoptosis ratio of cells in diffe-rent groups were detected.Methods K562 cells were cultured in vitro,and logarithmic phase cells were used for study.MTT-inhibitory test and cell morphological analysis were employed to examine the effects of extractum trametes robiniphila murr alone or in combination with hydroxyurea on proliferation of K562 cells.The effects of extractum trametes robiniphila murr alone or in combinated with hydroxyurea on apoptosis of K562 cells were examined by flow cytometry.The expressions of bcr-abl,bax and bcl-xl mRNA levels were detected by way of reverse transcription polymerase chain reaction(RT-PCR).Results Extractum trametes robiniphila murr could inhibit proliferation and induce apoptosis of K562 cells,and extractum trametes robiniphila murr combinated with hydroxyurea had a synergistic effects on cell prolife-ration and apoptosis.The RT-PCR displayed that either extractum trametes robiniphila murr alone or in combinated with hydroxyurea could down-regulate the expression of bcl-xl mRNA and up-regulate the expression of bax mRNA and the effect of the 2 drugs had a synergistic effect on bcl-xl and bax mRNA expressions.Extractum trametes robiniphila murr alone could also down-regulate the expression of bcr-abl mRNA,but hydroxyurea alone did not show any effect on the expression of bcr-abl mRNA and the 2 drugs had no synergistic effect.Conclusions The effect of extractum trametes robiniphila murr combinated with hydroxyurea on the proliferation and apoptosis of K562 cells has a synergistic effect.The mechanism of antiproliferation and inducing apoptosis of extractum trametes robiniphila murr is probably related to down-regulating the expression of bcl-xl and bcr-abl mRNA and up-regulating the expression of bax mRNA.The synergistic effect of antiproliferation and inducing apoptosis of extractum trametes robiniphila murr combinated with hydroxyurea on K562 cells is probably related to down-regulating the expression of bcl-xl mRNA and up-regulating the expression of bax mRNA.

Methods The standardized questionnaires were answered by the parents of 2 986 school children during 2004 to 2005. Objective To explore the mechanism of relieving symptoms of angina pectoris with transmyocardial laser treatment. Methods Twenty mongrel dogs were divided randomly into 4 groups (5 dogs each): I. sham-operated negative control animals undergoing thoracotomy and pericardiotomy only; II. ligation of the coronary arteries to create acute myocardial ischemia; III. 40 minutes after production of acute myocardial ischemia, transmyocardial punctures were made with high power CO_2 laser; IV. chemical destruction of cardiac nerves by application of 85% phenol to the epicardium after myocardial ischemia. The myocardium was harvested for examination 2 weeks later. Cardiac sympathetic afferent nerve function and innervation was assessed with immunohistochemical staining of tyrosine hydroxylase (TH), which was the specific enzyme of sympathetic nerves, and computer image analysis was done. Histopathological changes in myocardium were also evaluated with haematoxylin-eosin staining. Results Immunohistochemical analysis showed positive staining in I and II groups but negative or weakly positive in III and IV groups. Objective To evaluate the value of 3-dimensional dynamic contrast-enhanced magnetic resonance angiography (3D DCE MRA) in the diagnosis of adrenal and renal diseases.Methods 3D DCE MRA was performed on 22 patients with adrenal neoplasm,23 patients with renal neoplasm and 2 patients with stenosis of renal artery. Comparison with surgical findings or DSA was made.Results 3D DCE MRA clearly reflected the vascular anatomy,satisfied diagnostic demands.Simultaneously,The extravascular lesions were also detected.Conclusion The 3D DCE MRA is a reliable non-damaged imaging diagnostic method.

The scavenging activities of Ophiopogon japonicus for superoxide anion (O_2~(·-)) radicals by HPLC-DAD coupled with chemiluminescence(CL) was studied and the active constituents in it were screened.The aqueous extracts of Ophiopogon japonicus were divided into the ethyl ether fraction and the n-butanol fraction.Their activities were investigated respectively and the reduction of CL with the pyrogallic acid 3-amino-phthalhydrazid (luminol) Na_2CO_3/NaHCO_3 buffer (pH 11.0) system worked as an activity index of the antioxidant constituents.Major active components in Ophiopogon japonicus could be identified by HPLC-DAD-MS/MS and the reference substances.The antioxidant activities of different ingredients were evaluated by standard potency equations using quercetin as positive control.The experimental data indicated that the scavenging activities of Ophiopogon japonicus for superoxide anion (O_2~(·-)) radicals are mainly from homoisoflavone in the ethyl ether fraction.It has shown that the method for on-line detection is an advantageous tool to rapidly determinate antioxidants from complex herbs.

Objective To observe normal range based on normal adult echocardiography with data mining retrospectively.Methods Analyzed the echocardiography data of 15 553 patients who came to Peking university People's Hospital from Nov 2012 to Nov 2014 without obvious abnormalities of cardiac structure and function.The reference range of 15 parameters were calculated and analyzed in correlation with gender,age and other factors for each group.Results The reference ranges of basic measurement parameters in two-dimensional echocardiography:main pulmonary artery diameter(MPA) (2.14 ± 0.22)cm,diameter of ascending aorta (asc Aorta) (2.87 ± 0.35) cm;The reference ranges of basic measurement parameters in M-mode echocardiography:aortic root diameter (Ao root diam) (2.82 ± 0.35) cm,left atrial dimension (LA dimension) (3.14 ± 0.37) cm,interventricular septal end-diastole thickness (Ⅳsd) (0.86 ±0.12)cm,left ventricular internal diameter at end-diastole (LVIDd)(4.67 ± 0.41)cm,left ventricular internal diameter at end-systole (LVIDs) (2.83 ± 0.34) cm,left ventricular posterior wall end-diastolic thickness(LVPWd) (0.85 ± 0.12) cm,end diastolic volume (EDV) (101.21 ± 19.83) ml,end-systole volume (ESV) (31.09 ± 9.07) ml,ejection fraction (EF) (69.55 ± 5.70) ％;The reference ranges of basic measurement parameters in pulsed doppler echocardiography:mitral A wave velocity (MVA) (73.14 ±16.42) cm/s,mitral E wave velocity(MVE) (84.18 ± 16.17) cm/s,E/A (1.21 ± 0.35),aortic peak velocity (Ao V2 max) (124.95 ± 19.87) cm/s.The measured parameters of males were higher than those of females and value of measured was increased with age for example measured parameters in LVIDd (r =0.225,P =0.00),IVSd(r =0.216,P =0.00),LA dimension (r =0.231,P =0.00) and E/A(r =0.212,P =0.00),with age were positive correlated.The range of 15 parameters in this study and the ASE 2015 guide are similar but have slightly different,such as this research shows that men's left ventricular end-diastolic diameter (4.77 ±0.41) cm,the ASE recommend (5.02 ±0.41) cm which is smaller than the result of this study.And this study for women (4.60 ± 0.40) cm is higher than the ASE recommended (4.50 + 0.36)cm.Conclusions Based on preliminary analysis of large data from normal adult echocardiography,the rang of measured values could be used as a normal reference value of echocardiographyfor routine practice in China.

Objective: The demand of new rural cooperative medical system in Jiangsu province of the quantitative research is analyzed. Results: The farmers of Jiangsu for medical need a lower tendency and middle-income households below the expenditure on basic medical needs. In the premise of the farmers’ live, increase the income of medical and the demand will increase too. Conclusion:It needs to carry out effective propaganda work, improve the level of financing of the new rural cooperative medical system, implement the reform of the medical system.
