1.Effects of CMTM5 on EG-VEGF in prostate cancer cells

Objective To investigate the effect of CKLF-like MARVEL transmembrane domain contai-ning member 5(CMTM 5)on EG-VEGF expression in prostate cancer cells , and to detect the potential mechanism of CMTM5 inhibiting prostate cancer .Methods The relative expression of CMTM 5 and EG-VEGF in prostate cells and prostate cancer cells was detected .Prostate cancer cells were given Plasmid transfection to overexpress CMTM5 and EG-VEGF expression was again detected .Then CMTM5 and EG-VEGF were compared between be-fore and after CMTM5 plasmid transfecting prostate cancer cells .Results Compared with the relative expresion of EG-VEGF and CMTM5 in prostate cells , prostate cancer cells showed high expression of EG-VEGF and low ex-pression of CMTM5, which were statistically significant , P<0.05.Compared with prostate cancer cells , the rela-tive expression of CMTM5 were obviously upregulated and EG-VEGF obviously decreased in prostate cancer cell after transfected by CMTM5 plasmid, which were statistically significant , P<0.05.Conclusions Prostate canc-er cells shows higher EG-VEGF expression and lower CMTM 5 campared with normal prostate cells .EG-VEGF is suppressed significantly when the prostate cancer cell is transfected by CMTM 5 plasmid and shows highlevel of CMTM5 expression , suggesting high expression of CMTM 5 may inhibit the development of prostate cancer by downregulating EG-VEGF expression .
2.Nursing of a pediatric patient with serious skin problems at the puncturing site of peripherally inserted central catheter after methotrexate use

Objective To summarize the nursing experience of a child with serious skin problems at the puncturing site of peripherally inserted central catheter (PICC) for methotrexate (MTX) infusion.MethodsA pediatric patient from the Hematology Ward of Beijing University People''s Hospital was found to be with serious skin problems at the puncturing site of PICC for MTX infusion.Local irradiation with ultraviolet, disinfection of local skin with iodine volts and 0.9% sodium chloride injection, coverage of the broken skin with alginate dressing, and fixation of PICC with sterile gauzes were applied.Results After 22 days of nursing, the pain and itching at the broken skin around PICC disappeared.The skin returned normal, without exudates and prolapse of catheter.Conclusions After MTX use via PICC, the children can easily become irritable due to local symptoms and their own feelings after skin allergy.Meanwhile, prolonged treatment and nursing may make the child cry and increase the stress of parents.Caregivers needs to explain the related factors of allergy to the children and their families, and give the correct nursing intervention, so as to improve local allergy and facilitate the retention of PICC.
3.Arthroscopic reconstruction of posterior cruciate ligament using hamstring tendon transfer and e-val uation of its clinical evaluation

Objective The present was to report the methods of arthroscopic reconstruction of poste-rior cruciate ligament by a two-strand semitendinosus tendon autograft and evaluate its clinical results.Method From June1998through June2002,28cases had undergone arthroscopic restoration of a torn pos-terior cruciate ligament,including22female and6male patients with an average age of21years(range,18to31years).The time-interval between the sustained injury and the surgery was25days to3months.The causes of injury of posterior cruciate ligament were sport activity in6cases,traffic accident in18cases and fall down in4cases.All of the patients suffered from knee pain,knee laxity and atrophy of quadriceps femoris.The posterior drawer test was positive in26cases preoperatively,in which6cases were associated with positive anterior drawer test.The Lachmans test was positive in all of28cases preoperatively.The piv-ot shift test was positive in12cases preoperatively.The concomitant injuries consisted of anterior cruciate ligament injury in6cases,meniscal tear in8cases and medial collateral ligament injury in5cases.Results Follow-up period extended from27to48months (mean,33months).According to the international knee documentation committee(IKDC)activity grades,grade D were26cases,and grade C were2cases preoper-atively,while postoperatively,grade A were9cases,grade B16cases,and grade C3cases at the last fol-low-up.The clinical assessment judging by Lysholm knee functional scoring system,the average scores be-fore operation was65.5?3.6;however the scores increased to an average of92.7?3.6postoperatively.Con-clusion Arthroscopic reconstruction of injured posterior cruciate ligament using hamstring tendon transfer passes through the tunnel more smoothly than the bone-patellar tendon-bone because of no bone blocks,furthermore,the tunnel disappears after the screw is absorbed to avoid the impact of the ligament the inside tunnel and prevent the tunnel extending gradually to cause the ligament laxity and knee instability.The at-tachment of the tendon on the outlet of tibial tunnel and the rigid anchoring of ligament on the outlet of femoral tunnel could provide enough strength for constructed posterior cruciate ligament.So the patients could begin postoperative rehabilitation as soon as possible and recover functional activity earlier.
4.Surgical treatment of pelvic malignant tumors with individualized hemi-pelvic

[Objective]To discuss the surgical extension and clinical effects of CAD custom-made hemipelvic for treating malignant pelvic tumors.[Method]The pelvis model was generated with its CT data by rapid prototyping.Simulated bone resection and prosthesis design were performed on the models.Surgical extension and made hemi-pelvic were designed.Nine patients received resection of pelvic tumor and reconstruction by individualized hemi-pelvic from June 2003 to June 2007.The series comprised three males and six females,four were diagnosed having giant cell tumor,two having chordoma,two having chondrosarcoma and one having osteosarcoma.According to Enneking's division,region Ⅱ(n=3),the region Ⅱ and Ⅲ(n=5),involved region Ⅰ,Ⅱand Ⅲ(n=1).[Result]The resection of tumor the implantation of prosthesis was easily accessible.The amount of bleeding was 5700 ml on an average.One patient suffered from infection and recovered after debridement.All patients began non-weight bearing walk with double crutches 4~6 weeks after operation and gradually abandoned crutches in 4 months.Three patients had local relapse,one osteosarcoma patient died of lung metastasis at 18 months after operation,one chondrosarcoma patient relapsed locally at 13 months after operation and died at 17 months after operation.One chordoma patient relapsed locally at 6 months operation and received the second operation.It was relapsed again at 6 months after the second operation and died at 3 months.Bolt was loosened in one patient,but the patient can walk with one crutch.No loosening was found in the 5 patients.The position of bilateral hips was symmetric.[Conclusion]The surgical extension of tumor designed by computer aided technical makes thorough resection possible.The custom-made hemi-pelvic is highly similar to the pelvis of patient in appearance,which makes the operation easy and produces good clinic results.The custom-made hemi-pelvic offers a good promise for the reconstruction of pelvic malignant tumors.
5.INVESTIGATION AND ANALYSIS OF PREVALENCE RATE OF INPATIENT NOSOCOMIAL INFECTION

Objective To understand the situation of nosocomial infections, the distribution of important pathogens of affected patients and the uses of antibacterial drugs.Methods The cross-sectional survey was ap-plied.Before the survey, the investigators were trained by the professionals of the department of hospital infection management.Medical records were reviewed one by one, checked with bedside physical examination.Results In this survey, 283 inpatients were investigated, with an actual check rate of 100%.Two inpatients were affected with nosocomial infection with the prevalence rate of 0.71%.There were two cases of infections (0.71%).Conclusion Investigation of the prevalence rate of nosocomial infection was an important means to monitor the hospital infection, which can monitor specifically, minimize the incidence of nosocomial infection and ensure medical safety by obtaining the information of the departments with high-risk nosocomial infection, the main infectious sites and pathogens.
6.Clinical significance of hypersensitive C-reactive protein in patients with acute coronary syndrome

Objective To study the clinical significance of hypersensitive C‐reactive protein(hs‐CRP) in acute coronary syndrome (ACS) .Methods 45 patients with ACS were selected as the ACS group ,including 26 cases of acute myocardial infarction (AMI group) and 19 cases of unstable angina pectoris (UAP group) ;30 patients with stable angina pectoris(SAP) were selected as the SAP group and 30 health people with healthy physical examination in the outpatient department as the control group .The latex‐en‐hanced immunoturbidimetry was adopted to detect the serum hs‐CRP levels in all selected subjects and the detection results were compared .Results The serum hs‐CRP level of ACS and SAP groups were significantly higher than that of the control group with statistical difference (P< 0 .01) .The hs‐CRP level of AMI and UAP groups was higher than that of the SAP group with statistical difference (P< 0 .05) .Conclusion hs‐CRP is an independent risk factor for ACS and one of the most powerful predictors ,able to quickly and accurately conduct early diagnosis ,risk stratification ,condition monitoring and prognosis judgment in the patients with ACS .
7.Changes in resistance of Staphylococcus aureus to vancomycin and linezolid in Sichuan Province in 2011-2015

Objective To investigate the resistance of Staphylococcus aureus (S.aureus) to vancomycin and linezolid in Sichuan Province, and provide reference for clinical anti-infection treatment.Methods S.aureus and methicillin-resistant S.aureus(MRSA) from 71 hospitals in Sichuan Province in 2011-2015 were collected, minimum inhibitory concentration(MIC) values of vancomycin and linezolid against S.aureus and MRSA in each year were calculated.Results A total of 51 976 strains of S.aureus were collected in 5 years, 14 361 of which were MRSA, isolation rate of MRSA decreased from 36.02% in 2011 to 25.56% in 2015, which showed a downward trend (x2=160.72,P<0.05).From 2011 to 2015, MIC50 of vancomycin against S.aureus were 1, 0.5, 0.5, 1, and 1μg/mL respectively,MIC90 increased from 1μg/mL to 2μg/mL;the mean MIC50 of linezolid was 2μg/mL,MIC90 increased from 2μg/mL to 4μg/mL.Change in MIC50 and MIC90 of vancomycin against MRSA in 2011-2015 were obvious, which increased from 0.5, 1μg/mL to 2μg/mL respectively;the mean MIC50 of linezolid was 2μg/mL,MIC90 increased from 2μg/mL to 4μg/mL.Conclusion The isolates rate of MRSA had a decreasing trend,but there is a upward trend of MIC50 and MIC90 of vancomycin and linezolid against MRSA, bacterial resistance surveillance needs to be strengthened to provide evidence for rational clinical antimicrobial therapy.

9.Penehyclidime hydrochloride in treatment for patients with moderate and severe organophosphorus poisoning

One hundred and forty-nine patients with moderate and severe organophosphorus poisoning were treated in Fangshan District Hospital, Beijing during July 2005 to July 2007, 77 cases of them with intravenous atropine and intramuscular pyraloxime methylchloride and 72 with intramuscular penehyclidime hydrochloride plus pyraloxime methylchloride. Time of atrepinization, number of medication injection, time of symptom disappearance, recovery time of cholinesterase, length of hospital stay in those with penehyclidime hydrochloride plus pyraloxime methylchloride were all better than those with atropine and pyraloxime methylchloride (P<0.05). Percentage of atropine poisoning, atropine dependence, organophosphorus poisoning rebound and adverse reaction in those with penehyclidime hydrochloride plus pyraloxime methylchloride were lower than those in those with atropine and pyraloxime methylchloride (P< 0.05).
