OBJECTIVE: To provide reference for the application of automatic tablet packing machine in the inpatients dispensary.METHODS: The working principle and flow,the main structure and function of the automatic tablet packing machine used in our hospital were introduced,and its application value was evaluated and the problems remained to be dealt with were pointed out.RESULTS: The automatic tablet packing machine had the advantages of high accuracy and efficiency,and reduced working intensity,however,the costs for the packaging and consumables,and the low drug quality were the problems remained to be solved.CONCLUSIONS: Automatic tablet packing machine is suitable for the dispensing of oral single dose of tablets for inpatient patients,and it helps to improve the work quality and the service level in the dispensary for inpatients.
2.Study on clinical anatomy of the accessory nerve,the great auricular nerve and the transverse nerve of neck in neck levelⅡ-Ⅳ

OBJECTIVE To study the anatomicallandmarks of the accessory nerve (AN) , the great auricular nerve (GAN)and the transverse nerve of neck (TN) in neck level Ⅱ-Ⅳ and provide a reliable surgical basis for treatment of functional neck dissection in neck level Ⅱ-Ⅳ. METHODS The courses of the AN, GAN and TN were studied in 42 patients (56 sides) who underwent lateral neck dissection. Meanwhile, the correlations among the three nerves above and their surrounding structures were observed. RESULTS After emerging from the inferior border of the posterior belly of digastric muscle, the AN descends obliquely downward and backward before the transverse process along with the sternocleidomastoid branch of occipital artery to the upper part of the deep surface of the sternomastoid muscle. The distance from the mastoid process to the point where AN enters the sternocleidomastoid muscle is (4.93?0.75) cm. The point where AN emerges from sternocleidomastoid muscle (point A) located above the point where the GAN exits the sternocleidomastoid muscle (point B). The distance between the two points is (1.04?0.59) cm while the distance from point A to the middle point of clavicle (point C) is (8.09?0.65) cm. The GAN leaves the cervical plexus at the posterior border of the sternocleidomastoid muscle and run anteriorly between the lateral surface of this muscle and the deep surface of platysma. It is (6.37?0.73) cm long from point B to point D where the GAN send out posterior aurem branches, auricular lobule branches and anterior auricular branches. The distance from point B to point C is (7.67?1.00) cm. Most of GANand external jugular vein run nearly in parallel and the distance between them is (1.02?0.61) cm. The point where the TN emerges from the posterior border of the sternocleidomastoid muscle is below point B. Then the TN which is below the external jugular vein runs almost horizontally over the surface of the sternocleidomastoid muscle to sector branches in the lateral neck and part of its branches are distributed in the lateral neck after going across the anterior jugular vein. OBJECTIVE:To prepare compound Weifu emulsion and to observe its therapeutic effect on infantile eczema METHODS:The emulsion was prepared with a formulation consisting of Scutellaria,Chinese Angelica,Chuanxiong and dimethyl silicone oil etc and the quality control method was established RESULTS & CONCLUSION:This preparation is stable in quality and feasible in quality control;The effective rate in treating infantile eczema has reached 90% and no obvious adverse reactions were found This emulsion is worthy of recommendation for clinical use
4.Clinical Observation of Acupuncture plus Medicinal Application for Dysmenorrhea Due to Blood Stagnation Led by Cold

ObjectiveTo compare the efficacies between conventional acupuncture plus acupoint application and conventional acupuncture plus TDP in treating dysmenorrhea due to blood stagnation led by cold.MethodSixty patients were randomized into two groups, 31 cases in the treatment group and 29 cases in the control group. The treatment group was intervened by conventional acupuncture plus acupoint application, while the control group was by conventional acupuncture plus TDP. Before and after the intervention, McGill Pain Questionnaire was adopted to compare the Pain Rating Index (PRI) score, Visual Analogue Scale (VAS) score, and Present Pain Index (PPI) score, and the therapeutic efficacy was also compared.ResultThe recovery rate was 58.1% in the treatment group, significantly higher than 24.1% in the control group (P<0.01); the total effective rate was 90.3% in the treatment group, significantly higher than 62.1% in the control group (P<0.01), indicating that the therapeutic efficacy of the treatment group was more significant than that of the control group. Before the intervention, there were no significant differences in comparing the PRI, VAS, and PPI scores between the two groups (P>0.05), suggesting the comparability. After the intervention, there were significant differences in comparing the PRI, VAS, and PPI scores between the two groups (P<0.05), and the PRI, VAS, and PPI scores were markedly decreased in the treatment group.ConclusionConventional acupuncture plus acupoint application can improve dysmenorrhea symptoms and reduce patient’s pain, and can produce a more significant efficacy compared with conventional acupuncture plus TDP.
5.CLINICAL APPLICATION OF THE MEDIAL MULTIPLEX FLAP PEDICLED WITH THE POSTERIOR TIBIAL VESSEL

Objective To investigate the clinical results of the medial multiplex flap pedicled with the posterior tibial vessel. Methods Twelve cases with soft tissue defects and bone defects of limbs were treated with the medial multiplex flap pedicled with the posterior tibial vessel from September 1992 to May 1999. Among them, bone and soft tissue defects following opened fracture in 7 cases, chronic ulcer following chronic osteomyelitis in 2 cases, melanoepithelioma in 2 cases, bone and soft tissue defects following osteoma resection in 1 case. The bone defect area was from 2.5 cm×5.0 cm to 4.5 cm ×11.0 cm. Free graft was performed in 5 cases, bridged transposition in 3 cases and reversal transposition in 4 cases, among them, periosteal myocutaneous flap with autogenous or allogeneic bone grafting in 8 cases, myocutaneous flap in 4 cases. The area of the flaps from 6 cm ×8 cm to 12 cm×25 cm. Results All flaps were healed by first intention, but in the distal fragments of bigger flaps were partially necrosed in 2 cases. In 10 cases bone healing were obtained after 16 weeks of operation according to the X-ray photos. All cases were followed up from 6 to 18 months. All cases achieved satisfactory result but 1 case died because of lung metastasis of osteoma. Conclusion The multiplex graft pedicled with the posterior tibial vessel is an ideal graft for repairing the large soft tissue defects and bone defects, because it has such advantages as adequate blood supply, big vascular diameter, long pedicle and big dermatomic area.

We suggested that we should bring into full play the tutors and their groups function and excavate the connotation of groups’ morality education on the basis of the scientific research groups to develop the medical students' ideological and political education.
7.Discussing about the method of using center loaded pressure to produce the cleaning stomach machine of once for all

Objective To discuss the critical effect of studying and producing the practical cleaning stomach machine of once for all. Methods The process of the cleaning stomach by using the center loaded pressure system equipment was imitated, and the time of the liquid inside and outside, the whole cleaning time and the problems appeared during the cleaning stomach process was recorded. Results After finishing designing the equipment,the mimic stomach was used for critical experiment, all kinds of connecting tubes were airtight and no leaking air and water phenomena, the cleaning process was successful and it could save time to achieve the designing goal.Conclusion The cleaning stomach machine of once for all produced by using the center loaded pressure system had well solved the tube sterilized problems appeared by cleaning machine on hand, and could reduce the risk of hospital inflecting, the cleaning stomach effect was comparatively good,it had practical value and deserve to critical expanding.
8.Clinical analysis of postoperative radiotherapy in type B3 thymoma patients

Objective:To evaluate the clinical value and the prognostic factors of postoperative radiotherapy in type B3 thymoma pa-tients. Methods:A total of 159 patients with thymoma were treated by surgery and postoperative radiotherapy. According to Masaoka staging system, 12, 33, 62, and 52 patients had stageⅠ,Ⅱ,Ⅲ, andⅣlesions, respectively. Myasthenia gravis existed in 38 patients. Altogether 58 patients underwent chemotherapy. Overall survival, disease-free survival, and local control rates were calculated by Ka-plan-Meier method. Prognostic factors were analyzed by Cox regression model. Results:With a median follow-up of 52 months (8-125 months), the overall 5-year survival rate was 81.6%. The 5-year progression-free survival rate was 76.2%. The 5-year local control rate was 82.6%. The recurrence rate was 32.6%, and the metastatic rate was 9.3%. In the univariate analysis, tumor size, Masaoka stage, re-section margin, radiotherapy, and chemotherapy were significantly associated with 5-year overall survival and progression-free surviv-al (P<0.05). In the multivariate analysis, Masaoka stage, resection margin, and radiotherapy were independent prognostic factors of 5-year progression-free survival (P<0.05). Radiotherapy could improve the regional control rate and the overall survival of patients in Ma-saoka stagesⅢ-Ⅳ. Conclusion:The major failure mode for type B3 thymoma is the recurrence of pleure. Radiotherapy can improve the regional control rate and the overall survival of patients in advanced stages. Masaoka staging, surgical margin, and radiotherapy are the independent prognostic factors for type B3 thymoma treated by postoperative radiotherapy.
9.Clinical features and drug selection in 54 patients with inflammatory bowel disease and comorbid autoimmune disease

Objective To explore the differences in disease location,pathological feature,disease severity,extraintestinal manifestations and drug treatment between inflammatory bowel disease (IBD) patients with comorbid autoimmune disease (AD) and simple IBD patients.Methods From January 2009 to December 2014,the clinical data of 54 IBD patients with comorbid AD and at the same period 74 simple hospitalized IBD patients were retrospectively analyzed.According to IBD type and whether combined with AD,patients were divided into Crohn's disease (CD)+AD group (n=16),CD group (n=26),ulcerative colitis (UC)+AD group (n=38) and UC group (n=48).Chi square test was performed to compare the differences in disease severity,location,extraintestinal manifestations and drug treatment between IBD patients with and without AD.Results There was no statistically significant difference in location among four groups (all P>0.05).The most common concomitant AD of IBD was rheumatoid arthritis (20.4%,11/54) and ankylosing spondylitis (13.0%,7/54).The proportion of mild active patients of CD+ AD group was lower than that of CD group (2/16 vs 53.8% (14/26),x2 =7.180,P=0.007),while the proportion of severe active patients was significantly higher that of CD group (6/16 vs 0,x2 =8.519,P=0.004).There was no statistically significant difference in moderate active patients between the two groups (P=0.808).Main type of patients of UC+ AD group (76.3 %,29/38) and UC group (68.8 %,33/48) were moderate active patients.There was no statistically significant difference in disease stage and location (all P>0.05).The incidence of extraintestinal manifestations of IBD+AD group (55.6 %,30/ 54) was significantly higher than that of IBD group (9.5 %,7/74,x2 =32.279,P<0.01),and the main manifestation was arthritis (37.0% (20/54) vs 5.4% (4/74),x2=20.504,P<0.01).The rate of glucocorticoid and immunosuppressant application in IBD+AD group was higher than that of IBD group (40.7% (22/54) vs 17.6%(13/74),x2 =8.438,P=0.004;20.4%(11/54) vs 0,x2=14.000,P< 0.01).Conclusions The condition of patients with IBD and comorbid AD is more severe,and the incidence of extraintestinal manifestations is higher.Early treated with glucocorticoid and immunosuppressant could effectively achieve remission.
